CAPG Names Facey to its 2016 list of Elite Performers We're proud to announce that this month, the California Association of Physician Groups (CAPG) released the results of its seventh annual Standards of Excellence (SOE) member survey. Facey Medical Foundation again received the prestigious SOE status of Elite, as we have every year since The SOE program inventories the tools used by sophisticated health care systems to deliver the Institute for Healthcare Improvement s Triple Aim of better patient experience, better population health, and better affordability. Some of the initiatives and tools at Facey that contributed to earning Elite Status with CAPG include: 1. A coordinated effort between Facey Medical Foundation s Case Managers and Facey Medical Group s Hospitalists to develop an effective transition of care of patients from the hospital to the primary care providers, most of whom are in Patient Centered Medical Homes. 2. The expansion of our hypertension initiative, utilizing top of license concepts to allow greater medical assistant involvement in helping our patients control their blood pressure. 3. Use of our patient registry to identify patients who need preventive care services such as mammograms, colon cancer screening, and childhood immunizations or needed care for patients with chronic conditions such as diabetes, affording them access to our extensive diabetes education and wellness programs. 4. An extensive and transparent patient satisfaction survey program that provides feedback to all providers and caregivers, coupled with our training programs in patient communication and customer service. Our journey to create an integrated health care system that benefits the communities we serve has come a long way We're staying focused on the patient experience with our PCMH roll-out and HRO training programs. Every caregiver at Facey plays a part in this measure of our success! Thanks for all of your efforts over the last year. Teresa David Chief Operating Officer New Tarzana Clinic Set To Open In Late 2016 Everybody loves an upgrade. Later this year, we're bringing a better experience to Tarzana with the opening of a brand-new Facey clinic inside the soon to be completed Ventana Medical Building at Ventura Boulevard. The new clinic will combine Facey's existing Tarzana ob-gyn and pediatric clinics and add primary care, lab, radiology, and space for additional providers in the future. Seven physicians will work out of the new office when it opens. Three Northridge doctors will join the existing Tarzana team: Dr. Daniel Lewis, Dr. Jeffrey Shukhman, and Dr. Dafna Trites. Additional features of the environmentally-conscious Ventana Medical Building include a large onsite parking structure, a pharmacy, and a cafe. Last month we recognized and celebrated our caregivers who received the most nominations during 2015 at a special luncheon at the Odyssey restaurant. These "All-Star" caregivers continually stood out to patients in 2015 because of their positive attitudes and outstanding customer service Over 1900 nominations were submitted in 2015 and 500 of those were for this amazing group of 21 people. 3 Meet Our Newest Providers Nona Ahankoob, M.D. Porter Ranch Plaza Internal Medicine Dr. Ahankoob is the newest member of the Porter Ranch team. She did her residency & internship at UCLA Olive View and went to school at Northwestern University. Adriana Cedro, NP Mission Hills Internal Medicine, Spanish Adriana Cedro is a Family Nurse Practitioner, board-certified by the American Association of Nurse Practitioners. She graduated from California State University Los Angeles and completed her training in East Los Angeles. Her scope of practice includes health promotion, disease prevention, diagnosis and management of common and complex health care problems across the life-span. Adriana believes that strong trusting relationships between healthcare providers and patients and their families lead to better health outcomes. Brian Chang, M.D. Mission Hills Ophthalmalogy, Mandarin, Spanish Dr. Chang practices comprehensive ophthalmology, providing medical and surgical treatments for eye conditions such as cataracts, glaucoma, and diabetic eye disease. Dr. Chang completed medical school and his ophthalmology residency at UC San Diego, where he also served as Chief Resident. He grew up in Taiwan and his hobbies include hiking, reading, and spending time with his wife, one-year-old son, and threeyear-old dog. Tom Chiang, M.D. Mission Hills Infectious Diseases Dr. Chiang was trained at NY Presbyterian Hospital-Columbia/Cornell. After completing his infectious disease training, he spent the next eight years at the Veteran's Affairs New Jersey Healthcare System where he was Assistant Professor of Medicine at New Jersey Medical School. During this time, he