It was not as elaborate as the national conventions and we were more fiscally responsible. Congratulations to Angel Petznick from VA Medical Center elected as our Vice President We welcome back Nancy Martin from KU Medical Center as our Secretary and Rebecca Doane from Truman Medical Center as the EBP, Research and Journal Club member at large. They will have a busy year to make a difference to our component. The Board of Directors recently had a productive transition meeting held July 9. Changes were made to our strategic plan, bylaws and the scholarship policy. Special thanks go to Cindy Ladner, our Representative Assembly member who was instrumental in the revisions made. Also kudos to Carol Cooper and Jan Lopez for their extra efforts to make changes to the scholarship policy. The changes made will give members greater opportunity to earn points to apply for the scholarship. We plan to focus on improved education to all members of the benefits of ASPAN membership. The Gold Leaf award is presented annually at the national conference to a component whose members have participated in exemplary work efforts including community service, newsletter, website, scholarship and education to members and promotion of Evidence Based Practices. 2 P A G E 2 Certification Review Kim Kraft MSN RN CPAN CAPA On August 20 th, MOKAN PANA hosted a Certification Review Seminar at the Hilton in Kansas City, presented by Kim Kraft MSN, RN, CPAN & CAPA, and former ASPAN President from Topics covered included ASPAN Standards, Anesthetic Agents, care of special populations and test taking strategies. A total of 20 nurses attended in hopes of sitting for their certification within the next year. According to Kim Satariano from St. Luke s East, The information gained at the ASPAN PeriAnesthesia Certification Review course was a great review of various perianesthesia practices and standards, as well as providing information on the certification process itself. I would recommend it to any perianesthesia staff person, even if you aren't going to work on the certification. Lots of information given followed by multiple sample questions to review. The small group of attendees made for a comfortable environment to ask questions and interact with other peers. The presenter gets an A+!! By the end of the course, it gave me a little kick start to the certification process and more confidence about taking the exam. Well worth the time and money invested. Registration for Fall Exams Has Been Extended to September 19. CPAN and CAPA Exams will be administered October 3 November 25. The Fall 2016 Recertification window is open through October 31. \ M O K A N P A N A N E W S

Sheryl Montford Gwendolyn Black Diane Wikoz Robin Barber Kathrin Caudle Lynne Close Regina Davis Debra Dixson Alicia Hanson Wanda Grubb Stacey Hardin Stephanie Look Shannon Henkel Amy Marouk Teresa Rosslein Natalie Roeder Maria Shade Journal Club In June, a Journal Club meeting was held at North Kansas City Hospital in which the topic of Antifibrinolytis was discussed. The article selected was Antifibrinolytic Use in the Perioperative Setting: Aminocaproic Acid and Tranexamic Acid (Golembiewski, 2015). The article debated if the use of Antifibrinolytics such as Aminocaproic Acid and Tranexamic Acid (TXA) would decrease the number of post-operative transfusions in patients undergoing spinal surgery or orthopedic surgeries such as knee and hip replacements. It then went on to evaluate the possible side effects of these medications including intravascular thrombosis and acute renal failure. Interestingly, the risk of thrombosis and renal failure did not appear to increase with the use of these meds (Golembiewski, 2015). One participant, Cari Vittorino, stated that very few surgeons use antifibrinolytics outside of TXA prior to surgery. They only use techniques that have been tried and true. It would be interesting to find out what type of training and information has been promoted by the manufacturer. According to Lori Ackley, another participant, those patients who have received TXA appear to have less pain and an easier rehabilita- tion time. What about your facility, do they utilize antifibrinolytic therapy prior to surgery? 6 P A G E 6 Medical Missions Jennifer Yack and Nadesche, an orphan she sponsored. Short term medical service trips (MSTs) aim to address unmet health care needs of low and middle income countries. (Sykes, 2014) Each year there are several missions trips to countries such as Haiti, Guatemala, India and other third world countries supported and organized often by faith based organizations. Jennifer Yack of St. Mary s Medical Center has participated in medical missions for the last four years. Supported by Northwest Haiti Christian Missions, she travels to Haiti once a year in February to work with a team of physicians including Emergency Room physicians, Ophthalmologists and occasionally pediatricians, in addition to OR nurses and other various professionals to provide care. The team, which consists of 60 people or more, travels to St. Louis De- Nord in Haiti for approximately 10 days. Half of the team consists of people who have already participated in a missions trip and the other half is new to missions. Jen stated that during that time we see an estimated 800 people in the clinic and perform around 200 ophthalmic surgeries. The first five or six days is spent doing surgeries such as cataracts, glaucoma, strabismus and enucleations. When asked what the most exciting part of participating in missions trips, she stated that there is such a variety of things to do and it is rewarding. I love the cultural aspect of it. Local priests will pass out tickets to villagers and inform them of what day to come back to be seen. Due to the fact that there is not formal health care system provided, villagers line up days in advance to participate. There s always kiddos to snuggle PACU. We re lucky if we can get the patients to sit and stay for 10 minutes. M O K A N P A N A N E W S

