Essentially yu will wrk clsely with frnt line staff, taking a hlistic apprach t supprt ur participants t gain emplyment thrugh better management and self-management f their physical and/r mental health and wellbeing needs. Abut yu: Yu will need t be registered as ne f the fllwing: A Physitherapist with psych-scial experience, registered with the HCPC An Occupatinal Therapist, registered with HCPC. A cunsellr/psychtherapist registered as an accredited practitiner with the British Assciatin fr Cunselling and Psychtherapy (BACP); An accredited Cgnitive Behaviural Therapist registered with the British Assciatin fr Behaviural and Cgnitive Psychtherapies (BABCP). 3 Main Duties and Respnsibilities 1. T prvide supprt, advice and guidance t frnt line staff t enable them t effectively supprt and guide Wrk and Health Prgramme participants n their jurney t sustainable emplyment. 2. T prvide guidance and supprt relating t nward engagement in lcal health prvisin t meet the needs f participants fllwing Supprt Managers assessment f participants needs. 3. T undertake prfessinal assessments f the health and wellbeing needs f participants with cmplex r multiple needs using agreed dcumentatin and supprt nward engagement in lcal health prvisin t meet the needs f participants. 4. Prvide face t face, telephne and wrkshp based interventins fr participants with multiple and cmplex health and wellbeing needs as required. 5. Participating in 3-way cnference calls with frnt line staff and participants r emplyer t give direct advice and guidance. 6. T rganise, facilitate and if required directly deliver an agreed set f cre and specialist individual interventins t supprt the participant jurney under a bi-psych-scial apprach. This will include: a. Brief slutin fcussed therapy b. Lw level CBT c. Occupatinal perfrmance - Functinal skills - ADL d. Mtivatinal therapy e. Strengths / asset fcussed therapy f. Wellbeing and healthy lifestyles 7. T supprt and develp a grup f lcal prgramme vlunteers fcussed n health and wellbeing activities. 8. T wrk with frnt line staff t embed health and wellbeing appraches and ensure fidelity t ur practice mdel is maintained at all times. 9. Prvide training, caching and supprt fr existing and new staff / vlunteers n an individual and/r grup basis t supprt agreed develpments. 10. T deliver against ur framewrk f reflective practice, supprting frnt line staff t better understand the cmplexities f wellbeing needs participants experience and hw they can better supprt them. 11. T establish prgrammes t deliver effective health and wellbeing supprt wrking as an integrated member f the lcal teams and blending activity with that prvided by ther rganisatins. 12. T develp and maintain effective wrking relatinships with, and an in-depth knwledge f, the lcal Health and Wellbeing sectr t ensure cllabrative wrking and timely participant access t a wide range f services T wrk within a quality and clinical gvernance framewrks ensuring that services delivered meet the standards required f lead bdies, the cmmissiners, ISO 9001 (Quality Versin April 2017 Page 3 f 6
