Medical and Surgical Services Cardiac (2 West) Post-Cath (6 Central) Cardiac Progressive Care (3 West) Congestive Heart Failure (3 East) Orthopedic Unit (5 North) Surgical Unit (6 North) Med / Surg / Telemetry Unit (5 West) Neuroscience Unit (5 Central / 5 Midwest) Spine Center (3 Central) Renal / Medical Unit (6 East) 39-bed telemetry unit promoting recuperation from open-heart surgery and other cardiovascular surgeries. 6-bed stepdown unit. 8-bed unit providing care for patients post-invasive cardiac procedures. 39-bed telemetry unit for patients with cardiac chest pain, OBS to telemetry stepdown. 30-bed patient care department telemetry beds for CHF and ACS patients and post-cardiac catheterization. 32-bed unit providing care for patients after orthopedic surgery and those with musculoskeletal disorders. 32-bed unit focusing on inpatient, general surgery, trauma, and palliative care patients. 39-bed unit for patients requiring rapid admissions to Hospitalist services and providing overflow cardiac telemetry monitoring. 32-bed unit providing care for patients with stroke and other neurological diseases. 8-bed unit providing care to outpatient neurosurgery patients requiring extended observational recovery time from short stay procedures. 30-bed unit with telemetry specializing in the care of patients with renal and/or diabetic diseases. 2 Specialized and Operative Services Pediatrics Unit / Pediatric Intensive Care Unit (6 Midwest) Cardiac and Vascular Catheterization Hemodialysis / Apheresis Unit 2 nd Floor Operating Rooms 3 rd Floor Neuro Operating Rooms Post Anesthesia Care Units Endoscopy Short Stay Center Internal Medicine Teaching Service Outpatient Clinic 18-bed unit providing medical-surgical nursing care for infants and children, with an additional 4 intensive care beds. 10 labs, including a lab dedicated to EPS procedures and one dedicated to peripheral vascular procedures, provide 7,200 diagnostic catheterizations plus 1,900 PCI / stent placements annually. 9-bed unit for inpatient hemodialysis and plasmapheresis procedures. 16 major operative suites to provide care for patients requiring surgery from the simple to the complex, on a 24-hour basis. 4 major operative suites to provide care for patients requiring surgery from the simple to the complex, on a 24-hour basis. 14-bed Acute Care PACU receives patients from the main OR suites, and 4-bed Neuro PACU receives patients from the Neurosurgery OR suites. Treatment or diagnostic procedures for patients with GI disorders. Short Stay Surgical is divided into Pre admissions ( Pre admissions testing for Surgical patients, such as lab work, EKG, CXR, and pharmacy medication reconciliation, if not reached by phone), and Pre-operative ( Preparing Patient's the day of their surgery, and Interventional Radiology, if having anesthesia, with IV access, Blocks, etc). Short Stay Procedural is divided into Medical Day ( patient's receiving Blood transfusions, Medication injections/infusions), and Day Cath ( Patient's going to Cardiac Cath Lab, Vascular Lab, and Radiology patient's not requiring anesthesia). 21 exam rooms encompassing several subspecialty clinics (Anticoagulation by Pharmacist, Pediatrics, and Internal Medicine). Heart Failure Clinic Rehabilitation Services Rehabilitation Units (4 West & 4 Midwest) 38-bed referral center specializing in intensive and comprehensive rehabilitation of patients with spinal cord injuries, CVAs, and multiple traumas. ALSO ON THE MOSES H. CONE HOSPITAL CAMPUS Urgent Care Center Cone Health Sports Medicine Center 11-bed facility for patients with non-emergent medical complaints and minor injuries. Services include analysis of gait, orthotics, joint injections, bracing and casting, rehabilitation modalities, biomechanical evaluations for skill sports, preparticipation examinations, exercise evaluation prior to beginning an exercise program. Physicians see sports medicine patients and patients with musculoskeletal issues. Staffed by 2 physicians and 3 sports medicine fellows. Updated 11/2016 Page 2 of 6
3 Family Practice Center Regional Center for Infectious Disease Day Surgery Center Heartland Extended Care Center Patient-centered (medical home) practice provides a model office environment for medical resident training. The center houses 18 exam rooms, a laboratory, physician/faculty and sees approximately 100 patients per day. The multidisciplinary health care team includes, nursing, pharmacy, nutrition, social work as well as business office professionals. Drug Therapy Clinics offer specialized pharmacotherapy services provided by AHEC faculty pharmacists, residents and students for; smoking cessation, lung disease, diabetes and hypertension (ambulatory blood pressure monitoring). Outpatient clinic for HIV and other infectious diseases, located at the Wendover Medical Center. Pharmacists are consultants to the ID team. Provides outpatient surgical procedures. 138-bed skilled nursing care facility specializing in geriatric care. Heartland provides long-term residential nursing care and short and long-term residential rehabilitation services. BEHAVIORAL HEALTH HOSPITAL (80 beds) Behavioral Health Hospital Adult and adolescent facility utilizing an interdisciplinary team to providing inpatient and outpatient behavioral health care. WESLEY LONG HOSPITAL (175 beds) Emergency Department Intensive Care / Stepdown Unit Orthopedic / Medical / Surgical Unit Medical / Psychiatric Unit Telemetry / Urology Operative Services Endoscopy Suite Inpatient Oncology Unit Short Stay / Medical Day Unit 25 ED beds in addition to 10 psychiatric ED beds and an 8-bed holding area. 20-bed intensive care unit for medical, surgical, & pulmonary patients. 