1 VSPAN Virginia Society of PeriAnesthesia Nurses Serving the PeriAnesthesia Nurses of Virginia Volume XXX, Issue I, Spring 2015 Spring is here and with that being said, I hope that everyone had an opportunity to spend time with their families and friends during the Holiday season. I would like to use the spring edition of the as a time to reflect. The Miriam Webster dictionary defines reflection as the consideration of some subject matter, idea or purpose. As Peri -Anesthesia nurses, I would like for you to reflect on all the accomplishments you have done personally in your profession, reflect on the accomplishments you have done in your work area and reflect on what you have done to promote yourself as a Peri- Anesthesia nurse. As we begin the New Year, I have been able to reflect on all the great many things you are doing personally and professionally in your components and at work. VSPAN is so grateful to have so many wonderful members that are doing such great things in their Peri-Anesthesia areas. As we all know, Peri- Anesthesia Nurse Awareness Week (PANAW) week is observed during the first week of February (February 2-8, 2015). This years theme for PANAW week was, Peri- Anesthesia Nurses: A Vital Role in Patient Care. This week was an opportunity to recognize the extraordinary work that Peri- Anesthesia nurses perform around the country throughout the year. This includes caring for PRESIDENT S MESSAGE patients before and after procedures requiring anesthesia, sedation, and analgesia. PANAW week was an opportunity to educate nursing colleagues and the community about the professionalism and passionate care that we deliver daily to our patients. ASPAN s mission is to produce evidence-based research, promote education, optimize patient care, establish national standards, and serve as an advocate for peri-anesthesia nurses. As PANAW week ended, it was my opportunity to reflect on the many tasks that we have completed. No matter how insignificant it may have seemed to you, it was significant to someone else and I am sure that it was greatly appreciated. I hope you and your team had a great week to celebrate all of the good work that you do daily and throughout the year. In closing, I would like for you to please reflect on the simple smile you gave to someone the warm hand you held or by helping your co-worker with a complicated patient YOU made a difference. As a Peri-Anesthesia nurse, I would like to challenge you to use 2015, as a new chapter in your professional career and reflect on completing another level of education, become CAPA or CPAN certified, develop a quality improvement project for your facility or write an article or book, participate in your legislation or join VSPAN. WOW! A tremendous amount of the care that was once performed in the hospital by hospital staff is now accomplished by the patient, family or other caregivers in the home. Although hospitals have made tremendous strides in patient care during the surgical process, patients who are noncompliant in their care can derail surgical outcomes. Not only does this affect the patient s recovery, but it also can have financial implications for the hospital in lost reimbursements related to pay for performance. Part of the PeriAnesthesia nurse s role is teaching postsurgical self-care to the patient and significant others. Engaging the patient and care team is critical to post-operative success. A patient who is not educated or engaged may not properly care for their surgical site or achieve good control of their blood sugar making them at greater risk for a surgical site infection or other complications. Of equal importance is their understanding of the relationships of smoking cessation to surgical wound healing, as well as ambulation to prevent a deep vein thrombosis. Fischer et al. (2010) noted in a large study of newly prescribed e -prescriptions that a mere 72% were filled, suggesting that the rate of those patients taking the medication correctly was even lower. Certainly the compliance with instructions hurriedly given to patients as they leave the hospital after anesthesia would further diminish potential compliance with post-surgical medication needs. 2 New methods are needed to help educate and engage patients and caregivers. Some hospitals use videos, educational materials or classes to support post-surgical learning needs, but there is much opportunity for study to determine the most effective approaches. Phone calls can prove useful for follow up to make sure the patient has the needed equipment and medications and is following the plan of care. Many primary care physicians are scheduling follow -up visits if their patient has been hospitalized or had surgery to insure their patients are on the right track. There is a positive impact on patient outcomes created when surgical staff partner with the patient and caregivers to prepare for home management. Barriers such as poor support systems for the patient and lack of commitment need to be addressed. One surgeon suggested that in her facility, elective surgery will not be done until the patient stops smoking for at least thirty days. This type of