2 Welcome from Nnenna Osuji I hope this edition of GP e-news finds you well as we near the end of this eventful year. We know that every NHS Trust has a place at the heart of its local community, but never more so than when something tragic happens, such as the Croydon tram derailment near Sandilands on 9 November. Thirty one people came to Croydon University Hospital by the emergency services following the incident, and a further seven brought themselves to the hospital for care. The response, from all of our staff was calm, professional and unified, but above all it was caring, as we looked after those injured from the crash and their families. The incident received a huge level of media attention, including questions from the Evening Standard on how we coped with something affecting so many in our community. In this newsletter, some other things we look at are: 1) An update on our clinical administration transformation. 2) Our maternity service. 3) Our one year anniversary of the Edgecombe Unit and a pilot which sees LAS bringing patients directly to the service. 4) Our cancer waiting times being the best in London. 5) Improving care for our dementia patients, 6) our quality summit, held to improve patient safety and 7) a published Sustainability and Transformation Plan for South West London. 3 Improving Access The PODS are coming early next year! We know, from speaking to local GPs, that we need to make it easier for you to access information about your patients so we are changing the way work! From early 2017, we are bringing our clinical and administrative teams together by groups of specialties to create Point of Delivery teams or PODs. The PODs will make it easier for us to keep in contact with you and for GPs to access information about their patients. Bringing clerical and admin teams together within their specialties will also help us to manage patient journeys and waiting times much more consistently right across the Trust. Contact telephone numbers and s for clinical specialties will change very soon To make these improvements we will need to change our contact telephone numbers and addresses. This will give you one consistent point of contact for each specialty, which will be grouped by PODs. We will share all of these contact details with you early in the New Year before making these changes. Look out for more information in the next edition of GP e-news.

4 Service in focus Nine out of ten women would recommend having their baby at Croydon Health Services Nine out of ten women surveyed about her birth experience would recommend having a baby at Croydon Health Services to their friends and family. Friends and Family tests are carried out at four points in women s care antenatal, birth, postnatal ward and postnatal community care. Taking into account all four aspects, the overall number who would recommend CHS this year has consistently been above 90%. In fact, every woman asked across many months of the year, would recommend our birth services to their friends and family (100% FFT, Jan-Mar 2016, and May 2016). Every year, more than 4,000 babies are born at Croydon University Hospital or at home with the support of our Crocus Home Birth team. The Trust has employed more midwives to meet the national gold standard of one midwife to 28 births. We have also strengthened senior leadership within the maternity team to ensure women referred to CHS get consistently high-quality care. Following major refurbishment: the hospital s birth centre and delivery suite have new facilities include birthing pools, larger rooms with beanbags and yoga balls to aid active birth and calm relaxing lighting

5 In the postnatal ward, there is a dedicated ensuite facility for women who have had a caesarean There are reclining chairs next to each bed so that partners can stay overnight And even free wi-fi The CQC rated maternity and gynaecology services at Croydon Health Services as "good" overall after a routine inspection in Oct The CQC reported: "There had been continued and sustained improvements to [the Trust's] maternity services. Women who had previously given birth at the hospital commented positively on the improvements to maternity services and told us they were caring, responsive and knowledgeable." Croydon has also been chosen as one of the pioneer sites giving women greater control over their care. The scheme, announced by NHS England, means that pregnant women in Croydon will be offered a personal budget and be able to use it to make choices around their antenatal, delivery and postnatal care. Seven pilot sites were announced across the country. Choosing to have a baby with Croydon Health Services We have made lots of information available for expectant parents to help them make an informed decision if they choose to have their baby with Croydon Health Services. This includes the care we offer when having a baby at home, in our homely birth centre, or our delivery suite for more complicated pregnancies and births by Caesarean section. To find out more you are welcome to attend our maternity GP education evening on 23 February To book your place, please CH- Latest News Our Edgecombe Unit is one year old and is celebrating with a new service Our Edgecombe Unit is now one year old and has cared for some 27,000 patients. It has been instrumental in helping to ease the pressure in the Emergency Department to improve our performance overall throughout the year. Last year, we asked staff at CHS what we could do differently to manage the busy winter months. The Trust s Acute Medical Consultants suggested combining four services into one (Ambulatory Emergency Care Unit AECU, Rapid Assessment Medical Unit-RAMU, Acute Care Of the elderly Unit Frailty Unit and the COPD hot clinic. The Edgecombe Unit means that patients, especially the frail and elderly, now have rapid access to expert medical care without having to wait in A&E. When patients who need any of the services that Edgecombe provides arrive at A&E, or are referred direct from their GP, clinical staff fast-track them straight to the Unit, where they are met by an experienced nurse or and medical team. To celebrate its first birthday in November, the

