Herbert Doubek, MD, 79, is a retired family physician from Belleville, Kan. His daughter Debra Doubek, MD, 50, currently practices family medicine in Manhattan, Kan. His granddaughter and Dr. Debra Doubek s niece, Danae Doubek, 25, is in her third year of medical school at the University of Kansas School of Medicine in Wichita, Kan. While growing up, he and his three brothers worked in their father s blacksmith shop. He also worked at the Coats, Kan., pharmacy and found he loved working around medicine. Eventually, he went on to medical school and was a family physician. At one time I used to be a clinical professor at KU Wichita, he said. I helped teach residents, 29 of whom spent time in training in Belleville. He says the biggest changes in medicine over the years are the technology and the paperwork. Also, he says there seems to be less personal time to spend with patients. His daughter, Dr. Debra Doubek, earned her medical degree from the KU School of Medicine and spent two years as an assistant clinical professor at KU School of Medicine, Department of Family Medicine. She has been practicing medicine in Manhattan, Kan., for 16 years full time. She is board certified in family medicine and bariatric medicine (specialty of obesity and overweight medicine). Doubek says she has a full, rich life and is thankful every day to be who she is. If given choices, I would not choose to do anything different with my life, she said. I get to wake in the mornings and go to a job that I absolutely love. Her love of science and a desire to help people are the joys of her profession. Not a day goes by that I don t feel I have changed someone s life in a positive way. Perhaps in a small way with a hug, or in a huge way by saving someone s life in making an early diagnosis. Valentine EDITOR Marena Frazier EdITOR /GRAPHIC DESIgNER Amanda Gardner 2 Kansas c o n n e c t i o n s Plainville hospital opens new facility Rooks County Health Center of Plainville, Kan., opened the doors of its new facility for the first time in September, allowing local residents to tour the 59,000 squarefoot building. In attendance at the official dedication, held Sept. 7, 2008, were Congressman Jerry Moran and Governor Kathleen Sebelius, each giving addresses to the overflow crowd. Construction of the $19 million project began in December 2006, and staff and patients relocated to the new facility, on Sept. 22. Both inside and out, the new facility displays iconic materials found within the rural communities of Rooks County. The exterior color palette is also representative of the area. Red was chosen to represent barn buildings, lean-tos or other construction you might find on a farm. The green siding was used to represent the green fields and grass. The design uses material indigenous to this area of Kansas like galvanized metal, brick, concrete, steel and limestone. These things are frequently seen in farm settings all around Rooks County. The rationale behind the new hospital s aesthetics was when someone uses this hospital there would be a sense of familiarity. The new hospital will enable Rooks County Health Center to expand its physical therapy department and launch an occupational therapy program. The radiology department also will be expanding, and the hospital now has additional facilities for obstetrics and an additional operating room. Rooks County Health Center, Plainville, Kan., ribbon cutting at new facility It is hoped that the space increase of 30,000 square feet and enhanced capabilities will allow for more specialization and future growth. Congratulations to Rooks County community members for the newly-constructed hospital. AAFP introduces a rural Web site The American Academy of Family Physicians (AAFP) launched the new rural online community for rural physicians on July 7, With this new resource, rural FPs can stay on top of news that is important to them and their patients. They can share information with one another and can access other resources to support quality rural health care. The Rural Online Community features: Links to headlines from national and local publications on topics of interest to rural FPs, as well as access to the CMS Quarterly Provider Update and news releases An discussion list for community members An online library community that members can use to post resources to share with others Rural FP profiles, giving rural FPs a chance to learn about one another and giving individuals considering a career in rural medicine a glimpse into the lives of rural FPs Updates on pertinent legislative issues A calendar of events for rural FPs across the country Rural health resources on topics including reimbursement, coding, practice management, and AAFP policies. 