According to the Kansas Youth Empowerment Academy (KYEA), young people with disabilities have more opportunities and more challenges than at any other time in the nation s history, and the need for this education and advocacy for youth with disabilities has increased. The Kansas Youth Empowerment Academy (KYEA), is a non-profit organization established to house leadership forums and other programs to empower young people with disabilities. KYEA provides youth with disabilities the encouragement and information about resources that will enable them to develop as leaders and become agents of change. The mission of the KYEA is to promote and support the development of youth with disabilities to become empowered community leaders. This is achieved through the implementation of various programs that target youth with disabilities through education, mentoring, and peer support. KYEA, through Sunflower Foundation grant funding, launched the Creating Youth Agents of Change program to help youth identify health-related issues and develop advocacy strategies that put them in the driver s seat for promoting health-related change. Participants of the Kansas Youth Empowerment Academy discuss the challenges they face with regard to health-related issues. The program began with an informationgathering phase among past participants of the Kansas Youth Leadership Forum, a program sponsored by KYEA. Jean Hall, PhD, and Barbara Starrett, MHA, Center for Research on Learning-Division of Adult Studies, University of Kansas, were integral in distributing a written survey and assisting with focus group sessions about health-related issues that concerned youth with disabilities, such as health insurance, employment, education, and transition from school to community life issues. Thirty youth participated in the survey and ranged in ages years. Participants represented 19 of Kansas 105 counties, and 30 percent (9 participants) represented rural counties. Those organizations include: Bethany Home Association, Lindsborg, Kan.; Dooley Center, Atchison, Kan.; Medicalodge of Eureka, Eureka, Kan.; Minneola District Hospital Long-Term Care Unit, Minneola, Kan.; Newton Presbyterian Manor, Newton, Kan.; Hillsboro Community Medical Center, Hillsboro, Kan.; and Pleasant View Home Inc., Inman, Kan. The PEAK program is an initiative of the Kansas Department on Aging to support nursing homes instituting non-traditional models of care with progressive environments. This award recognizes homes that adopt more home-like settings along with resident control, staff empowerment, and community involvement. Heart of Healthcare Awards The 16th Annual Nursing: The Heart of Healthcare awards honored 10 registered nurses on Oct. 14, 2006, for their commitment and dedication to serving Kansas communities. This event, coordinated by the University of Kansas School of Nursing, recognizes outstanding registered nurses in Kansas. Among those honored, two nurses representing rural communities were recognized. Michele Correll, RN, Meade County Health Department, Meade, Kan., has worked tirelessly to turn the community s small health department into a highly respected and professional organization. Often working nights and weekends, Correll has worked diligently to ensure that each community member has access to health care. As stated in her nomination for the honor, she treats each person and everyone who enters the clinic with dignity and compassion. Correll is committed to educating, empowering and improving the health of her clients and providing the community with the resources to make their lives better. Carol Smith, RN, ICU staff nurse, McPherson Memorial Hospital, McPherson, Kan., has taken her commitment to the community one step further in support of those impacted by Hurricane Katrina. When Hurricane Katrina hit the Gulf Coast, Smith, was dedicated to assisting nurses, many of whom had lost everything, get back on their feet with the donation of scrubs and other much-needed supplies to the area. Smith was nominated by John Jones, associate administrator, Medical Center of Louisiana, who states, Her tireless efforts on behalf of the nursing community all across the Gulf Coast was met with a welcome sigh, as nurse after nurse reported back to work in new scrubs. Her efforts were nothing less that heroic, the full effects of which will never be measured. 