1 Second Annual Meeting of the Collaborative Care Learning Network February 23-25, 2017 University of Texas Health Science Center San Antonio San Antonio, Texas February 23 Thursday Afternoon Preconference Workshop (UTHSCSA Emergency Department Conference Room) 2:30-5:30 Preconference Workshop Introduction to Collaborative Care The preconference workshop is designed to warmly welcome newcomers into the Collaborative Care Learning Network. Of course, everyone else is welcome, too! The workshop is a great way to learn about collaborative care, make new friends, and establish shared foundations for the meeting. During the workshop, mentors with experience in collaborative care will facilitate group discussions and simulations. A copy of the Field Guide to Collaborative Care (Paul Uhlig and Ellen Raboin, 2015) will be provided to all participants. The workshop will highlight essential principles of collaborative care and connect new attendees with supportive colleagues. There is no extra cost for the workshop. Joanne and her ACGME colleague, Bud Baldwin, were pioneers in establishing collaborative care in their respective fields of Gerontology (Joanne) and Pediatrics (Bud). Joanne will talk about these early experiences with collaborative care, 1

2 reflecting about lessons learned and what the future may hold including promising benefits and unsolved challenges of this still-pioneering work. Mary and Erin will demonstrate how HealthSim United weaves this principle into every interprofessional simulation activity. 2

Facilitators Jane Taylor and Jeff Brown This reflective session will continue developing the Things that Matter Checklist" that was started earlier in the day. The goal of this session is to enrich and deepen the "Things that Matter Checklist" based on the presentations and discussions we have had throughout the day. The session will utilize a small group/large group discussion format to reflect collectively on what was seen, heard, and felt during the program presentations. The Things that Matter Checklist will be refined and examples from the presentations will be highlighted as part of the discussion process. 5:00-5:15 Special Presentation: First Footprint Award 2017 Presented by Paul Uhlig and Ellen Raboin The First Footprint Award honors people who have made important early contributions to the establishment of collaborative care in their care environments. Recipients of the First Footprint Award for 2017 are Kana Kornsawad and all members of Team Awesome for their dedicated work implementing collaborative care on the Hospital Medicine Service at University Hospital in San Antonio, January :15-5:30 Restaurant Choices and Transportation Planning for Group Dinners Facilitator Rebecca Eschete of CE Group 5:30 Adjourn for Group Dinners 3

People will form into dinner groups and make transportation arrangements together at the end of the meeting day on Friday, facilitated by Rebecca Eschete of CE Group. Dinner and transportation costs will be the responsibility of each person. February 25 Saturday Morning (UTHSCSA Emergency Department Conference Room all events) 7:30-8:00 Continental Breakfast (provided) 8:00-8:10 Welcome/Reconvene 8:10-8:30 Appreciation Exercise Facilitators Ellen Raboin and Ron Estrella We will begin this last day of by taking time to invite people to connect with and thank others whose presence and contributions have been particularly meaningful to them during the meeting. 8:30-9:30 Patient and Family Perspectives Moderators Sean Garcia and Irini Kolaitis Collaborative care is best differentiated from traditional care by the deeper ways that patients, families, and health professionals relate and connect. As these connections develop, new relationships have the effect of shifting the focus of care from medical interventions, to restoring and enriching lives of everyone involved in reciprocal ways. 4

This session looks at two research projects designed specifically for collaborative care, in San Antonio and Wichita. 5

What was especially meaningful? What should we do again? What could be changed or strengthened? The meeting will have an enduring impact for many people, and leave a legacy. An appreciative, evaluative closing session will help create pathways to a new lasting future. 12:00-2:00 Lunch (provided) and informal networking time All Although not everyone will be able to stay for this closing time together, it will be important for many.
