The person appointed will be expected to have a primary research commitment in conjunction with teaching and clinical service responsibilities. The job plan will encompass 5 University and 5 NHS programmed activities, provided at the Nottingham University Hospitals NHS Trust. The Nottingham Digestive Diseases Centre (NDDC) provides a wide range of research opportunities in gastrointestinal and liver disorders including a National Institute for Health Research (NIHR) funded Biomedical Research Centre (BRC) specialising in these themes. This post offers the opportunity to join the NDDC, one of the largest and internationally excellent divisions comprising academic gastroenterology, hepatology and gastrointestinal (GI) surgery. The NDDC has 8 Professors, 7 clinical/ 2 non-clinical associate professors, 4 clinical lecturers, 13 research & teaching staff, 5 technical and 8 administrative staff within the Gastrointestinal and Liver Disorder Theme of the NIHR BRC. The BRC builds on the NIHR-Nottingham Biomedical Research Unit established in 2008 and the only NIHRfunded BRU focussed on GI, liver and pancreas-related areas to have sustained its funding and theme during the most recent NIHR funding cycle. The GI & Liver Disorder theme is the largest of the five themes included in the NIHR-Nottingham BRC funded 23.6M for early translational programme of research for the period The major purpose of this appointment is to expand the critical mass of world leading and internationally excellent research performed within the NDDC and GI & Liver Disorder Theme of NIHR-Nottingham BRC that has GI & Liver Infections (C difficile, Viral Hepatitis and H pylori), Disorders of GI Physiology & Liver Metabolism and Innovative Methodologies as priority Research Areas. We have strong links with: - the Hepatitis C Virus (HCV) group that co-ordinates the world s largest HCV cohort (n >11,000) - the Clostridia Research Group, underpinned by the BBSRC/EPSRC centre and - the internationally leading Helicobacter group. Our long-standing collaboration with the world-class Sir Peter Mansfield Imaging Centre (SPMIC) will be considerably strengthened through the BRC s cross-cutting Imaging theme. The EPSRC/ MRC funded Nottingham Molecular Pathology Node (NMPN) is a joint venture between NDDC, the Respiratory Research Unit, Molecular Pathology and the Advanced Data Analysis Centre (ADAC) of the University of Nottingham (UoN). Our strong links with the MRC/ARUK Centre ( 2M) for Musculoskeletal Ageing Research, which is internationally leading in in-vivo human, whole body physiological investigations, and with the Food Sciences Page 1 of 12
2 Research Group ranked 2 nd in England in the RAND bibliometric analysis, provide outstanding collaborations that will underpin the development of new treatments for disorders of GI & liver physiology and metabolism. Our Stem Cell Biology Group, with its specific stem cell based disease models, the FRAME Alternatives Laboratory, a state-of-the-art cell biology facility expert in culturing primary human cells, and the UK top-ranked School of Pharmacy will translate GI and liver drug discovery/formulation programmes through our BRC theme. Our UK-leading GI epidemiology group provides expert methodological support and the Nottingham Clinical Trials Unit has recently expanded to provide improved support for early patient-based translational studies. The successful candidate is expected to have strengths in translational research in one of the research areas of the NDDC and the GI & Liver Disorders theme of the NIHR-Nottingham BRC. In particular, they will bring existing research strengths to devise, and undertake, their own research programme developing novel therapeutic and preventative strategies within their specialist field of interest. In addition, the person appointed will deliver excellent care to patients through the Gastroenterology/ Hepatology/ Nutrition/ Endoscopy Services of the Division of Surgery at the Nottingham University Hospitals (NUH) NHS Trust and contribute to the development and delivery of undergraduate and postgraduate teaching within the School of Medicine, UoN In terms of research, the appointee will be expected to: develop a research portfolio, gain external research funding. publish in prestigious journals, disseminate research findings and impact on clinical practice. supervise BMedSci, DM and PhD students. The person appointed will be required to devote the whole of their time to the duties of the post and give them priority at all times; the proportion of the time committed to the clinical services will be proportionate to the design of the clinical academic position and specified in the job plan