3 Kansas Physician Assistant Program Preparing students for practice in rural areas Established in 1972, the Department of Physician Assistant at Wichita State University (WSU) is one of the oldest physician assistant programs in the United States and the only PA program in Kansas. The PA program prepares students to practice medicine under the supervision of a licensed physician. Wichita State s College of Health Professions offers one of the largest university-based programs for health professionals west of the Mississippi. The college provides training in the area s many medical facilities and offers students a chance to learn from some of the top professionals in the state. Kimberly Dansel, PA-C, attended WSU for her PA education and now practices with Greeley County Health Services in Tribune, Kan. Wichita State University prepared me well for my roll in primary care, she said. As a student, I selected rural practical rotations versus those in urban areas, as it was my intention to return service to those types of areas. Her practical experience in rural settings allowed her to have a broader experience in ER, surgery, hospital, and clinical settings. She says it gave her the to see diseases first hand and to practice a wide variety of clinical skills. Dansel was raised 10 miles from one of the clinics she currently provides services in. I am privileged to live back in my hometown of Weskan, Kan., just a few short miles from my parents farm. I love the rural lifestyle and my involvement in my community on more than just a professional level. Rural life has a more personal and intimate feel; people genuinely care for one another. Dansel says PA s have an important role in rural health care. Mid-level care can help make services more accessible when physician numbers are few, and they provide a more personal health care experience as PA s are promoted as having more time to spend with their patients. More than just returning home, Dansel chose to practice in rural Kansas because she liked the autonomy it gave her. What Dansel loves most about practicing in a rural community is that every day is different and has its new for learning. She says she tries to make it a point to learn at least one new thing each week or to at least polish up on something that was seemingly forgotten. I am amazed at how frequently that goal is met, she said. I appreciate the people of rural Kansas and enjoy that I can truly empathize and relate to their lives. I am also blessed with an amazing group of physicians to learn from and to grow under as I expand my practice. 4 Generations continued from cover And she enjoys sharing stories with family members when they get together. My family is very close, and we make it a priority to all gather together for holidays, she said. We love to tell stories and laugh. Doubek says she loves to hear her father reminisce about what medicine was like in the 1950s, 1960s and 1970s. He has some wild stories that he tells! From her father, Doubek says she 4 Kansas c o n n e c t i o n s has learned how to balance multiple aspects of her life as well as how to lead by example. He was not defined by simply his role as a physician, she said. He was so involved in all aspects of our community in Belleville, where I grew up, including church, the school board, Boy Scouts, County Coroner, Bridge Club, High School team physician, teaching medical students and residents, as well as attending all of his children s school activities. By his example, Doubek says, she has been involved in many areas of volunteerism and leadership in the Manhattan community. Her husband of 24 years was Mayor of Manhattan last year, so one of her most recent job descriptions was First Lady of Manhattan. Danae Doubek just started her third year of medical school clinical rotation at the VA Hospital in Wichita. She already has some original, cute stories about taking care of the VA Veterans, said Dr. Debra Doubek, Danae s aunt. Danae s sister, Cheryl Doubek is marrying a doctor. My older sister Diane is a nurse. We can all identify with one another. Dr. Debra Doubek says she s encouraged to see Danae and her classmates going through medical school 20 years after her training. I see bright, energetic, kind students who seem to be in the field of medicine to help others be healthy. Danae Doubek made the decision to study medicine when she was in college. She has always loved to travel and she thought she might be an international business major. But Doubek soon found she had little interest in business-related subjects. She did, however, enjoy the general biology course required for My grandfather and aunt are two of the most compassionate people I know, not just in medicine, in life. Danae Doubek, third-year medical student graduation. As a result, she switched her major and found her passion for medicine while working at her aunt s office. I had a job there stocking the exam rooms with supplies, and sometimes I was allowed to bring patients back to the exam room to get their vitals, said Doubek. It was there that I found I loved working with people and was constantly curious about the diagnoses the doctors worked on, and was really interested in the handouts the patients would receive about their condition. Three years later, I was accepted into medical school. At medical school, students asked her if she knew Dr. Doubek from Manhattan and then would tell her what a great teacher she was. She s been a support to me during