The new Heartland Telehealth Resource Center (HTRC) was recently created through a $980,000 grant from the U.S. Department of Health and Human Services. Almost 90 percent of the counties in the tristate area of Kansas, Oklahoma, and Missouri are rural, which means many residents have limited access to health care. The Heartland Center aims to help alleviate those gaps in access by creating resources and market strategies to expand telehealth services. A recognized leader in telemedicine, KUMC will share strategies it has used for 20 years to connect rural areas with high-quality care using videoconference technologies to visit faraway doctors. Almost any tool a health professional would normally use can be used through telemedicine, said Ryan Spaulding, director of the Center for Telemedicine and Telehealth at the University of Kansas Medical Center, who is principal investigator for the grant. Doctors are able to listen to patients hearts with stethoscopes, look in their ears with otoscopes, and check their skin with dermascopes. When KUMC started its telehealth programs in 1991, it was one of the first such programs in the country and the first one known to provide school-based telemedicine for children. Since its inception, the program has held consultations with tens of thousands of patients. The Heartland Telehealth Resource Center will combine expertise from KUMC s Center for Telemedicine and Telehealth, the Missouri Telehealth Network at the MU School of Medicine, and the OU Center for Telemedicine at the University of Oklahoma Health Sciences Center. 3 In 1910, Dr. John Axtell, a Newton surgeon, met with representatives from privately owned and religiously affiliated hospitals to discuss forming an organization to promote the efficiency and economy in the management of hospitals in Kansas. A century later, the resulting organization the nonprofit Kansas Hospital Association has grown to 197 members, including 125 community hospitals, which work together toward improving access, quality and affordability of health care for all Kansans. Celebrating 100 Years of Kansas Hospitals Working Together is the theme for the centennial celebration, which culminated at KHA s annual convention and trade show in Topeka Nov The 100th Anniversary Gala, from 7 to 10 p.m., Wednesday, Nov. 10, featured broadcaster and Kansas native Bill Kurtis as master of ceremonies. The current KHA board chair, Jim Reagan, Jr., Ph.D., CEO of Morris County Health Hospital in Council Grove, Kan., sees the most important role of the organization as advocating for Kansas hospitals on the state and federal levels. I think they do an excellent job of that, he said. There is no way that a rural hospital or even a small group of rural hospitals could have their own lobbyist or staff to interact with legislators. KHA s educational programs also are a valuable service, Reagan said. Among Celebrating 100 Years of Kansas Hospitals Working Together topics addressed are electronic records, risk management, and leadership training for those promoted to management. KHA also provides frequent webinar updates about regulatory changes at the state and federal levels. With health reform, that becomes even more urgent, Reagan said. Managing hospitals is much more complex than it was a century ago, and KHA has evolved to meet the ever-changing challenges of its members by providing resources and education on management, technology, and regulatory issues, as well as being an advocate for Kansas hospitals. One of the organization s early victories was a 1924 Kansas Supreme Court decision stating that hospitals could not be taxed since they were charitable organizations. KHA supported hospitals during the Great Depression and Dust Bowl days, and was able to hire its first employee in That year also was the first time the 99 member hospitals were represented at the Kansas legislature, which two years later passed a law barring establishment of a hospital without a license from the Kansas Board of Health. In 1949, KHA established a central office, developed district organizations, and began offering educational programs for hospital personnel in such Jim Reagan, Jr., Ph.D., KHA Board Chair, CEO of Morris County Health Hospital in Council Grove, Kan. areas as food service, medical records, operating and emergency room techniques. A decade later, professional liability and reimbursement were big issues facing the organization. The staff increased to 18 by 1980 as KHA increased efforts to provide services in areas such as reimbursement, data, financial planning, public relations, and personnel management. The Kansas Hospital Education and Research Foundation (originally established in 1969) was redesigned in 2000 to identify, create, and support projects and solutions that improve health delivery and support opportunities for education