TransforMED seeks to address the rapidly changing needs of health care throughout the United States and focuses on a transformative practice redesign in primary care organizations. The TransforMED project is based on the concept of a relationship-centered, personal medical home and seeks to provide a more patient-centered, consumer-friendly model of care. The $8 million practice redesign initiative seeks to provide consultation and support to the physicians looking to transform their practices to meet the needs of health care in today s society. As the health care system becomes more consumer-driven, TransforMED, has launched a demonstration project that will test the methods believed to enhance primary health care. This practice redesign project is a realworld test that has potential to change the way primary care is delivered in the United States. Hays Family Medicine was chosen to incorporate a new practice design model, as part of a TransforMED demostration project. Thirty-six family medicine practices across the country were chosen to incorporate the new model as part of the TransforMED demonstration project. Over 500 applications were received from interested practices across the country and evaluated based on the organization s commitment to practice transformation, incorporating all components of the new model. The practice s need for change as well as the willingness to change, also influenced the potential participation per organization. Hays Family Medicine was chosen as one site for the implementation of this demonstration project. Terry McGeeney, MD, MBA, FAAFP, President/CEO TransforMED, explains that the Hays Family Medicine was chosen because of their awareness, interest, and willingness to change, to incorporate the TransforMED method of care. Chosen by the Kansas Academy of Family Physicians as the 2006 Kansas Family Physician of the Year, Moser has been integral in positively influencing health care in rural western Kansas. As a family physician at Greeley County Health Services, Moser embraces numerous qualities that contribute to his ability to provide valuable health care to western Kansas. In a statement nominating Moser for the award, Todd Burch, CEO, Greeley County Health Services states, Dr. Moser is the true embodiment of what all family physicians in rural America should strive to emulate. Moser attended Robert Moser, MD the University of Kansass Medical School, Wichita, Kan., and completed his residency training at Smoky Hill Family Medicine Residency, Salina, Kan. In addition to serving as medical chief at Greeley County Health Services, Moser is medical director for Greeley County s LTC, Hospital lab, Home Health and EMS, and is the district deputy coroner. Being a family physician means having the ability to care and treat a wide range of illnesses and variety of patients. The peers, patients, and community members who nominated Moser would attest that his abilities far exceed those minimum requirements, as he has helped shape health care for rural western Kansans. We congratulate Dr. Moser on his accomplishments and thank him for his contributions to health care in rural Kansas. Welcome! The Kansas Recruitment Center (KRC) was established to assist Kansas rural communities in recruiting and retaining physicians and other health care providers; conversely, KRC assists health care candidates in finding the right practice or career opportunity, and ensuring they receive the support and assistance needed to enjoy their life in Kansas. I am proud to say that KRC has been successful in assisting both communities and candidates in finding the right match. Since January, KRC has assisted in the recruitment of eleven physicians and health care professionals to rural Kansas. I would like to welcome each of them. Neil Goodloe, MD, will begin practice at Labette County Medical Center, Parsons, Kan., this summer. Nancy Rushing, RN, originally from Cedar Vale, Kan., accepted a position with Anthony Hospital, Anthony, Kan., early this spring. Jorge Guillen, MD, family physician, is practicing at the United Methodist Mexican- American Ministries, Dodge City, Kan. Amy Madril, MD, family physician, will practice at Ashley Clinic, Chanute, Kan., this summer after completion of her family practice residency at Wesley Medical Center. Brandi Bowen, ultrasound technologist, began her position with Bob Wilson Memorial Grant County Hospital, Ulysses, Kan., in March. Elizabeth Hineman, MD, will begin her practice at Scott County Hospital, Scott City, Kan., in the fall, after completion of her family medicine residency at Baptist Lutheran in Kansas City, Mo. 3 Nurse-midwifery Increasing health care options in rural Kansas iving in a rural community offers L numerous advantages less traffic, fresh air, and friendly neighbors; but, living in a rural community can also present challenges. Access to resources in health care is one of these challenges. Numerous patients in rural communities are influenced by this challenge each day; including persons receiving treatments for an illness, a patient seeking emergency care, and perhaps even expectant mothers seeking prenatal and birthing care. Each day mothers learn that they will be welcoming a new member to the family. The excitement and nervousness that surrounds this realization can certainly overwhelm