2 Applications are invited from enthusiastic and dynamic team players for the appointment of 3 full time Consultant Thoracic Radiologists with special interest in cross sectional imaging and intervention, to work in a dynamic team in a large Thoracic Tertiary Centre at the Manchester University NHS Foundation Trust. These posts give the opportunities to be involved in many other thoracic services at the North West Lung Centre, North West Thoracic surgical centre, The National Aspergillosis Centre and the North West Heart-Lung Transplant Centre. Additionally, the posts provide an abundance of research opportunities including participating in delivering the broad portfolio of research studies through NIHR Biomedical Research Unit and Thoracic Oncology Research Hub (TORCH) The lung cancer team at Wythenshawe have won numerous awards and have award outcomes pending including for their work around the RAPID Programme and Lung Health Check/Lung Cancer Screening Programme. Following a major expansion in Respiratory services due to increase in lung Cancer diagnosis and treatment, we are seeking 3 enthusiastic, patient facing Consultant colleagues to join our existing team of 7 consultant Thoracic Radiologists (Dr Melanie Greaves, Dr Elaine Smith, Dr Anna Sharman, Dr Rebecca Duerden, Dr Klaus Irion, Dr Devinda Karunaratne and Dr Marie Stokes). One post is a replacement following retirement and there are 2 new posts. Due to the wide range of specialist diseases and imaging performed, any additional training will be provided if required although we are also interested in existing specialist thoracic radiologists. This post has duties primarily based at Wythenshawe Hospital however the new city wide PACS system under procurement will allow reporting across the whole Trust, indeed across the city and also from home, which we see as an important aspect to improve work-life balance. In particular, applicants who are seeking a job-sharing or part-time appointment are welcome to apply. If successful, the job content will be modified in consultation with colleagues and the needs of the service. The appointee must be on the Specialist Register of the GMC, or be within 6 months of CCT at the time of interview. The team provides a high quality, patient-centred delivery of both diagnostic and therapeutic thoracic imaging in close collaboration with clinicians of the North West Lung Centre, North West Thoracic Surgical Centre, the National Aspergillosis Centre and North West Heart-Lung Transplantation Centre. The Trust is seeking to improve cancer diagnosis following a nationally acclaimed pilot in lung cancer screening and provide tertiary review of external imaging to facilitate patient care across all thoracic services. A significant focus of the post-holder s work will be to ensure sustainability of the award-winning RAPID diagnostic thoracic oncology hub that provides prompt radiological reporting and same day histological sampling where necessary. Collaborative reporting of external imaging from South Sector (Leighton/Macclesfield) partners to facilitate improvements in regional working will also be required. In addition, the successful applicant will provide radiology support to the first NHS UK lung cancer screening programme, ensuring high quality reporting of low-dose CT scans, assessment day imaging for positive screens and prompt reporting and next-day biopsy, where appropriate. We have procured CAD nodule follow-up tools to provide triage and ensure ease of follow-up comparisons. P a g e 2

3 The applicant will be expected to contribute to the interventional service that currently performs approximately 40 image guided biopsies (lung & pleural) per month including those referred via the regional Difficult Biopsy Service and in particular to develop fiducial marker placement and thermal ablation of early stage malignancy. Commitment to a broad range of thoracic MDTs will be required, specifically preparation and presentation at twice weekly thoracic oncology MDTs including the regional mesothelioma MDT. Additional MDTs available for regional services include interstitial lung disease, transplant, ECMO, emphysema and asthma. The appointees will be expected to provide cross-cover and take part in regular departmental audit and research activities. The post holder will contribute a single session to acute general radiology and participate in the on-call rota. The thoracic imaging section participates in a broad portfolio of research studies through the NIHR Biomedical Research Unit and the Thoracic Oncology Research Hub (TORCH). It encompasses research of benign thoracic disease and thoracic oncology, the latter focused on early detection, surgery, adjuvant chemotherapy and mesothelioma. The majority of input is through reporting of research scans to RECIST criteria and collaborators in clinical research, but opportunities exist for the post holder to pursue a complementary portfolio, supported by a trials infrastructure, if required. With excellent academic links to the University of Manchester and research into cancer, airways disease and infectious diseases there are substantial opportunities to lead or develop regional and national collaborations. An evidenced interest in teaching is desirable but not essential. Previous experience of establishing/supporting a post-cct thoracic radiology clinical training fellowship would be advantageous. The successful candidates will be required to work effectively and positively within the multidisciplinary team adopting and developing innovative working practices for the benefit of the patient experience. This creates a new and exciting opportunity for the future running and development of services that P a g e 3