32-bed unit for medical / general surgery including bariatric care 20-bed unit for patients requiring additional psychiatric monitoring. 45-bed unit for patients requiring cardiac monitoring. 11 OR suites, 2 outpatient OR suites, 12-bed PACU and 12 outpatient recovery beds. 4 procedure rooms, 6-bed recovery area for patients with GI problems. 32-bed unit providing comprehensive care to patients receiving cancer-related treatments. 30-bed unit for preadmission, surgery, radiology, and other patients admitted the morning of procedures. 4 CONE HEALTH CANCER CENTER Involvement in weekly multidisciplinary clinics (i.e. thoracic) Cone Health Cancer Center 10-physician medical oncology practice includes the main center on the Wesley Long Hospital campus and 2 satellite centers at High Point and Randolph providing specialized inpatient and outpatient services. Pharmacists and technicians provide chemotherapy and clinical services to patients and are involved in weekly multidisciplinary clinics. Primary training site for PGY2 hematology-oncology specialty resident, PGY1 oncology rotations, and PharmD student oncology rotations for 3 NC pharmacy schools. Cancer Center Anticoagulation Clinic Specialized anticoagulation management by pharmacists with an emphasis on inherited coagulopathies and thrombophilias as well as anticoagulation treatment protocols. WOMEN S HOSPITAL (134 beds) Maternity Admissions Birthing Suites High Risk Obstetrical Unit Mother-Baby Suites / Women s Unit Neonatal Intensive Care Unit (NICU) Women s Unit Women s Hospital Clinics Operative & PACU Services 12-bed unit providing evaluation of obstetrical emergenices, labor assessments & antenatal testing. 18-bed birthing suites providing family centered care. 14-bed unit serving antenatal and adult intensive care populations. 48-bed mother-baby unit that provides couplet care for mothers and newborn infants. 36-bed unit with LEVEL III neonatal care & neonatal transport services. 17-beds for postsurgical OB/GYN patients. High Risk OB, GYN, Developmental and Medical (Pediactrics). 7 OR suites providing surgical care to women. ANNIE PENN HOSPITAL (110 beds) Emergency Department Intensive Care Unit Dept. 300 Orthopedic / Medical / Surgical / Pediatric / Concentrated Care Annie Penn Cancer Center 23 treatment areas providing care to all patient populations presenting with a variety of emergency and non-emergency conditions. 12-bed unit provides care to critically ill or injured patients. 41-bed unit specializing in the care and treatment of patients of all ages with total joint replacements, knee surgeries, abdominal pain, asthma, diabetes, pneumonia, dehydration, medical surgical, oncology, respiratory, and cardiac patients. 5 ANNIE PENN NURSING CENTER (99 beds) Skilled nursing care facility for older adults and those with acute and chronic conditions CARELINK MOBILE CRITICAL CARE UNIT 6 ambulances transport critical and non-critical patients into, within, and out of Cone Health, serving a 150-mile radius. CareLink provides 7,000 transports per year. Although CareLink s primary responsibility is the transport of patients into Cone Health, CareLink routinely provides transport of patients to other facilities. CONE HEALTH MEDICAL GROUP CHMG HeartCare and LeBauer HealthCare Cardiovascular Risk Reduction Clinic 30 physicians and 16 APPs including 3 PharmDs with 8 locations within 3 counties. We service an average of over 600 patients each day within our ambulatory clinics. Pharmacist- managed specialized clinics for management of anticoagulation, hyperlipidemia, hypertension, and antiarrhythmic therapies. HOSPICE AND PALLIATIVE CARE OF GREENSBORO Beacon Place 12-bed facility providing care to hospice patients with any type of diagnosis (heart failure, cancer, HIV); priority admission is given to patients with HIV/AIDS. Beacon Place offers a comfortable, homelike setting for residents. CONE HEALTH MEDCENTERS / OUTPATIENT CARE Kernersville Location High Point Location Nutrition & Diabetes Management Center 40,000 square-foot outpatient facility offers family medicine, OB/GYN, cardiology, gastroenterology, outpatient rehabilitation, imaging, laboratory, behavioral health, Urgent Care, Orthopedics, Oculofacial Surgery and occupational health services. This MedCenter provides 24/7 stand-alone emergency and radiology services to western Guilford County. The center also provides satellite oncology services, primary care and physical therapy rehab services, OB/GYN, Behavioral Health, Carolina Neurosurgery and Spine, Cardiology, Pulmonology, Pharmacy,, Sports Medicine, and Massage Therapy. Also serves patients (indigent, oncology, etc.) discharged/seen at any Cone Health facility. Pharmacies located adjacent to Cone Hospital and Wesley Long Hospital, at Alamance Regional Medical Center Employee Pharmacy, and in MedCenter High Point. COMMUNITY HEALTH CENTERS Community Health and Wellness Center The Cone Health Community Health and Wellness Center provides a medical home for those that are uninsured or underinsured by offering comprehensive primary and behavioral health care services including chronic disease management. The Community Health & Wellness Center uses the medical home model to provide care.
