2 UCSF Transplant 2014: Rid OVERVIEW This two-day conference has been coordinated through the University of California San Francisco, Division of Transplant Surgery and the Heart & Lung Transplant Program. This program is designed to provide a unique opportunity to hear UCSF Transplant experts present a comprehensive review and update on recent advances in adult and pediatric transplantation. Leaders in the field will share their medical and surgical expertise in solid organ transplant and living donor topics. Plenary sessions include the future of transplantation, the impact of allocation changes and the Affordable Care Act on transplantation, immunosuppression withdrawal, and other contemporary and controversial topics. Organ specific clinical tracks will focus on current issues and patient management strategies appropriate for each area of specialization. TARGET AUDIENCE The target audience is a broad cross-section of health care professionals who care for pre- and post-transplant patients. Heart, lung, liver, kidney, pancreas, and small bowel transplantation will be discussed. The symposium content is designed for community/referring physicians, transplant nurses, coordinators, administrators, nurses, advanced practice nurses, pharmacists, social workers, and all members of the health care team who are interested in improving their clinical practice or patient management during transplantation. The course will: Illustrate and examine the impact of transplantation on the lives of patients, their families and the donor families for the community health provider. Examine the latest developments in transplant management to optimize patient outcomes. Provide a forum for current trends and opinions in transplantation that can be readily applied to their practice. LEARNER OBJECTIVES At the end of this symposium, the participant will be able to: Adopt new informational technologies to improve communication between health care providers and patients. Integrate into the community of practice and determine appropriate pathways for patient care. Differentiate and design individual education plans for patients. ACCREDITATION The University of California, San Francisco School of Medicine (UCSF) is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. UCSF designates this live activity for a maximum of AMA PRA Category 1 Credit(s) TM. Physician should claim only the credit commensurate with the extent of their participation in the activity. This CME activity meets the requirements under California Assembly Bill 1195, continuing education and cultural and linguistic competency. Nursing For the purpose of recertification, the American Nurses Credentialing Center accepts AMA PRA Category 1 Credit TM issued by organizations accredited by the ACCME. Licensed Clinical Social Workers and Marriage and Family Therapists This course meets the qualifications on an hour-for-hour basis of continuing education credit for MFTs and/ or LCSWs as required by the California Board of Behavioral Sciences.
