2 Welcome to Gerrard Ward Your care and treatment We hope you find the information in this pack useful during your stay on Gerrard Ward. You may not find all the answers to the questions you have, so please ask a member of staff if there is anything else you would like to know. We aim to deliver care of a high quality based on your individual needs in a warm, friendly and safe environment. We have 18 beds for adults aged 18-65, including people with learning disabilities and occasionally adolescents aged We aim to: Provide a safe and helpful environment for your care Provide an individual care plan that is planned with you about your needs. Provide a full programme of therapy and activities on and off the ward Provide education and advice about mental illness, as well as information about the treatment and services available to suit your needs Provide care that meets all the requirements set by the Department of Health such as the Care Programme Approach and the Mental Health Act, 1983 Provide advice and support from the Homeless Prevention Initiative regarding accommodation and repatriation issues Prepare you for discharge and provide a care plan involving your community workers/support What is a triage ward? A triage ward has a dedicated multidisciplinary team who will be able to assess your needs. The team will find out how you are feeling and what has been happening to you before you come to hospital. This will help the team to put together a care plan with you. The length of stay is different for each patient, depending on your clinical need. We aim for time-limited admission of no more than 14 nights, although this may not always be possible. Depending on your assessed needs you may be transferred to another facility or discharged before the 14 days. We will discuss this with you. Physical health checks Within the first 24 hours of your arrival, a nurse will talk to you about your lifestyle and do a physical health check with you so that we can understand if you have any physical health problems. Later on, a doctor will physically examine you and take blood and urine samples as well as carrying out an electrocardiogram - or ECG - a simple and useful test which records the rhythm and electrical activity of your heart. We will give you a Healthy Lifestyle Advice booklet and also offer a therapy programme to support you in your lifestyle choices. Your nurses You play a very important role in your wellbeing and recovery. We will allocate a primary nurse and an associate nurse when you are admitted to the ward. The primary nurse is usually the nurse who manages your admission onto the ward. You will be offered regular one-to-one meetings with your primary and associate nurse
3 during your stay. They will support you in developing a treatment plan that suits your specific needs. There will always be a specific nurse who is allocated to take care of your needs. Their name will be written on the board opposite the nurse s station at the start of each shift. We have weekly community meetings and coffee mornings on Gerrard Ward. These are run by our associate practitioners. You do not have to attend these meetings but they are a good way of getting to know each other and an opportunity to air your concerns about any issues on the ward that you may wish to discuss with each other or the staff. However, if you would prefer to raise any issues with just the staff or any other time this would be fine too. There are a number of nurses, and other staff who will be looking after you during your stay with us.we also have ward doctors known as a senior house officer (SHO) and Specialist Registrar (SpR), psychologists, occupational therapists, peer support workers, associate practitioners, pharmacists and a social worker. This ensures that you receive the necessary care during your stay with us. The staff do not wear uniforms, but all staff wear name badges. There are five staff based on the ward on each shift during the day and three at night. The ward also has student nurses/medical students, student occupational therapists, a Ward Clerk and domestic staff. Clinical reviews As part of your admission process you will be seen by a junior doctor for physical and psychiatric assessment We offer a review by a senior psychiatrist (specialty registrar or consultant) within 24 hours of admission. This senior clinical review allows us to develop your care plan and treatment plan with you. Further clinical reviews are offered by both ward doctors and more senior staff as your admission continues. We may (with your consent) include family members in such reviews or workers who know you from the community, such as your care coordinator. We hold daily clinical reviews on Gerrard Ward similar to what you may have experienced on other wards. These reviews are arranged when required on the day or when it is suitable for you and anyone else who you would like to attend. In the review, we will discuss your progress care and treatment plan.we aim to assess and formulate treatment and discharge plans with you.we would also refer or signpost you to relevant services that may be helpful to you