in discussion with the School of Medicine, UoN and NUH. The person appointed will be expected to fulfil the following clinical service duties within the agreed Job Plan including: provision of reciprocal cover for periods of leave continuing responsibility for the care of patients in his/her charge and the proper functioning of the clinical department maintain CPD, annual appraisal and revalidation. These duties are subject to regular review in the light of the changing requirements of the academic and clinical service. If alterations to the described duties are required these will be mutually agreed between the successful candidate, their academic and NHS Consultant colleagues, the University and the NHS Trust. The successful applicant s line manager will be the Head of Nottingham Digestive Diseases Centre, School of Medicine, University of Nottingham. For clinical matters, line management will be devolved to Nottingham University Hospital s Head of Service for Gastroenterology. The Medical Director, Dr Keith Girling, has overall responsibility for the professional performance of clinicians working under Honorary Contracts within Nottingham University Hospital NHS Trust. NUH have recently developed a Trust mentoring scheme for new and existing consultants and this would be available to the new post-holder as will the UoN School of Medicine s Mentoring programme for clinical academics. The appointee will be encouraged and supported to attend the RCP New Consultants Development Programme. Main Responsibilities 1. Research: Formulate ideas & undertake high-quality clinical /laboratory research in conjunction with established research in the Department. To conduct peer reviewed clinical and basic research in Gastroenterology or Hepatology of sufficiently high quality to be returned in the University s evaluation of research quality. To obtain independent external funding to conduct such research To secure funding for, and provide supervision of, postgraduate research students studying for MD/PhD degrees. % time per year 40% Page 2 of 12
3 Contribute to some of the management systems of the School of Medicine e.g. Divisional and School committees, as requested by the Head of School or Head of Division, engage in research leadership and perform appraisals of School staff. 2. Teaching and School Management: To supervise postgraduate students undertaking higher degrees (e.g. MSc, MRes or similar). To develop curricula, teach, set exams, examine and otherwise participate in Clinical Phase 2 of the medical undergraduate curriculum. To participate in other phases of the curriculum from time to time. To participate in teaching on postgraduate taught courses. To make a contribution to School of Medicine management systems in undergraduate & postgraduate teaching which is in line with balance of other responsibilities 3. Clinical: - To participate as a clinical academic consultant in delivering excellent care to patients through the Gastroenterology/ Hepatology/ Nutrition/ Endoscopy Services of the Division of Surgery at the Nottingham University Hospitals (NUH) NHS Trust as determined by the agreed job plan. -To contribute to teaching & training of junior medical staff. Most of the Trust s emergency care pathway is provided on the QMC campus with specialist registrar on-call during the day and out-of-hours under the supervision of a dedicated consultant-led team. In-patient care is also provided on the QMC campus as dedicated consultant-led care. Scheduled, and ambulatory, care pathways are provided at QMC and City Hospital campuses as well as through the independent sector Treatment Centre. The post holder will have an annualised job plan working closely with other colleagues in delivering the service across both QMC and NCH campuses. They will have their own dedicated weekly sessions and be able to work flexibly with other luminal gastroenterologists and the emergency gastroenterology service. Booking rules for out-patient clinics are set by the individual consultants based on a point system (16-18 points per 4-hour session, 2 points per new & 1 point per follow up patient). We expect active discharge of patients to their primary care physician wherever possible; however, we acknowledge that as a tertiary centre of expertise some complex patients will need life-long specialist follow up and monitoring of new: follow up ratios is not always appropriate. Teaching Both campuses within Nottingham University Hospitals NHS Trust are major centres for undergraduate and postgraduate specialty trainee education and teaching. The successful candidate will be expected to play a full role in undergraduate and postgraduate teaching. Governance