1 SECTION 1 SUMMARY OF POST Job Title: Specialty Doctor in Obstetrics and Gynaecology Grade: Specialty Doctor, Full-Time Managerial Responsible to Clinical Director Dr Judith Hyde Base/Department(s): Obstetrics and Gynaecology Location: Southmead Hospital SECTION 2 ABOUT THE TRUST North Bristol NHS Trust (NBT) is a progressive and ambitious provider of health services, delivering for a large local population and with specialist regional services for major trauma, neurosciences, vascular centre, urology, plastics and burns, orthopaedics and renal services. With a turnover of 550m and 9,000 staff, our clinical teams have built a strong reputation for exceptional healthcare. Our acute services moved into a brand new, state of the art, hospital building in May This new 850 bed facility at the Southmead site offers patients single rooms with en-suite facilities, greatly improving privacy and dignity and reducing the spread of infections. SECTION 3 THE WORK OF THE DEPARTMENT The department provides a full range of obstetric and gynaecological services. The maternity department is one of the largest in the country, with more than 6,700 deliveries per annum with a 15 bed Consultant led delivery suite, a co-located birth unit performing over 900 deliveries per year and a stand alone birth centre at Cossham Hospital with 500 deliveries per year We have extensive maternal medicine services for specialist endocrine, epilepsy, haematology and renal clinics and run additional specialist AN clinics for women including substance misuse, mental health and blood borne viruses. We also have an active fetal medicine department and tertiary level 3 neonatal unit. The Obstetrics services are well recognised. We currently work closely with interventional radiology in the management of placenta accreta/percreta providing a 24 hour service accepting tertiary referrals for potential cases. Pan Bristol services for both vascular surgery and Urology have been centralised at North Bristol Trust since the opening of the Brunel Building in May 2014 which has helped with the

2 development of this service for women with abnormal placentation. We provide tertiary level gynaecology services in: reproductive medicine, recurrent miscarriage, management of fibroids, vulval disorders, urogynaecology, pelvic pain and advanced laparoscopic surgery, including severe endometriosis. We are an accredited Endometriosis centre and have applied for accreditation as a Urogynaecology Centre. We undertake 11 gynaecological outpatient clinics and 15 gynaecology theatre lists per week in a dedicated gynaecology theatre suite. We have pioneered the use of ultrasound scanning in the Gynaecology clinics, helping to streamline patient management. Strong links exist with Urology and Colorectal Surgery through both our advanced endometriosis and complex pelvic floor surgery work. SECTION 4 - MANAGEMENT ARRANGEMENTS The appointee will be expected to be fully involved in the developments of the Speciality, to take account of the Trust policies, including Risk Management Strategy and developments in Clinical Governance. The appointee may be asked to participate in other managerial and administrative work and will be expected to be aware of the broader context within the NHS in which Specialty doctors operate. It is a requirement that if the employee has concerns about the professional performance of a member of the medical staff, they have a duty to speak to the person concerned. If the matter is potentially serious or satisfaction is not obtained with a direct approach, such concerns should be discussed with the Lead Clinician and, if necessary, the Trust s Medical Director. The appointee will be expected to be aware of the general and local budgeting Issues within the NHS.

4 SECTION 5 DUTIES OF THE POST Clinical Duties Title of Post Specialty Doctor in Obstetrics and Gynaecology The post is funded by NBT for the postholder to participate in the Registrar rota to reduce junior doctors hours. The daytime duties will include a combination of gynaecology clinics, antenatal clinics, Gynae theatre, and a share of the registrar cover for both the labour ward and the gynaecology ward for emergencies. The out of hours rota will be a 1 in 7 shared with the other registrars as a full shift In addition there will be an opportunity for the postholder to undertake up to 2 RCOG ATSM (Advanced Training Skills Modules) if they are eligible. The chosen module will be agreed with the Educational Supervisor at the start of the post and will be decided according to the needs of the other senor trainees in the department. Study Leave Specialty Doctors are entitled to study leave and may claim a maximum of 450 pro rata. The postholder should submit requests to Ms Donna Paddon, Study Leave Administrator at the Medical Education Centre, Southmead Hospital. This post is not a rotational post and therefore does not lead to career progression. Educational Supervision will be provided and the postholder will be expected to formulate an educational plan. Annual Leave Locums are not employed to cover the absence of medical staff who are away on leave. The number of doctors who are away on leave at any one time will be agreed with the Trust speciality policy. During the absence of colleagues on leave, colleagues are expected to provide agreed levels of cover. Speciality Doctors are entitled to five weeks leave until the completion of two years service. Thereafter they are entitled to six weeks leave plus appropriate Bank Holidays. Annual leave is calculated on a 5 days week. Research There is a strong research element in our obstetric Unit led by Professor Draycott. The work on Multi professional Training is world renown with the PROMPT programme now running in a large number of units across the UK and the World. The unit also has the support of Mr Dimitrios Siassakos as Consultant Clinical Lecture funded 50% by the University of Bristol. Currently there are also two academic clinical lecturers and 5 academic clinical fellows, 4 are funded by NIHR. In addition there are 4 research fellows undertaking OOPR programmes in the department. The postholder will be encouraged to contribute to research in collaboration with the current members of the department or to develop new and related research topics. The post-holder will be required to submit all research development plans which have resource implications to the Clinical Director prior to implementation. There are professionally staffed Medical Libraries in the Medical Postgraduate Centre at Southmead Hospital, with a rapid service between this and the University Library situated in the centre of Bristol city. A computer literature search facility is available for which a charge is made and the library has links with other networks. The libraries are at the forefront of developments in electronic knowledge delivery.