4 we offer our patients and will also allow for closer working relationships across Manchester, leading to opportunities for service development and research between the departments. THE TRUST Manchester University NHS Foundation Trust (MFT) achieved Foundation Status in October The Trust provides secondary and tertiary services for a population in the North West of England of up to 5 million and the Oxford Road site is located 3km south of Manchester City Centre with its rich cultural and sporting heritage and excellent road, rail and air links to the rest of the UK, Europe and internationally. Radiology services across all sites are delivered at both the Oxford Road and Wythenshawe sites having merged to form one large imaging directorate with regional and national aspirations of excellence. The directorate has applied for ISAS accreditation which should be achieved over the next 2 years. Common directorate meetings are being held and cross site PACS access should be available in late 2018/early The Nuclear Medicine departments merged in January 2018 with joint reporting lists and on-call being implemented. Four hospitals are based on the Oxford Road site; previously Central Manchester Foundation Trust: Manchester Royal Infirmary, Manchester Royal Eye Hospital St Mary s Hospital for Women and the Royal Manchester Children s Hospital. The site has seen completion of a 450m New Hospital Development (NHD) with the rebuilding of the Royal Eye Hospital, St Marys Hospital for Women and new medical wards of the Manchester Royal Infirmary in addition to the opening of the Manchester Children s Hospital. Manchester Royal Infirmary provides all major adult medical specialities including Rheumatology, Gastroenterology, Hepatology, Nephrology, Respiratory and Cardiology, Diabetes and Endocrinology. Surgical units include Orthopaedics, Hepato-Biliary, Colorectal, Upper GI, Urology, Renal transplantation, Cardiothoracic surgery, ENT and vascular surgery and has recently been designated as a complex vascular centre. The Manchester Heart Centre sits within Manchester Royal Infirmary and is a major provider of cardiac care with over 10,000 new patients a year not only from Greater Manchester and the wider North West region but also North Wales and the Isle of Man. The centre provides specialist care for adult congenital heart disease. It houses 5 cardiac catheterisation labs and was opened in 2006 as part of the NHD. The Wythenshawe and Withington sites, (previously University Hospital of South Manchester NHS Foundation Trust), provide district hospital services for the local communities and many specialist tertiary services for Greater Manchester and the whole of the North West. The specialist tertiary services include cardiology and cardiac surgery, thoracic surgery, heart and lung transplantation, complex respiratory services, vascular services, burns care and plastic surgery, cancer services and breast care. In partnership with the University of Manchester and other education providers both the merging trusts are recognised regionally and nationally for the quality of our teaching and research programmes. Our strong links with the University of Manchester ensure that we support excellent research in cancer, lung, vascular and gastro-intestinal diseases, tissue and wound management, and medical education. 5 The capital investment programme at Wythenshawe and Withington Hospitals has supported our ambition to develop the site as a major health campus. In 2007 we opened the 15M Nightingale Centre and Genesis Prevention Centre, Europe s largest breast cancer prevention facility. The 20M expansion in our North West Heart Centre, opened in May 2008, provides a state-of-the-art intensive care and investigation facilities, in July 2008 we commissioned a cardiac MRI scanner and the Board approved a further investment of 8M to expand our regional cystic fibrosis service. As a consequence of the changes outlined in Making It Better relating to maternity and children s services, the Board approved a 20.3m investment for a substantial investment in expanding our maternity services in March The Directorate of Clinical Radiology The principle Radiology Departments within the Radiology Directorate are as follows: Wythenshawe and Withington sites The department is sited in several locations across the site and contains the following facilities: Radiology Acute (A) Block (next to the Emergency Department): This provides the main acute radiology service to the site with 2x128 slice CT scanners, 1 large bore Aera MR Scanner, 3 Interventional Rooms, 2 Ultrasound Rooms, CR and DR rooms for plain film radiography and a 4 bed Recovery Area. Most Consultant offices are here with Directorate Manager, Admin & Clerical staff, Radiology Trainees room, PACS Reporting Area, Seminar Room, Offices of Lead Radiographers for MR/CT, and Staff Room. Radiology F Block: This represents the outpatient focus with 1 CT scanner, Nuclear Medicine Department with 1 Gamma Camera about to be replaced with a SPECT-CT camera, Fluoroscopy Room, 1 Ultrasound Room, Plain film rooms, including a DR Chest Room also being replaced in Maternity & Womens Health Department: 4 Ultrasound rooms. Withington Community Hospital: Plain Film room, Fluoroscopy room, Room for Ultrasound, Pad for mobile MR Scanner. Nightingale Centre & Genesis Breast Cancer Prevention Centre: Digital Mammography Units, Ultrasound, Clinic Assessment Rooms, Reporting Area. Offices for Consultants, Admin & Clerical Offices. Cardiac unit: 2 cardiac MRI units and one dedicated Toshiba cardiac scanner. Two hybrid theatres opened in July 2014 based within the theatre complex for interventional cases, in particular EVARs and TAVIs. P a g e 5