and Audit Nottingham University Hospitals NHS Trust is committed to the development of Clinical Governance. The approach taken is to develop actions plans at a directorate level. Each member of the medical staff is expected to take an active role in clinical governance activities within their Clinical Service Unit and each Clinical Service Unit has a Consultant nominated as Clinical Governance lead. The activities include, but are not restricted to, audit, incident reporting, review of complaints, risk management, CPD and Evidence Based Practice. Other Duties as Agreed The person appointed may eventually wish to take on other roles. Positions such as Clinical Service unit Lead are offered by the Trust on fixed-term contracts. Any positions offered by anyone outside the Trust (e.g. Training Programme Director, Regional College Advisor, Royal College Representative etc.) must be discussed with the Clinical Director, Head of Division within the University s School of Medicine and colleagues in advance of the position being accepted, so that consideration can be given to the job plan and whether this needs to be revised should the additional role be accepted. All the latter roles come under the University of Nottingham s External work policy. The University and Trust are moving towards annualised job plans to ensure the maximum flexibility and responsiveness of the service. Time off in lieu is granted for weekend and evening working. The weekly job plan outlined below is the most likely starting point although it is likely that sessions will change both in location and type to fit with service needs. The new appointee will oversee inpatient care for about patients in rotation with other Gastroenterologists. We are committed to provide consultant-led service and aim to have daily senior ward rounds. Each consultant aims to do four ward rounds (including 1 teaching round) per week while covering the ward. One day of the week, the Speciality Registrar leads the ward round with appropriate discussion with the consultant. During time on the ward, fixed morning commitments will be cancelled to allow 4 hours of dedicated ward time to include case discussion, meeting with relatives, junior doctor supervision and ward related admin. 6 Office accommodation and administrative support will be provided by the Trust and University, as will a PC with and Internet access. All consultants are required to check their hospital regularly. The post holder will be assigned a named Patient Pathway Administrator (PPA) who manages patient queries, tracks results and pathways, types clinic letters and handles post. In addition, there are speciality PAs on both campuses, who perform more complex administrative functions & manage diaries, rotas and clinics cancellations. The post holder will be assigned a named PA on the City campus as well as use of the pool of speciality PAs on the QMC campus. Both named PPA and PAs are shared by multiple consultants and cross-cover occurs during times of leave. To find out more about Nottingham, use the following links: Nottingham County Council Tourism University of Nottingham Zoopla (Guide to local properties) My Nottingham (information on schools, term dates, school transport etc.) Appendix 2 Academic activity in the Nottingham Digestive Diseases Centre Academic hepatology, gastroenterology & GI surgery in Nottingham comprises of medical and surgical gastroenterologists, hepatologists and non-clinical scientists, which we believe to be the largest clinicallybased gastro-intestinal research grouping in the UK. At its core are 12 senior academics (8 full Professors and 6 associate professor/senior Lecturer) with extensive, mature, well-funded research programmes and expertise ranging from molecular and cellular biology through patient-based research to clinical trials and GI epidemiology. They work closely with the NHS gastroenterologists/hepatologists, many of whom also have active research programmes. Nottingham Digestive Diseases Biomedical Research Unit (NDDBRU) is the only BRU in the UK that is funded by the National Institute for Health Research (NIHR) to deliver translational research both in gastrointestinal and liver specific areas of research. Established with 5.3M funding in 2008, NDDBRU successfully secured further 7.2M funding for the period The current research programme of NDDBRU is focussed on Gastrointestinal and Liver Infection, Inflammation and consequences. NIHR NDD BRU has 4-bedded area suitable for intensive monitoring and interventions, research endoscopy facility, patient assessment rooms and tissue processing and storage facility. We have access to state-of-the-art laboratories in the University of Nottingham. Our collaboration includes over 50 Principal Investigators and also we have the largest dedicated academic training programme in gastroenterology in the UK, with 15 Research Council Training Fellows and 34 non clinical PhDs. As part of the implementation of the research strategy University of Nottingham has identified 24 Research Priority Areas (RPA) and grouped them around five Global Research Themes (GRT) so that we can use creative interdisciplinary approaches to deliver world-class research on a global scale. Chronic inflammatory disease is one of the RPAs in combination with Respiratory research groups under the Health and Wellbeing GRT. Page 8 of 12