any family. With a myriad of decisions to address during pregnancy, delivery and child care, many families are uncertain as to where to begin. In addition, the options for seeking care must be established. Traditionally, choices for prenatal care have been specific to an obstetrician or family physician, but living in a rural community has potential to limit these options. With a lack of health care providers, or physicians who do not provide obstetric care in these areas, families are faced with limited care options. With an increase in certified nurse midwifery care, however, women throughout Kansas, specifically rural communities, may have additional options for seeking health care during pregnancy. Nurse midwifery, while not traditionally a common choice for women, is certainly not a new concept for care. An increased interest in natural childbirth stemmed from the 1970 s, as there was an amplified popularity for individualized care that can oftentimes be provided by a nurse-midwife. A certified nurse-midwife (CNM) can provide a full range of primary health care services to women, including routine gynecological checkups, family planning services, preconception care, prenatal and postpartum care, and delivering babies. Today, most CNMs attend births in hospitals, birth centers, and occasionally in private homes. Common misconceptions regarding the care of a nurse-midwife have been addressed in many states through increased educational requirements and certifications. The common concern that midwives deliver babies only at home and perhaps don t have the training and background necessary in the event of an emergency, has been alleviated through increased training and education. The American College of Nurse-Midwives (ACNM) has addressed the concern by defining criteria for nurse-midwifery to oversee the standards of the field. As a result, a certified nurse midwife must be a highly trained health care provider who has been educated in both nursing and midwifery and is certified by the ACNM. In Kansas, a certified nurse-midwife must complete a master s degree from a pre-accredited or accredited university. A CNM must also possess a state board of nursing license, pass a national certification examination administered by the ACNM Certification Council, and prove continuing competency to practice as an advanced practice nurse, dually recognized by Kansas state boards of nursing. Midwives are an appealing option for women who seek a more individualized, less routine, approach to childbirth than many traditional obstetricians provide. Midwives focus on helping expectant mothers learn about the physical and emotional changes that occur during pregnancy. Specifically, the additional care from a nurse-midwife allows expectant families in rural communities additional options. Kay Garman, FNP, CNM, a certified nurse-midwife in Norton, Kan., states her patients, appreciate the time in which she spends with them during the prenatal and beyond. As a family nurse practitioner in addition to being a certified nurse midwife, Garman also states, I find being a midwife is the best part of my job. 4 Nurse-Midwifery CONTINUED FROM PAGE 3 As a nurse-midwife, Annette VanAnne, FNP, CNM, increases health care options for families in southeast Kansas. For numerous families in rural Kansas, nurse midwifery has become a viable option. Annette VanAnne, FNP, CNM, serves expectant mothers in southeast Kansas. VanAnne has been practicing in the area for nearly a year, and indicates that the idea of utilizing a certified nurse-midwife is becoming more popular. Ginger Breedlove, PhD, CNM, ARNP, FACNM, director, Nurse Midwifery Education Program, University of Kansas School of Nursing, indicates that the popularity has grown [in Kansas] as a direct result of the KU School of Nursing graduate specialty track. To date, 25 students have completed the program of study, states Breedlove. The graduates of the program are now working in many rural and urban areas in Kansas and as small communities continue to be faced with health care challenges, nurse midwives have increased the capabilities of health care to provide further options for women seeking health care in these communities. Breedlove states, Nurse midwives positively impact maternity care in rural communities across the U.S. by providing increased access to cost effective health care that in-turn reduces perinatal mortality related to low birth weight and pre-term birth rates. Adequate prenatal care is an issue for women who are literally hundreds of miles from a qualified provider. With the challenges that continually face rural health care providers, the concept and services surrounding midwifery becomes increasingly valuable. Breedlove states, Just as certified registered nurse anesthetists serve rural settings, CNM s have a significant contribution to make to improve health outcomes of communities. 