4 At Regional Hospice and Home Care of Western CT we help patients and families discover what end-of-life care can be what can be gained, what can be realized and what can be recaptured. Mom s greatest joy is her family and talking with others about her life story. With Regional Hospice and Home Care, she enjoys being surrounded by everyone she loves. Our team of doctors, nurses, hospice and home health aides, clergy, social workers and volunteers are all focused on giving you and your family the physical and emotional support needed in order to live those special moments together. We provide expert care in pain control and symptom management so that you live more fully and spend quality time with each other. Real hope for peace of mind.

8 The right time to begin hospice care is personal and can only truly be determined by you, your family, and your physician. We encourage you to discuss hospice with your physician and ask for a referral if you feel it is right for you. Hospice is available to anyone with an advanced illness and a limited life expectancy of six months or less who has chosen comfort care treatment for care, not cure. Hospice care is a covered benefit under Medicare, Medicaid, and many private insurance carriers.
9 Any person with a life-limiting illness and life expectancy of six months or less, regardless of age, is eligible for our programs. Many different diagnoses may be considered, including Alzheimer s disease and related dementia, pulmonary and heart diseases, and cancer. Early referrals are vitally important the more time Regional Hospice and Home Care has to work with you and your loved ones, the more you will benefit from all of the services we provide. Anyone can make a referral a patient, family member, physician or friend. Our services include: An individualized care plan developed with you by the Regional Hospice and Home Care team that meets each patient s needs for pain management and symptom control. Clinical staff available 24 hours a day, 7 days a week for advice or counsel. Emotional, psychological and spiritual support. Needed medications, medical supplies and equipment. Support for loved ones on how to care for the patient. Special services like speech, physical, and occupational therapy when needed. Hospice care in a skilled nursing or assisted living facility under contract with Regional Hospice and Home Care. Short-term respite care. Bereavement support and counseling. Trained Hospice Volunteers available to give caregivers a break, for companionship, to run errands, or share special skills including music and integrative therapies such as massage, reiki, and pet partner visits.
10 What Palliative Care means to you Palliative Home Care is an option for those with a serious or advanced illness, that integrates expert pain and symptom management. When hospice care is not the right option, Palliative Home Care can provide patients with relief from the symptoms, pain and stress of an advanced illness. The Palliative Home Care Program works in collaboration with your physicians. This care is appropriate for patients of any age, who are homebound and who have a skilled need for nursing care. Palliative care is especially helpful when symptoms are difficult to control. Palliative Home Care also can offer psychosocial support for both patients and families through social worker and clergy services. Other services such as physical therapy, speech therapy, home health aide services and occupational therapy are also available as appropriate to the patient s plan of care. A Palliative Home Care consultation can be provided at any point during an advanced illness. We believe through skilled and supportive care, people can approach an advanced illness with comfort and dignity.
11 Our Palliative Home Care Program offers specialized medical care and treatment that allow people to live their life more comfortably with relief from the symptoms and pain of an advanced illness. This results in better quality of life and plans for your future needs.

There is hope in knowing that Regional Hospice and Home Care s Healing Hearts Center will be there to support your family and friends as they cope with their feelings of loss. Our professional staff and trained bereavement facilitators will be there to listen and offer comfort.