engagement should be our goal. 3 National Conference For those of you that are thinking about or attending the National Conference on April 26-30, 2015 in beautiful San Antonio, TX, we would like to encourage your participation in these fun activities: Component night: Boogie back to Texas! Sunday, April 26, 2105 at 7 p.m. The Dream Walk is Sunday, April 26, 2015 at 6:30 a.m. This year they are calling the walk Treasure Quest. This is a treasure hunt that you will need to solve the clues that will lead you to the finish line or the treasure! There is a $25.00 registration fee. The monies that are raised from the Dream Walk are used to sponsor scholarships, nursing recognition awards, and materials for education and public awareness through the ASPAN Foundation. Roanoke Team s First Mission Trip to Guatemala was a Success! I was fortunate and very blessed to receive the Humanitarian Scholarship Award from ASPAN. I lead a small team in January to Chocola, Guatemala, it was our first mission to work at Hospital Santa Fe de Cristo. The surgeon we worked with (Dr Sergio Castillo) started in Guatemala City, was asked to do a medical mission in the village of Chocola, Guatemala, fell in love with the area, in 1998 rented a 3 cinder block building which later became The Hospital Santa Fe de Cristo. We assisted the local surgeons by doing Pre-op-Intra Op and Post Op. I was fortunate enough to work in all aspects of nursing for the patients we saw. I worked in the Pre-op area, assisted with the help of a translator, to get patients medical history, and asked questions for the anesthesiologist regarding anesthesia history, starting IV's, and inserting foley catheters. We had a team of 3 RN's Lisa Hunt RN, Anita Gooding RN, from Pre Surgical Testing at Carilion Clinic members of ASPAN/ VSPAN, and Nancy LeHardy from Roanoke Ambulatory Surgery Center in Roanoke. 1 NP-Lorrie Saville from Carilion Clinic Hospitalist Department, 1 CRNA-Lois Stewart from Blue Ridge Surgery Center in Roanoke, and Virginia Commonwealth University. 1 PA Evie Alhfield from Martha Jefferson Hospital in Charlottesville, Va, Mark Parrish-medical technician at University of Virginia, and our driver and translators Adolfo Santizo, and Daniel Dominguez. Dr. Sergio see's over 8000 patients per year, and more than 350 major surgeries, C-Sections, Cholecystectomies, Appendectomies, Hernia Repair, Hysterectomies, Tubal Ligation, Prostatectomy s, and Kidney stone removal. Surgery has spared many women from cervical cancer, which is common and often fatal. Patients are also hospitalized for medical illnesses. If not for Dr Sergio these patients would not have affordable medical care. We have agreed to return in February of 2016 with a full surgical team to run our own Operating Room, while also assisting Dr Sergio. The scholarship I received was also instrumental in helping an operating room nurse go to Guatemala. With her being in charge of the OR, and teaching us how to assist, our surgical mission was a huge success, with many successful surgeries. 4 REQUEST FOR VSPAN NURSES: NURSES WEEK IS MAY 6 TH -MAY 12 TH As part of nurse s week I would like to ENCOURAGE, and challenge all VSPAN chapters to make donations to their local food banks. As a member of the board of VSPAN and working in the mission s field, the need in our community and across the state are at all time highs. Our local food bank: Feeding America Southwest Virginia is working very hard to feed Southwest Virginia s hungry through a network of partners and engage our region in the fight to end hunger. Feeding America Southwest Virginia has served the 26 counties and associated municipalities that make up Southwest Virginia for over three decades. As the leader in hunger relief in this region, FASWVA distributes more than 20 million pounds of food annually to nearly 400 partner agency soup kitchens, rescue missions and food pantries. 5 Volume XXX, Issue I, Spring 2015 Celebrating PANAW Week Perianesthesia Week was celebrated in Presurgical Testing CMC Carilion Clinic. Julie Jackson, Debby Hodges, and Cheryl Bober did an outstanding job of planning educational and fun activities. We had a cross word puzzle on perianesthesia terminology, movie trivia of famous movie quotes, a potluck celebration, a breakfast treat, daily door prize drawings, and an educational session on EKG updates presented by Courtney Peery. We enjoyed the moments of fun and refreshing out knowledge of Perianesthesia Nursing. Brenda Gilliam MSN, MHA, RN, CAPA The PACU staff at Sentara Norfolk General celebrated PANAW with a special lunch and cake catered by their manager Sharon Ward, RN BSN CPAN. Carolyn Tucker, RN, BSN The CVOR Pre-op/PACU at INOVA Fairfax Medical Campus celebrated with balloons, posters, pen give away's and brownies. Sally Schermer, RN, BSN, MBA Our Mission The Society exists to promote quality care for patients and their families, by encouraging professional education, development of standards, research, certification, and specialization in all phases of perianesthesia nursing, and by providing a forum for exchange of ideas between health care providers involved in perianesthesia care.

