6 SECTION 3: DUTIES & RESPONSIBILITIES OF THE POST POST INFORMATION This is a replacement post based at the James Cook University Hospital, Middlesbrough and serving a population of approximately 1.3 million in Teesside, North Yorkshire and County Durham. The candidate will lead Neurorehabilitation services. The main priority for this post will be to deliver the highest clinical standards in the practice of neurological rehabilitation, for the adult (16+) population. The post holder will be jointly responsible for initiating and carrying through appropriate service developments and further improvements in these services. It is advertised on a whole time (10 PA basis), although any candidate who is unable to work full time will be eligible to apply and, if appointed, modification of the job content will be discussed in consultation with the Clinical Director and the Medical Director. Clinical responsibilities The consultant will provide a clear leadership role within the discipline of neurorehabilitation in order to deliver the best in multidisciplinary and interdisciplinary care. Within Neurosciences, the consultant will join an established MDT, providing inpatient and outpatient care, including a spasticity/botulinum toxin therapy service and prosthetic limbs service. The MDT meets formally every week to plan and evaluate patient s care and progress. The post holder will be named consultant for an inpatient caseload, and provide cross-cover for absence with their colleague. The consultant will provide an acute assessment service for early rehabilitation within Neurosciences Services and jointly oversee the post-acute ward-based neurorehabilitation service, and associated outpatient clinics. The ward is 18 beds, currently supported by one staff grade doctor. Junior doctor cover is provided by the neurosciences roster. The ward has a full team of therapists included dedicated physiotherapy, occupational therapy, speech & language therapy, dietetics and neuropsychology input. The ward is currently registered as a level 2b unit and fully participates in UK-ROC. The consultant will liaise with the various specialist teams involved to ensure smooth flow through patient care pathways, facilitate discharge and assist with the delivery of national targets. The consultant will be encouraged to develop innovative approaches to deliver rehabilitation services and provide guidance in relation to the implementation of recommendations contained within the NSF for Long Term Conditions and other relevant national policy and clinical guidelines. The consultant will provide an assessment and advisory service for inpatients at JCUH and other trusts referred for specialist rehabilitation opinion and input. Page 6 of 20

7 The consultant will assist in the development of care bundles, care pathways and treatment protocols for the rehabilitation and disability services. The consultant will be encouraged to develop specialist interest compatible with service needs. The consultant will be flexible in the work plan to accommodate changing service needs. Job Plan A formal job plan will be agreed between the appointee and the Clinical Director for Spinal Cord Injuries & Neurorehabilitation on behalf of the Medical Director for Specialist Care Centre and the Trust Executive Medical Director, three months after the commencement date of the appointment. The job plan will be reviewed annually. It will cover all aspects of a consultant s professional practice including clinical work, teaching, research, education and managerial responsibilities, including a clear schedule of commitments, both internal and external. It should include personal objectives, including details of their link to wider service objectives and details of the support required by the consultant to fulfil the job plan and the objectives. Please refer to Appendix A further details. Teaching Teaching and training of junior medical staff, therapists and nurses is encouraged. The services are becoming increasingly involved in the undergraduate medical curriculum and the teaching of medical students is also encouraged. There is opportunity to attend a weekly Neurosciences academic session (Friday morning) which includes formalised teaching for FT/core trainees, audit, journal club, clinical case presentations and neuropathology demonstrations. The successful candidate is expected to support and facilitate informal as well as formal teaching opportunities. Research Clinical research is encouraged and supported. The School of Health at Teesside University has an on site facility - the Teesside Centre for Rehabilitation Science. Neurosciences services have good links with the University of Durham (Department of Cognitive Sciences, Department of Molecular Biology), for example. Consultant Leave Advanced notice of leave (6 weeks) is required. Authorisation of annual leave will take into account the needs of the service. Any leave requested with less notice must be specifically approved by the Clinical Director and Service Manager. Rotation of Duties The successful candidate will work at the James Cook University Hospital, Middlesbrough and however as the service develops they may undertake duties delivering a satellite clinic at another site. 8 It is expected that the consultant will lead and participate in directorate and multi-disciplinary audit activity. CPD The successful candidate will be expected to take advantage of continuing professional development. The Executive Medical Director holds the budget centrally and all reasonable requests for study leave have been honoured to date and funded within the allowances contained with the Medical