of Kansas hospitals leadership and personnel. Today, various councils, committees, and task forces assist KHA s staff and 21-member board of directors with the core functions of advocacy, education, information, collaboration, products and services, governance, and operations. Current president Tom Bell joined the staff in 1986 and became president and CEO in They work for us (the hospitals), Reagan said. 4 Kansans with Disabilities CAN be Healthy Living Well with a Disability Workshop Living Well with a Disability, an eight-week workshop designed for Kansans with disabilities and chronic health conditions, uses goal setting and problem solving as the framework for a healthy lifestyle. Kansans can take the workshop online or in their communities with a trained facilitator. The classes are usually offered by Centers for Independent Living, which serve people with disabilities, said Val Renault, project coordinator for the KU Research and Training Center on Independent Living. We train their staff members to be facilitators, and they do their own student recruitment. The online course is an abbreviated version of the course, but has the same essentials. The more than 1,000 individuals in 32 states who have completed the workshop reported a 37 percent decrease in limitation due to secondary conditions, a significant decrease in depression levels, and a 10 percent decline in costs for medical services. In Kansas, about 18 percent of the population has a disability, which makes them prone to developing secondary health conditions. KU s Research and Training Center on Independent Living and the University of Montana Rural Institute on Disabilities developed the Living Well program to help prevent and reduce those secondary conditions. Now the Centers for Disease Control and Prevention and KDHE provide support so the course is available to Kansans. According to Renault, The course has been given [in Kansas] in 14 towns and one tribal reservation, and it will soon be presented to soldiers and their children with disabilities at Fort Riley. Those assisted by the program in Kansas range in age from their teens to their 70 s with varying disabilities including: traumatic brain injury, spinal cord injury, mulitple sclerosis, diabetes as well as learning disabilities. With the Living Well with a Disability workshop, they all share the chance to maintain and improve their physical, social and emotional health. Facilitators at seven community-based organizations throughout the state are trained to teach the course. Free course facilitator training is provided to staff of Centers for Independent Living or other community-based organizations. Overall, these institutions offer telehealth expertise that few regions can match, Spaulding said. While the Kansas, Missouri and Oklahoma telemedicine programs are all robust and active, there are still significant gaps in coverage in each state that the HTRC will target and address. The HTRC will also provide technical assistance and resources to new telehealth programs, use new social media tools such as Facebook to provide resources to patients and professionals, and test other marketing strategies to drive consumer demand for telehealth services. Patients were early adopters they know that a telehealth visit is much easier than driving several hours to get care, Spaulding said. Doctors have been slower to take advantage of the technology. 5 Kansas Optimizing Health Program Education for management of chronic illnesses Since the Kansas Optimizing Health Program (KOHP) was adopted in Kansas in 2008 from a chronic-disease self-management program developed by Stanford University, more than 425 Kansans have benefited from 60 programs around the state, said Cindy Winters, master trainer for the KOHP program and Arthritis and HTN Project Specialist at the Kansas Department of Health and Environment. The six-week classes help people with a variety of chronic diseases learn the skills necessary to self-manage their conditions. Classes are ongoing in as many as 12 to 15 communities at a time, with sessions offered in Chanute, Coffeyville, Colby, Coldwater, Ellsworth, Great Bend, Hays, Mayetta, Manhattan, Newton, and other cities. Deb Kiker, RN, MSN, who has been a nurse with Community Health Ministries in Wamego for four years, was impressed with the program after she took the free, four-day leadership training in 2008 in Manhattan. As a nurse who works in a rural safety net clinic, I believe in the power of educating people about their issues, said Kiker, who also was able to use the leadership training class to fulfill state board of nursing requirements for relicensure. The program is especially important in smaller communities where the local providers stay so busy they don t have time to teach. Kiker taught her first KOHP class in Wamego in 2009, then one in