2 CARER SUPPORT & ASSESSMENT CO-ORDINATOR (Rapid Response Support-Carers & Older People) JOB DESCRIPTION Hold in house expertise on behalf of Carers FIRST engaging in national best practice, working collaboratively with statutory and voluntary sector partners Build an understanding of Carers / Older People to identify support needs and keep abreast of changes within this field of support Attend training opportunities to broaden skills and experience Network within the locality to gain a knowledge of the support available in the wider community Work within the health environment, working closely with health and social care colleagues to identify Carers / Older People and provide them with navigation through the system Professionally present the face of Carers FIRST within the hospital and community health settings Provide a single point of access for professionals to provide them with information and advice around support available and promote referrals into community services Work within the multi-disciplinary discharge team to assist seamless discharges Develop strong working and communication relationships with the health and social care professionals Provide guidance and training to enable health and social care professionals identify Carers/ Older People that would benefit from the service Provide a personalised service to Carers/Older People based on the principles of trust, respect and dignity Provide Information, Advice and Guidance to Carers /Older People to make informed decision about their situation Using assessment and triaging tools provide Carers/Older People with support that is proportionate to their needs Maintain a professional relationship with Carers /Older People, enabling them to build trust in the quality of the services Carers FIRST provides Conduct carers assessments and other initial assessments to identify need and provide appropriate support planning and reviews Using a variety of Assessment tools, assess the needs of the Carers /Older people, providing support that is appropriate and proportionate to their needs Conduct high quality Assessments in the Carer / Older Person s home environment to assess their need for support to prevent,or delay, hospital admission Utilise non Statutory assessment tools (Carers Star / Older Person s Star) to understand needs and provide them with support and guidance Provide Carers/ Older people with appropriate support plans that enable them to remain in their own homes Conduct reviews of the assessments and support plans that allows changes in need to be recognised and relevant support put in place
3 Provide a responsive and reliable service to help prevent Carers / Older People that are in need of care from requiring residential or hospital stays Take referrals for the Rapid Response Service - Carers and Older people from professionals (Monday Friday) Contact Carers / Older people within 24 hours to assess the level of support required Conduct home visits as required to fully assess the support needed to prevent hospital admission Work with the team to co-ordinate and broker for practical support in the home to maintain the Carer / Older Person s independence and reduce likelihood of hospital admission Work with colleagues to identify providers within the DGS area that can offer high quality support to individuals in their own homes Develop partnerships with a range of providers that are able to deliver support services across the geography to meet a variety of support needs Build on existing processes to manage the brokerage of services within a 24-hour time period Work with colleagues within the Hospital Discharge Teams to identify people that would benefit from the service to encourage timely hospital discharge Promote the service within the hospital environment and to colleagues to encourage referrals Identify through initial assessment what is required to enable a timely discharge home and broker / put in place appropriate support services Work with carer support and care navigation services to ensure that ongoing support is offered once the service ceases Work within the Carers Hub To work, as required, to deliver the Carers Hub triage system Develop a strong understanding of the database to enable accurate reporting, ensuring that accurate database records are maintained and provide evidence to report on the services we provide Record all contacts with people accessing the service onto the database daily to ensure that information about the support given is up to date Utilise the database to build an understanding of the support given and identify areas of need Keep appropriate records and take part in regular supervision, appraisal and training Accurate and factual accounts of interventions and support given are maintained at all times All contacts with those accessing the service are recorded on the Blue Box database Participate in a monthly supervision with line manager to discuss workload, the support of people using the service and potential role development Participate in a yearly appraisal Maintain personal development and knowledge by attending relevant training Use outlook calendars and s to maintain contact with colleagues within the Community Working Model To work as part of the Carers FIRST organisation and from time to time undertake work as directed by line manager Carers FIRST expects all staff to be flexible to the needs of the service As the needs of the organisation change from time to time, staff are expected to be open to changes in their work to support such changes Staff are expected to be supportive of their colleagues and, if appropriate, to take on additional tasks to support the team
4 Work within all Carers FIRST policies, procedures, accreditations and standards All staff should work within the ethos of the Charity, showing respect to staff, professionals, carers and members of the wider community PERSON SPECIFICATION SKILLS AND EXPERIENCE A good all round education including GCSE s (or equivalent) or NVQ level 2 or have extensive experience in a similar role. Working knowledge of social care and health, particularly as it relates to working with carers and adults Knowledge and experience of working with adults and children Experience of working within a health or social care setting Experience of conducting holistic or person centred assessments Experience of supporting people 1-1 and in groups Experience of providing emotional support Ability to work within professional boundaries Ability to work as part of a team Highly competentin own use of IT (Microsoft Office Applictaions Word, Excel Outlook and mobile telephony Experience of multi-agency working Ability to compile records and reports and gather data as evidence to develop services Good geographical and working knowledge of Carer support within the locality Desirable PERSONAL QUALITIES Excellent interpersonal skills Good public presence Able to stay calm in a crisis and be measured in your response Good organisational and time management skills Commitment to excellence Values people and the contribution of all stakeholders High expectations of self and others Non-judgemental Supportive Commitment to values Focussed Proactive