and Dental Whitley Council and its successors. Appraisal and Revalidation Annual appraisals are mandatory for all Consultants and are carried out by the Clinical Director. Appropriate time and assistance will be provided for preparation for appraisal and revalidation. Mentorship Newly appointed consultants are strongly encouraged to identify and engage with a mentor, and assistance will be provided with this. The Trust maintains a list of Deanery-approved mentors and more informal arrangements can also be facilitated. Managerial Duties/Service Development In addition to the expected managerial aspects of participating in/leading the MDTs and service provision, consultants are encouraged to take an active role in the wider management of the directorates with opportunities to become clinical leads and represent the service area on the various committees that exist within the Trust On-Call On call is not a core part of the job role but may be negotiable. Accommodation and Secretarial Support Office accommodation with access to a named medical secretary and pool typing support will be provided. This may be provided on a shared/pro rata basis. Consultants are usually required to share an office. A PC and access to relevant IT systems will be provided. Privacy and Dignity Responsibility for protecting privacy and dignity does not lie with one individual or group but with staff at every level. All staff members must adhere to the Policy G112 Privacy and Dignity. Hospital Infection Control (HIC) The Trust regards control of healthcare-associated infections as a high priority. The appointed Consultant will be expected to promote high standards of infection control practise among the junior doctors he/she supervises. The post holder must follow the Trust s hospital infection prevention and control (HIC) policies and procedures to protect patients, staff and visitors from healthcare-associated infections. The post holder must perform the correct hand hygiene procedures (as described Page 8 of 20

9 in HIC 14), when carrying out clinical duties. He or she will use aseptic technique and personal protective equipment in accordance with Trust policies. Records Management The post holder will ensure that Trust policy on records management is followed and ensure and compliance with the NHS Code of Practice for Records Management. Page 9 of 20

10 SECTION 4: NEUROSCIENCES SERVICES Neurosciences Services Within the Specialist Care Centre, Neurosciences provides specialist tertiary services to a population of approximately 1.3 million. Based at the James Cook University Hospital, Neurosciences includes the departments of Neurology, Neurosurgery, Neurophysiology, Sleep, and Neurorehabilitation (including Wheelchair and Prosthetics Services). Following a restructure (April 2016) this portfolio will also include Stroke Services, Chronic Pain Services, Spinal Surgery and the Regional Spinal Cord Injuries Centre and associated services. The Neuroradiology department is located within the neurosciences footprint. The close proximity of each element of the neuroscience service, as well as co-location with critical care and a full range of secondary and tertiary services, has led to excellent working relationships and, undoubtedly, facilitates high quality patient care. The clinical management structure within the Centre comprises of a Medical Director who represents the Centre at Trust Management Group level supported by Clinical Directors of Neurology & Stroke Services, Neurosurgery & Spinal Surgery, and Spinal Cord Injury & Neurorehabilitation (structure from 1 April 2016). Departments of Neuropsychology and Neuroradiology are within the Diagnostic and Clinical Support Services Centre however the working relationships with Neurosciences are well established. Neurosciences delivers outpatient care to the population through a flexible hub and spoke approach. Neurology and neurosurgery satellite clinics and neurology ward rounds are held variously at hospitals in Darlington, Bishop Auckland, Hartlepool, Peterlee, Northallerton, Redcar and Whitby, and in several GP practices. Neurophysiology provide an outreach clinic in Darlington. All remaining diagnostic and inpatient services are based at the James Cook University Hospital. The clinical area comprises of a Neurology ward (21 beds), a Neurosurgical ward (31 beds, including 8 Level 2 HDU), a Neurorehabilitation ward (18 beds), 3 Sleep diagnostic beds and a dedicated Neuroscience outpatient clinic with 10 consulting rooms. All departments are within close proximity of one another over two floors. The Directorate of Neurology has seen rapid expansion over the past five years. The 9 consultant neurologists provide general and specialist services supported by a team of Nurse Specialists in PD, Epilepsy, Multiple Sclerosis and MND. The Directorate of Neurosurgery has 2 dedicated neurosurgical theatres housed within a theatre complex of 23 operating suites. These provide all standard equipment together with Leica operating microscopes, image intensifier, Leksell frame stereotaxy, image guided neuronavigation (Treon S7), and KTP laser. The Neuroradiology department has state of the art equipment including X-ray, CT, MRI and 3D Angiography and provides a full interventional service. There are strong links with the department of Nuclear Medicine for Isotope based imaging. In collaboration with the University of Durham a 3-Tesla functional MRI scanner is available. All radiological images are stored on the Trust s PACS system and all consultants may direct access to this through their office and clinic PCs. Page 10 of 20

