Ever run a code or treat a dialysis patient, and despite everything you re doing correctly his/her presentation doesn t improve? Remember the H s and T s? Come participate and we ll discuss signs, symptoms, physical presentation, ECG changes and other things to consider.

8 Advancing Care in New York State - How our protocols have advanced for our patients presented by Dr. Dailey Medical care is in constant evolution - an EMT in 1983 could get two little rockers that went under the EMT patch - "EMT-D" and "MAST"...one is now ubiquitous and one is no longer a part of EMS care. Teams of doctors, educators, and EMS providers have been advancing the protocols in upstate New York as the collaborative protocols have developed. Some protocols have been added, some have been removed, and others have changed: cardiac arrest care in place is encouraged and termination of futile resuscitation rather than transporting patients with CPR ongoing is expected, nitro paste has been taken out of the care for acute pulmonary edema, and the removal of etomidate facilitated intubation. Bring your questions, Dr. Dailey will discuss the process of protocol development and how it impacts patients and providers. Treat the Monitor AND the Patient: Advanced Scenarios Using Diagnostic ECG Interpretation presented by Dan Batsie, BA, NREMTP ECG interpretation is an important step in advanced patient assessment and an essential element in forming a treatment plan for a wide range of critically ill patients. Although, no care should ever be based solely on an ECG, being ready to make accurate interpretations and recognize lethal dysrhythmias is a skill that requires practice. This class is designed to hone that skill. Students will be presented with real world scenarios that require rapid ECG interpretation and will work through a process of problem based learning to develop real time treatment strategies. Debriefing will focus on electrophysiology, pathophysiology and current treatment best practices. Although all providers are welcome, this class is geared toward advanced providers with ECG interpretation training. In an Instant presented by Christopher Ebright, B.Ed., NRP One time or another, we take the call for a patient that we know all too well. Sometimes it's a family member, a friend, and in one terrible instance - a co-worker. Based on an actual EMS call, this presentation correlates real timeline history, events, and preventative treatments for a fellow colleague with the underlying pathophysiology of a pulmonary embolism. Various signs, predisposing risk factors and origins of emboli formation are all tied together to give a global perspective of the acute and, in this case, fatal event that shocked and stunned an entire EMS service. Time AND Brain - Shifting Paradigms in Acute Stroke presented by Dr. Hashem Masoud Stroke therapies continue to evolve, and more patients are eligible for life-saving therapy than ever before. The recent publication of two pivotal trials expanding the treatment window will be discussed, providing a focus on revisiting the time paradigm in patient selection. Dr. Masoud will also review the neuroanatomy behind large vessel occlusion, and compare scales used to assist in the early recognition of patients that can benefit from thrombectomy. Primary Assessment and the Deadly Triad of Trauma presented by Dan Batsie, BA, NREMTP Almost every medical provider at some point learned that the primary assessment was set of linear steps focused on airway breathing and circulation. Although the ABC s still form the foundation for quality care, recent advances in trauma care have shown us that the primary assessment should be neither linear nor limited to these three elements. This class will focus on the deadly triad of trauma and discuss the role of acidosis, hypothermia and coagulopathy in the mortality of trauma patients. We will further discuss how practitioners can evolve their primary assessments so as to account for these dangerous situations earlier in the care of a trauma patient. All levels are welcome, but this class will discuss some advanced topics.

9 Managing Respiratory Distress Before ALS Arrives: Assessing and Treating the Dyspnea Patient at the EMT Level presented by Dan Batsie, BA, NREMTP Respiratory distress is both a common and frequently life-threatening complaint. Many prehospital assessment techniques and interventions are focused on advanced providers, but far too often we lose sight of the importance of good basic skills. EMTs play a vital role in early diagnosis and treatment of the dyspnea patient and proper initial care can significantly impact the outcome of these patients. This class is designed to enhance the EMT s understanding of the pathophysiology of respiratory distress and will focus on rapid assessment skills for the BLS provider. We will further discuss key interventions that can be completed by the EMT to improve the outcome of the various causes of respiratory distress. Including lung sounds. Hormones gone wild! presented by Christopher Ebright, B.Ed., NRP This presentation deals with emergencies of the thyroid/adrenal glands and the pancreas. Signs and symptoms that manifest in these patients can mimic other ailments. Knowledge of these problems will enable the EMS provider to accurately differentially diagnose these patients and render the best possible care. EMS 2018: A Positive Outlook on Our Challenges and Opportunities presented by Steven Kroll EMS providers are dedicated to meeting the expectations and needs of communities. Changes in our society have added to the complexity of fulfilling our responsibilities. Steve will discuss strategies for managing some of the challenges and opportunities facing EMS agencies in upstate New York communities. Until All the Pieces Fit presented by Christopher Ebright, B.Ed., NRP As a pre-hospital care provider, on average, you are seven times more likely to come in contact with a person on the autism spectrum. It is not one condition, but rather a broad range of conditions characterized by challenges with social skills, repetitive behaviors, speech and nonverbal communication. As a result, these patients can present EMS providers with unique assessment and management challenges. This presentation will explain the autism spectrum, the uniqueness of each part, as well as certain notable characteristics of these patients to consider when performing an assessment and rendering medical treatment. The Naloxone is In... Now What? - Care of the Overdose Patient after Resuscitation presented by Dan Batsie, BA, NREMTP The wave of overdoses associated with the opioid crisis has pushed naloxone into the hands of everyone from bus drivers to school teachers. This important countermeasure in the battle against opioid deaths is an important step, but far too often overshadows the real work that follows the initial administration of the drug. This class focuses on care of the overdose patient after the initial administration of naloxone. In particular, we will focus on the decision to administer additional doses, the side effects of naloxone and recognizing common co-morbidities associated with drug overdose. More importantly, we will describe best practice care for those patients who are successfully resuscitated and include information on social service advocacy and quality provider patient interaction and intervention for the addicted patient. All levels are welcome. REMEMBER WHY presented by Randolph Mantooth Modern technology has given emergency responders incredible tools that help save more lives more than ever before. The Internet and social media have allowed us to instantly share ideas and information with others from all over the world. Johnny and Roy would be envious. Or would they? Randy's presentation will remind us that it's not so much about the "business" as it is about people. Don't miss his inspiring presentation that will lead us back to the heart of why we do what we do.