6 Winter is a busy time of the year in Richmond. The General Assembly was in session and the Virginia Nurses Association (VNA) held their annual Legislative Day. During the General Assembly, the legislators completed a relatively calm session, ending a day early. This was aided by revenue projections being better than expected in December, allowing more funds to be available for the biennial budget. This lead to salary increases for state and public university and college employees and more money for Health and Human Resources. Accomplishments out of the General Assembly for nursing and healthcare include: - Modified Good Samaritan 911 bill passed-bill allows affirmative defense for individuals calling for emergency assistance for an individual with suspected overdose of drugs or alcohol. - Minors will no longer be allowed to purchase nicotine vapor products - Virginia Alliance on Medication Access and Affordability backed measures to decrease the cost of specialty tier medications and expand coverage for applied behavioral analysis for treatment of autism to the age of 10 years. - HB 1435 allows physician assistants and nurse practitioners to be appointed as Medical Examiners. Government Affairs Update - HB 1476 provides for nurse practitioners to be added to definition of health care provider in medical malpractice statutes for the purpose of providing expert witness testimony. An opportunity missed was Medicaid expansion. The General Assembly did approve to expand the Governor s Access Program to provide for outpatient mental health and medical benefits for uninsured adults between 21 and 64 with mental illness. Also dental services will be provided for pregnant women receiving Medicaid. The cost of these two programs for July 1, 2015-June 30, 2016 is estimated at 200 million dollars. With Medicaid Expansion under the Affordable Care Act (ACA), Virignia would have been eligible for close to 2 billion dollars in FY 16. The last day for action by the Governor for all the bills submitted is March 18, The April session of the General Assembly will begin April 15, 2015 to address any gubernatorial actions. The Virginia Nurses Association (VNA) held its annual Legislative Day Amplify the Voice of Nursing on February 3, 2015 at the downtown Richmond Marriott. This event was well attended by a variety of nurses and nursing students from across the state. Opportunities were provided for touring the state capital and time to visit with their representatives. Attendees also had the opportunity to view and meet with nurses sharing their best practices via over 40 poster presentations. The speakers for the day included local, state and nursing representatives. Dr. Jennifer Lee, 6 Deputy Secretary of Health and Human Resources for the Commonwealth of Virginia spoke about Medicaid expansion. She also shared tips for nurses serving as advocates. James Pickeral, VNA lobbyist provided legislative updates and Kevin Shimp, MSN shared basic advocacy and lobbying information. The final morning session was Janet Haebler, MSN, Associate Director, State Government Affairs for the American Nurses Association talking about the top health issues for the 2015 Congress. Kathy Baker PhD, Co-Lead for the Access to Care Workgroup of the Virginia Action Coalition (VAC) and Patricia Selig, PhD, member of Access to Care Workgroup for the VAC spoke about the barriers to practice for Virginia Nurses. There are many opportunities for nurses to engage in the legislative process, so please find what motivates you, research the facts and share your thoughts with your legislators.