11 The Neurophysiology department delivers a full range of diagnostic services. There are two EEG recording suites, a multifunction room for EMG, nerve conductions and evoked potentials, and offices. There is spilt screen EEG videotelemetry linked to two dedicated ward cubicles as well as equipment for ambulatory recording. The Sleep department has significant expertise in both sleep disordered breathing and neurological disorders. A dedicated diagnostic service delivers oximetry, polysomnography, sleep latency tests, and a range of other diagnostic tests. A specialist nurse-led service provides CPAP initiation and maintenance. The Neuropsychology department has also experienced significant growth in the past 3 years. It provides comprehensive diagnostic services. Therapeutic services are also increasing concomitant with the growth of the department. The Neurorehabilitation service has its own dedicated ward for post acute inpatient rehabilitation. It provides comprehensive physiotherapy, occupational therapy, speech & language therapy and Neuropsychology support. Rehabilitation following spinal injury is provided in the Regional Spinal Cord Injuries Centre on site. 13 SECTION 5: SOUTH TEES HOSPITALS NHS FOUNDATION TRUST South Tees Hospitals NHS Foundation Trust South Tees Hospitals NHS Foundation Trust provides health care from two main sites The James Cook University Hospital in Middlesbrough and the Friarage Hospital in Northallerton along with community hospitals and integrated community services in the South Tees locality. The Trust is part of the recently formed North of England Cancer Network which is made up of the old Northern Cancer Network and the Cancer Care Alliance. The Regional Spinal Injuries Unit provides a comprehensive spinal cord injury service for the North of England (population 3.4 million). The Trust provides a range of highly specialist tertiary services extending to 1.5 million people in Teesside and parts of Cumbria, Durham and North Yorkshire with leading expertise in heart disease, cancer, trauma, neurosciences, renal services and spinal injuries. The Trust s core business is also to provide secondary healthcare to: Around 270,000 people living in Middlesbrough and the local area of Redcar and Cleveland 122,000 people in an area stretching from the North Yorkshire Moors to the central Pennines, the borders of York district in the south and the borders of Darlington in the west. The James Cook University Hospital One of Europe s most modern hospitals, The James Cook University site is the largest of its type and has a wide range of specialities. This hospital was opened in October 2003 and has a capacity of around 1000 beds. Based on the original site of South Cleveland Hospital, all acute and elective medical and surgical specialties are accommodated. Tertiary services, in addition to cardiology and cardiac surgery, include renal medicine/haemodialysis, neurosurgery, vascular surgery and the regional spinal cord injuries unit. This site contains a modern central operating suite containing 20 well-equipped theatres, with two recovery areas. This is immediately adjacent to an Adult Surgical Day Unit containing a further three theatres and also to the Paediatric Day Unit. The 16 bedded general ICU is across the corridor. Two additional theatres for gynaecology and obstetrics are in the Central Delivery Suite and two more in the Ophthalmology Day Unit. The Accident and Emergency Department on this site is the designated Regional Trauma Centre for Teesside, South Durham and North Yorkshire. The hospital has a 24-hour Acute Admissions Unit. There is a 21 room imaging department equipped with a full range of mainly digital modalities, a trust-wide PACS system and a purpose built 6 bed Radiology Day Unit. A Medical Physics Department is also on this site together with isotope imaging. Page 13 of 20

14 The central clinical laboratory provides a comprehensive pathology service with electronic links to wards and departments. Electronic requesting is being piloted. There is a purpose-built Endoscopy centre. The hospital has a comprehensive tertiary level fertility service and houses the Gynaecology Cancer Centre (North of England Cancer Network). The hospital caters for just less than 4000 deliveries in purpose built delivery and low dependency suites. The birth rate is increasing. Fetal medicine and high dependency delivery referrals are accepted. There is a regional tertiary neonatal intensive care unit with 9 intensive cots, which accepts both local and regional admissions. It supports a neonatal transport service and has a strong academic record. Any babies born at the Friarage hospital (c1100 deliveries per year) requiring intensive care are transferred to this unit. There is an established Academic Centre embracing postgraduate medical education, Nurse Training School, the School of Radiography and library facilities. It encompasses two large lecture theatres with the latest audio-visual aids and a considerable number of classrooms and seminar rooms, including ward facilities for teaching and examining. There is a new library with comprehensive IT suite. A dedicated undergraduate teaching centre and simulation-training centre is on site. An accredited major trauma receiving hospital, it contributes to UK Trauma, Audit & Research Network (UK TARN). It occupies a leading UK position in its survival rates from blunt trauma as evidenced from UK TARN. The Friarage Hospital Northallerton The Friarage Hospital at Northallerton, situated approximately 23 miles from The James Cook site, merged with South Tees Hospitals NHS Trust in April It provides a wide range of secondary services to its local