REGISTRATION INFORMATION All registrations must be accompanied by a check payable to MPO and postmarked on or before September 18, You may also register on our website by September 18, A refund, minus a $30 processing charge, will be given if notification of cancellation is received on or before September 18, Cancellations after September 18, 2009 and no-shows will not be refunded. If a registered person cannot attend, a substitute is welcome. You may register at the conference, but there will be a late fee of $20 in addition to the usual registration fee. *To encourage teams of attendees from institutions or organizations, MPO is offering a $10 discount for each member of a team that registers together. The team must consist of at least 5 members and all team members registration forms must be received together. A block of rooms has been reserved at Breezy Point Resort. Room rates are $89.00 plus tax per room, for up to two people per night. More than two people per night may result in additional charges; please confirm rates with hotel upon registration. This block of rooms will be held only until Monday, August 24, 2009 so make your reservations prior to that date. Breezy Point Resort will take reservations via phone at (be sure to identify yourself as a participant in the Minnesota Perinatal Organization). Guest room check in is guaranteed by 5:00pm rooms may be available earlier, check with hotel staff to confirm; check out is 12:00 noon. Conference meeting rooms are often cool. We recommend you bring a sweater or light jacket for your comfort. In an emergency, participants can be reached during conference hours by dialing and asking for the MPO Conference Coordinator. After attending the conference, participants should be able to: 1. Describe the process to implement individual and population-based systems of care that are irrespective of the consumer s race, age, culture and outcomes-based research. 2. Discuss the design plans of inpatient and outpatient care for implementation by designated healthcare professionals, which address the unique nutritional, developmental, psychosocial, nursing and medical needs of mothers and infants. 3. Develop personal goals that will promote a positive outlook on their role as an essential member of the perinatal healthcare team. Successful completion of this activity requires that participants turn in all evaluation materials for attended sessions. Nurses: This three-day activity has been designed to meet the MN Board of Nursing continuing education requirements for up to contact hours. Physicians: This activity is being reviewed for Prescribed credit hours by the American Academy of Family Physicians. Final credit numbers to be provided on site. Only registered conference attendees are eligible to attend conference sessions, meals and activities. In consideration of conference participants, we ask that children not attend any conference sessions or activities. While we sympathize with last minute schedule changes, conference attendees bringing children will be asked to leave and will be refunded their conference registration fee minus a $30 processing fee.