3 100,000 Lives Campaign Improving health care in rural Kansas Nearly 50 hospitals in Kansas were instrumental in saving an estimated 122,300 lives through the 100,000 Lives Campaign, an 18-month campaign initiated by the Institute for Healthcare Improvement (IHI) in December of The Campaign challenged US hospitals to save 100,000 lives through the implementation of six proven health care interventions. As a result, more than 3,000 hospitals across the country joined the Campaign representing 75 percent of U.S. hospital beds. The Kansas Foundation for Medical Care (KFMC) served as the node for the 100,000 Lives Campaign in Kansas and received support from other health care partners to promote the program and encourage Kansas hospitals to participate. Hays Medical Center was one of more than 50 hospitals in Kansas that participated in the 100,000 Lives Campaign. The Hays Medical Center staff realizes the need to improve patient care each day. John Jeter, MD, MHA, FACEP, president and CEO, indicates that the Center is always looking for projects to improve the care delivered at Hays Medical Center and follows the lead of accredited institutions such as the IHI and KFMC. Hays Medical Center is always looking for quality initiatives, and the 100,000 Lives Campaign gave us direction as to what to address to improve patient care, stated Jeter. Connie Hubbell, senior vice-president, community relations, KFMC, indicates the 100,000 Lives initiative identified six scientifically- proven interventions that will save lives in Kansas hospitals. Each initiative provides guidance on implementation, which allows Kansas Hospitals to implement any one or all 6 of these initiatives in a coordinated effort. The project promotes the right care for every person, every time, states Hubbell. Hospitals participating in the Campaign agreed to implement one or more of six evidence-based interventions. Activate a Rapid Response Team at the first sign that a patient s condition is worsening and may lead to a more serious medical emergency. Prevent patients from dying of heart attacks by delivering evidence-based care, such as appropriate administration of aspirin and beta-blockers to prevent further heart muscle damage. Hays Medical Center, Hays, Kan., continues to implement evidence-based interventions, as a result of the 100,000 Lives Campaign. Prevent medication errors by ensuring that accurate and continually updated lists of patient s medications are reviewed and reconciled during their hospital stay, particularly at transition points. Prevent patients who are receiving medicines and fluids through central lines from developing infections by following five steps, including proper hand washing and cleaning the patient s skin with chlorhexidine (a type of antiseptic). Prevent patients undergoing surgery from developing infections by following a series of steps, including the timely administration of antibiotics. Prevent patients on ventilators from developing pneumonia by following four steps, in- Continued on page 4 3

4 100,000 Lives CONTINUED FROM PAGE 3 cluding raising the head of the patient s bed between 30 and 45 degrees. The Hays Medical Center was part of the initiative to save lives and set out to institute each of the six interventions. Jeter indicated that the Center had already accomplished two of the initiatives prior to the campaign, but continued to implement the additional preventions and was very happy with the results. Hubbell stated, Generally rural hospitals are able to implement at least five of the six interventions and there are times when the rural hospitals made adaptations to the intervention so it would work in their setting. For the Hays Medical Center, each initiative was valuable, but the institution of the rapid response team was the most significant change. Jeter indicates that this prevention is probably the most important in rural hospitals. Kansas Hospitals have realized the impact that adopting these 6 interventions will have on their quality of care, stated Hubbell. Kansas Hospitals have had opportunities to learn from each other, share their successes with other hospitals and know that they have made a difference in the quality of care that they are providing to their patients. Bob Cox, MD, medical director, Hays Medical Center, feels the 100,000 Lives Campaign was a great opportunity for dialogue. The future will move forward from what we re trying to do and it s important to have those building blocks, states Cox. Adoption of these six interventions by Kansas Hospitals has definitely saved lives in Kansas, stated Hubbell. KFMC strongly encourages Kansas hospitals to continue to work on and adopt the six interventions established through the campaign. It is important to carry on the efforts to improve quality of care and reduce the number of unnecessary deaths in our health care systems. More than 30 physicians and Brandi Bowen, Sonographer 4 Kansas c o n n e c t i o n s allied health professionals have benefited from KRC services; Brandi Bowen, sonographer, is one such health care professional. Bowen received the assistance she needed from KRC, and joined the staff at Bob Wilson Memorial Grant County Hospital, Ulysses, Kan., in March Bowen, a graduate from Southern Medical Corporation, School of Ultrasound, Baton Rouge, La., was seeking a position as a cardiac and vascular ultrasound technologist, with the potential for growth. Once Bowen began her job search, KRC was able to provide the necessary guidance and input to help Bowen