9 Current research interests in gastroenterology/ hepatology/ academic surgery The research interests of the NDDC are broad covering luminal gastroenterology, hepatology and gastrointestinal surgery. There is specific expertise in H. pylori associated diseases, C. difficile infection, diverticular disease, inflammatory bowel disease, neurogastroentreology, drug-induced liver injury, nonalcoholic fatty liver disease, chronic hepatitis C, cirrhosis and biomarkers of liver disease, perioperative nutrition and metabolism, diverticular disease, anal fissure, colorectal cancer and anaemia/ iron deficiency. Prof Guru Aithal is the Head of the Division, NDDC and the GI & Liver Disorder Theme lead for NIHR Nottingham BRC. He also jointly leads Chronic inflammatory disease RPA and is the Deputy Director of NPMN. He has led recent discoveries in genetic susceptibility to DILI; he is the Chief-investigator for Prospective European DILI Registry supported by European Association for the Study of Liver. He also established a translational research group in Nottingham investigating novel tests to stratify NAFLD, life style and pharmacological interventions in NAFLD. In collaboration with SPMMRC, there are a number of work streams developing MR methodology to evaluate chronic liver diseases. Prof John Atherton has led translational and basic research in Helicobacter pylori virulence and has contributed to seminal work in the area of H. pylori gastritis and ulcer diathesis. Dr Karen Robinson an immunologist with a focus on immune mechanisms underlying both pathogenetic as well as protective effects of H. pylori. Prof Robin Spiller s research programme in neurogastroenterology focuses on pain and motility disturbance in IBS. He is currently investigating the hypothesis that constipation is caused by excessive cyclical retrograde propagated contractions (CRPC) leading to mechanism-based treatments. The role of CRPCs in diarrhoeapredominant IBS and its modulation by 5HT3 antagonists is a subject of another trial that Prof Spiller is leading. Prof Yash Mahida is interested in mucosal biology (including GI stem cells) and C. difficile pathogenesis. Prof Krish Ragunath is an academic endoscopist interested in advanced imaging, pre-cancer detection and treatment in the GI tract. Prof Chris Hawkey is interested in inflammatory bowel disease, non-steroidal anti-inflammatory drugs, cancer prevention and the conduct of clinical trials, which currently include the HEAT trial examining the benefit of H. pylori eradication in the prevention of aspirin associated GI bleeding. Dr Gordon Moran has broad research interests in inflammatory Bowel Disease and has expertise in the neurohumoral regulation of appetite in relation to inflammatory disorders of GI tract. Dr Luca Marciani is a Physicist focussed on developing novel MR based imaging techniques to evaluate in vivo the effect of food and GI function. Dr Maura Corsetti, is a clinical associate professor interested in disorders of GI physiology and motility an expert in high resolution manometry. Dr Neil Guha, is an expert in the development and evaluation of biomarkers in liver disease. His current work includes stratification of chronic liver disease using tissue, serum and imaging markers both in primary care and hospital based cohorts. Dr Neil Guha and Prof Guru Aithal were winners of the National NHS Innovation Award in 2013 and finalists of BMJ gastroenterology team of the year. Dr Aloysius Dominic Aravinthan has investigated the role of hepatocyte senescence in liver fibrosis and has a focus on hepatocellular carcinoma. Dr Brian Thomson is the Director of Nottingham Health Science Biobank (NHSB) which provides a centralised, quality assured, biofluid and tissue resource to underpin translational studies and to provide added value our clinical trials. Dr Tim Card and Dr Joe West conduct a wide range of GI epidemiology and observational studies in many areas including IBD, GI bleeding, celiac disease and liver diseases.
