Job Purpose: This is an exciting joint full time post for an experienced, registered and qualified Nurse. The successful applicant will spend the first part of their working work with the Disabled Children s Intervention Team (DCIT) and the second part working with children and young people from Beaulieu House Residential/Respite Unit. Undertake timely clinical learning disability nursing assessments for children referred for continuing health care. 4. Deliver clinical interventions following assessment; this includes direct work with children and young people. 5. Contribute to group work and training and other opportunities where appropriate. 6. Liaise and link with all other relevant children s services e.g. CYP and families services, colleagues in arly Years services, Primary Health Care Teams, other learning disability staff, staff in statutory and independent sector provider agencies and those that relate to the transitional period with adult care. 7. Co-ordinate clients with complex health needs whose care packages are either funded jointly by health and social care or solely with health. Work closely with the Children s Community Nursing Team and access clinical supervision to ensure safe and supported practice. Initiate and support any nursing procedure, process, therapy or treatment that may be recommended by a medical practitioner and or needed by the child/ young person that is considered to fall within safe practise of the nursing profession. Devise intervention plans that promote the health and wellbeing of the individual. Interventions will broadly follow a nursing process model with planned outcomes described, discussed and recorded with the client in advance, reviewed and summaries prior to case closure Assess situations/ information / cases. Develop and recommend solutions which ensure compliance with appropriate regulations / legislation. Assess and mitigate any risks associated with the recommendations made. Undertake nursing assessments for children with learning disabilities and provide written reports to commissioners as required. 1. To ensure that any hazards are dealt with promptly or reported to the manager as appropriate. 3. To contribute and carry out quality monitoring and risk assessment procedures as appropriate. 2. To co-ordinate and supervise staff on a day to day basis. 3. To ensure staff have regular updates on specific procedures, changes in protocol and new medication and conditions. 7. To contribute to the assessment of training needs of the staff. 8. To advise staff on matters relating to hygiene and infection control. 9. To provide health-related information/support to staff as necessary. 10. To attend and facilitate staff development courses, training courses, seminars etc. as and when required. To ensure that the Health and Safety and Child Protection policies for the Isle of Wight Council and those specific to Beaulieu and DCIT are adhered to at all times. Support improvements in own area through recommending improvements in procedures/ process. Support the delivery of improvements in own area. Contribute to the planning and delivery of projects / specialist initiatives as part of a project team. Carry out all duties and responsibilities with reasonable care for the health and safety of ourselves and others and report any potential hazards or unsafe practices to line manager. To participate in any statutory, required or recommended training that may maintain or improve performance as a nurse and that, which is essential to maintain NMC registration. 1. To assess, deliver, facilitate and evaluate the health and intervention focus of children and young people and work with other health care professionals to manage health complications of children with severe, complex/multiple disabilities and provide the necessary support to staff. 2. To observe record and monitor health needs. 3. To undertake, develop and maintain care plans for each child as appropriate. 4. To seek further medical advice when necessary 5. To oversee the maintenance of the AD 6. To inform and maintain communication with parents/legal guardian in all aspects of the child s health issues. 7. To provide, administer and monitor the effects of, all regular medications 8. To maintain adequate stock levels and arrange for safe disposal of medication

4 Co-ordinate the day to day allocation of tasks to others, including where appropriate supporting and managing volunteers. Assist with the preparation and submission of bids for short and long term funding. Monitor and report on service standards / performance indicators /expenditure as required. Follow all policies and procedures which apply to the job and understand the reasons for this. Monitor or process financial or budgetary information. Contribute to budget planning as required. Mental Health, Paediatric or General nursing] with appropriate experience supporting children/adults with learning disabilities. Be able to work independently and autonomously. Motivate both self and others to promote and enable driving forward change agenda. Demonstrate practice at a high level of competence in assessment and care planning. Demonstrate good interpersonal and negation skills to manage conflict where change is required. Be able to impart to families highly contentious information around their child s care.