St. Deb Kiker, RN, MSN, Community Health Ministries, Wamego, Kan. George, and is now leading one in Westmoreland. She particularly remembers one woman in the class who had significant arthritis in her feet and ankles and liked to garden. Her action plan the first week was to try stretching exercises and she was able to significantly decrease her pain and enjoy gardening again, Kiker said. Another woman who had fibromyalgia learned during class how to visualize her pain and then perform relaxation exercises. She noted significant decrease in the pain right away, Kiker said. Topics covered during the six weeks include developing an action plan, using the mind to manage symptoms, feedback and problemsolving, fitness and exercise, breathing to help manage pain, nutrition, how to communicate better with health care providers, how to take medications effectively, and how to deal with depression. As a nurse who works in a rural safety net clinic, I believe in the power of educating people about their issues. Deb Kiker, RN, MSN, Community Health Ministries, Wamego, Kan. Chronic conditions are a big umbrella, Kiker said, covering issues such as asthma, arthritis, diabetes, and heart and lung disease. Family members and care givers also can take the classes. In tracking participants for five years, researchers at Stanford University found they spent fewer days in the hospital, had fewer outpatient visits, communicated better with their doctors, and reported an improvement in their general health as well as less fatigue. They also used less pain medication and solved problems better. Teaching participants about problem-solving is a big piece of the program, Kiker said. Groups meet once a week for two and a half hours for six weeks. During classes, two trained facilitators lead participants through action planning and learn hands-on strategies to better manage their health. Participants cover a broad range of ages and conditions, Kiker said. The only charge is $20 for a textbook, but scholarship funding is usually available. Kiker is always on the lookout for class locations a senior center, church or community center. To spread the word, she contacts physicians in that community and the local newspaper, puts inserts in utility bills, and speaks to different groups. 6 Electronic Health Records Funding for technical support to critical access hospitals and rural hospitals U.S. Health and Human Services Secretary Kathleen Sebelius in mid-september announced nearly $20 million in new technical support assistance to help critical access and rural hospital facilities convert from paperbased medical records to electronic health record (EHR) technology. Kansas received one of the largest amounts, with $1.14 million allocated to the Kansas Foundation for Medical Care. More than 1,600 critical access and rural hospitals in 41 states will benefit from this assistance, which can help qualify them for substantial EHR incentive payments from Medicare and Medicaid. The of health information technology can be especially important for patients and clinicians in small and rural health care facilities, yet these facilities face high hurdles as they look toward joining in the transition to electronic information, Sebelius said in making the announcement. The funding we are announcing today is a new category of support, aimed specifically at assisting critical access and rural hospitals with their particular needs and challenges. The new funding is provided under 6 Kansas connections The of health information technology can be especially important for patients and clinicians in small and rural health care facilities Kathleen Sebelius, U.S. Health and Human Services Secretary the Health Information Technology Economic and Clinical Health Act, part of the American Recovery and Reinvestment Act of The Medicare and Medicaid EHR incentive programs will provide incentive payments to eligible professionals and hospitals that adopt and demonstrate use of certified EHR technology. Incentives totaling as much as $27.4 billion during 10 years could be expended under the program, which is administered by the Centers for Medicare & Medicaid Services. Funding was awarded to 46 Regional Extension Centers (RECs) and is part of the Critical Access Hospitals and Rural Hospitals (CAH/Rural Hospital) Project, which provides additional technical support to critical access and rural hospitals with fewer than 50 beds. The added level of support we are announcing today will enable the RECs to offer greater field support to these communities as they deal with the financial and workforce constraints, and work to achieve access to broadband connectivity and to overcome other barriers that critical access hospitals and other rural hospitals may confront, said David Blumenthal, M.D., national coordinator for Health Information Technology.