also dedicated himself to research on carbapenem resistant Enterobacteriaceae (CRE) or commonly known as the "super bugs." He is an expert in this field and has numerous peer reviewed publications. For the past two years, Dr. Chiang has been the medical director at the AIDS Healthcare Foundation (AHF) in Beverly Hills. Besides taking care of HIV positive patients he also treated patients with viral hepatitis and was involved in many clinical research trials. Rohit Kedia, D.O. Mission Hills & Valencia Specialty Center Endocrinology Dr. Kedia grew up in Cincinnati, Ohio before going to medical school in Pennsylvania. After completing his internal medicine residency at Wright State University in Dayton, OH he then went to the University of Nebraska for his endocrinology fellowship training. Dr. Kedia has special interest in the thyroid gland and does fine needle aspirations as well as treat thyroid cancer. In addition to treating diabetes, he also is well-trained to treat pituitary, parathyroid, adrenal and bone metabolism diseases. He prides himself in listening and creating goals with the patient that will empower them to be healthy. He enjoys exploring the great state of California while spending time with his wife and young daughter. Mya Levy, M.D. Porter Ranch Plaza & Valencia Specialty Center Urology Dr. Mya Levy is excited to be joining Facey after completing her fellowship in Reconstructive Urology at the University of Minnesota and residency in Urology at UCLA. Her scope of practice includes male and female voiding dysfunction, incontinence, erectile dysfunction, urethral strictures and neurogenic bladder. Her surgical interests also include upper and lower urinary tract and genital reconstruction due to congenitalism, trauma, prolapse and radiation for both men and women. Mya takes pride in her work and is looking forward to collaborating with you in the care of your patients. Lauren Lubin, M.D. Burbank Pediatrics Dr. Lubin was born and raised in NY. Dr. Lubin moved to Los Angeles to complete her pediatric residency at LAC+USC. Since then, she has served the pediatric community in Burbank and the surrounding area. As the daughter of two elementary school teachers, Dr. Lubin focuses on educating her patients families and on preventive medicine. When not in the office, Dr. Lubin will be found most often on soccer fields throughout Southern California cheering on her two sons. She also enjoys running, Zumba, pilates, traveling and singing in a choir.

4 Mina Moussavian-Assadi, M.D. Tarzana Pediatrics, Farsi Dr. Moussavian received her medical training at the University of Montpellier School of Medicine in France and served her residency at the Monmouth Medical Center in New Jersey. Prior to joining Facey, she worked as a managing partner at the Pediatric Associates of Hampden County (Massachusetts) for 13 years. She believes in building strong relationships with patients and parents by providing compassionate care and raising their awareness around their health concerns. Away from the office, Dr. Moussavian enjoys spending time with her husband and son. She loves traveling, reading, swimming and eating ice cream! Jeffrey Shukhman, D.O. Northridge (Tarzana in Fall 2016) Internal Medicine Dr. Jeffrey Shukhman is a graduate of Midwestern University-Arizona College of Osteopathic Medicine and completed his residency training at Cedars-Sinai Medical Center. He enjoys treating a a wide variety of conditions, ranging from comprehensive preventive medicine to complex acute and chronic disease management. As an Osteopathic physician, he also enjoys performing osteopathic manipulative medicine to treat common musculoskeletal complaints and add to standard medical treatment. Dr. Shukhman believes in a holistic approach to patient care, with an emphasis on compassion and finding the root of every problem. When not practicing medicine, he enjoys playing hockey, spending time with his family and traveling. Dafna Trites, D.O. Northridge (Tarzana in Fall 2016) Ob-Gyn Dr. Trites grew up in the San Fernando Valley and has been a practicing Ob-Gyn here since She is skilled in providing a broad range of Ob-Gyn services (including the latest in minimally invasive and roobotic-assisted procedures) with compassion, patience and kindness. She prides herself on careful, expert attention to all of life's stages, has a keen interest in menopausal therapy, and integrating other forms of complementary medicine as healing tools. She also enjoys spending time with her family and two children, cooking and traveling. Building a Positive Environment Together By Gregory Kirsch, Director of Mission Leadership As we enter summer, many things may stir children out of school, less traffic at rush hour, extra time for family, friends, self, vacation, days with more sun and