5 TransforMED CONTINUED FROM PAGE 1 Hays Family Medicine is a rural health care provider in Hays, Kan., and encompasses a staff of 32 health care professionals, including physicians. Richard Rajewski, MD, Hays Family Medicine physician, indicates that the initiative is valuable to not only the 36 practices involved in the project, but health care as a whole. For Hays Family Medicine, the TransforMED initiative has great potential to provide efficient, cost-effective results that will potentially encompass not only family medicine, but medicine in general, states Rajewski. The components of the TransforMED project include several aspects. According to McGeeney, The TransforMED model of care includes such elements as office redesign, access to care, quality and safety, sustainable reimbursement, and advanced information systems. The new model of care involves providing patients with a personal medical home emphasizing a patient-centered care approach, eliminating barriers to access. A team approach to care is also proposed in this new model, utilizing advanced information systems (including a standardized electronic health record); redesigned, more functional offices, and a whole-person orientation that focuses on quality, safety, and care provided in a community context. These practices seek to provide patients a defined realm of services, which will help to clarify patients expectations and understanding of a family medicine practice. McGeeney states that the new models will allow health care organization the ability to implement best practice guidelines, and provide family practice organizations the tools necessary to provide quality care. Participants of the project will engage in the transformative process in two separate groups: 18 practices will implement the model as a facilitated group, the remaining 18 practices will be provided the information to implement the model of care in a self-directed manner. Hays Family Medicine is working in the facilitated group and therefore will received support from TransforMED in the form of tailored implementation strategies, discounted software technology, training, support, and ongoing consultation throughout the project. Evaluation of the successes and failures of the implementation process will be provided throughout the project period. Key findings will be evaluated and disseminated on an ongoing basis among the demonstration sites and within the larger practice community. Once the demonstration project is complete, a more intense evaluation will be reported to examine the project effectiveness in enhancing health care in a primary care setting. While there are challenges that Hays Family Medicine may encounter while implementing the TransforMED model of care, Rajewski indicates that a team approach will empower providers to do more. As with anything, the challenge lies in being profoundly busy. There is risk involved, as it is challenging to remember that the number one priority is in taking care of the patients, all the while implementing new processes in a new model of care. Through the implementation of the TransforMED model of care, however, Rajewski indicates that the model truly has the capacity to give care to everyone who needs care. For Hays Family Medicine, the TransforMED initiative has great potential to provide efficient, cost-effective results that will potentially encompass not only family medicine, but medicine in general. This is truly unchartered territory. AAFP is committed to this effort to transform family practice with limited resources. TransforMED s mission is to lead, empower, and assist family medicine practices as they transition to a patient-centered model of care that offers every patient a medical home within a family physician s practice, states McGeeney. For Rajewski and the staff at Hays Family Medicine, this means a lot. The end result not only promises to make processes simpler and provide benefits to the patients, but has potential to provide benefits to us as health care providers and to our families. The first event will be held at the University of Kansas Medical Center Kansas City, Thursday, October 26, from noon to 3:00 pm, in the Hixson Atrium. Lunch will be provided for those attending. With over 50 percent of Kansans living in rural communities, one might wonder what it is that is so great about living in a small town. Susan Baker, Baldwin City Chamber of Commerce, would say it s how everyone pulls together in times of trial and in celebration! As part of the six-part series highlighting unique attractions in areas throughout the state, with the help of Marci Penner s book entitled, The Kansas Guidebook, this issue will provide a small taste of the sites and sounds of northeast Kansas, a part of the state known for its barbeque, its people, and rich history. To begin the journey, it s interesting to note that Horton, Kan. boasts the first ever power pole in Kansas. The first electric power pole was erected in Horton on November 10, 1937, for the Brown- Atchison Electric Cooperative. It was the first rural electric project to energize Kansas and was financed by a loan from the Rural Electrification Administration. Atchison, Kan., contributes to the history of the country in a different way, as it was home to Amelia Earhart and houses the Amelia Earhart Birthplace Museum. Amelia was born in Atchison on July 24, Despite living in many different cities, Amelia considered Atchison her hometown, having spent more of her life in Atchison than anywhere else. The Amelia Earhart birthplace represents the most tangible remaining link with the famous female aviator. In contribution to the taste of the area, the Holy-Field Vineyard and Winery can be found in Basehor, Kan. The vineyard and winery