find the right match for her and her family. The KRC recruiter took charge of the situation of finding me a better match. She [the recruiter] did her very best to make me happy with my choice of relocation, stated Bowen. KRC staff used Bowen s clinical experience and professional profile, which includes involvement in the Society for Vascular Ultrasound, and the American Society of Echocardiography, in addition to her desire to work with a diverse patient case load, as a tool in making the right match for her at Bob Wilson. Although the move from southern Mississippi to Kansas has been a transition in and of itself, Bowen states the employees at Bob Wilson expressed a genuine warmth and friendliness, unlike any I ve seen in other hospitals. KRC has more than 550 active candidates looking for practice opportunities, and assists in the efforts to recruit and retain physicians and other health care professionals in rural areas of Kansas. Health care professionals may utilize the KRC as a career service center and receive placement assistance with openings throughout the state. 5 KYEA CONTINUED FROM PAGE 1 the participants conveyed personal insights and experiences regarding access to health care, as well as employment and educational issues related to having a disability. It was reported through the study, that 23 percent of the participants were not satisfied with their current medical care, and 30 percent reported barriers to seeing the doctor, including expense, lack of transportation, lack of insurance, and attitudes of the medical staff regarding disability. Young adult Kansans who participated in the health survey were asked if they had discussed their changing health care needs as they transitioned to adulthood. Forty-seven percent indicated they had not, while 33 percent said they had, and 20 percent replied they did not know. When asked if they had developed a transition plan for their changing from adolescent to adult health care needs, 40 percent said they had not, only 17 percent indicated they had, and 43 percent did not know. Participants responded that most doctors see their disabilities first and feel compelled to fix them, rather than see them as individuals who have the same needs and desires as everyone else. This concern is illustrated through the fact that half of the participants said they had never received any information about birth control, pregnancy or STDs from their doctors, yet, according to the study, 77 percent of the participants plan to marry, and 70 percent expect to have children. Starrett indicates that this factor is a large concern. It is apparent that these youth are not being provided with information about sexuality, STDs, and AIDS issues. It is essential that parents and doctors create an awareness for these youth and get the information out. Starrett also states, Adults will find a way to engage in sexual activity, so it is imperative that they receive information regarding issues surrounding this activity. As a result of the focus group, it was suggested the medical providers have a difficult time getting past the medical model of disability and feedback indicates that these young adults feel as though medical providers need to learn more about people with disabilities and to see them as It is apparent that these youth are not being provided with information about sexuality, STDs, and AIDS issues. It is essential that parents and doctors create an awareness for these youth and get the information out. Barbara Starrett, MHA, Center for Research on Learning-Division of Adult Studies, University of Kansas individuals rather than as their conditions. The KYEA program, Creating Agents of Change, has provided insight into the needs of youth with disabilities regarding health care. This opportunity for youth to share their experiences with health care related issues reveals that many youth with disabilities are not able to, or were not provided access to services and information that could improve their health and well-being. With these findings in hand, both Hall and Starrett indicate they hope the survey will be conducted again next year to determine if attitudes or access has changed for these young adults with regard to health care. Hall feels as though the survey and focus group have provided an awareness and insight into the challenges that many youth with disabilities face. Hall encourages health care providers to listen to the person, make the health care process a team effort, be aware, and don t neglect to consider the individual s perspective the person with the disability is the expert. Creating Agents of Change program has provided insight into the needs of youth with disabilities regarding health care. 5