population and is well known for its friendly atmosphere. A fully equipped district general hospital with around 300 beds, it provides a 24-hour accident & emergency service and offers inpatient and outpatient facilities in general surgery including colorectal, breast (including mobile screening) and urology services, orthopaedics and trauma. Medical specialties include acute medicine, diabetes and endocrinology, rheumatology, gastroenterology, respiratory and rehabilitation medicine. A midwife-led maternity unit, outpatient obstetrics/gynaecology, paediatric assessment and adult psychiatry services are on site Cardiology, encompassing CCU and cardiography, functions as a self-contained unit but is part of the cardiology directorate. The hospital is furnished with a well-equipped medical library with facilities for electronic searching of medical databases and access to the Internet. A full programme of postgraduate educational meetings is held on this site. Support services on this site include haematology, chemical pathology, histopathology, microbiology, physiotherapy, occupational therapy and speech and language therapy. Patients requiring cardiothoracic or neurosurgical services, nuclear medicine or MRI scanning are referred to other hospitals including The James Cook University Hospital and Leeds General Infirmary (45 miles). 15 The Ministry of Defence Hospital Unit (MDHU) has been based at the Friarage since July 1999 with contribution of military doctors, nurses and other staff who are integrated into the hospital s services. Education, Teaching and Research The modern Academic Centre at The James Cook University Hospital is housed within two linked buildings. They contain two large lecture theatres, which have audio-visual links. There are many seminar rooms of varying sizes, which are used for teaching, demonstrations, lectures and conferences. There is a new library which is well equipped with most of the general and specialist journals relevant to the Trust s clinical activity. There is a large IT suite of over 40 stations within the library complex. The Academic Centre also houses the Clinical Trials Unit which has recently been extended with links to the unit at the Royal Victoria Infirmary in Newcastle. Clinicians who are interested in developing a clinical trials programme are encouraged to contact the Research & Development Department. There are close links with the School of Health at Teesside University for the training of nurses and midwives, radiographers and physiotherapists. There is an active postgraduate district and divisional curriculum on site based in the postgraduate education centre under the direction of a university clinical tutor. There is an upto-date medical library within this centre. Consultants are expected to undertake activities suitable for continuing medical education in accordance with Royal Colleges. As part of the national expansion of medical student numbers, the Universities of Newcastle and Durham have formed a partnership increasing the undergraduate intake to 340 students. The first two years of the curriculum are being taught at the Queen s Campus of the University of Durham in Stockton as well as the Medical School in Newcastle. Approximately 100 students are based in Stockton and 245 in Newcastle including 25 in the accelerated programme. At the end of year 2 the entire cohort of students are divided up for their third year between base units of acute and community trusts and PCTs around the Northern Region. South Tees has been designated the lead trust for the Teesside Unit which currently comprises Bishop Auckland and Darlington, North Tees and Hartlepool, as well as South Tees and the corresponding Mental Health Trusts of Tees, Wear and Esk Valley and South Durham and the associated PCTs. Research is actively encouraged. The Trust employs several visiting Professors (Orthopaedics (2), Anaesthetics (2), Academic Emergency Medicine (1), Paediatrics (1), Medicine (1), Surgery (1) and Maxillofacial Surgery (1). The Sub-Dean of the Medical School (Professor Bilous) is also based at JCUH. As a result of these activities the Trust was designated a University Hospital in All junior doctors posts are anticipated to be compliant with the 48hour working week. It is expected that consultant staff will contribute to the education supervision, appraisal and assessment of junior medical staff and support the on call arrangements that exist in their Directorate. The Trust also supports the changes to competency-based assessments of trainees as envisaged by MMC. The James Cook University Hospital has an on site family friendly crèche. 16 Teesside is surrounded by the delightful countryside of the North Yorkshire Moors National Park, the North East Coastal beaches and the attractive countryside in South and West Durham, and North Yorkshire, with excellent rural pursuits. It also provides one of the largest petrochemical complexes in Europe and is also the major steelworks site of Corus (previously British Steel). Northallerton, one of England s best-loved country towns is on the doorstep of the Yorkshire Dales. Leisure/Recreation There are major shopping centres at Teesside Park (Stockton), Middlesbrough and Darlington. Newcastle and Gateshead Metro Centre are within reach, as are the historical Cities of York and Durham. Hartlepool Marina and Hartlepool Historic Quay and Maritime Museum are part of a multi-million pound development also only a short distance away. The historic spa town of Harrogate is within easy reach of Northallerton. There are excellent facilities in the area for most sports, including golf, swimming, squash, rugby, cricket