consists of a 14-acre vineyard, wine production cellar, tasting room, and gift shop. The carefully tended grapes are harvested and pressed, fermented, aged, and bottled in the cellars beneath the winery s tasting room. Visitors are encouraged to visit the small winery and experience the literal taste of Kansas. While Kansas is known for its contribution to agriculture, referred oftentimes as the bread basket of the world, farmers in Whiting, Kan., contribute to agriculture in a different way. A small town with fewer than 210 residents, is home to Schlaegel s Home-Grown Popcorn, a popcorn farm that feeds visitors and residents alike. Located near Manhattan, in Riley County, the Tuttle Creek State Park also offers visitors a broad variety of outdoor recreation and plenty of room to roam. Tuttle Creek Reservoir, the state s second largest impoundment, offers 12,500 acres of water and about 100 miles of rugged, wooded shoreline to explore. A swimming beach, boat ramps, nature trails, a mountain biking trail, and a scenic equestrian trail offer explorers a variety of routes to experience the aesthetic Flint Hills environment. Scenic picnic areas, an 18-hole disc golf course, volleyball courts, horse shoe pits, and facilities accommodate park visitors. If seeking something unique, Bonner Springs, Kan., is home to the Moon Marble Company. The store is located near the intersection of K-32 & Hwy 7 in Bonner Springs and stocks machine-made marbles in a multitude of colors and designs. In addition, artisan and owner Bruce Breslow frequently demonstrates his marble-making abilities. The Moon Marble Company also exhibits and sells the work of several dozen prominent glassworkers from across the country. The northeast corner of Kansas has a lot to offer travelers. Even if a visitor is merely passing through to seek another destination, the Flint Hills Scenic Byway provides a taste of the beauty and nature found in Kansas. The Flint Hills Scenic Byway stretches 47.2 miles across the Flint Hills of Kansas on K-177 between Council Grove and Cassoday. The byway is a two-lane, paved road easily reached from the Kansas Turnpike, I-70, U.S. 50 and U.S. 56. The drive features the panoramic views of the tallgrass prairie, ranches, and even dips down into the creekside to follow magnificent stone walls around farmsteads still in operation. Historic sites along the way include the city of Council Grove, home to the Kaw Mission State Historic Site and Museum, Council Oak, Santa Fe Trail, Old Cowboy Jail, Post Office Oak and Museum, KATY Depot, Bonner Springs, Kan., is home to the Moon Marble Company, where visitors can watch the process of glass-working during demostrations by the owner. Hays House, and the Last Chance Store. Marci Penner s book, The Kansas Guidebook, explains that Council Grove has been designated a National Historic Landmark because of its significance to the Santa Fe Trail. She states, in 1860 alone, 3,000 wagons, 7,000 travelers, and 60,000 mules passed through the city. Council Grove, Kan., boasts numerous tourist attractions, and listed on the town s Web site are the top ten reasons visitors should come to the area: shade, lakes, music, food, development, river walk, easy access, central location, friendly people; to learn the best reason to come to Council Grove, however, the city urges visitors to come find out for themselves. With numerous sights and sounds located in northeast Kansas, finding the best reason to visit isn t a bad idea. Northeast Kansas has a lot to offer a visitor, but even more to offer a resident. Emma Boltz, Sabetha Chamber of Commerce, would say there are several reasons to live in a rural community. In a small community people know each other, support each other, and come together in times of need. People are here for each other, states Boltz. 15

The Hays Kiwanis Club has assisted the Area Health Education Centers (AHEC) in helping to alleviate the impact increased cost has had on the health care delivery in Hays, Kan. The Kiwanis Club provided an opportunity that would allow the AHEC Northwest clinic to purchase a vital signs monitor allowing staff members to gather additional information Lucas Stouffer helps demonstrate how the Welch Allyn Vital Signs Monitor works. from the patient, accurately and effectively. The Welch Allyn Vital Signs Monitor, which will be purchased with the donation from the Hays Kiwanis Club, has the capability to monitor blood pressure, pulse, blood oxygen levels, and temperature. The club s donation of approximately $3,200 to purchase the equipment will assist in the clinic s ability to be more consistent and provide further information about the patient. Ruby Jane Davis, director, Northwest AHEC, indicated that the AHEC clinic has always made do without such advanced technological equipment, however, the purchase of this equipment will enhance the assessment of the patients. The previous equipment being used consisted of a manual blood pressure cuff, which did not provide temperatures and oxygen saturation levels. The vital signs machine will enhance the care provided through the consultation clinics, as Davis states, with prompt and accurate readings of blood pressure, pulse, temperature, and oximetery. The AHEC Northwest, Hays, Kan., provides consultation clinics to many low income children and families