TAIHS's main operating income comes from Commonwealth and State funding bodies; including the Commonwealth Department of Health, Queensland Department of Health, Queensland Department of Communities, Child Safety and Disability Services, Dept of Prime Minister & Cabinet, and Queensland Department of Housing & Public Works. Funds are also generated through Medicare billing of patient health services. TAIHS provides a holistic primary health care service incorporating a bulk-billing general practice medical clinic, maternal and child health clinic, chronic disease program, a dental clinic and social and emotional well-being services. TAIHS also provides a range of community services, including crisis accommodation, family support services, parenting support, child safety programs, and other community services. In addition to the operation of comprehensive primary health services and the provision of a range of community services, TAIHS seeks to improve health and social outcomes for the community through; strategic partnerships, advocacy for needs, promotion of awareness and education in improved health and social practices, promotion of knowledge and understanding of underlying issues and unique needs and development of the evidence base. Our Vision TAIHS More than a Health Service is to strive to provide a truly comprehensive model of care that responds to the physical, social, emotional, cultural and spiritual needs of our people Purpose of Position The position is responsible for the provision of day to day early intervention and or intensive support practical services to Aboriginal and Torres Strait Islander children and families, with a primary focus primarily on the care and protection of vulnerable children and young people. The role requires working from an Aboriginal and Torres Strait Islander/strengths based perspective to support vulnerable and at risk families to; strengthen their capability, parenting skills, and resilience to prevent problems from developing or escalating to crisis point in order to avoid entry into the statutory system or when exiting from the statutory system. 2 Support includes but is not exclusive of; building on parenting knowledge; information and support for healthy child development and wellbeing; access and participation in education; skills development; building connection to community networks; access to relevant services. Support is delivered through a case management model underpinned by the Family Wellbeing Service Practice Manual and Family Wellbeing contractual agreement standards. Outcomes for families include Children and young people are reunified or remain with family and community Families improve their capacity to meet their children s care, protection and development needs Families are supported to care for their children and young people Fewer children and young people in the tertiary system and in out-of-home care Aboriginal and Torres Strait Islander families have access to support to strengthen their capacity to nurture and care for their children s wellbeing Reduction of the over-representation of Aboriginal and Torres Strait Islander families in the child protection system. Key Responsibilities and Performance Indicators Area Responsibility KPIs Ensure secure management of Evidenced in individual client all program files and compliance files & ARC with applicable privacy Case notes evidence work with legislation and policies Assess and monitor client needs, identify risks, develop a case plan and monitor and review the family in working towards and or achieving goals and strong linkages to culture and community case plan Case files demonstrate effective case management processes demonstrated through internal and external audits & 30 output hours per week Case conferencing / supervision evidence progress and informs future planning / exit strategies Outcomes reflect positive shifts Participate in all aspects of family support and case management practice as per the Human Services Quality Standards & TAIHS Family Wellbeing across wellbeing domains Evidenced in case conferencing documentation and Complete ARC assessments - initial and closing for nonstatutory cases - closing only for Statutory cases 1. 3 Employment assessment for all family members 3. Subsequent and closing Wellbeing Domains Assessment (initial completed at intake) Case management Advocate for the provision of appropriate services for the child and parents and support efficient referrals to salient services for children and their family Evidence in case notes and clients file Data entries evidence internal and external referrals and participation in group works activities Submit weekly predictive planning to line managers before close of business each Thursday for upcoming week Evidenced in case conferencing documentation and Case load may include increased number of complex and or statutory cases requiring more intensive interventions Evidenced through case management Administration Administration Maintain accurate and complete client records in accordance with case management guidelines. Provide the Family Wellbeing Manager & Program Coordinator with - all relevant data and information regarding service delivery as required and / or directed - Complete all quality management tasks by the required date in line with quality management procedures and case management processes. Contribute to continuous quality improvement Minimum 30 hours per week entered into ARC data base Monitored through weekly audit of entries All reports completed within a time framework Less than 10% of tasks overdue in LogiQC Recorded on LogiQC Action directions from Coordinator are and the Family Wellbeing Manager Monitored through LogiQC Progress Note entered on LogiQC when task cannot be actioned / completed within time frame Doc_1286_PD_Wellbeing Worker L4 _v1 3 of 6
4 Maintain current professional knowledge and understanding of; - The Child Protection Act Child safety & Wellbeing - And all other related legislative and regulatory requirements for the provision of services to Aboriginal and Torres Strait Islander children Further skills by attending training and development and contribute to continuous quality improvement, e.g. identify improvement to policy, work instruction, or the work environment - Case management demonstrates sound application of the responsibilities of mandatory reporting and increasing protective behaviours. - Individual training plan developed - Measured through annual Participation in internal and external Professional Development Recorded on LogiQC Risk Management / OHS Staff Management & Team Contribution Staff Management & Team Contribution Staff Management & Team Contribution Stakeholder Relationships Perform all work to reflect TAIHS Vision; Mission and Values Foster effective relationships between all TAIHS Work teams and staff Perform work in accordance with organisational policies & procedures Participate in the Performance Planning and Review process (PP&R). Participate and contribute to team meetings; morning musters & other meetings as requested Develop and document good working relationships and/or collaborative partnerships with relevant agencies Work reflects awareness of duties and responsibilities of TAIHS work performance and behaviour policies. - with all policies and procedures (reviewed at PP&R). - Follow all safety procedures and contribute to a safe work environment Draft document provided to the Coordinator within a minimum of 2 weeks of due date. Monitored through LogiQC Demonstrated through participation in case conferencing and other collaborative activities. 5 Stakeholder Relationships Participate and contribute to joint community events, Actively participate in scheduled Family Wellbeing community based activities to provide brief interventions for information, advice and referral and promote and support families participation in evidence based community programs Delivery of community promotions and education programs as required in community and at the TAIHS Wellness Hub Documentation and case studies to evidence activities and purpose Evidenced in ARC ENQUIRIES (non - case activities) Outcomes are recorded in case notes and wellbeing domains Please note that the duties outlined in this position description are not exhaustive, and only an indication of the work of the role. Required Qualifications and Experience An appropriate certificate relevant to the work required to be performed; eg Certificate IV in Community Services or equivalent Previous experience in a relevant industry, service or an equivalent level of expertise and experience to undertake the range of activities required Sound knowledge of child development and parenting skills models and theory, Attachment theory and Strengths based service delivery Ability to operate within a case management model Ability to work with a team in ways that are respectful, transparent and acknowledging of individual s skills, abilities and capacities. Ability to work autonomously and display initiative, self-motivation, time management and solution focused skills. Demonstrated oral communication skills and competence in use of business technology and desktop applications; internet, word, spreadsheet and database packages Practical Requirements Hold a current Suitability Blue Card for working with children and young people Hold a current Queensland Driver s Licence, Satisfactory Police Check no Serious/Criminal/Court Record Work out of normal hours of duty, including some week-ends, may be required Travel across the region will be required Doc_1286_PD_Wellbeing Worker L4 _v1 5 of 6