a host of other positive opportunities. Recently I was reviewing the book, The Why of Work, by Dave Ulrich and Wendy Ulrich. One chapter focuses on the positive work environment in action. The authors biases are rooted in leadership actions, but still let s consider for a moment their research: Pay attention to the work environment as patterns of how things are done. Listen for cynical or deficit-driven routines vs abundant affirming attitudes Pick a couple deficit driven routines and focus on their movement to positivity Ask new Caregivers to your team and work environment their impressions of what is positive and what is not (even ask a seasoned Caregiver the question) Leader and team members make public statements as a team about shared commitment to shaping a positive work environment. Our tradition is rooted in engagement at the level where the relationship takes place Each Caregiver has the power to shape our daily work into a positive experience. How might these ideas help you shape a positive environment? New FMF Intranet Site Is Now Live On July 5 th Facey workstation web browsers began directing users to our new Facey Intranet, which is built on the same foundation as the current Providence Intranet. More than just a switch to the Providence look and feel, this change improves the overall flexibility of our intranet. The upgraded site introduces a new feature: content owners. Select caregivers will be able to manage documents and information resources that are "owned" by their teams. Content Owners will also be responsible for keeping this information up-to-date.

5 "Everyone s feedback about the design & functionality of the new site has been very helpful," says Applications Analyst Bryan Fox. "I'd also like to give special thanks to the Managers who guided us in build-up of each site." HRO Update: Training Classes Roll Out Across Clinical Sites HRO training classes are being scheduled at many of our clinics over the next two months. This four-hour program is mandatory for all caregivers and physicians at Facey. If you're signed up for an upcoming session, please arrive promptly! You will be asked to attend a future session if you are not seated in class within 5 minutes of the scheduled start time. Remember, one component of the 2016 Caregiver Incentive Program is to train 60% of Facey caregivers by the end of Your participation counts! Canyon Country Team Takes Top Spot at Golf Tournament The 2016 Facey Golf Invitational was quite a success with 41 players participating this year under perfect conditions! The lowest score of -9 was achieved by three teams, with tiebreaker going to the Karen Henriquez Foursome (Karen Henriquez, Tim Ornales, Glenn Mitchell, and Frank Rodriguez). Congratulations! They will be immortalized on our plaque of annual winners. Closest to the pin went to Linda Garcia and Michael Luna, while longest drives went to Bjorn Gustavsson and Lonnie Aguinaga. There was also a special appearance by the Avelino family, lead by Dr. Chris s daughters, Katie May and Jasmine. Their 2 over par was quite impressive! Thanks to all who participated and we hope to see you next year. Dear Facey Porter Ranch, Dr. Alaleh Sareh was very responsive to all of my questions and went out of her way during my visits. After I told her about the pain that I experienced in my ears she examined me and ordered tests. She was very knowledgeable and I felt very at ease with her No more ear pain! I definitely appreciated her expertise and she was very attentive to my needs. Thanks Dr. Sareh. I am now sending the rest of my family to you! Jon Dear Facey Canyon Country, Today this Facey location has earned five stars in my opinion. They were very knowledgeable and experienced in the lab. The medical assistant who check me in was friendly and pleasant. Nurse Alicia was thorough and understanding. Dr. Hasratyan was extremely professional, friendly and caring as usual, which explains why her service is full and she can no longer take new patients. She sees almost our whole family and we are blessed to have her as our physician. While I do enjoy my time with the good doctor, I don't plan on seeing her again until this time next year. Tina

6 Dear Facey Copper Hill, I was so pleased with my first visit with Dr. Daisy Markley today. She is personable and patient, taking time to go over my concerns and answer my questions. She is very knowledgeable in her field and was open to talking about more natural, as opposed to solely allopathic, means to good health. The front desk gals greeted me with a smile and were very friendly. Dr. Markley's nurse was also wonderful (sorry I didn't get her name). I had been with my previous doctor for about 20 years and recently felt it time to make a change. Good decision! Very impressed with Dr. Markley and her staff!
