15 From Fee For Service to Managing Populations 29 Lessons Learned There needs to be strong collaboration and shared vision between administration and physicians to design a system that accomplishes the 3 Wins. Empowering staff in a Team Based Care model is the best way to accomplish effective population health management, and to share the care for complex patients. Team support for EHR work is needed to alleviate clinician burnout. Team based care is an effective way(the best way?) to put joy back into the practice of medicine, and leads to better patient care. 15
