National Safety Day celebrations The National Safety Day/Safety Week Campaign being conducted to mark the Foundation Day (4th March) of National Safety Council which has significantly contributed to reduction in the rate of industrial accidents and created wide spread safety awareness. The campaign is held every year across Tata Power and some of the glimpses / highlights are below. Bharat Oman Refineries Limited (BORL-Services division) Flag hoisting ceremony & safety pledge being administered for own & contractor employees Safety Quiz competition Prize distribution for various competitions held during National Safety Week

12 Wind Operations Various programs were conducted on the occasion of National Safety Day / week across wind sites. Some of the glimpses are shown below from various sites Safety Pledge was undertaken in Poolavadi, Gadag, Samana, Khandke, Satara, Dalot & Ahmednagar region wind farms (Poolavadi & Gadag shown in picture) Safety Quiz competition at Gadag (in picture) & also conducted in other wind farms Prize distribution for Safety competitions at Satara (in picture) & also others wind farms Mr. Safe prize at Samana wind farm and also same was done in other wind farms Prize in Slogan competition at Supa

14 2. Annual Safety Meet (ASM)-2014 ASM-2014 was conducted on 3 rd & 4 th Feb 14 which was graced by the presence of Mr. Anil Sardana (MD-Tata Power) & Padmanabhan S (ED (O) -Tata Power). The participants are from all Tata Power Group i.e., including JVs & Subsidiaries. The meet started with opening ceremony by MD & ED (O), release of Suraksha Geet, followed by address of MD & ED (O). During the meet, participants have shared challenges faced and also good practices followed at their respective divisions. Lessons Learned Incident Date & Time : 12th February 2014 & 12:30 Hours Location : Boiler House MDC # 3 RAV Incident : Medical Treatment Injury Description of Incident: While trying to clear the jam by hitting over the RAV, The jam got opened and the hot ash material spell over the body of the operator and he has received burn injury over his chest, right wrist, right neck and below abdomen Root Cause Analysis: Human Factors: 1.5 Improper position or posture for the task - The operator was standing in close proximity to the open line 3.2 Personal protective equipment not used - The operator was not wearing cotton boiler suit which is standard PPE for operator and issued to all operator Physical Factors: 6.3 Improperly prepared equipment - Equipment was not positively isolated / shutdown 7.6 Temperature extremes - Hot Ash Material splash over his body 8.1 Restricted motion - Area was congested and movement was difficult
16 System Factors: 5.4 Inadequate reinforcement of critical safe behaviors - use of cotton boiler suit by operator was not enforced 8.1 Unclear assignment of responsibility - The operator has carried out maintenance personnel job. This was the second incident of similar nature 8.4 Inadequate identification of job hazards PTW was not issued for the job HIRA / JSA was not prepared 11.1 Inadequate work planning - The clearing of jam was carried out without proper isolation / shutdown 14.2 Inadequate development of PSP - The SOP for the subject activities was not available Recommendations: 1. Written procedure (SOP) to be developed and followed 2. Permit to Work (PTW) system shall be followed by Operations team for such Non-Routine Tasks 3. Ensure that the no critical maintenance works shall be taken without proper isolation / shutdown 4. HIRA/ JSA shall be used for all jobs 5. Provide awareness session 6. All Boiler Operators must wear Boiler Suits only 7. Identify proper and suitable gloves for such operations & replace the cotton gloves Click here for detailed report

Safety Training - Feb 14 Safety standards and procedures training, first-aid and fire fighting, DMP awareness, defensive driving training, LOTO, working at heights, e-ptw etc were conducted at Trombay Session on JSA, HIRA, Basic First-aid, Safety at Work, LOTO were conducted at Poolavadi, Samana, Gadag & Agaswadi wind farms respectively Training on Excavation, concerting, working at Height, conductor installation & personal health and hygiene was conducted at 28.8 MW Solar Power Project Transmission Line camp area
18 5. Emergency Exercises-Feb 14 Fire Emergency exercise was conducted in Nivade & Supa wind farms Emergency exercise on Strong & Severe winds was conducted at Sadawagapur Wind farm 6. Corporate Safety Highlights Feb 14 Arranged Annual Safety Meet with Tata power group companies including JVs & Subsidiaries at Hotel Ambassador, Mumbai. Conducted Office & Fire Safety training for Bombay House personnel. Participated in Level II mock drill conducted at Trombay CHP area, Station A and necessary observations for improvements were made. Carried out audit of Trombay U#8 TG deck Scaffolding and the observations were communicated. Conducted interview along with CFT to certify O&M Engineers of Transmission projects & DCS. Coordinated with Corporate Communication for displaying Office & Fire safety posters on Sangam and Hard copies of Safety posters were also circulated to all divisions. Voices Closure session of Corporate Safety with Corporate HR was conducted. Attended fire service week meeting at Maharashtra Fire Academy. Conducted meeting with JV s for finalizing action plan for implementation of DMP & BCP.