6 A patient-centered guide Implementing language access services According to a Kansas Health Institute report, Racial and Ethnic Minority Health Disparities in Kansas: Data and Chartbook, Kansas minorities like minority populations nationwide are disproportionately affected The Patient-Centered Guide provides practial suggestions for the implementation of language access services by health care providers. by disease and other health conditions caused, in part, by socioeconomic status 6 Kansas c o n n e c t i o n s and lack of access to quality health care. The Kansas Department of Health and Environment (KDHE), indicates the minority population in Kansas doubled between 1980 and 2000, from 223,637 to 454, percent of the population in Kansas. As population trends adjust and communities grow, the need for addressing potential language disparity in relationship to education, economy, and health care, becomes increasingly important. In an effort to assist health care organizations in addressing language barriers in health care, the Office of Minority Health (OMH) has released, A Patient-Centered Guide to Implementing Language Access Services in Healthcare Organizations. The guide is intended to help health care organizations implement effective language access services to meet the needs of limited-english-proficient patients, and increase access to health care. The Patient-Centered Guide, indicates that health care delivery and success is primarily based on the patient and provider communication, regarding the health care complications present at the time of diagnosis. Patients who struggle to communicate with the providers How to work effectively with interpreters Face the patient and speak directly to the patient. Speak slowly and clearly. It is not necessary to raise your voice and shout. Avoid metaphors, slang, idioms, and jargon. Explain all medical terms in simple language. Watch the patient during the translation to allow observation of nonverbal cues. Try not to interrupt the interpreter, and allow the interpreter to ask questions to clarify what the patient says. Allow time for patients to ask questions and seek clarification. If the interpreter seems to answer for the patient, inquire may experience inappropriate care. In addition, it is oftentimes difficult for the patient and provider to establish a continuum of care, as communication barriers inhibit the creation of trust and rapport, needed to build a relationship. The primary goal of the Patient- Centered Guide is to provide practical, ground-level suggestions for the implementation of language access services by health care providers. The guide is designed to serve as a resource for organizations of all sizes. It may be especially helpful for smaller organizations with resource constraints, as it provides useful information solicited from health care professions who work in smaller organizations, such as family practices, health clinics, and health care specialists with limited resources, that seek alternative means of implementing language access services. The guide seeks to assist in improving the quality of care delivered in health care organizations and improve patient satisfaction. Use diagrams, pictures, and translated written materials to increase understanding. Always ask patients to repeat instructions back to you to be certain they were understood. Some patients who request an interpreter may understand English well. The patient may understand any comments made to other providers or to the interpreter. 14 A journey through Kansas: A Closer look at the Southeast Visitors to Columbus, Kan., have many sights to see, including a Hot Air Balloon Regatta, held each October - one of only 4 balloon rallies in the state. The state of Kansas covers 82,282 square miles of terrain. As the 15th largest state in the U.S., the historical and agricultural contribution is great, and the way of life for numerous Kansans is unique in each region of the state. As part of a six-part series, highlighting unique attractions in areas throughout Kansas, with the help of Marci Penner s book, entitled, The Kansas Guidebook, this issue will provide a closer look at the southeast region of the state. According to Jean Pritchett, executive director, Columbus Chamber of Commerce, southeast Kansas is a beautiful area of the state, with lots of trees, green grass, hills, agriculture, and numerous industries in the area. It s enough to keep her rooted. I stay because I like it. I still have the city close by to do activities, but have all the small town advantages, states Pritchett. Much of the Flint Hills can be found in southeast Kansas, as the landscape spans hundreds of miles. Visitors to the area may take advantage of the wildlife in the area through hunting. The Flint Hills Adventures, Inc., offers trophy Kansas deer hunting in remote areas with great whitetail deer populations. Visitors interested in hunting at the Flint Hills Adventures, may also plan quail and turkey hunting, as the ranches have limited hunting pressure and numerous game to hunt. Visitors interested in visiting the Flint Hills for the aesthetic qualities may do so with an authentic