in Ellis County. In an effort to bring additional care to patients, rather than forcing them to drive out of town, many specialists from the University of Kansas Medical Center travel to Hays to provide specialized care. These clinics include: pediatric cardiology, pediatric behavior, pediatric endocrine, pediatric rheumatology, and orthopedic. The Hays specialty clinics see more than 400 patients each year. The Kiwanis Club s gift will definitely make a difference. Davis concludes by saying, We appreciate the gift of this equipment from the Kiwanis club. It will certainly enhance our Hays clinics, in assuring that we have accurate and consistent readings. We are most grateful for this gift. 17 Warren provides leadership as statewide AHECs director The Area Health Education Centers (AHECs) have been integral in addressing the state s health and health work force needs by providing educational programs and health care services throughout the state. The organization s contribution has changed the face of health care in Kansas and will continue to do so through the leadership of Mary Beth Warren, RN, BSN, MS, as she has been named the AHECs statewide director. Through Warren s dedication and desire to serve Kansas, she has been vital to the success of the organization. Dave Cook, PhD, assistant vice chancellor of External Affairs, states, Mary Beth is dedicated to the mission of the AHECs in enhancing the availability and quality of health care in rural Kansas. As a result, I am excited to see how her capabilities will influence the AHECs organization further as she serves in this capacity. Warren has served rural Kansas in some facet for most of her life. Born and raised in southeast Kansas, she has experienced, first hand, the challenges associated with receiving quality health care in a rural community. Warren completed her Bachelor of Science degree in nursing from Pittsburg State University, a Master of Science in adult education from Kansas State University, and contributed to health care in two separate rural health care organizations for over 15 years. This experience has provided Warren a firm understanding of the needs rural communities face in regards to health care. I learned, through experience, how important a strong health care system is to the overall wellbeing of rural communities. I experienced the rewards, challenges, and struggles of providing health care in these communities, states Warren. As a member of the AHEC team since 1994, Warren has seen changes occur within the AHECs, similar to those within the health care system as a whole. As with most organizations, the AHECs operate with a limited budget. This factor is a priority for Warren, as it equates financial stability for services provided through the AHECs organization. An additional priority as director is to continue assessment of opportunities, to consolidate functions and streamline processes. As interim director for the previous 18 months, Warren has begun the process of centralizing financial management, and capturing usable information on the services, partners, and customers utilizing the AHECs programs. Warren has amplified her focus within the AHECs to encourage success and increase capabilities of programs and services provided. Her anticipated evaluation of the AHECs programs include examining the most important functions and services provided through the Mary Beth is dedicated to the mission of the AHECs in enhancing the availability and quality of health care in rural Kansas. As a result, I am excited to see how her capabilities will influence the AHECs organization further as she serves in this capacity. Terri Schmitz, RN, BSN, continuing nurse education coordinator, states this about Warren s leadership: I have always found Mary Beth to be a supportive leader. She allows her staff to grow right along with her and the organization. Her vision is far reaching and she works diligently to promote the health care of all Kansans. 18 Millie Meck of Girard, Kan., says the Kansas Audio-Reader is a godsend. 18 Kansas c o n n e c t i o n s Audio-Reader Rural Kansans benefit from radio reading services The average morning routine can oftentimes include a cup of coffee and the local newspaper; this is true of someone living in an urban or rural setting. The local newspaper keeps residents informed of the community events and happenings and assists in maintaining the culture of a community. The morning routine for a person with a visual disability may be somewhat altered according to these activities, but with assistance from the Kansas Audio-Reader, blind, visually impaired, and print disabled individuals in Kansas have the ability to engage in the local newspaper and have the opportunity to stay connected to the community in which they are involved. The University of Kansas Audio- Reader Network is a reading and information service for blind, visually impaired, and print disabled individuals in Kansas and western Missouri. Any person whose eyesight or physical condition makes it difficult or impossible to read a newspaper or book may receive the broadcasts. There is no charge for service. Established in 1971, Audio-Reader was one of the first radio reading services in the world. Audio-Reader s original program is a radio reading service for anyone who has difficulty reading