2 Psitin Overview The psitin is respnsible fr individual supprt in a safe and secure residential envirnment fr peple wh have experienced trauma, have multiple cmplex needs and/r a diagnsed mental health issue r disability. The psitin is respnsible fr daily supprt, crisis interventin, infrmatin prvisin, case ntes, reprt writing, prgram develpment, structured daily activities, administering f medicatin, husehld duties alng with a range f persnal develpment prgrams supprting peple t assist with their cnnectin back int the cmmunity. Service Delivery Cntext The peple wh will be targeted by this service will require intensive supprt packages tailred t: Prmting the capacity f the client/participant t maximise their cntrl ver their envirnment Enable the client/participant t live ptimally within a supprtive cmmunity Facilitate the develpment f supprtive and respnsive cmmunities Facilitate cnstructive links with the range f cmmunity services and resurces. The brad bjectives f Ermha 360 (Barwn Services) are: T assist peple we supprt t acquire/secure apprpriate accmmdatin and supprt in the cmmunity, T prvide intensive supprt t enable the peple we supprt t maintain their accmmdatin, minimise harm t themselves and thers, achieve their stated gals and address their psychscial, emtinal and physical needs, T ensure the prgram wrks cperatively with ther service prviders t imprve access t apprpriate, relevant and timely services, T prvide cntinuity f care and supprt, T prvide an apprpriate rganisatinal envirnment including staff supprt, develpment, training and supervisin. Practice Principles The prgrams endrse the key wrker mdel, where the develpment f a trusting relatinship is seen as pivtal t the change prcess, and cmplementary t the rles f advcacy and supprt. Prgrams als endrse the Prgrammatic r Team apprach t direct service delivery, where a high degree f cllabratin amngst team members will cntribute t the assessment f client need and the design, review and delivery f packages f supprt. 3 The prgrams aim t best match cmmunity resurces t the peple we supprt s needs. Service prvisin is flexible and recgnises that individual needs change ver time. The prgrams have a cmmitment t enhance the lng-term health and wellbeing f peple we supprt and recgnise that peple may need supprt f varying intensity and duratin. The prgram aims t be respnsive t this need. The prgram recgnises that due t cnstant r recurring crisis, peple develp cping strategies, which may inhibit the attainment f a stable lifestyle. Ermha seeks t validate these experiences and encurages mre sustainable and effective slutins t achieve independent living, thrugh the implementatin f an Individual Prgram Plan (IPP) develped fr and with each cnsumer. ERMHA is cmmitted t the principal f empwerment via the practice f enabling. Key Respnsibility Areas T undertake and maintain a direct service caselad as determined by Prgram Management. T prvide intensive supprt t: Prmte the capacity f the peple we supprt t maximise their cntrl ver their envirnment. Enable the peple we supprt t live ptimally within a supprtive cmmunity Facilitate the develpment f supprtive and respnsive cmmunities, and Facilitate cnstructive links with the range f cmmunity services and resurces. T cntribute t the delivery f services which facilitate the develpment and/r rehabilitatin f peple and their cnnectin int the cmmunity. T assist with practical living skills including meal preparatin and planning, laundry and persnal hygiene and administer medicatin. T maintain an apprpriate level f safety within their residence fr bth staff and the peple we supprt. T assist with the planning and implementatin f a range f structured daily & weekly prgrams and activities t assist with cnnecting back int the cmmunity. T develp and maintain effective prfessinal wrking relatinships with peple wh may exhibit challenging behaviural traits, t help supprt and maintain psitive change. Act as an advcate and mediatr with ther relevant services and prfessinals in regard t the needs f the peple we supprt. Page 3 f 8

4 Adhere t bth internal and external legislative requirements and prtcls in relatinship t referral, assessment and direct care tasks. T establish and maintain a strng wrking relatinship with family, significant thers, cmmunity supprt ptins apprpriate t the peple that we supprt needs and participate n cmmittees and in wrk grups which are relevant t the wrk f the prgram. Planning and Reprting T ffer prfessinal supprt t targeted peple that we supprt within a case management framewrk including assessment & referral, establishment and implementatin f cmmunity supprt and input int the develpment and implementatin f Individual Prgram Plans (IPP). Participate in nging prgram evaluatin and refinement and cntribute t the strategic planning prcess f the prgram. Fllw and adhere t DHS incident reprting and quality f care guidelines and ensure that internal incident reprts are cmpleted and submitted t the relevant cntacts within the stipulated time frames. Prepare thughtful and well written reprts t the Team Leader and/r Prgram Manager r Directr reflecting achievements, challenges, exceptins and actins. Team Wrk Prvide input t the team s wrk and ther Ermha Prgrams actively participating in the maintenance f a cllabrative and harmnius prfessinal envirnment, and perate in accrdance with the aims, bjectives, philsphy and plicies f Ermha. Maintain case management recrding systems including case planning dcumentatin, case ntes, reprts and data cllectin in accrdance with Ermha prcedure, plicies and data recrding requirements. Ensure all wrk undertaken cmplies with Ermha s values, plicies, prcedures, cdes f cnduct and legislative/regulatry requirements and recgnised accreditatin standards, with particular reference t risk management and duty f care. Engage in frmal supervisin and reflective practice. Develp a Prfessinal Develpment Plan with yur supervisr and be practive abut btaining the bjectives described within it. Page 4 f 8

