The District Nursing Team District Nurses are registered nurses with specialist training. We lead a team of healthcare staff that includes: registered nurses; community nursing assistants; and administrative staff. We are based in community health centres and clinics. We provide nursing care to people over the age of 16 in their own homes and residential care homes. The service operates 24 hours a day, 365 days a year. 3 Our values Our District Nursing Service is based on traditional beliefs and values: we treat everyone as an individual; we are open, honest and do what we say we will do; and we are continuously listening, learning and improving. What you can expect from us Your Neighbourhood District Nursing Team consists of: District Nurse Team Leader Royal box blue with white piping on the collar and sleeves A member of the nursing team will talk to you, your family or carer to assess how we can help you with health and social care. We will work with your doctor and other professionals involved in your care. We will: care for you at home but sometimes we may need to see you at your health centre; support you, your family or carers with information and advice; work with you to develop an individual care plan around your health and social care needs; help you to become independent with your care needs and live a healthier lifestyle; and encourage you to involve family and surrounding social support networks in maintaining your independence at home. 4 Working together We are guests in your home and we will: treat you as an individual; be respectful and value your privacy and dignity; be polite and helpful; keep your personal information secure. We may need to share some information with other professionals involved in your care with your consent; make sure your patient records stay with you throughout your care; offer advice and support about healthy eating or stopping smoking; and show you our ID badge. If we forget, It s OK to Ask!* (see back page) You can help us. If you need to cancel a visit, please let your nurse know or leave a message with the contact number provided. We expect to work in surroundings that are smokefree, safe and free from abuse. We need to use facilities to wash our hands before we care for you. Please make liquid soap and paper towels (kitchen roll) available. It is the responsibility of you or your carer to collect prescriptions. Your local chemist may offer a delivery service. We train nursing and medical students, who may help with your care. If you would prefer students not to be involved, please talk to your district nurse. We provide specialist nursing skills: Wound prevention, assessment and management Post operative care Diabetic care Stoma care Information and advice on continence and catheter management Support with prescribed medication Nutritional advice Advice on leading a healthy lifestyle End of life / palliative care including care for the terminally ill patients Bereavement support We can prescribe for your nursing treatment Advice on the prevention of falls The District Nursing Teams are experienced in caring for those who are terminally ill or have a serious or life-threatening illness. We help people receive care at home for as long as possible. We provide an evening and night district nursing service across the city between the hours of and Following assessment we are also able to offer overnight care and support for people approaching end of life. We also work alongside GPs and specialist nurses to manage your symptoms and keep you safe and comfortable. Sharing your information We routinely share you clinical information with other services where it is necessary to provide treatment. 5 *It s OK to Ask! Leeds Community Health Care NHS Trust has signed up to the National Patient Safety Agency Clean Your Hands Campaign ( cleanyourhands/). A part of this campaign is to encourage patients to ask doctors and nurses if they have washed their hands before caring for them. If you feel uncomfortable with asking staff if they have washed their hands/ used hand gel, then feel free to invite staff to wash their hands before caring for you. Don t forget it is OK to Ask. Leeds Carers Charter A carer is a person who provides care for someone else who, because of long term illness, mental illness, impairment or old age, is not able to care for him or herself. A carer can be: A parent, a son or daughter, a brother or sister, a husband, wife or partner, a relative, friend or neighbour. A carer is someone who is not paid for the care they provide. Young carers are children and young people who take or share responsibility for the care of another person (Carers Act 1995).