postdischarge. What is the purpose of these meetings? To review your progress and on-going care To listen to your views and any concerns you may have related to your care To receive feedback on any ward activities you have been involved in To review your medication To review your leave arrangements To plan ahead including what sort of follow-up and care you might need after leaving hospital. Please remember that sometimes the meetings can be quite large. Medication You might be prescribed medication to help you while you are on the ward. Please ask a nurse if you would like some more information about the medication you are on. You can also talk to the ward pharmacist about your medication and any side effects that they might cause. The Choice and Medication website www. choiceandmedication.org/ cnwl/ provides information on around 20 mental health conditions and over 110 different medicines. There is a medicines helpline that can support you with medicines once you have been discharged from hospital. The helpline number is and is open Monday to Friday from 9.00am to 5.15pm (outside of these hours a message can be left and they will call you back). You can also contact them by medinfo.cnwl@nhs.net Your medication will be given to you at: 8am, 12pm and 6pm. Observation levels You will be checked at least once an hour by a member of nursing staff, including at night. This is to ensure the safety of
4 all patients on the ward. Some patients will require an enhanced level of observation. This may be intermittently (approximately every 15 minutes) or on close observations where a member of nursing staff will be within eyesight at all times. The decision to place a patient on an enhanced level of observations is a multidisciplinary team decision and will be discussed with you. Nursing staff will always endeavour to observe you in a discreet and therapeutic manner and will always try to respect your privacy and protect your dignity. Your health records Information about your stay here is kept on our computer system. We have a legal duty to keep your information confidential. For more information about how we use your health records you can read the leaflet Your Information that you will be given. Occupational therapy The unit has an inpatient therapies team that runs a variety of groups for inpatients. You are encouraged to attend groups to make your stay on the ward more fulfilling and help your recovery. A separate leaflet detailing these therapy groups will be provided by the Occupational Therapist on admission. We also have a variety of self occupying activities including board games, novels, X- Box 360, and daily newspapers which the staff encourage patients to make use of. These can be found in the therapy room. There are also a number of DVDs available to watch and Freeview TV. If unsure please speak to a member of staff. The associate practitioners coordinate activities on the ward. They will meet with you in the first few days of your stay and provide you with the therapy timetable and talk to you about your interests. A range of smaller activities are also available on request so please speak to the associate practitioners or a member of staff about this. We are open to new ideas or requests of what you would like to happen on the ward so please let Gerrard Ward team know. You can also speak to the team about the CNWL Recovery College and what it can offer you, as well as your support network (friends, family and carers). On the ward Meals Meal times are: Breakfast: 8.30am Lunch: 12pm Dinner: 5pm In addition, there is a beverage bay on the ward that is open for patients to make hot or cold beverages, snacks and fresh fruit at any time. The menu for each day is displayed in the dining room. Please let us know if you have any special dietary requirements or preferences. We welcome any suggestions you have about the meals and we will do our best to act on your suggestions. Visiting times Your family, friends and/or carer(s) are welcome to visit you on Gerrard Ward. They can do so at the following times: Monday to Friday: 4.30pm to 8pm Weekends: 2.30pm to 8pm If these times are unsuitable for your visitors please speak to a member of staff. Please note that the ward reserves the right to refuse visitors or to ask any visitors who are unruly to leave. We want all patients and visitors to feel welcome and safe whilst on Gerrard Ward. Some items cannot be brought onto the ward, some of these are listed in the Restricted Items section on page 8 of this leaflet. If you are unsure whether an item is restricted or not, please ask a member of staff. All visits take place in the communal areas of the ward or in the quiet room. Visitors are not allowed in your bedroom or in any other restricted areas on the ward. Visits with children Your children are welcome to visit you while you are staying on Gerrard Ward. There is a family room on the ward where visits with children take place and where you will have more privacy in a pleasant environment. Please let a member of staff know if you want to use the family room so it can be booked for you. We do not allow children into the main ward areas.
