1 Independent Living Reablement Service Creating quality choices for communities

2 Helping you to regain your independence at home Our Independent Living Reablement Service is designed for people who need assistance to regain their independence. We provide a planned reablement service to help you to remain at home and maximise your independence. Our aim is to enhance the quality of life for everyone who uses this service. What does the service provide? We provide planned, short term and intensive help to you. The service will enable you to learn how to accommodate your illness or condition and remain independent at home. The length of the service will vary, depending on your needs. You may only need a few days to regain your skills but the service can run for up to a maximum of six weeks and during this time you will not be charged for this service. What do we help with? Assistance to enable you for washing and personal grooming Enabling you to dress/ undress Assistance with toileting Enabling you to prepare a meal Enabling you with medication administration Enabling and supporting social interaction or attendance at groups/ clubs Assisting you to access other agencies/organisations Enable you to cope with household duties Enabling you to fill out forms, write letters, finances, claiming benefits and the processes Responding to out of hours calls to provide low level appropriate assistance (6 week free trial of Telecare Service if taken up) Working with other professionals such as your occupational/physio therapist, GP and locality teams. What we can t help with End of life needs Nursing/long term care Requirement of 24 hour care Any long term Intensive Manual Handling needs Care of nails this should be undertaken by a qualified chiropodist Who we work with Adults over 18 years of age, who are residents of shire Adults where an event has led to a decline or change in skills/confidence Adults where a period of hospitalisation has taken place Adults where assessment has indicated reablement benefit (i.e. care reduction) Adults who are medically stable Adults who have ability to engage Adults who consent to participate in rehabilitation with shared goals Adults who have all appropriate equipment/adaptations in place/or planned. Adults where reablement interventions will reduce levels of dependency Adults who are at risk of losing previous levels of independence 2

3 Assessing Needs Before we start working with you, one of our assessment team will contact you to complete an assessment of your needs. Our fully trained team will put support in place for you to learn or relearn the skills necessary for daily living. We will review your progress regularly with you and if you need further support in the longer term, we will help you to access the appropriate services. As a user of the service, you will have a copy of your own personal support plan, which sets out how the service will be delivered to ensure your desired outcomes are met and your lifestyle is sustained. A risk assessment will also be carried out at our first visit. What are the circumstances that may result in services stopping? The service may be withdrawn where there is a serious risk of harm to you as the user of the service and/or reablement workers. The potential harm may result from environmental factors, contamination, infection or service user/third party behaviour. We are required to carry out a detailed risk assessment of each home to establish whether measures for the reablement workers safety are identified. In exceptional circumstances a meeting would be arranged with all relevant people, issues discussed and risk reducing actions agreed. Core service elements Our Independent Living Reablement Service is built around 7 core elements: Sensitivity Dignity Person centred Choice Compassion Reliability Satisfaction Our experienced reablement team What are the management and reablement worker arrangements? The Independent Living Reablement Service is run by Independence Trust, through the shire Housing Group. We ensure that managers are responsible, capable and qualified. We support on-going training especially through the Diploma route. This ensures that all colleagues are competent and skilled to do the job. Additionally, our recruitment process is in line with the National Minimum Standards. Care Quality Commission (CQC) Registered manager The manager of the service is a CQC registered manager, with an ILM endorsed level 5 certificate in management and leadership. This formal qualification is supported by many years of experience working with older people, people with physical disability and older people who suffer with mental frailty. This knowledge and experience leads to stability and continuity in the service offered and the value base to ensure consistence in quality and performance. Reablement workers Our Reablement workers either have or are working towards a formal qualification in Health and Social Care. And have relevant training in: Adult Safeguarding/Abuse awareness Professional Boundaries Data protection and confidentiality Dementia care Emergency first aid Health and safety Manual handling 4 5

4 How are complaints dealt with? A leaflet explaining the complaints procedure will be provided to you at the commencement of services. The leaflet gives advice about how to complain and how the procedure works. It also explains how to give more general feedback about the service you have received. The complaints procedure ensures complaints are fully investigated, normally within 28 days. The investigating officer will normally interview you and carers as part of this process. Following this, complainants are notified in writing of the outcome of the investigation and any action to be taken. The procedure also explains what to do if they are not satisfied. Records will be kept by the Registered Manager of all complaints, outcomes and actions in the previous 12 months.