5 Audit The postholder will be required to take part in a Clinical Audit Programme. The Trust has an audit department which supports and monitors such activity. There is an active Trust Clinical Audit Committee. SECTION 6 LOCATION Your principal place of work will be Southmead Hospital Other work locations including off site working maybe agreed and incorporated in your job plan where appropriate. You will be expected to undertake your programmed activities at the principal place of work or other locations agreed in the job plan. Exceptions will include attending official meetings away from the workplace. You may be required to work at any site within the Trust including new sites provided that they are within a reasonable travelling distance from your home address. Please note that the base could change as a consequence of clinical requirements, therefore, you may in the future be required to change base. SECTION 7 ASSOCIATE DUTIES AND RESPONSIBILITIES Whilst on duty a doctor has clinical and professional responsibility for their patients or, for doctors in public health medicine, for a local population. It is also the duty of a doctor to: keep patients (and/or their carers if appropriate) informed about their condition; involve patients (and/or their carers if appropriate) in decision making about their treatment; maintain professional standards and obligations as set out from time to time by the General Medical Council (GMC) and comply in particular with the GMC s guidance on Good Medical Practice as amended or substituted from time to time; A doctor is responsible for carrying out any work related to and reasonably incidental to the duties set out in their Job Plan such as: the keeping of records and the provision of reports; the proper delegation of tasks; maintaining skills and knowledge. Doctors will be expected to be flexible and to cooperate with reasonable requests to cover for their colleagues absences where they are safe and competent and where it is practicable to do so. Where doctors undertake duties in accordance with this paragraph and such duties take place outside of their contracted hours they will receive either an equivalent off duty period or remuneration. Where this adversely impacts on the Job Plan and/ or opportunities for individual doctors a temporary Job Plan will be agreed for the period of cover.

6 SECTION 8 MAIN CONDITIONS OF SERVICE (i) The appointment is subject to the Terms and Conditions of Service for Specialty Doctors England (2008) which may be amended by collective negotiation from time to time. (ii) As a senior and professional employee it is essential that you and we work in a spirit of mutual trust and confidence. You and we agree to the following mutual obligations in order to achieve the best for patients and to ensure the efficient running of the service: to co-operate with each other to maintain goodwill to carry out our respective obligations in agreeing and operating a job plan to carry out our respective obligations in accordance with appraisal arrangements to carry out our respective obligations in relation to the organisation s policies, objectives, rules and working practices and protocols. (iii) (iv) (v) Any practitioner who is only able to work part time for personal reasons such as domestic relationships or ill health is entitled to apply. The timetable may be determined by negotiation prior to commencement of employment. The post is pensionable within the NHS Pension Scheme unless the appointee opts out of the scheme or is ineligible to join. The salary scale effective from 1 April 2017 is: 37,923-70,718 (pro rata for part time). There will be an annual progression up to point 4 of the scale. In order to progress from point 4 to point 5 practitioners will be required to pass through Threshold 1. Progression between point 5 and point 7 will be at 2 yearly intervals. To progress from point 7 to point 8 of the salary scale, doctors will need to pass through Threshold 2. Progression between point 8 and point 10 will be at 3 yearly intervals. The criteria for passing though the Thresholds is set out in Schedule 15 of the Terms and Conditions of Service (vi) The postholder will normally be covered by the NHS Hospital and Community Health Services Claims of Medical Negligence. However, in certain circumstances (especially in services for which a separate fee is received) the indemnity may not apply. The Department of Health therefore advise that membership of your medical defence organisation is maintained. (vii) The appointee is entitled to receive three months notice of termination of employment and is required to give the employing Trust three months notice. CONDITIONS OF APPOINTMENT

The postholder must be fully registered and from November 2009 be licensed to practice with the General Medical Council or General Dental Council as appropriate and ensure they maintain their registration. Health. The passing of a medical examination is a condition of appointment to all medical and dental posts in the hospital service. The Trust will require the successful candidate to provide a certificate, signed by a medical practitioner, stating that the postholder is free from any medical defect or disease which would impair his/her capacity to undertake the duties of the post. Hepatitis B. The successful applicant will be required to provide documentary evidence of natural or acquired immunity to Hepatitis B. Where this is not possible, the postholder will be required to demonstrate, by recent (within the last year) evidence of serology showing the absence of Hepatitis B surface antigen. Criminal Records Bureau (CRB) The Criminal Records Bureau provides access to information about criminal convictions and other police records to help employers make informed decisions when recruiting staff.