covered wagon trip. Since 1979, the Flint Hills Overland Wagon Train has provided trips through the Kansas Flint Hills. The trips allow visitors, or pioneers, the opportunity to relive experiences of those who were part of the Westward movement in the mid-19th century. Many indigenous animals have made much of the Flint Hills their home, but others, exotic animals included, find the Emporia Zoo a much safer, comfortable place to settle. As one of the country s smallest accredited zoos, visitors may view the animals while walking or driving. The Emporia Zoo is open daily from 10:00 a.m. to 4:30 p.m., but visitors may drive through the large animal drive-through until dark. Fort Scott, Kan. is located midway between Fort Leavenworth, Kan., and Fort Gibson, Okla., on the route historically known as the Military Road. The 14 Kansas c o n n e c t i o n s

15 fort at Fort Scott was established in 1842 and named for Lieutenant General Winfield Scott, then, General-in-Chief of the U.S. Army. As a result of this rich history, the National Park Service has deemed 20 historic structures throughout Fort Scott as well as the Fort Scott National Cemetery, located on the outskirts of this small town. Eureka, Kan., is home to nearly 3,000 residents. With a plethora of sights to see and enjoy, visitors may experience horse racing, as Eureka is known as the Horse Racing Capital of Kansas. Horses have been racing at Eureka Downs since 1862 and while the grandstand has been renovated over the years, visitors can still experience the excitement surrounding a good race. Erie, Kan., the county seat of Neosho County, is on highway US59 near the Neosho River. This small town with nearly 1,200 residents is famous for its annual Old Soldiers and Sailors Reunion held during the third week of July. This event includes the annual free Beanfeed. During the Beanfeed, more than 1400 pounds of beans are cooked in more than 50 iron kettles on the courthouse lawn by the American Legion Post. Through this annual event Erie has earned the title, Bean Feed Capital of Kansas. If beans aren t the food of choice for visitors to southeast Kansas, one doesn t have to travel far to find Crawford County, the Fried Chicken Capital of Kansas. According to Penner, Crawford County probably has more fantastic nonfranchise restaurants than any other county in the state. Although visitors may find more fried chicken in this area than in most, anything from classic diners to Brazilian-style steaks are on the menu in this county. Visitors to southeast Kansas will realize quickly that the area is rich in history. Contributing to the historical value of the area, West Mineral, a town with fewer than 250 residents, is home to the second largest electric shovel in the world, more affectionately known as Big Brutus. This piece of machinery is 16 stories high (160 feet), weighs 11 million pounds, 150 feet long and could scoop enough rocks and topsoil to fill three railroad cars, in one fail swoop. Big Brutus ran continuously between 1963 until 1974, removing soil and rocks. Dedicated as a museum and memorial to the rich coal mining history in southeast Kansas, Big Brutus is a certainly a site to be seen in West Mineral, Kan. Galena, Kan., has also memorialized mining history and is home to the Galena Mining and Historical Museum. According to Penner, photos and artifacts capture the story of the region s mining days from 1877 to the 1920 s. During this time, lead and zinc were the heart and soul of Galena. The museum is located in a KATY railroad depot and provides visitors with an appreciation of the town s history. The habitat in southeastern Kansas is enough to support numerous wildlife, including the safari animals found at the Safari Zoological Park, Caney, Kan. Walk-through guided tours of the park are available for an educational and entertaining experience. If visitors are interested in the historical attributes of the community, self-guided, driving or walking tours through 26 historic sights in Caney are available. As the economy grows, population trends move toward larger cities and Lauren Lambert of Wichita, Kan., visits Big Brutus in West Mineral, Kan. This electric shovel is the second largest in the world. away from rural areas, Elgin, Kan., has a motto that most small communities can relate to: A town too tough to die. Eighty residents call Elgin home and pronounce their dedication to their small town with that motto clearly painted on a sign in front of the community building. There are numerous reasons Kansans have chosen to live in the southeast region of the state. Aside from the aesthetic beauty mentioned by Pritchett, she also indicates that she has chosen to live in Columbus, Kan., because the pace is much slower and ultimately, because the people are friendly and easy to get to know. 15