22 8. Security Highlights JOJOBERA Operational achievements - Successfully Conducted a number of training programmes to enhance the operational skills of the guards. The sense of security awareness has also been created among contract workers. All are adhering to the security instructions. Training activities - A)Security awareness lecture/training were given to 88 contract workmen by the security officers of Tata power to make the contractors workmen aware of the Do's and Don ts in respect to security, while working inside the Plant area. B) 41 security personnel of SIS were imparted training on Fire Fighting and Awareness on Pollution Due to Plastic. One Security Officer and 10 Guards were imparted training on First Aid. Social activities Mr Sunil Kumar, Head Security and Mr A K Srivastav actively participated in organizing Homeopathic camp for villagers of the nearby villages on 28/02/14. Reward-Recognition- A)Mr A K Srivastava was recognized for his activism in Greenolution & Sustainability initiatives for plastic free environment. He was also rewarded for Providing logistic support for organizing Intra division Volley Ball tournament. C) One Inspector and two guards of SIS were rewarded for successfully organizing Intra division Volley Ball tournament. Security Meeting- Head Security attended meeting with SSP on 20/02/14 at his residence. Mr Sunil Kumar. Head Security, attended joint meeting with Mr Mallik, DCIO-I, IB and Circle inspector, Telco on 12 & 24 Feb 14. Regular meeting with Circle Inspector, Telco and I/C PS Govindpur was attended by Head Security. Security Related Functions/ Events Mr Sunil Kumar, Head Security with the help of Mr A K Srivastava and other security personnel organized Intra Division Volley Ball Tournament.
23 Achievements - A) The well planned security sessions and its execution will certainly go a long way in achieving the aim of creating security awareness amongst the employees and contract workers. B) Security Department won 5 awards/rewards during the month of February, Highlights of the visit of Group Chairman Cyrus Mistry at JOJOBERA The Chairman was received by Station Head Mr Dinesh S Kudalkar and President of union Mr Rakeshwar Pandey. Group Chairman garlanded the statue of founder Late J N Tata Chairman paid visit to operational control room. Did plantation in side the plant Inaugurated a green garden made of fly ash. 24 Chairman inaugurated Fly Ash Brick Making Machine & Handed over four Fly Ash Brick Making Machine to four groups from community. Trombay Addressed the employees of Tata Power, Jojobera 05 Feb 2014 Monthly Security Officers Meet held at Security Conference Room duly presided over by Col. P B Dalvi (Retd). 08 Feb Managing Director Mr. Anil Sardana visited the plant to oversee the U # 8 restoration progress and ongoing U#6 outage. Necessary security arrangements were provided for the said visit. 10 Feb ISPS Code compliance training for security personnel deployed at Coal Jetty carried out by PFSO/Dy PFSO on Security aspects. 20 Feb ED (O) & Chief Corp Security & Vigilance interaction with all Security Officers through Video Conferencing was held on various security aspects duly attended by Trombay Security Officers.. 22 Feb Managing Director Mr. Anil Sardana visited the plant to oversee the U#8 restoration progress and ongoing U#6 outage. Necessary security arrangements were provided for the said visit. 24 Feb 2014 His Excellency Mr. PIUS DUNAISKI, High Commissioner of Namibia visited the plant. Necessary security, traffic control and escort provided during the visit.
25 24 Feb CFT for taking review of MO Security for FY13-14 and finalization of security agencies for FY was held at Conference Hall No. 1, Trombay duly attended by Security Officers from MO Area. 25 Feb Disaster Management Plan preparedness Mock drill was conducted. Evacuation of all the personnel of plant and accounting of personnel were carried out at respective Safe Assembly area by Security. Necessary traffic regulation/security/qrt arrangements were made for emergency vehicles, Incident Site & Safe Assembly Zones. Arrival of QRT/Security/Fire/Ambulance at Incident site Cordoning of Incident site by Security staff Search of casualty by Security/Fire/Rescue staff

The following points were covered. (a) Discipline (b) Procedure for Handing and taking over of duty. (c) How to communicate on Walkie-talkie. (d) Leave procedure (e) Action to be taken during different kinds of alert level. (f) Handling of Security equipments 28 Feb Quarterly Meeting of Security Officers organized by Corp Security was held at Dharavi RS. Representative of Trombay attended the VC meet. 27 Transmission & Distribution The training workshop for security supervisors and guards of T & D was conducted at Borivali Rec Stn on 11 Feb 14. Total 31 security personnel s attended the workshop.