6 Trafford General Hospital and Altrincham General - also form part of MFT Situated 12km to the east of the Oxford Road site this department has 3 general rooms, a 4 room ultrasound department 16 slice CT and a 1.5T MR system. Manchester Royal Infirmary This provides the main adult radiology services for the Oxford Road campus and there are over 20 examination rooms. The department was originally opened in 1990 and enlarged in 1995 and In November 2014 a 16-bedded recovery and day case area opened allowing for patient admission under the direct care of Radiology unit and space for radiology clinics. A new MR unit will open early in 2018 and plans are underway to redesign the interface with the Accident and Emergency department incorporating a CT scanner with A & E adjacent to the x -ray department. Royal Manchester Children s Hospital This department was opened in June 2009 as part of the new Children s Hospital. It is an 11 room department and has a full range of facilities including a 1.5T MR system, 128 Slice CT and a dedicated paediatric gamma camera. St Marys Hospital This department was also part of the New Hospital Development and opened in July There are 7 Ultrasound rooms providing a comprehensive obstetric and gynaecological service including the Regional Foetal Management Unit. The Department of Nuclear Medicine is a separate directorate and there are 3 Nuclear Medicine physicians having close links with Radiology; the department has a significant role in supporting radiology training. The nuclear medicine service includes Both PET-CT and SPECT-CT. PET MR is available on site by April 2017 and will be only the second institute within the UK to offer access to this service on site. The Nuclear Medicine Centre performs approximately 7,000 tests annually, covering a very wide range of procedures and provides a radionuclide therapy service for non-malignant conditions. It is a well-staffed specialist department in which the physics support and radiopharmaceutical services are fully integrated within a multi-disciplinary team of 40 people. This is a training site for nuclear medicine physicians, radiologists, cardiologists, non-medical scientists and technologists. The Directorate also has responsibility for the single camera nuclear medicine unit at Wythenshawe hospital. The five gamma cameras include facilities for SPECT-CT, nuclear cardiology and mobile imaging. In addition there is a self-contained paediatric area with its own gamma camera within the centre which is run by a specialist paediatric team from the Children s Hospital. The PET-CT suite is equipped with the Biograph mct scanner with 64 slice CT. P a g e 6

7 The Wythenshawe Cardiology Unit also has 5 catheter labs as well as a Toshiba cardiac CT and 2 Cardiac MR units. The Wythenshawe site provides district general hospital services for its local communities and many specialist tertiary services for Greater Manchester and the whole of the North West. Its specialist tertiary services include cardiology and cardiac surgery, thoracic surgery, heart and lung transplantation, complex respiratory services, vascular services, burns care and plastic surgery, cancer services and breast care. Although no longer a major trauma centre the hospital continues to receive patients who are transferred in by the North West Air Ambulance Service. In addition the hospital provides a District General role for our local community in collaboration with our south sector providers. The Dental Hospital is located in the University. There is a Department of Dental Radiology which provides the majority of Dental imaging. However patients attend Manchester Royal Infirmary for MR and CT examinations. The Welcome Trust located in the CMFT campus offers research opportunity. Clinical and research Cardiac MR scans are done on the 3T MR system located here. Structure The Central Manchester Foundation Trust has set up Clinical Sciences Services as a virtual organisation providing radiology and other facilities across all its sites and the combined thoracic radiology team will thus provide thoracic Radiology to the City of Manchester. Radiology services across all sites are delivered by one large directorate with regional and national aspirations of excellence, the directorate has applied for ISAS accreditation which should be achieved over the next 2 years. The Radiology Manager is Ms Catherine Walsh. The Director for imaging is Dr S O Shea (tel ) and the Clinical Director for adult imaging is Dr R Sethi (Tel ) and Childrens Radiology (Dr R Hawkes/Dr N Wright) The Directorates of Radiology, Anaesthesia, Laboratory Medicine and Nuclear Medicine, form the Division of Clinical Scientific Services. The Medical director of the New division is Dr Jane Eddlestone and the Chief Executive is Mr I Lurcock who reports directly to the Trust Executive Management Team. P a g e 7