15 Educating the public about Kansas and encouraging travel in the state is the purpose of the 8 Wonders series organized by the non-profit Kansas Sampler Foundation. After selecting the overall 8 Wonders of Kansas, the Sampler Foundation set out to choose 8 Wonders in each of eight categories: architecture, cuisine, geography, art, customs, people, history, and commerce. In future issues, we ll feature one of the categories to help inspire day trips around the state. For the customs category, KU s Rock Chalk Jayhawk cheer beat out 24 finalists to be named an 8 Wonder of Kansas Customs. To see it in action, attend a football or basketball game, or visit the student union to learn the background of the chant. Other winners include: Cottonwood Falls brings together musicians every Friday night for a jam session at the Emma Chase Café. On summer evenings, a couple hundred people in lawn chairs listen to the tunes of as many as 25 musicians. Every Friday night features a different genre. The custom of clicking your heels three times and saying, There s no place like home, earned awards for Wizard of Oz attractions in Wamego and Liberal. The custom of commemorating Veterans Day started in Emporia when a local citizen helped change Armistice Day into Veterans Day, making Emporia the Founding City of Veterans Day. Lindsborg was recognized for its display of ethnic handicraft through the many traditional and contemporary Dala horses seen around the city, which was founded by Swedish immigrants. Kansas rich history of soda fountains earned an award for the 37 soda fountains still operating across the state. In the first half of the 20th-century, most Kansas towns claimed at least one soda fountain. At the turn of the 21st-century, Kansas had about 50 soda fountains. Today, there are 37. The custom of riding a carousel brought accolades to the famous C.W. Parker carousels in Abilene and Leavenworth. Building fence posts out of limestone was a custom used by many in the Smoky Hills soon after the state was founded. Learn more about them at the Post Rock Museum in Lacrosse. 16 Jennifer Koontz, M.D., Newton, Kan. Editor s note: This is the fifth in a series of articles highlighting a rural health provider. If you know someone who would be an interesting subject, contact Susan Belden at sbelden@kumc.edu. Dr. Koontz assists a patient in the temporary clinic after the Haiti earthquake earlier this year. Volunteerism has always been a priority for Newton physician Jennifer Scott Koontz, MD, MPH, so it wasn t long after the devastating Jan. 12 earthquake in Haiti, which killed 200,000 people, for her to find a medical relief organization that was putting together a team of physicians from Kansas. After Kansas City-based Heart to Heart International asked if she could leave the next day, Dr. Koontz made arrangements at Pinnacle Sports Medicine & Orthopaedics to cover her appointments, then started packing clothes and camping supplies while Newton Medical Center staff bought three large suitcases and filled them with donated 16 Kansas connections medications, splints, and wound supplies for her to take to Haiti. She said good-bye to her husband and three young children and boarded a flight to Santo Domingo. She arrived The need was just overwhelming and the infrastructure was just gone. Jennifer Koontz, M.D. in Haiti only a few days after the earthquake with no idea where her group would stay. While she felt well-prepared in her medical training, she had no idea what to expect otherwise. The need was just overwhelming and the infrastructure was just gone, she said. This was my first experience with disaster relief and obviously it was a catastrophic one. They found crumbled buildings, no roads or electricity, and people living in the streets because they were afraid to go into the structures that were still standing. Only one cell phone provider had service, and all banks and gas stations were closed. By the time the group from Kansas arrived, Heart to Heart International had established a relationship with a Nazarene seminary, which had a building where they were able to stay. There was such great concern for aftershocks, however the seminarians insisted they sleep outside under mosquito nets for several nights. We teamed up with a medical team from Israel in the national stadium downtown where they normally play soccer, Dr. Koontz said. We set up a temporary clinic with a triage line and a pharmacy, which consisted of our suitcases in the corner. The group got up at 5:30 a.m. every day and ate breakfast together before traveling to the temporary clinic, which was half an hour away. They d stop work by nightfall because experienced disaster relief workers warned them about traveling after dark without any electricity. The only light, Dr. Koontz said, came from car headlights or an occasional candle on the sidewalk with people huddled around it. It was very surreal, she said. Dr. Koontz estimates her group saw 400 to 500 patients every day. The most indelible memory for her is of an elderly woman who was brought to the temporary clinic in a wheelbarrow from the hills surrounding the city several days after the earthquake. They had realized they needed to go to Port-au-Prince if they were going to get help, Dr. Koontz said. Continued on page 17

