The Surgical Unit comprises two wards: Ward 7 with 31 in patient beds and Ward 8 which has been redesigned in the last five years to become an elective admissions/day case unit. As well as General Surgery ward 7 provides inpatient accommodation for ENT, Ophthalmology and Urology patients. Within the surgical unit there are five Foundation Year 1 House Officers, two Specialty Registrars on rotation from the South East Scotland (General Surgery) Training Scheme, Three Associate Specialist / Specialty Doctor Surgeons and one GPST Specialty Registrar. They all assist with the dayto-day management of in-patients and provide emergency medical cover. Facilities at the Borders General Hospital provide a high standard of both diagnostic and interventional radiology support including easy access to Nuclear Medicine, Spiral CT and MRI scanning. POST INFORMATION The Post Ideally we are seeking to recruit a Specialty Registrar in General Surgery, Locum Appointment for Training (LAT) from 02 August 2017, for a period of one year until 01 August Appointment may be made for a lesser period by mutual agreement but a minimum of 4 months applies to Locum Appointment for Training. You should have evidence of achievement of foundation competences from a UKFPO-affiliated foundation programme or equivalent, by time of application in line with GMC standards/good Medical Practice You should also have evidence of achievement of core surgical (ST2 level) competences via one of the following methods: 3

4 Current employment in a UK Core Surgical Training programme and on track to successfully complete the training programme by the advertised post start date, or Successful completion of a UK Core Surgical Training programme, having completed the relevant competencies. Acceptable evidence includes ARCP or RITA documentation, or Evidence of achievement of core surgical competences at time of application through equivalence.. Acceptable evidence is only permitted via the standard Alternative Certificate of Core Competence. This post may be most suitable for ST3 level doctors seeking out of programme experience, to support future career intentions. The post is suitable for experienced aspiring surgeons who are seeking further experience to prepare for a career intention in general surgery or related discipline. We can be flexible about the experience obtained to match your career needs. The post offers a wide experience in all aspects of acute general surgery in a District General Hospital that has an excellent postgraduate educational programme, good library facilities on site and protected formal teaching. The Postgraduate Dean confirms that LAT placements (4 months duration or more) have the required educational and Postgraduate Dean s approval. There may be a possibility to extend this appointment to the end of the training year. The extension may be at any of the hospitals in South East Scotland on rotation. If appointed to a relevant Specialty Training Programme through open competition, documented competences achieved through a LAT can count towards CCT or towards a CESR application for those not entering an approved Specialty Training Programme. There is an excellent Postgraduate educational programme, good library facilities on the Borders General Hospital site and protected formal teaching in the surgical unit. This is a full shift working pattern with a Band 1A / 2B (50%) supplement and a 1 in 6.5 commitment to night duty and weekend on-call. 5 Health and Safety Staffing within Unit You are required to comply with NHS Borders Health and Safety Policies. You will share responsibility for the training and supervision of the Junior Training Grade Medical staff (Foundation Trainees, GPSTs) from various specialties who work with you. The training grade staff in the Department are as follows: 2 Specialty Registrars +1GPST: Two at StR3+ or StR2 level on South East Scotland Surgical rotation scheme and from time to time one General Practice Specialty Trainee. Foundation Year 1 Five Posts + one FY1 in Anaesthetics who contributes to the General Surgery rota. However, some 30% of the population live outwith towns of 1,000 population or more (compared to Scottish average of 12%). The proportion of the population over 65 is the highest 6

7 in Scotland years 20.84% (19.29% Scottish average) years 15.83% (13.98%) 75+ years 8.86% ( 7.09%) (e) There are 4 Community Hospitals providing GP acute, intermediate care, palliative care, slow stream rehabilitation and NHS continuing care beds primarily for frail elderly patients. DUTIES AND RESPONSIBILITIES Main Duties Under the direction and supervision of the Consultants: 1. Clinical responsibility for the investigation and treatment of patients in surgical wards and shared responsibility in the children's ward; this includes daily ward rounds, daily admission of list cases and emergency admissions which take place on a 24 hour day / 7 day week basis and assisting in theatre and Outpatient clinics. 2. Clinical responsibility for general surgical emergencies in the emergency department. 3. Independent minor laesion clinic 4. Assisting consultants in theatres. If/when required competencies achieved, independent theatre lists. 5. Supervision of FY House Officers' work in the wards and emergency department. 6. Preparation of theatre lists; preparation of discharge summaries to general practitioners. 7. Obtaining medical opinions on patients from other Consultants in other specialties. 7

