4 BREAKING THE SILENCE Freedom from Distressing Pelvic Floor Conditions For seven years, I suffered from urinary incontinence before I finally told my doctor. I was too self-conscious to tell anyone what I was going through even my husband or my friends. I wish I had gotten help sooner. Now, I feel like a new person. Irene*, 67-year-old pelvic floor surgery patient Your age and race are irrelevant. So is whether or not you have had children. Pelvic floor disorders such as incontinence, pain and pressure can affect any woman by limiting her quality of life. And the worst thing you can do is to stay silent. Pelvic floor conditions occur when muscles and ligaments that support the pelvic organs become weakened and lose their elasticity. As a result, the uterus, vagina, bladder and rectum can slip, or prolapse, from their normal position causing urinary or fecal incontinence, difficulty in passing urine, or a feeling of pressure in the pelvic area. Many factors, including repetitive heavy lifting, chronic constipation, pregnancy, vaginal childbirth, obesity and atrophy of pelvic tissues during menopause can contribute to pelvic organ prolapse, says Kathleen Sterling, MD, Urology, Holy Cross Germantown Hospital. Fortunately, there are effective treatments for pelvic floor disorders including pelvic floor physical therapy, medication, a pessary (plastic support device) and surgery. And, the time to seek treatment is when your condition begins to affect your quality of life such as when you are avoiding exercise or social engagements. IRENE S PATH TO NEWFOUND HEALTH Irene s story is typical. For years, she silently dealt with an almost constant urge to urinate. Before leaving home, she would calculate when to stop drinking liquids. Anxiety about having to go to the bathroom governed how she planned her day. Then, her symptoms worsened, and she started feeling pressure and bulging in her vagina. The feeling of urgency and pressure was now complicated by obstruction and inability to urinate, resulting in urinary tract infections. By finally telling her primary care physician about her symptoms, Irene s journey to freedom from her ailments began. Holy Cross physicians, Ellen Goldmark, MD, Urology, and Shobha Sikka, MD, Obstetrics and Gynecology, with a specialty in urogynecology (L-R) Ellen Goldmark, MD; Rami Makhoul, MD; Anita Pillai-Allen, MD; Shobha Sikka, MD; Kathleen Sterling, MD. and minimally invasive gynecologic surgery, determined that Irene had severe vaginal wall prolapse. This diagnosis meant the bladder and rectum were displaced, causing urinary frequency and urgency. Irene underwent robotic surgery at Holy Cross Germantown Hospital where I performed a minimally invasive procedure called sacrocolpopexy to reconstruct and repair the damaged tissues and correct the prolapse, explains Dr. Sikka. Sacrocolpopexy is considered the gold standard to correct severe vaginal and uterine prolapse. During the same surgery, I inserted a bladder sling to restore Irene s urinary functioning and prevent future incontinence, says Dr. Goldmark. Minimally invasive surgery means less pain and faster recovery. Most patients are able to go home the day after surgery. 6 Sisters Tackle Breast Cancer Prevention and Treatment Together A double mastectomy is a difficult choice for any woman to make but sisters Katie Creely and Jennifer Howard say the decision for each of them to undergo breast surgery and reconstruction was made easier with compassionate counseling and their shared confidence in the expertise of the Holy Cross breast care team. Katie, now 39, was diagnosed with early stage breast cancer in 2014, after finding a suspicious lump and being referred to Eric Oristian, MD, medical director, Breast Center, Holy Cross Hospital. Because of her family history (a paternal aunt developed breast cancer, twice), Dr. Oristian advised her to undergo genetic testing before her personalized treatment plan was created. Katie was found to be positive for the BRCA1 genetic mutation, which predisposes her to greater risk of breast and ovarian cancer. With that knowledge and the support of her husband Tom, she chose to treat her cancer at Holy Cross with the most aggressive treatment available: a double mastectomy followed by chemotherapy. Her sister Jennifer also decided to take the genetic test, although she was only 34 years old at the time and had not had any abnormal mammograms or screening results. When Jennifer tested positive for the BRCA1 mutation as well, she and her husband Brett discussed their options with Dr. Oristian and decided on a prophylactic (preventive) double Eric Oristian, MD Ali Al-Attar, MD mastectomy with breast reconstruction. There was more than an 80 percent chance I would get breast cancer at some point, most likely while I was young, says Jennifer. We want to have a family, and enjoy that time, and not be constantly scared about breast cancer, so we decided that despite losing the ability to breastfeed, our family s overall health was worth that trade-off. Both Katie and Jennifer are grateful to Dr. Oristian and their reconstructive surgeon on the breast care team, Ali Al-Attar, MD, Plastic Surgery, for working with them and their families in scheduling treatment and surgery. In fact, the sisters were able to have the second phase of their breast reconstruction surgery on the same day. It was incredible to be able to go through this difficult time with my sister, says Katie. I felt like everyone was trying to make things easier for us, and worked together to get to know us and our families, and what we needed. 8 EXPERT CARE FOR COMPLICATED PREGNANCIES LIKE DESMA S I was in a beautiful private delivery suite at Holy Cross Germantown Hospital and the care team was excellent, says Desma, shown here with daughters Alyssah and Alaiviah. Four years ago, New Yorkers Desma and Moses Nesmith and their now 6-year-old daughter Alaiviah embarked on a new journey. They picked up their lives and moved south to Montgomery County and to exciting opportunities for their family. Moses made great strides in his banking career. Desma was working as a nursing assistant and on her master s degree in health administration. Best of all, by mid-2015 they were expecting their second child. Halfway through the pregnancy, Desma experienced unexpected complications, and when premature labor sent her to Holy Cross Hospital s emergency room more than once, Aliya Poshni, MD, Obstetrics and Gynecology, was at her side. Desma was beginning to dilate long before she should, Dr. Poshni explains. My goal was to delay delivery for as long as possible. So I put Desma on bed rest weeks before her due date. We were nervous, but I felt so confident about my doctors and I was looking forward to giving birth close to home at Holy Cross Germantown Hospital, Desma says. Desma had also developed high Eric M.