standard printed material. Broadcasting 24 hours a day, Audio-Reader uses a large pool of volunteers to read daily and weekly newspapers, magazines and books, as well as news, interviews and other programs dealing with disability and aging issues. Audio-Reader is generally broadcast on the subcarrier of an FM radio station. This means a special radio is required in the home. The radio is provided by Audio-Reader, on loan, for as long as it is wanted. In some locations Audio- Reader may only be available through the local cable company. In those cases, Audio-Reader will provide a special radio if one is required, but the listener will have to subscribe and pay for basic cable service. Listeners receive a monthly program guide in large print or Braille. Today, with the help of several media outlets reading by radio is available to thousands of people all across Kansas and western Missouri, with some programming available via satellite through other radio reading services across the United States. The service has proven to be an invaluable tool for those utilizing it. For Millie Meck the program is a godsend. Meck is a resident of Girard, Kan., and has used the services of Audio-Reader since At age 88, Meck is mostly home-bound and relies on the help of someone to get her out and about. She gets excited about the service, because it has something for everyone. She listens to Audio-Reader mostly for the news, but admits she enjoys reading/listening to a good book every once in a while. Meck is an advocate for the program and depends on the Audio-Reader to get her the local news. She listens to the Wichita Eagle and states, it s wonderful having the Pittsburg news as well. The Kansas Audio-Reader serves approximately 3,500 registered listeners in Kansas and Missouri. Kansas Audio-Reader covers many rural areas in northeast Kansas, including the area between Topeka and Kansas City; a large portion of central Kansas between Hays and Salina; and out into western Kansas, including Hill City, Wakeeney, Garden City, Satanta, and numerous towns in between. Areas not covered by the radio reader are covered through internet broadcasts and special request (cassette) recording services. Diana Frederick, Development Director, University of Kansas Audio-Reader Network states, Our regional newspaper broadcasts, featuring local news articles from dozens of small-town Kansas newspapers, are especially popular with rural listeners. The Audio-Reader service provides an option for print-disabled and visually impaired residents to engage in media outlets they may

19 not otherwise engage in without the service. The service is invaluable to rural communities in several different ways, states Jim Boyd, University of Kansas Audio-Reader, Telephone Reader Supervisor/Listener Liaison. For one, we broadcast all the local newspapers we can get a hold of in Kansas and western Missouri, from the Holton Recorder to the Tonganoxie Mirror to the Russell County News and all points in between. So it s very likely that rural listeners can get to listen to their hometown newspaper, which puts them in touch with what s going on in their community. If the local paper is not broadcasted, the Kansas Audio-Reader will take requests, and often have a volunteer record the newspaper and send it to the listener on cassette. On the other side, Audio-Reader is good for rural communities in that it brings the wide world of books, national newspapers, and magazines directly to our listeners. For most of our listeners, Audio-Reader is their one of their only links with the outside world, and in terms of the printed word, often the only link. Audio-Reader fills that gap for them, providing entertainment, news and information, explains Boyd. The service not only provides the traditional newspaper content for listeners, but content such as local grocery advertisements, department and discount store ads, catalogs, and other information is read to the listeners as well, so that they might use it right away. For print disabled residents in south central Kansas, the Wichita Radio Reading Service Audio-Reader is good for rural communities in that it brings the wide world of books, national newspapers, and magazines directly to our listeners. For most of our listeners, Audio-Reader is one of their only links with the outside world, and in terms of the printed word, often the only link. Jim Boyd, University of Kansas Audio-Reader, Telephone Reader Supervisor/Listener Liaison (WRRS) is also available. The WRRS currently broadcasts on a 60 mile radius of Wichita, and has listeners in the area smaller towns and communities within this area. WRRS has been working in the southern Kansas region for 30 years and provides each listener with a pre-set closed-circuit receiver tuned to readings which are broadcast 24 hours a day, 7 days a week. Volunteers read several publications for listeners of WRRS, including the Wichita Eagle, USA Today, magazines, novels and other materials. While this service covers a smaller region of the state, it also services approximately 3,500 listeners. WRRS is broadcast as a sub-carrier of the public radio station operated by Wichita State University. Kansans with a print disability interested in receiving the Kansas Audio-Reader service can request an application to be sent through the mail.
