ACTION ITEMS Apprval f Minutes The minutes fr the January 21, 2016 were mved and apprved unanimusly by the bard first by Jack Rgers and secnded by Paul Raffer. Henry Tuttle and Philip Deming abstained. Adjustments t minutes: Alternate Jenifer Mendel discussed P.E.R.T. and a questin was asked by HSAB member abut Federal Funding and answer was negative. Recmmended t pursue as a demnstratin prject.

Henry Tuttle prvided in a handut frm the Cuncil f Cmmunity Clinics Member Health Centers n Oral Health Services Bard member Bb Prath ntified that Brenda Schmitthenner with Aging & Independence Services (AIS) is retiring. Kristen Smith is replacing Brenda and she will be prviding the LTCIP updates beginning in April. The Cunty f San Dieg decided that the cst f Medi-Cal wuld escalate and the Bard f Supervisr asked t submit three ptins: 1) Netwrk f Care-manage yur wn care. 2) Team San Dieg- 3) Crdinated Care Initiative-nt successful and were tld by Sacrament hw t handle. Brain Health Strategy BL will be ging t the bard n March 1, 2016 fr apprval. Public hearings will cnduct n H-Friendly (Health-Friendly) Initiative thrughut the Cunty f San Dieg which is a five year plan. This is a livable cmmunity fr all ages. Bard member Philip Deming prvided infrmatin frm Behavir Health Advisry Bard and nted that the Mental Health Advisry and Drug and Alchl Advisry Bard cmbined in They are wrking n integrating resurces and strengthening ur effrts in verall behaviral health care system. Behaviral Health is currently reviewing suicide preventin and recmmendatins thru wrkgrups.. Presentatins: Cnnect Well San Dieg- Carrie Hff Carrie Hff prvided a pwer pint presentatin n CnnectWellSD and explred the lng-term plan t advance the health and verall well-being f the regin. Visin: Enrich the lives f individuals and

Cnnect Well t Live Well: Persn-Centered Service Perspective Infrmatin Hub CnnectWellSD will bring tgether infrmatin frm acrss the Cunty: Aging & Independence Services, Alchl & Drug Services, Benefits Eligibility, Child Welfare Services, Husing, Mental Health, Prbatin, and Public Health. CnnctWellSD Prgram is tw flded; the Persn- Centered Service Strategy and the CnnectWellSD Technlgy. The Persn-Centered Service Strategy wrks with custmers t identify needs, preferences, gals, strengths, and pririties, cnnects individuals with resurces, and cllabrates acrss disciplines t empwer psitive custmer utcmes The CnnectWellSD Technlgy prvides infrmatin hub t pull data and a mre detailed view t each individual served, plus a secure platfrm fr multi-disciplinary teams and functinal Capabilities CnnectWellSD will implement new technlgies and plicies t enable autmated r imprved abilities t perfrm cllabrative service delivery thrugh referral management system, ntificatins and Alerts, and ppulatin-based shared analytics Access is cntrlled by a three-dimensinal mdel, based n each individual user and the type f data, n matter where it cmes frm. CnnectWellSD will allw a wrker t see infrmatin frm multiple prgram databases as needed and help them prvide better custmer service. But, staff can nly see what they allwed t see, depending n their rle, laws, and custmer apprval. CnnectWellSD Benefits Imprved custmer experience thrugh mre tailred interactins. Mre detailed view f custmers, t help with servicing multiple needs. Easier access t custmer infrmatin. Custmer is part f their wn team,

We are rlling ut CnnectWellSD in stages. System data, functinality, and staff participatin will increase ver time. Phase One Rll Out: users User rles define data access. Limited functinality. Ptential new abilities: Q & A: Judith Yates: Hw will be cnnectivity between CnnectWellSD and Health Infrmatin Exchange? Carrie Hff: With permissin with Health Infrmatin Exchange (HIE), CnnectWellSD wuld like t d a hub t hub cnnectin. We have been wrking with Dan Chavez s staff which data fields they are using t identify their master persn index. Dr. Krnrich: Hw many custmers d yu anticipate in engaging in the first year? Carrie Hff: We currently have ver 1 millin clients in the system. The system will accmmdate an accunt fr every resident in the Cunty. Greg Knll prvided a PwerPint presentatin n Medi-Cal Managed Healthcare in San Dieg Cunty. The presentatin supplemented with a handut that included an rganizatinal structure f the Healthy San Dieg and the missin and visin f the rganizatin. In regards t Healthy San Dieg peratins, as f August 2015, there are mre than 700,000 Medi- Cal beneficiaries in San Dieg Cunty and 89% enrlled in a Medi-Cal managed care plan (nly 54% in 2011). Overall managed care beneficiary increase > 35% in past year.