16 16 Kansas c o n n e c t i o n s Rural hospitals identify funding for construction Many rural health care organizations face the same concerns that Wilson County Hospital, Neodesha, Kan., staff were facing, in that the best possible care is difficult to provide when a building begins to age. Increasing health care capabilities is also a challenge, as Rooks County Health Center, Plainville, Kan., staff has learned, when building constraints prohibit the staff and services from expanding. Deanna Pittman, CEO/Administrator, Wilson County Hospital, indicates, Accomplishing the task of providing the best possible medical care in Wilson County and surrounding areas, in a building built in 1916 is a daunting and frustrating undertaking. For that reason, both health care organizations have sought alternatives for providing the best possible care to patients in rural Kansas. The board of trustees at Wilson County Hospital (WCH) spent several years gathering and examining data to determine how the needs of patients could be met more effectively. Senator Derek Schmidt, Wilson County Hospital Board of Trustees, Administration, and County Commissioners broke ground Tuesday, November 7, 2006, on their new hospital and medical office building in Neodesha, Kan. Concerns regarding the need for additional space to accommodate the Hospital s Center for Wound Care and Hyperbaric Medicine, as well as other new services, and the inability to add on to their 90-year-old building led the board to determine that a replacement facility would be the best option for meeting the needs of current and future health care provided at WCH. For Rooks County Health Center (RCHC), the past five years has provided consistent growth in revenues and an increase in the level of care provided. We have four family physicians and three mid-levels that provide excellent care to this area of Kansas. We have increased our ambulatory surgeries, obstetrics, cardiac rehabilitation, lab and therapy services, stated Kathy Ramsay, RN, communication and development director, Rooks County Health Center. The construction of a new facility for Rooks County Health Center will provide ample space for the Center s growth. As with any major undertaking, the changes, triumphs, and challenges are many. For Rooks County Health Center, locating the most cost effective financing was the biggest challenge. Rooks considered several options, as did Wilson County Hospital. The HUD 242 program and the USDA loan program were identified, but not taken advantage of by either organization. Instead, both organizations will utilize tax-exempt hospital revenue bonds. Ramsay states, The best alternative came from an opportunity with United Missouri Bank (UMB) to have a $19 million, tax-exempt, fixed rate, and hospital revenue bond to assist in funding the construction. While a majority of the funding will come from the hospital revenue bonds, additional funding for the new facility in both communities will come in a variety

17 of ways. Wilson County Hospital will use donations from the capital campaign and tax-exempt revenue bonds for the replacement hospital. Funds from a block grant, tax credits and donations from the capital campaign will be used to build a new medical office building next to the hospital. In Plainville, Kan., a voluntary hospital employee payroll deduction program, in addition to the same at Dessin Fournir (an organization within the community), has been established to assist in raising the necessary funds to build the new hospital. Rooks County Health staff will apply for various grant opportunities and organize numerous fundraising efforts to contribute to the construction costs. Community members have already shown their support with many individuals sponsoring fundraising efforts such as a Pampered Chef party, Cook Book project, Turkey Trot 5K (run or walk), and various other events. Funding for the new hospital in Wilson County, has come from a variety of sources. Wilson County recently issued approximately 10 million dollars in tax-exempt revenue bonds through UMB, Kansas City, and another two million dollars in additional bonds will be issued in January of The Wilson County Hospital Foundation also exceeded its goal of $2.2 million for the capital campaign, including 100 percent voluntary participation from WCH employees, and a lead gift of $1 million from a trust fund within the community. Financing has been secured in Neodesha, and a groundbreaking was held November 7, 2006, as a ceremonial kick-off for the construction. With the construction period estimated at 14 months, hospital staff anticipates moving into the new facility in the first quarter of Rooks County Health Center will share the same success and has plans to complete their new hospital by April The construction of a new hospital will make an impact on the communities for which they serve. Pittman states, Construction of a new replacement facility will greatly improve our efficiency, patient comfort and the opportunity to add additional services to meet the needs of our community.
