4 Project Goals The purpose of the Meds at Beds project was to; Store medications at the bedside Reduce the delivery time of medications to the residents Reduce medication incidents Reduce staff injuries by October

5 Objectives 100% of residents will have their medications stored at the bedside by June 2014 Have a 75% reduction in medication incidents by October 2014 Reduce the time delivering and administering medications to 40 minutes by October

6 Planning Instrumental for the success of the project Planning included: Project team meetings Cocktail party Evaluation and measurement tool Communication strategies Including residents and family The pilot Sharing results and celebrating success 6

7 Our Pilot Planned to pilot meds at beds for 6 residents and then roll out to full nursing home once issues were identified and then rectified Having two processes (old and new) led to an increase in medication incidents during the trial period Staff were frustrated at having two medication processes Data collection tool used was inaccurate 7

8 Where are we up to now Full roll out of the project was completed and embedded as the routine No medication errors for August 8

11 How are our staff and residents better off? Less medication errors Timely medication administration Less workplace muscle strain as a result of not pushing heavy medication trolley Increased satisfaction levels with medication process from staff, residents and family members 11

12 What have we learnt about change? Identify where staff are at before you start the change Important to have a plan Stay focused on the goals and objectives Regular reviews Minimise time between trial and final rollout of project Important to bring your people along with you 12

13 The rider/elephant/path habits we have built include: Ensure riders identify processes required for change Elephants engage the belief in others that the change is positive Identify the path and remove any barriers to a smooth change journey Teamwork is essential to effective change Communicate Communicate Communicate 13
