1 Services for Children and Young People with Special Educational Needs and Disabilities Lancashire s Local Offer Lancashire s Health Services 1. Name of the service and what the service provides Complex Care and Treatment Teams (CCTTs) provide high quality support and treatment for people over the age of 16 years who are experiencing complex mental health and social care needs. With the support and care of the team, people will be assisted to maximise their quality of life by promoting independence, recovery and social inclusion. The person s needs are assessed and the care plan is developed with the person and reviewed regularly. The person is allocated a named member of staff (a Care Coordinator) who will coordinate their care and will act as a key point of contact for the person and their family and carers. Complex Care and Treatment Teams consist of a variety of staff groups including; Nurses Social Workers Psychiatrists Psychologists Occupational Therapists Support Workers Pharmacists Specialist Practitioners People use our service for varying lengths of time, depending on need. 2. 3 Tel: Tel: Care coordinators will ensure that you have a clear plan of who to contact Out of Hours. 3. The service is for Complex Care and Treatment Teams provide high quality support and treatment for people over the age of 16 years who are experiencing complex mental health and social care needs. We will not exclude anyone who needs our service on the basis of other disabilities or needs that the person may have. We work closely with a range of statutory, voluntary and other organisations to ensure through the Care Programme Approach to meet needs that cannot be by CCTTs alone. 4. Accessing the Service Referrals to CCTTs are made from other teams within Lancashire Care NHS Foundation Trust, such as Single Point of Access, Crisis Resolution and Home Treatment Teams, In- Patient Units and Early Intervention Services. CCTTs do not accept self-referrals. However referrals can be made by a person s GP, but they will be assessed initially by the Single Point of Access Team to ensure they are referred to the correct team. There is no charge for users of CCTTs. We do not operate waiting lists to access CCTTs 5. Decision making processes used to determine who is eligible to receive a service. Some young people are transferred to our teams from a Child & Adolescent Mental Health Service (CAMHS), and the young person and parents/carers will be involved throughout this process. If not already involved with CAMHS, the decision to refer to mental health services will 3
4 usually be one made by a young person and their parent/carer with their GP, who will make the actual referral. Another health or social care professional may be involved. An assessment team will review the referral and meet with young person and parents/carers to make sure that we have a good understanding of what it is that you want some help with, and that we involved the right people to help. You will receive a copy of the letter that we send to referrers after all assessments which summarises what we have will offer and the reasons for this. 6. Methods of communication with service users/patients and how they are involved in decision making/planning. We regularly seek feedback from service users and use this to both check where we are providing a good service and improve where people say that we can improve. If needed, care and treatment will be supported by interpreters and signing. We recognise the key role that parents and carers play in the Recovery of service users, and ensure that carers are offered their own carers assessment and care plan. 7. Service Accessibility. Our buildings are wheelchair accessible and disabled toilet facilities. 8. Workforce Skills and Training Our teams have traditionally been accessed by people aged 18 or over, and our teams are undergoing further training in working with young people and their families to ensure that we offer the best service possible to 16 and 17 year olds. As well as the range of professionals, we have a range of specialist skills within our teams, with training in interventions such as Cognitive Behavioural Therapy and Psycho-Social Interventions. 9. Contacts for further information If you think that you may benefit from our service, we recommend that you first speak to your GP he is able to make a referral to mental health services. This will make sure that you are directed to the right service to meet your needs. 4
5 10. Feedback Feedback is regularly sought from all of our service users. We also welcome complaints, comments and compliments, all of which help us to improve our service.