7 Edgecombe Unit has begun piloting a new initiative which sees London Ambulance Service (LAS) bringing patients, where appropriate, directly to the RAMU (Rapid Assessment Medical Unit), rather than going to the Emergency Department. Led by Dr Reza Motazed, the new service is aimed at further improving patient experience by having patients at the right place at the right time with early consultant input. The referral process happens directly between LAS paramedics and the RAMU Consultant. This is completely unique in the NHS. Currently this service is available Monday to Friday 9am 6pm. The pilot is being monitored closely before it will be fully evaluated to consider the next steps. How GPs can refer to the Edgecombe Unit GPs can refer patients directly to a single point of referral on the ambulatory emergency care unit and rapid assessment medical unit (RAMU) number, which is The Ambulatory emergency care unit is open from Monday to Friday, from 9am to 6pm and the rapid assessment medical unit (RAMU) is open 24 hours a day, seven days a week. The COPD hot clinic is open Monday to Friday, between 9am to 5pm. Referrals for both remits of the ACE (Acute Care of the Elderly) service can be made Monday - Friday from 9am- 5pm. The age criteria for the service has been lowered from 75 to 70.

8 CHS best in London for cancer waiting times Recent figures show that CHS has met all cancer targets, even seeing many urgent cancer referrals within a week halving the national expected waiting time! Published data shows that CHS was the top in London (September 2016) for patients beginning their first cancer treatment within two months of being urgently referred by their GP (62 day standard). For urgent referrals for suspected cancer, we are even managing to see 40 per cent of cases within seven days this is half the time specified in national standards. Our cancer performance over the last three months: 96.8 to 99% of patients referred with suspected cancer were seen within 14 days above the national standard of 93%; 96 to 100% of those diagnosed with cancer started treatment within 31 days; 88 to 100% of people with cancer were treated within 62 days of their initial urgent referral by a GP; at 94.6% - the best in London in September The national standard is 85%. Improving care of patients with dementia One of our elderly care wards (Queens 3), along with our Workforce Development team have piloted a This is Me Patient Passport system, as part of a staff training programme, to enhance care for all patients with a formal diagnosis of dementia. This is Me, a national initiative, contains important information on what works best when a person is anxious or upset. It can help in both the engagement of our patients and the de-escalation and reduction of distress, often experienced by people with dementia. The passport can also reassure family members that all efforts are being made to care for their loved ones well. Other patient experience improvements in recent

9 months include regular lunch clubs for inpatients and new, easy to understand information included in our 'Your stay at CUH' inpatient booklet. Local children look inside our X-ray department for World Radiography Day To celebrate World Radiography Day, on Tuesday 8 November, our X-ray department at Croydon University Hospital opened its doors to some young visitors, who had a chance to learn about bones and how X-rays are taken. The rare opportunity presented to children in the local community, and accompanying adults was a chance to also discover the important role that radiographers at the Trust play in helping diagnose and treat patients. More information on our diagnostic imaging service can be seen here. Our new Emergency Department for Croydon Our new Emergency Department for Croydon, due to open in 2017 is progressing at speed. Our clinicians, who designed the new facility went to check on its progress. Some staff are pictured in the soon to be resuscitation area, which will have better sight of all patients. The new Department, when complete will have: a dedicated children s emergency care department, with its own reception, waiting room and play area;

10 more beds to treat the very ill or injured; a bigger resuscitation unit to care for critically-ill patients; doors on cubicles rather than old-style curtains, to improve privacy and dignity for patients; a brand new Urgent Care Centre, for more minor injuries and illnesses For more information on the changes to the Emergency Department, follow #NewCroydonED on Twitter or visit Sustainability and Transformation Plan for South West London published The Sustainability and Transformation Plan for South West London has been published, along with many others from across the country. STPs are being developed locally across the NHS to address the challenges of meeting rising demand from an ageing population within the funding and resources we have available. This means we have to build our services around the needs of local people, and invest more in out-of-hospital services to make it easier to access the right care and support. The full details of the STP are still being developed by clinicians across South West London and these will be subject to full public consultation. Clinicians have suggested that we need more clinical networking and possibly one less hospital providing acute services like A&E, obstetrics and specialist paediatrics. Equally, they have not ruled out retaining these services in all five acute hospitals in South West London. Croydon clinicians will be very much involved in the next phase of modelling to ensure that any proposed changes are safe and in the best interests of our patients and community. The STP does not affect the ongoing construction of our new Emergency Department at Croydon University Hospital, which will open in late 2017.

11 Spreading best practice & learning from mistakes Improving quality and safety at the Trust Recently we commemorated National Clinical Audit awareness week, by promoting our quality improvement work, to encourage staff to use clinical audit as a tool and to become patient safety champions. During Clinical Audit awareness week, our medical director Nnenna held a Quality Summit at the Trust, with more than 60 staff in attendance. The main theme was Learning from Never Events. At the Summit, three Never Events that had occurred in the Trust and the learning and action to prevent these events from reoccurring were looked at. The importance that the WHO checklist played in these incidents was discussed and the changes that have been put in place since their occurrence. The use of a patient story to make learning real and relevant to staff, along with audience participation, achieved with a real time, electronic interactive feedback tool, helped to make the Summit a success.