With re-configuration of the healthcare provision throughout Greater Manchester there is a move to more collaborative working between the Trusts and cross-site working between MFT and North Manchester General Hospital is encouraged, with plans for NMGH to merge with the trust in late Combined PACS procurement is currently in the process of tendering to allow a more versatile PACS system to integrate the current Greater Manchester systems. Office and Secretarial The appointee will be allocated an office space and will be allocated a specific secretary for personal use. They will be provided with a personal computer and internet connection according to the Trust policy. Information Technology The appointee will be encouraged to pro-actively seek to work with the Trust s information technology resources, to exploit benefits in delivering improved patient care. 9 Remote access to the radiology CRIS and web based PACS is provided from home by the use of a personal VDI fob/ access code. The Trust is fully equipped with an integrated RIS (HSS) and PACS (GE) system. A voice recognition dictation system (Dragon Dictate) is in place. There is a well-developed Trust patient management system which contains a radiology request module (ICE). All radiology requests for hospital patients are generated using this electronic pathway which will subsequently be integrated into the electronic patient record which is being procured. Teaching The Trust currently provides training places for 29 SpRs in radiology and is one of the key centres in the largest radiology training scheme in the country. The Dean of Postgraduate Medical Studies for the North West Region, based at the Medical School, is Professor Jackie Hayden. The candidate will, along with all the other consultant staff, play a role in SpR training and education within the Trust. The Manchester Training scheme has a comprehensive programme of lectures and tutorials and participation in these would be encouraged. Training is also provided leading to the Postgraduate Diploma in Medical Ultrasound. MFT is the largest undergraduate teaching Trust in the city. For the present the two main hospitals will retain their separate identities for undergraduate purposes. Students work within the Radiology Directorates during special study modules, research options and problem based learning sessions. The Trust plays a major role in Undergraduate Medical Education with around 800 students at any one time in years 3, 4 and 5 of the Undergraduate Medical Curriculum for the University of Manchester and many of these students go on to become our Foundation Year doctors. As two teaching hospitals with strong affiliations to the University of Manchester both recruit large numbers of patients into research studies and clinical trials. 10 JOB DESCRIPTION Job Title: Responsible to: Accountable to: Full Time: Consultant in Thoracic Radiology Clinical Director in Radiology The Clinical Sciences Services Medical Director 10 PAs Salary: 76, , Introduction This post is a 10 PA whole time equivalent new post based at Wythenshawe Hospital as part of Manchester University NHS foundation Trust. The appointment is subject to the terms and conditions of the new Consultant Contract. The duties of the post are to contribute to and develop the thoracic radiology service, in collaboration with thoracic medicine & surgery, including providing a difficult biopsy service across Greater Manchester and beyond to ensure and ensure a prompt, efficient and patient orientated service across all work domains, including reporting, intervention and other patient facing work. As a large Tertiary centre MDTs are undertaken collaboratively across GM and include multiple MDTs: a. Twice weekly lung cancer MDT including regional mesothelioma MDT b. Twice weekly regional interstitial lung disease MDT c. Monthly joint rheumatology interstitial lung disease MDT d. Twice weekly transplant MDT e. Monthly severe asthma MDT f. Twice monthly emphysema MDT g. Monthly occupational lung disease MDT h. Weekly ECMO MDT i. Weekly general respiratory non cancer meeting The Lung Cancer Screening reporting will be predominately be done by external Radiologists with quality assurance undertaken and completed by in house Radiologists The post holder will be expected to use Computer Aided Detection (CAD) and nodule software to aid reporting and standardise reporting. Interventional skills are advantageous including complex/high risk lung biopsies with an expectation of performing lung biopsies a week, as a team. The North West Lung Centre has a dedicated USS machine to aid the delivery of same day stage 4 biopsies. NB: Support will be provided if the successful candidate has limited experience of performing such procedures. P a g e 10