school also and has listened to me at times when I was stressed and needed help with balancing life and medical school. Doubek says it s amazing having other family members in the health field. I love hearing my grandpa s stories about the way things were when he started practice, said Doubek. A while back, we went to a KU basketball game together, and he told me about everything from medical school to the way he practiced before he retired, and it was one of the most special times I can remember. He recently gave me some of his books from medical school, so I was able to look at his notes and read about his classes. Doubek says from her grandfather and aunt she has learned compassion. My grandfather and aunt are two of the most compassionate people I know, not just in medicine, in life. My grandpa will send me articles about medicine or books about the medical school just because he s thinking about me, and it always brightens my day. I ve seen from being a part of their family why their patients love them. My aunt is a fireball, and I ve seen how she puts so much of that energy into her patients wellbeing, and it has been inspiring to me. I know that I will be a better physician because of lessons I ve learned from them. Doubek was proposed to in September and her fiancé, Kyle Goerl, is also a third-year medical student at KUSM-W. Upon completion of her medical degree, she intends to stay in Kansas and practice family medicine or pediatrics. Dr. Herbert Doubek s oldest daughter is the Director of Nursing at Republic County Hospital in Belleville, Kan. He has a granddaughter who is a registered nurse at the Registered Hospital in Kansas City and a granddaughter who is a case manager at the nursing home in Belleville. Danae s sister is a graduate of KU School of Psychology and is now a practicing psychologist. She is also engaged to a fourth-year medical student at KUMC. Dr. Debra Doubek has two boys who may possibly end up in the medical field as well. One is a sophomore in college at KU and the other a junior in high school in Manhattan. It gives me pride and satisfaction to know that some of my family is following in my footsteps, said Dr. Herbert Doubek. There is still a great in the medical world for doctors. It s a good profession to be in and there is room for more good doctors in the medical field.

5 J-1 Visa waiver program Bringing skilled physicians to rural Kansas Several rural areas are receiving much needed help from foreign-born physicians who fill the needs of many rural health care facilities. The physicians are part of the J-1 Visa waiver program. Hassan Shafeeq, MD, from the Maldives Islands, went to medical school in Katmandu, Nepal. Dr. Shafeeq moved to the United States in 2002 for residency, which he completed in 2005 at Mount Sinai Hospital in New York. He was then placed by the Kansas Recruitment Center with Meade Hospital in Meade, Kan. Without Dr. Shafeeq, the 20-bed hospital in Meade might not even have a doctor. He is one of more than 100 foreign-born physicians from countries such as Egypt, Pakistan and Croatia who have served in Kansas communities since 1994 under the J-1 Visa waiver program. As with other states, Kansas is authorized to provide waivers for up to 30 physicians a year. The program is crucial in small farming towns such as Meade, Greensburg, and La Crosse, where it has become nearly impossible to recruit physicians. Though Shafeeq said living in Meade took some adjustment after living in New York, he says he s been pleasantly surprised by the warmth and openness of the community. It s laid back working in a rural area, said Shafeeq, whose wife and two young children also moved to Meade. The J-1 Visa allows an international medical graduate to come to the United States under an educational exchange program for up to seven years. Normally, the physician must return to his/her own country for at least two years before applying for a permanent visa in the United States. The J-1 Visa waiver program eliminates the two-year home residency requirement, allowing the physician to remain and practice in the United States. State government agencies may also sponsor J-1 Physician waiver requests which are called Conrad State 30 programs. There are five statutory bases upon which a person can apply for a waiver of the two-year foreign residence requirement: No objection statement from the host government, if needed, referred to as a No Objection Letter; it takes an average of four months to obtain. Request from an interested U.S. Government agency on the person s behalf, referred to as an International Government Agency Waiver; it takes an average of four months to obtain. Claim that you will be persecuted if you return to your country of residence, referred to as an Asylum Waiver; it takes an average of nine months to obtain. Claim of exceptional hardship to a U.S. citizen or permanent resident spouse or child if you are required to return to your country of residence, referred to as a Hardship Waiver; it takes an average of nine months to obtain, and Request by a designated State health agency or its equivalent; it takes an average of five months to obtain. The J-1 Visa Waiver program has been important and helpful for rural communities to sustain access to health care in many rural communities when other recruitment efforts have been unsuccessful. The J-1 Visa Waiver is beneficial for both the doctor and the community, because the program is dependent on the physician working in a Health Professional Shortage Area or Medically Underserved Area, the physician is able to stay in the United States to practice, and an underserved community gets a much-needed doctor. 5