8 8. Take part in formal teaching of various under-graduates attached to the Hospital. 9. Participation in Clinical Audit will be expected. 10. Outpatient clinics alongside consultants and independent clinics when adequate competencies achieved. Borders General Hospital The Borders General Hospital is the district general hospital serving the Scottish Borders Region. It has 282 beds of which 12 are in a medical high dependency area with monitoring and telemetry facilities. The hospital offers services in General Medicine, Haematology, Palliative Care, Medicine for the Elderly, Stroke Medicine, Paediatrics, General Surgery, Orthopaedic Surgery, Ophthalmology, ENT, Obstetrics and Gynaecology. There is a 6 bedded ITU and an active outreach programme for critically patients. The opening of the Margaret Kerr Unit in January 2013 has provided palliative care services in the Scottish Borders with a purpose-built environment from which to deliver specialist palliative care. Within the Department of General Medicine there are specialists in Cardiology, Respiratory Medicine, Diabetes and Endocrinology, Gastroenterology, Neurology and Rheumatology. Visiting consultants provide clinics for Oncology, Dermatology, Renal Medicine and Clinical Genetics. The Emergency Department provides facilities for the reception, resuscitation, examination and treatment of patients in the Scottish Borders who require emergency admission, or immediate care and discharge. The ED service is well integrated with the Borders Emergency Care Service (BECS), which is the Primary Care out-of-hours service for Borders. The department is managed by a consultant emergency physician and the Specialty Doctors, Salaried GPs, training grade doctors (FY2s, GP, orthopaedic and surgical trainees) manage ED patients supported by speciality medical staff from within the hospital.. There is a 6 bedded ITU run by the Anaesthetic Department who also run an acute haemodynamic support service with outreach staff from ITU liasing with ward staff. There is an up to date laboratory service covering Haematology, Microbiology and Clinical Chemistry with ready access to more specialised investigations in Lothian laboratories and other laboratories in Scotland. The Radiology Department is well equipped for routine radiology, CT scanning, ultrasound, Doppler studies, nuclear medicine and MRI. CT coronary angiography and cardiac MRI are well established in the department. There is an active post graduate programme under the direction of the Director of Medical Education, Dr Jane Montgomery, with excellent facilities in the Education Centre. 8

9 Educational Facilities / Study Leave The Postgraduate Dean confirms that this placement has the required educational and Dean s approval for the full duration if a LAT post. There is a well equipped library within the hospital grounds available for study and reference purposes. The Board's Librarian is also available if required. You will have access to a clinical supervisor (consultant within the unit). Post graduate and continuing medical education is actively encouraged and supported by Dr Jane Montgomery, Director Medical Education. There is an extensive postgraduate programme including weekly educational/audit meetings within the Department of Medicine, weekly X-ray meetings, monthly educational half days and a monthly peer review meeting. There is a weekly Clinical Meeting. Each consultant has a formal weekly Teaching Ward Round. Each month there is an PG educational half day when clinics and other sessions are usually cancelled. The Borders General Hospital is a teaching unit for undergraduate students in medicine. Study leave is a maximum of 30 days in a year (including weekends in a continuous period of leave) for purposes approved by the South East Scotland Deanery and suitable for service commitments. Study leave can be used for approved training courses. For example the GIM training days, a joint initiative of RCPE and NHS Education for Scotland (South East Scotland Deanery), attract study leave. At least six training days are held at RCPE in Edinburgh in the period of a year and protected time on the rota is identified. Study leave can also be used for the purpose of studying (with Education Supervisor approval) and/or sitting an examination for a higher qualification. Study leave cannot be carried over to the next year. An educational supervisor (a senior clinician within the surgical unit) will be appointed. 9