and tennis. There is an excellent indoor tennis centre (David Lloyd) close to the Durham University campus in Teesdale (4 miles from the James Cook University Hospital) and there are excellent golf courses at Wynyard and Romanby near Northallerton with driving range facility. The Tees Barrage has allowed the creation of simulated white water canoeing and rafting centre in Teesdale that attracts canoeing enthusiasts from all over the UK. There is easy access to the coast, the North Yorkshire Moors National Park, the Pennines and Dales. There is an extensive range of recreational and cultural facilities in the immediate vicinity. The Cleveland hills offer opportunities for a variety of outdoor pursuits, including rambling, gliding and hang-gliding. There is active support for the arts in the region, both at professional and amateur level, with a wide range of cultural events and activities available. These include theatre, ballet, opera and all types of music, as well as a variety of museums. There are multi-screen cinemas in Middlesbrough, Teesside Park (Stockton) and Hartlepool Marina with bowling facilities at Teesside Park. Middlesbrough, Stockton, Yarm, Stokesley, Great Ayton and Northallerton have a wide range of family friendly eateries. The majority of small towns and villages surrounding Middlesbrough and Northallerton have excellent restaurants offering a variety of different cuisine. The enthusiastic spectator can follow all football, rugby and cricket. Middlesbrough Football Club has a superb new ground on the developing Middlehaven site, close to the banks of the River Tees (Riverside Stadium) regularly featuring Premiership football. Housing There is a choice of housing in comfortable residential areas in the many pleasant rural and urban areas in the surrounding countryside and within easy reach of the Trust. Stokesley, Great Ayton, Eaglescliffe, Yarm and the villages surrounding Northallerton offer a range of quality houses at sensible prices. A range of executive houses is available, including exclusive developments at Wynyard, Nunthorpe and surrounding villages. Marske and Saltburn offer the opportunity to live by the sea and there are many beautiful village locations at the edge of the Dales National Park area accessible from Northallerton. Page 16 of 20

17 Education There is a complete range of educational facilities ranging from nursery to higher education, with Universities at Teesside, Durham, Sunderland and Newcastle, offering a wide choice of degree courses. The local secondary schools are comprehensive and many in Nunthorpe, Stokesley, Yarm, Eaglescliffe and Northallerton enjoy an excellent national reputation with high positions in the league tables. There are also excellent local private schools in Yarm and Eaglescliffe as well as a number of first class preparatory and public schools in the area. Transport Road links are excellent in all directions allowing quick and easy commuting. Due to the good road system and ease of travel, many consultants live outside the traditional 10 miles distance. Road: The A19 links to the A1 (M) and the A66 links Teesside to the beautiful Lake District and Cumbria. Travel time to the Lake District is just 1.5 hours by car. Whitby and York are just 1 hour away Rail: Air: Darlington and Northallerton railway stations are only a short driving distance away, from which London is 2.5 hours away and Edinburgh 2 hours. Durham and Tees Valley International Airport is about twenty minutes from Middlesbrough and has frequent services to London, Amsterdam, Dublin and Aberdeen as well as a large number of holiday destinations in Europe. Newcastle and Leeds/Bradford International Airports are within easy driving distance. Page 17 of 20

The successful candidate will be offered the opportunity to undertake additional PA s in accordance with the guidelines within the new consultant contract. Note that should the successful candidate take on substantial additional teaching, research or managerial roles then additional SPA time may be included into the job plan by agreement. The post does not involve any on call commitment. MAIN CONDITIONS OF SERVICE The terms and conditions are identical to the new Consultant Terms and Conditions of Service of Hospital Medical and Dental Staff (England and Wales) and General Whitley Council Conditions of Service, as amended from time to time. 1 Any Consultant who is unable to work full time for personal reasons will be eligible to be considered for the post. If such a person is appointed, modifications of the job content will be discussed on a personal basis in consultation with consultant colleagues and the Medical Director. 2. The successful applicant will normally be required to reside within the traditional ten miles (or within 30 minutes) of their base hospital although reasonable requests to extend this are usually agreed by the Medical Director. 3. The appointee will be subject to a medical examination. 4. The current salary applicable to the post will be in line with the new Consultant Contract. Page 18 of 20

19 5. Relocation expenses will be offered and are negotiated on an individual basis with the successful applicant. 6. Secretarial assistance and office accommodation will be made available. Travel and Subsistence Travel and subsistence will be reimbursed for preliminary visits only to those candidates selected for interview. Reimbursement is restricted to two such visits, whether they are made before or after the constitution of a shortlist. In the case of candidates travelling from abroad, travelling expenses are normally payable from the point of entry into the United Kingdom.