The specific tasks performed as a physician assistant vary because of the different practice requirements mandated by geographical, political, economic, and social factors. Most of those underserved areas in Kansas are in rural areas. With more than 100 years of combined clinical experience among faculty members and more than 80 years of experience in health care education, the PA program is dedicated to training excellent primary care physician assistants. Of the more than 1,000 graduates of the WSU PA program, 52 percent are working in primary care settings and 15 percent are in underserved areas, both of which are above the national average. Dr. Moser says providers such as physician assistants and nurse practitioners are an important component of our health care system and in other rural and underserved areas of Kansas. Many communities are seeing the graying of their populations and that includes the physicians as well. Chronic disease management takes more clinic visits per year and provider time to manage and an adequate provider workforce remains unmet in many communities. The collaboration between providers is imperative to provide the physician directed medical home that can support effective patient care and the access patients require for their care, he said. Dansel and her husband, Jonathan, enjoy the community of approximately 200 people and have been influential in starting the Weskan Community Improvement Association to help promote their community. When she is not working she enjoys time outdoors with her 5-month-old son and watching her husband coach football for the Wallace County High School Wildcats. Scholarships and Programs for Providers Statewide Programs Kansas Medical Student Loan The Kansas Medical Student Loan (KMSL) program represents a generous and serious effort by the people of Kansas to encourage students in the University of Kansas School of Medicine to practice primary care medicine in the state of Kansas. The Kansas Medical Student Loan is awarded on an annual basis in return for agreements to practice medicine in the state. The Kansas Medical Student Loan provides payment of tuition and monthly living expenses during medical school for physicians who agree to practice primary care (including emergency medicine) in Kansas. Loan repayment assistance is provided in exchange for one year of service in a federally qualified underserved area in Kansas. Agreements are renewable annually based on the availability of funding. Contact: Kansas Board of Regents, Kansas Nursing Service Scholarships The Kansas Nursing Student Scholarship was enacted by the 1989 Kansas Legislature to provide financial incentives for students to study and practice nursing in Kansas with an emphasis on underserved rural areas. It was renamed the Kansas Nursing Service Scholarship by the 2001 Legislature. Minimum criteria include: 1. Acceptance to a Kansas nursing program (pre-nursing will not be funded) and, 2. Commitment by an eligible sponsor to fund up to half of the scholarship and provide full-time employment to the recipient within six (6) months after licensure. Contact: Kansas Board of Regents, Kansas Osteopathic Medical Service Scholarship The Kansas Board of Regents has the authority to grant scholarships to individuals pursuing a medical career in osteopathy. The program is designed to encourage doctors to practice primary care medicine in areas of Kansas where there is an insufficient number of medical providers. The Osteopathic Medical Service Scholarship provides an annual award for up to four years of study at nationally accredited osteopathic schools. Preference goes to first year students. Recipients return one year of practice for each year of assistance. NHSC offers a number of programs to encourage health professionals to practice in America s underserved communities. NHSC gives fully trained health professionals who are dedicated to serving the underserved and have qualifying educational loans the to apply for loan repayment if they choose to serve in a community of greatest need. Contact: NHSC, hrsa.gov National Health Service Corps Scholarship The NHSC Scholarship Program is a competitive scholarship open to eligible students in a variety of primary health care disciplines who have career goals that include practicing in underserved areas. Scholarships are awarded in exchange for a student s commitment to practice in a community with a shortage of health professionals. NHSC scholars receive payment of tuition and fees for up to four years of education; 12 monthly stipend payments per year of scholarship support; and payment of other reasonable expenses, such as books, supplies, and equipment in exchange for their commitment to serve in a community in need. Authorized by Section 846 of the Public Health Service Act, as amended, the purpose of the NELRP is to assist in the recruitment and retention of professional nurses dedicated to providing health care to underserved populations. 