7 V O L U M E 4, I S S U E 3 P A G E 7 When asked what was the hardest part of the trip, she responded with dealing with the poverty and the people you can t help. Often times there are people who have waited to be seen and you can t fit everyone in. Traveling to poverty stricken areas can be challenging. You don t always have the supplies you need so you learn to be resourceful especially in the OR and make due with what you have. Often times, supplies are shipped in advance or the team will utilize what is already there. This is where surgeries are performed, sometimes as many as three at the same time. Dr. Yi from Boston teaching a Haitian ophthalmologist how to do cataract surgery. If you are interested in participating in a medical missions trip or would like additional information, please MOKAN PANA at mokanpana@gmail.com and I will forward it to Jennifer. By: Angel Petznick Special Note: I wanted to say a personal thank you to Jennifer Yack for all her hard work and participating in medical missions. I appreciate you sharing with everyone. 8 P A G E 8 Volunteering at the Franciscan Mission On June 18, from 9 am to 12, MOKAN PANA volunteered at the Franciscan Mission Warehouse in Independence, Mo. The Franciscan Mission works with local hospitals and Dr. s offices to collect excess medical supplies and materials. Much of the equipment is still in perfect working condition but is being replaced in favor or newer models of equipment. (Franciscan Mission Warehouse, 2016) Rather than throwing the supplies out or paying to have these items destroyed through medical waste companies, the mission offers local hospitals a place to donate equipment such as stretchers, beds, bed side tables and commodes. Volunteers from local medical schools, nursing schools and professional organizations come to the mission and assist with sorting and boxing supplies. We hope to volunteer our time here again. If you would like to join us, please look for future postings of volunteer hours on the website and Facebook. 9 V O L U M E 4, I S S U E 3 P A G E 9 Region 2 Report Greetings friends, Already, four months since ASPAN s 35 th national conference in Philadelphia, PA! Indeed, it was a rich opportunity to network, learn, laugh and love! Never before have our nursing units been occupied by four generations of nurses all at once. Think about it, we span from Traditionalist to Millennials and soon Generation Z s will be in the house! The key is to blend, merge and fully embrace the best attributes each generation has to contribute. ASPAN s President Katrina Bickerstaff has brought an element of excitement by promoting her platform, Energizing Generations: The Race to Distinction As time marches ASPAN remains effective in meeting expectations of multiple generations of perianesthesia nurses. ASPAN is utilizing cutting edge technology to remain the premier source for perianesthesia nursing in education, research and clinical practice. Here are highlights of ASPAN activities thus far. A special meeting of the 2016 Representative Assembly was convened via to review and vote on A Position Statement on Air Quality and Occupational Hazard Prevention in the Perianesthesia Environment, resolution passed June 2, 2016 ASPAN, along with 61 other professional nursing organizations have signed on in support to allow advance practice registered nurses (APRN) full practice authority within the Veterans Hospital Administration (VHA). The Competency Based Orientation and Credentialing Program for the Unlicensed Assistive Personnel in the Perianesthesia Setting Edition is being reviewed and updated by editor Jennifer Allen. The Joint Commission invited ASPAN to identify a representative who has the clinical and technical expertise to serve on a technical advisory panel (TAP) that will advise the Joint Commission on contemporary pain management topics. Terry Clifford was chosen to represent ASPAN. ASPAN receives a vast number of clinical practice questions from nurses throughout the nation on topics regarding patient safety, staffing and best practice. The National Office staff works diligently to disseminate questions weekly, through the Clinical Practice Committee (CPC) and responses are sent promptly to the nurse that submitted the question. Standard and Guidelines Strategic Work Team is appraising the systematic literature review for Practice Recommendation 10 Obstructive Sleep Apnea in the Adult Patient. ASPAN Education has taped 7 new On-Demand modules from the 2016 National Conference (NC) presenters, along with 3 new modules to add to the library.

15 V O L U M E 4, I S S U E 3 P A G E 1 5 Evidence Based Practice Research in PeriAnesthesia Nursing Providing quality care is a perianesthesia nursing priority. Over the years, nursing care has progressively incorporated evidence-based research when defining the standards of patient care. Mallory (2010) discusses the knowledge that is gained through nursing research, and the many barriers to making changes to clinical practice. This is also true in perianesthesia nursing. Majid (2011) states that when clinical staff are involved in finding evidence to support a practice change they are more likely to support the change in practice. Incorporating evidence-based practice (EBP) changes must be supported by nursing leadership and encouraged at the unit level in order to overcome the natural barriers to any organizational change. One barrier that is often overwhelming is the translation of nursing research into practice. The evaluation of research findings is critical in locating the best research. An evidence hierarchy is valuable in finding the highest quality research (Polit & Beck, 2012). The systematic review offers the highest level of evidence, and utilizes the best methods of research. Established best practices come from this level of evidence. Randomized controlled trials and quasi-experimental studies help to establish appropriate interventions and they tend to incorporate methodical procedures. The lower down in the pyramid the study or research is located the less rigorous the results. It is important to consider new and emerging perianesthesia practices to stay up to date with new ideas and opinions within our specially.

16 V O L U M E 4, I S S U E 3 P A G E 16 It is very important for PeriAnesthesia nurses to understand research design. The design of a study can impact the validity of the results and failing to consider the validity of a research study could cause nurses to develop practices that are not truly reliable. I believe that PeriAnesthesia nurses should also ensure there is in-depth research out there to support any change in practice. An evidence hierarchy can be useful to all nurses who are interested in EBP. References Mallory, G. A. (2010). The topic is yet to be determined but will be posted on the MOKAN PANA website by November 1st.
