2 Intermediate Mental and Physical Healthcare Team (IMPs) We provide a specialist programme of physical rehabilitation and mental health support to older adults in the borough of Westminster. Together we work with service users and their carers to set achievable goals to help regain a more independent life. We offer an individual based programme of physiotherapy and occupational therapy, while also providing support and guidance with mental health problems. We receive referrals from professionals involved in your care, for example your GP.
3 We offer: A general assessment of your mental and physical health needs The opportunity to discuss treatment options with you Support to fulfill your full potential both physically and mentally and to maintain your independence in a safe environment Assistance in coming up with a care and goal plan that works for you An intense programme of physiotherapy and occupational therapy, while also providing support and guidance with any mental health problems Psychological assessment and treatment to work with you and those close to you Doctors to advise on things like a diagnosis and medication, if needed Referrals to other teams such as mental health services or day services in the local community
4 Who is in the IMPs team? We will discuss treatment options with you that can be given and ask for your consent. Job title Physiotherapist Occupational therapist Registered mental health nurse Clinical psychologist Psychiatrist Associate practitioner Rehabilitation assistant Area of expertise Assisting with mobility, balance, exercise, muscle strength, mobility aids, positioning, transfers, musculoskeletal related problems. Assessing your environment, practising Activities of Daily Living, equipment for the home. Encouraging engagement in the wider community. Assessing mental health state. Assessing mental health state, advise regarding medication, counselling and therapeutic support, information and advice. Special psychological assessment and treatment. Working with you and supporting you to engage in your rehabilitation and care plan. Assessment of mental health diagnosis, prescribing and monitoring medication. Social inclusion, engagement with the wider community. Carrying out treatment designed by the physiotherapist, mental health nurses and occupational therapist. Assisting on home visits.
5 What does this mean for me as a carer? Following consent from your relative/friend to discuss their case with you, the IMPs Team can: Work with you to improve manual handling techniques if you are using any Support you as a carer by putting you in touch with organisations such as day care centres, clubs, respite homes and family counselling services The IMPs Team can give you a wealth of information about your relative/friend s problems/needs What does this mean for me as a service user? Mobility: We can design a course of exercises to help balance, strength and prevention of falls. We can help you gain confidence, stability and stamina when walking outside. We can also help you practice transfers such as getting out of a chair or bed. Activities of daily living: We can visit you to assess your ability to carry activities of daily living and will order any equipment, if necessary, for your home to assist your independence, e.g. washing and dressing. Mental health: We will support you and monitor your mental health, advise regarding medication, offering counselling, offering information and advice, and providing any referrals to appropriate services if necessary.