10 The Scottish Borders The Hospital is situated on the outskirts of Melrose in the Scottish Borders. The Borders covers a large and scenically beautiful area of the Southern Uplands of Scotland. Predominately rural, it is historically a unique part of the country, the home of the Border Reivers, where annually each town in the Borders maintain its links with the past during the season of Common Ridings. Seven-a-side rugby originated in Melrose, and the Melrose event in particular draws large crowds each year. The Borders has tremendous facilities for sport and leisure. Glentress and Innerleithen mountain bike parks are world renowned for both cross country and down hill biking. The beautiful Berwickshire coast provides options for sea kayaking, surfing, diving and sea fishing. In addition there are facilities for fishing, golf, swimming, horse riding, cricket, football, hiking and many other activities. The Borders has excellent cultural opportunities in terms of music and art societies, drama, and small theatres in Melrose and Selkirk as well as amateur opera. There are excellent restaurants, cinema, shops and an excellent Fitness Centre in nearby Galashiels. There is a purpose built nursery in the grounds of the hospital for hospital employees children. The Scottish Borders offers all the benefits of rural life with very easy access to major cities such as Edinburgh (37 miles) Glasgow (75 miles) Newcastle (75 miles). Edinburgh is renowned for its cultural activities in music, including opera and ballet, theatre, cinemas (including a film theatre) and visual arts, and of course every year there is the world famous Edinburgh International Festival and Fringe Festival. Local transport links have improved greatly. After an absence of almost 50 years, the Borders Railway has now opened, with train services to central Edinburgh running every thirty minutes (journey time 50 minutes approx). Tweedbank Station is a few minutes walk from the Borders General Hospital. On opening the railway Her Majesty Queen Elizabeth said "The Borders railway brings so much promise for sharing and invigorating this most beautiful countryside as a place to work, live and enjoy. There are rail links to the rest of the country at Berwick Upon Tweed, and Carlisle and there is easy access to Edinburgh Airport (approximately 1 hour 15 minutes) and Newcastle Airport (approximately 1 hour 30 minutes). 12 TERMS AND CONDITIONS With the exception of pay and out of hours banding the Terms and Conditions of Service are from Hospital Medical and Dental Staff And Doctors And Dentists In Public Health Medicine and the Community Health Service Terms and Conditions of Service (2007) as amended. i. The appointment will be made by NHS Borders, with participation from the Training Programme Director or a representative from the South East Deanery. ii. The full-time salary is between 31,220-49,091 basic and a 50% supplement for Band 1A/2B rota. There is flexibility and applicants who wish to work lesser hours on a part time / flexible training basis are welcome to apply. iii. iv. The rota / working pattern will be agreed in association with the Consultant Surgeon / Education Supervisor. There may be an opportunity to take part in undergraduate and postgraduate teaching programmes. v. The person appointed will undertake clinical administrative duties associated with the care of patients on the unit. vi. vii. viii. ix. The appointment will be superannuable and subject to the regulations of the National Health Service. Superannuation Scheme and the remuneration will be subject to deduction of contributions accordingly, unless the appointee chooses to opt out of the Scheme. NHS Borders is legally liable for the negligent acts or omissions of the employees in the course of their NHS employment. It is no longer a requirement of employment within the NHS that medical staff be registered with a medical insurance organisation. Medical staff are however advised to ensure that they have defence cover for activities not covered by the Board s indemnity. All entrants to the NHS Borders must be certified medically fit and the appointment is conditional on such certification. Arrangements for medical screening (usually by questionnaire) are the responsibility of the Occupational Health Service. Termination of the appointment will be subject to a 1 month notice period of notice on either side. x. For locum appointments, depending on circumstances, support 12

15 Extract from Reference Guide for Postgraduate Specialty Training in the UK 5.30 LAT appointment carries no entitlement to entry to any further specialty training programme, which must be by competitive entry LAT trainees have several subsequent career options open to them: apply for a run-through, core or higher specialty training programme apply for another LAT appointment in the same or a different specialty seek appointment to a career post when eligible to do so 5.32 LAT appointments are undertaken in approved training posts, which can contribute towards a CCT once a trainee has been competitively selected for a relevant training programme. They can also be used by doctors in submitting their CESR application LAT appointees are required to register with the appropriate College/Faculty for the post to count towards specialty training. Retrospective credit for LAT posts previously undertaken will not be granted Enrolled LAT appointments of three months whole time equivalent or more should automatically count towards training where satisfactory progress is confirmed through the demonstration of achievement of competences (usually by ARCP), or unless otherwise notified by the host locality in HEE, NES, the Wales Deanery or NIMDTA. While up to 12 months can count towards training, where it can be demonstrated that curricular progression within a governed system has occurred, further time may be counted By definition, a LAT appointment must be a minimum of three months. On occasions, a trainee working in a LAT post may not complete this time before acquiring a numbered post or further LAT appointment. Under these circumstances, a period of less than three months worked in a LAT post will not count towards training unless it is linked seamlessly (i.e. no delay between exiting LAT and commencing new post) to an appointment to a ST3 post with a National Training Number (NTN), or another LAT post. 15