16 Wonders 8of Kansas The Kansas Sampler Foundation has chosen the 8 Wonders of Kansas. This is the third story of four in a series highlighting the wonders. The wonders were announced on Kansas Day, Jan. 29, by Gov. Kathleen Sebelius. The Kansas Sampler Foundation is a non-profit organization. Its mission is to preserve and sustain rural culture in part by educating the public about what there is to see and do in the state which all Kansas communities, regardless of size. Kansas Underground Salt Museum The fifth wonder of Kansas is the Kansas Underground Salt Museum. Located in Hutchinson, this wonder is one of 16 salt mines in the United States Kansas Underground Salt Museum, Hutchinson, Kan., courtesy KS USM 16 Kansas c o n n e c t i o n s and one of three salt mine museums in the world; the other two are in Poland and Austria. The Kansas Underground Salt Museum showcases the natural treasure of salt that exists 650 feet below the Kansas plains. While at the museum, visitors can explore an exotic environment carved from salt deposits formed 280 million years ago. People from across the country visit Hutchinson to catch a glimpse of this wonder. The tour adventure begins on board a double-decked elevator that takes visitors to the salt deposit 650 feet underground. Upon arriving, they board mined out areas with naturally occurring geologic formations and artifacts left behind by miners over a period of 85 years. Tourists then have the to wander through exhibits that tell the story of salt mining, the geological history of the area and how these vast salt caverns underneath Hutchinson have been used to store over 7 million documents. Salt Block, courtesy KS USM The world s oldest living organism, estimated to be about 250 million years old, is now living at the Kansas Underground Salt Museum. The unprecedented discovery of living bacteria found trapped inside a salt crystal, is part of the museum s newest exhibit. Also on display at the Kansas Underground Salt Museum is General Electric (GE) Engine No. 2, one of only three such engines ever built. The antique train was built in 1919 and ran along a short railway line from 1928 to 1963 that provided switching services for the Carey Evaporation Plant and Salt Mine. The rail line was less than six miles but was vital to the success of the Carey Salt Mine. 17 Mobile Homes and the Kelly Mills companies. Weighing 60,000 pounds, the locomotive was powered by a 260 horsepower engine. The Kansas Underground Salt Museum plans to add new windows and doors to the locomotive and then will begin restoring it to its original state. The area around the train will be landscaped and will include exhibit information for visitors to learn about the history of GE Engine No. 2. These are only a few of the many things to see and do at the Kansas Underground Salt Museum. For more information about the exhibits or to schedule a tour, visit org/ or call The Salt Museum is open to the public for tours Tuesday Saturday from 9 a.m. to 6 p.m. and Sundays from 1 p.m. to 6 p.m. Monument Rocks & Castle Rock The sixth wonder of Kansas is a combination of two sites Monument Rocks and Castle Rock. They were chosen as a wonder of Kansas because of the scientifically significant fossils these ancient chalk beds have produced and because they have been highly eroded into unusual spires and shapes, making them spectacular landmarks on the plains of western Kansas. Monument Rocks is a series of large, heavily sculpted chalk monoliths, Castle Rock photo by Marci Penner located on the western edge of Gove County. The site has been designated as a National Natural Landmark and is sometimes referred to as the Chalk Pyramids. The Castle Rock, a chalk spire that stands by itself in the valley of Hackberry Creek, is located in eastern Gove County. Immediately south of Castle Rock is an extensive outcrop of chalk, capped by the younger Ogallala Formation. The chalk was deposited during the Cretaceous Period, about 80 million years ago, when the central interior of the U.S. was covered by a seaway. The several hundred feet deep water contained single-celled animals that drifted to the sea floor, creating a mucky ooze over time. This material was perfect for trapping and preserving the remains of animals that lived in that ocean, such as fish, turtles, sharks, swimming reptiles called mosasaurs and plesiosaurs, swimming birds, gliding reptiles called pterosaurs, as well as invertebrate animals such as giant clams. Today, the chalk beds routinely give up these fossils. Probably the best-known fossil from these beds is the famous fish-withina-fish on display at the Sternberg Museum in Hays.