17 Spotlight continued from PAGe 16 The woman had a large open fracture on her ankle that was clearly infected and needed amputation. It was really terrible but she was so thankful for us being there, Dr. Koontz said. She asked if it could be cleaned and covered before she was taken to the temporary hospital. She was so grateful for that. Dr. Koontz added: That was probably one of the most impressive things: In spite of all of the tragedy and pain, Haitians were uplifted and calm and resilient. The biggest help to the English-speaking physicians were Haitian medical students, who volunteered and served as interpreters. Most people spoke Creole and some spoke French, Dr. Koontz said. The medical students could speak both languages plus English. Their schools were closed, and they really wanted to contribute. They accompanied us every day. By the time Dr. Koontz went home 11 days later, the trauma wounds had transitioned into infections and issues arose from lack of clean food and water. Before her experience in Haiti, Dr. Koontz s first volunteer stint came at a local nursing home while she was a high school student in Nickerson, Kan. She always wanted to be a doctor except for a brief time in first grade when she wanted to be a missionary and her mother, a nurse, made a point of exposing her to the possibilities of health care. While she was a student at KU Medical School, Dr. Koontz was founder and executive chairperson for JayDoc Free Clinic, a student-run clinic for the uninsured in Kansas City, Kan., which opened in 2003, and co-founder and student board member for JayDoc Community Clinic, a student-run clinic for the uninsured in Wichita, which opened in She continues to be a volunteer preceptor for the Jaydoc Community Clinic in Wichita. After graduating from Bethel College Dr. Koontz assists a young patient after the earthquake in Haiti. in 1998 with a psychology and natural sciences degree, she spent two years as a full-time volunteer at the North Hamilton Community Health Centre in Ontario with the Mennonite Voluntary Service. While there she coordinated the breakfast program, which served 60 school children each morning, and worked as camp counselor for adults and children with HIV. She completed the master of public health degree at KU School of Medicine and then went on to medical school. She received her MD degree in 2005 and served her residency at Via Christi Family Medicine in Wichita where she was chief resident during her last year. After earning board certification in family medicine in 2008, she completed a sports medicine fellowship at Via Christi Sports Medicine in Wichita. As a career, she s been able to combine her love of medicine with sports she was captain of the varsity volleyball and basketball teams at Bethel. She earned certification in sports medicine in 2009, the same year she began practicing family medicine and sports medicine with Pinnacle Sports Medicine and Orthopaedics, in Newton and Hutchinson, as well as serving as team physician and medical director for athletic training and student health at Bethel College, which presented her with its Young Alumni Award in She defines sports medicine as anything that helps people stay healthy and lead an active lifestyle, so it could include conditions such as a hurt shoulder to asthma, depression or chronic knee pain. She currently serves as a physician member of the Harvey County Wellness Coalition and is a facilitator for Ready, Set, Fit, a project through the American Academy of Family Physicians to present healthy fitness and nutrition ideas to 4th-grade students. In addition, she is a faculty member for Clarion, a multi-disciplinary health care class at the KU School of Medicine Wichita and Wichita State University. She is enthusiastic about the program that brings together students from different disciplines while they re still at the education stage. The students work in teams of four solving a health care dilemma or coming up with a way to prevent it in the future. While a student, she was involved in a variety of professional activities, including serving as president of the KU Medical School chapter of the American Medical Student Association. She received the University of Kansas Deborah E. Powell, M.D., Pride in the Profession Award, the Olive Ann Beech Scholarship for Primary Care, and KU Medical Center Academic Society Awards for Student Leadership and Community Service. She lives in Newton with her husband, Matt, also a Bethel graduate, and their three children -- Caleb, 6, Abby, 4, and Naomi, 1. 17