5 Attend any training, curses r seminars relevant t the rle and/r prgram requirements in cnsultatin with yur supervisr. Relatinships: Develp and maintain strng prfessinal relatinships with key peple/rganisatins, including family members, relevant service prviders and agencies and ther Ermha staff. T maintain a gd wrking knwledge f and relatinships with, external cmmunity supprt ptins apprpriate fr yung persn s needs. Represent the rganisatin in a psitive and prfessinal manner when liaising with the cmmunity. The cmmunity includes peple wh access the service, ther service prviders, key stake hlders and the lcal cmmunity. Occupatinal Health & Safety: T undertake ther duties as directed by the Practice/Team Leader, Prgram Manager r CEO, and maintain a clean and safe wrk envirnment cmplying with Ermha s OH& S plicies and prcedures. Ensure that safe wrking prcesses are undertaken at all times. Identify and reprt all hazards t yur supervisr and/r OH&S Representative. Key Selectin Criteria 1. A certificate IV in Disability r Mental Health r higher level tertiary qualificatin with relevant wrk experience. Or relevant experience in the field f adult and/r yuth wrk and the justice system and/r wrking with thse wh are disadvantaged in the cmmunity with particular reference t peple with cmplex care needs. 2. Demnstrated awareness f the full range f issues experienced by peple we supprt living with significant disability as a result f a severe mental illness, tgether with high level practice skill in the delivery f apprpriate services, strategies and interventins is desirable. 3. An understanding f the Trauma Infrmed Care framewrk and experience in brad case management practices wrking frm a recvery fcused and yung persn centred apprach. 4. Clear cmmunicatin skills bth written and verbal. The ability t prepare clear, thughtful written reprts, maintain file and data requirements and the capacity t represent the prgram at bth an agency and cmmunity level. Page 5 f 8

Other Relevant Infrmatin All psitins are subject t an annual perfrmance review which will be a dcumented reflectin f perfrmance against Psitin Descriptin and will be cnducted by the direct supervisr. Appintments t psitins are subject t a satisfactry result frm Plice checks, Wrking with Children Checks and cmpletin f the 6 mnth qualifying perid. 7 Emplyment Basis As per Emplyment Agreement. The psitin f Supprt Wrker is established under the terms and cnditins f the SCHADS Award. The successful applicant will be appinted t a Level within the award structure cnsistent with their cmpetencies, qualificatins and experience. Shifts will be rstered n an as required basis. Dependant n the peple that we supprt needs, a shift will cnsist f up t 8 hurs. Shifts lnger than 5 hurs will require an unpaid 30 minute meal break. Shift allwances, weekend penalty rates and public hliday rates will apply. Accuntability: The Casual Supprt Wrker psitin is accuntable t the Ermha Bard thrugh the Chief Executive Officer. This psitin reprts t the Team Leader Ermha 360 (Barwn Services). Why Wrk fr Ermha? Alng with ffices in Dandenng, Beacnsfield, Springvale, Claytn& Clac a scial inclusin Café at Funtain Gate Shpping Centre and 2 pprtunity shps in Nble Park, Ermha is cntinually grwing. Ermha is currently accredited against Department f Human Services Standards, Quality Imprvement Cuncil Standards, Hme & Cmmunity Care Standards. Ermha is cmmitted t further enhancing each emplyee s prfessinal qualificatins and skills by prviding a cmprehensive rientatin fr all new staff, a range f in huse training and external training pprtunities are prvided, prfessinal develpment plans, and regular supervisin are prvided t all staff. With cntinual increases in federal and state gvernment funding, Ermha has expanded in size and last financial year, Ermha prvided services t ver 1200 peple we supprt.