5 Mobile phones You can keep your mobile phone, laptop and other electrical items with you. The chargers for electrical items are kept in the nursing office and staff will charge them for you when requested. Mobile phones are not allowed to be used to film, record voices or take photographs whilst you are on the ward. This is to protect the privacy and confidentiality of all our patients. Telephone There is a public telephone in the corridor that takes incoming and can make outgoing calls. The phone number is: Female area ; Male area The office phone number is: If you have a mobile phone, please discuss with a member of staff whether you want to keep this with you. You will not always have access to the telephones in the nursing station so it s better to use the payphone which is operated using coins. Restricted items The following items are not allowed to be brought onto the ward. If you have any of these items with you, they can be kept safe in the nursing office or ward safe and will be returned to you when you leave the ward. Prescribed medications including ointments and creams Over the counter medicines and supplements Illicit drugs Alcohol Solvents and other toxic, hazardous substances Glass Any sharp implements including knives, scissors, razors Firearms and ammunition Lighters and matches Photographic equipment Any other items which could compromise the safety of the ward or of people on the ward. We appreciate your cooperation in keeping the ward safe for everyone. Smoking The law bans smoking in all enclosed public spaces, this includes hospital wards. There is a designated smoking area that patients are allowed to go to. The smoking area is open for the first 15 minutes of each hour between 6am and 11pm each day. Your visitors are not allowed to use the smoking area, it is for patients only. Religious and spiritual needs The hospital Chaplaincy services provides additional support to our patients.please inform a member of staff if you wish to have a meeting with any of our chaplains. We also have faith visitors of all denominations who also attend to provide lay support to our patients. They visit the ward throughout the week and run mass and other religious services. This is available on request so please approach staff for more information. In addition we have a fully equipped multi-faith room available on the ground floor for our patients to use. We respect any faith events or requirements that need to be observed and aim to provide as much facilities as possible to allow for this to happen for you. Laundry There is a laundry room that you can use. This room is kept locked and a member of staff will open it for you to use. You can use the laundry room from 9am to 10pm. Newspaper Daily newspapers are available on the ward for you to read. They can be found in the communal areas but please ask a member of staff if you can t find them. Shop round If you are not granted leave to go out, a member of the nursing team can buy things for you daily or sometimes twice daily. You can either speak to the nurse in charge or go to the daily planning meeting to request something to be bought for you. Property and Valuables Central and North West London NHS Foundation Trust (the Trust responsible for this Ward) does not accept responsibility for loss or damage to your personal property of any kind, including loss of money unless it has been kept in the ward safe. All property is checked on admission and contraband items will be confiscated. If you decide to keep any of your valuables with you, you will be asked to sign a disclaimer form. Nursing staff will ask to see what is on your person on return to the ward, and may need to asked visitors to show what s in their bags. Nursing staff will periodically conduct room searches and confiscate any prohibited items. Locked doors and CCTV The ward operates on a controlled access system. This means that admission to and from the ward can only be gained by asking a member of staff. This ensures patient and staff security. The use of any equipment for taking or storing images or sound on the ward is forbidden. This is to ensure that everyone s dignity and confidentiality is maintained.
6 Leave from the ward Escorted leave To make sure that you are as safe as possible when you leave the ward, sometimes you may need a staff escort. You will definitely need an escort when you first come onto the ward. If you are brought onto the ward under a Section of the Mental Health Act special permission from your consultant will also be needed. Leave for informal patients An informal or voluntary patient is someone who has agreed to come into hospital to receive treatment. Some informal patients may know they are vulnerable or maybe having suicidal thoughts and they may agree to have an escort with them when they go out. This can make you feel more settled and will be part of your treatment plan. All informal patients are asked to work with staff to come up with a care plan for when you leave the ward that will keep you, and other patients on the ward, safe. We will always try to provide an escort as soon as possible once you have asked for one but this may not always be possible straight away when the ward is busy. Mental Health Act If you are detained under the Mental Health Act 1983 (amended 2007), you will be told when you are first admitted to hospital. The Mental Health Act 1983 (amended in 2007) is the law in England and Wales that allows people with a mental disorder to be admitted to hospital, detained and treated without their consent either for their own health and safety, or for the protection of other people. You will also be given information about your rights. If you need a list of solicitors please ask a member of staff who can give this to you. You also have the right to an Independent Mental Health Advocate. Advocacy Service Advocates can help you if you are not happy with the service provided by a doctor, NHS professional or social services. They normally work for charities, so they are independent. They can help you to: Get what you are entitled to Get your point of view across Get the services that you need. Staff from Advocate for Mental Health visit The Gordon Hospital on Tuesday and Thursday from 9.30am-4.30pm and will visit Gerrard Ward during that time. You can also get in touch with advocates or ask staff to call between Monday and Friday 9.30am-5pm on Bed availability within The Gordon Hospital There are only a limited number of beds at The Gordon Hospital. Sometimes when patients are on leave and are not using the beds on the ward it is necessary for them to be used for other patients. If there is a lot of demand for the beds we occasionally have to ask patients to sleep on another ward or unit. This will only happen if your mental state is stable and you are close to being discharged. If you need to be moved temporarily to another ward, the staff will discuss this with you and support you so that your care and treatment are not disrupted.
