2 What is Oaktree Lodge? Oaktree Lodge is a continuing care unit run by Oxleas NHS Foundation Trust providing care for people over the age of 55 years, with long term mental health rehabilitation needs. There is a multidisciplinary team that provides assessment, treatment and active rehabilitation. The team includes: activities coordinator chef clinical psychologist consultant psychiatrist general practitioner (GP) healthcare assistants housekeeper nurses occupational therapist pharmacist physiotherapist social worker technical instructor ward administrator ward manager.
3 We provide Individual rehabilitation programmes to assist people to reach their maximum potential and so increase their quality of life. For some people this may mean looking for quite small improvements within Oaktree, and for others it may be possible to aim for a move to more suitable surroundings. We aim to provide the highest possible standard of care and treat people with dignity, respect and independence. We do this by offering: community contact a safe, secure and homely therapeutic environment social and daily living skills promoting mental and physical health and wellbeing stimulation. You will be allocated a primary nurse and associate nurse on your admission to the unit. They will be your link with the rest of the people involved in your care. When your primary nurse is not on duty, a nurse will be allocated to ensure continuity of care.
4 Activities on the unit To aid your recovery you will have the opportunity to be involved with a range of activities. This may include: community meetings exercise groups one to one sessions relaxation groups social and recreational groups. You will also be able to take part in therapeutic outings to the local community and occupational activities. There is also a TV room with newspapers and magazines.
5 Reviews Reviews are held regularly and you will be told a month before your review is due. Your primary nurse will discuss your progress and any concerns you may have prior to your review. The review team will include your consultant psychiatrist, nurse, psychologist and occupational therapist. Relatives are involved as much as possible (with your consent), and will usually be invited to attend. Relatives and carers are welcome to speak to nursing staff regarding any concerns. Visiting times Visiting times are generally between 2pm - 8pm; this stops organised programmes or meetings being interrupted. But if family or friends wish to visit outside of normal visiting times, please discuss this with your primary nurse so arrangements can be made. Catering arrangements: Breakfast Lunch Tea 8am - 10am 12am - 1pm 5pm - 6pm Drinks and snacks are available throughout the day and evening. Please let us know if you have a special dietary requirement.
6 Smoking Smoking is banned in all indoor public areas. Oxleas NHS Foundation Trust operates a smoke free environment and smoking is only permitted by service users in the designated smoking area. Please ask your primary nurse for more details. Cultural and spiritual needs Staff on the ward will try to meet any individual, cultural and spiritual needs. Please speak to your primary nurse if you require further information.
7 Your Opinion Matters. We welcome your views on the services we provide If you would like to comment, make a suggestion or make a complaint, please speak to the person you normally see, raise it in community meetings or ask to speak to the team manager.