11 The appointee will also be expected to take part in regular departmental audit and research activities. Job Plan The proposed job plan is attached. Whilst it is intended that the number and type of activities are reflected in this plan, the timing of sessions may be changed and will be agreed with the Clinical Director at the time of appointment. The job will be based at Wythenshawe Hospital but sessions at the other Centres may be incorporated on discussion with the successful candidate. The appointee will be contracted for 10 Programmed Activities as per the new Consultant Contract, 8.0 related to Direct Clinical Care and 2.0 for Supporting Activities. Part-time or job share working is possible. The appointee would be expected to cover for colleagues annual, study and sick leave. The Directorate is fully committed to skill mix for Radiographers. Radiographers currently undertake A&E reporting, nuclear medicine reporting, IV injections; perform micturating cystourethrograms, barium studies, Advanced Practice roles in CT, HSG and diagnostic Angiographic lists. The successful candidate would be expected to support further skill mix initiatives aimed at producing a responsive service. The Post 1. The post holder will be a member of the thoracic radiology team based in Wythenshawe hospital but supporting the whole Trust, acting as a source of advice for consultant colleagues and general practitioners. 2. Develop good working relationships with professional colleagues within the Radiology Department, the Trust, primary care and with Regional Specialty colleagues. 3. Support Trainees, Radiographers and Sonographers in delivering the thoracic radiology service to patients. 4. Adopt, where relevant, the guidelines and protocols produced by the Royal College of Radiologists and local approved Trust guidelines. 5. Will be responsible for presenting the results of radiological investigations, or ensuring that colleagues present those results to required MDTs. 6. With Thoracic Service Leads, and other senior staff in radiology, the post holder will determine the department s direction with reference to Trust tertiary services and patient needs. 7. 14 JOB PLAN CONSULTANT RADIOLOGIST Intervention 1PA MDT preparation average 1PA MDT attendance average 1PA Plain film reporting/checking 1PA Chest ultrasound including interventional procedures 1PA CT/MR Reporting 3PA SPA 2 MAIN TERMS AND CONDITIONS OF SERVICE The successful appointee will be employed by the Manchester University NHS Foundation Trust, subject to national terms and conditions as per the Consultant Contract 2003 as amended from time to time. Any changes to national terms and conditions determined at a local level are subject to the Trust s collective bargaining arrangements which include the Trust s Local Negotiating Committee for medical staffing issues. Tenure The appointment is a substantive, whole time position which, unless terminated, will be held until retirement. The notice period is three months on either side. The appointment is subject to the provisions of Schedule 14 of the Terms & Conditions. Salary The salary scale is as per the Terms & Conditions of Service as set out in the Consultant Contract The current scale is 76,761 rising to 103,490 per annum, for 10 programmed activities per week. Any agreed extra programmed activities, and on-call requirement will be paid in addition to the basic salary. Pay progression will be on the anniversary of appointment and is subject to satisfactory participation in annual appraisal, the completion of a job plan review and agreed personal objectives as set out in Schedule 15 of the Terms and Conditions. Starting salary will be determined according to the terms and conditions as per the Consultant Contract Where a candidate has service in or outside the NHS which they feels should be taken into account in determining the starting salary, this will be considered following the offer of appointment by the AAC. Annual Leave Your annual leave entitlement is as per Schedule 18 of the Terms and Conditions. At least six weeks notice is required before taking annual leave. Numbers of Years of Completed Service as a Consultant. Superannuation This post is superannuable under the Health Service Superannuation Scheme. P a g e 14

15 Registration You will be required to have full and specialist registration with a licence to practise with the GMC and to provide a valid certificate annually. Conditions of Employment The following conditions must be met before the Trust will confirm an offer of employment: Verification of ID and Right to Work All employees must provide the Trust with the following documentation to prove their ID and eligibility to work in the UK: UK Passport UK Birth Certificate (if passport is unavailable) National Insurance evidence (P60 / recent payslip / P45) EEA (European Economic Area) Passport Residence permit issued by the Home Office or UKBA Driving Licence (ID card + counterpart) 2 addressed bills or statements (Bank / utility bill / phone bill) Note: if you are a British citizen but do not hold a UK passport then we require a UK Birth Certificate combined with National Insurance evidence Disclosure and Barring Check: Protection of Children & Vulnerable Adults In accordance with Safeguarding Vulnerable Groups Act 2006, the Trust is required to seek disclosure of criminal records when considering employing an individual who will be working with children or have regular contact with vulnerable adults. Disclosure will include checks on the information held in the Police National Computer, local Police records and, where appropriate, lists held by the Department of Health and the Department of Education and Employment. Disclosure need not necessarily be a bar to obtaining the position you have applied for. The information you provide and the Disclosure documents returned by the Disclosure and Barring service (DBS) will be available only to those who need them as part of the recruitment process. Disclosure documents will not be retained for longer than six months after the recruitment decision has been made.
