Please provide corrections via fax or to Karen Lennon, (919) , The North Carolina Health Care Facilities Association (NCHCFA) is a non-profit North Carolina corporation with an office located at 5109 Bur Oak Circle, Raleigh, North Carolina The Association is composed of nursing homes providing professional skilled nursing care. The membership consists of 13

14 approximately 400 nursing homes providing professional care to more than 40,000 residents in North Carolina. As a professional trade association, NCHCFA works closely with state and federal agencies governing skilled nursing facility operations. In addition, the Association staff coordinates statewide programs and research projects, plans educational programs and membership meetings, and prepares various membership and consumer publications. The North Carolina Health Care Facilities Association is a strong and active association dedicated to providing quality long term health care to the residents we serve. 14

15 Staff Bios Samuel B. Clark, CPA Vice-President of Finance Phone Extension 206 Sam is primarily responsible for issues related to reimbursement of nursing facility services. In addition to acting as an information resource on matters pertaining to reimbursement, Sam is also the staff contact for the Health Insurance Portability and Accountability Act (HIPAA), Certificate of Need and Life Safety Code. He acts as staff liaison for the Association s Payment for Services Committee. Sam serves on the American Health Care Association s Reimbursement Committee and represents NC residents of the 2 nd Congressional District on the Medical Care Advisory Committee. Before joining NCHCFA in 2002, Sam was employed as a Senior Auditor with Myers and Stauffer PLLC. Sam holds Bachelor s and Master s degrees in Business Administration. He is licensed as a Certified Public Accountant in Kansas and North Carolina. Allison Dew Director of Communications and Major Events allisond@nchcfa.org Phone Extension 214 Allison manages the design, content, distribution, and maintenance of all NCHCFA marketing materials, publications, and Web site. She is also responsible for the selection, planning, and presentation of the Annual Convention & Expo, Summer Symposium, Autumn Classic benefitting the NCHCFA Political Action Committee (PAC), and additional special events. Allison is responsible for fundraising and all grassroots activities for the NCHCFA/PAC. She is the staff liaison to the Education and Programs Committee and Associate Advisory Committee. Allison is the managing editor of UPDATE, the Association's weekly newsletter, and assists with special projects as needed. 15

16 Prior to joining NCHCFA in 1999, Allison was a student at North Carolina State University where she earned a BA in Communications. Eric Kivisto Director of Policy Development erick@nchcfa.org Phone Extension 207 Eric coordinates the various projects that provide members with information, resources and direction as to how best to access various quality improvement programs. Eric assists district committees to identify opportunities and target initiatives and projects to continue LTC enhancement in NC. Eric is the staff liaison for NCHCFA Quality Committee and supports the Standards and Ethics Committee and Council on Provider Relations. Eric also serves on the NC Culture Change Coalition and the American Health Care Association s Quality Improvement Committee. Eric works closely with NCHCFA staff and members to help achieve their Quality Improvement goals. In addition, he helps coordinate AHCA's LTC Trend Tracker and Quality Awards program use among NCHCFA member facilities. Eric is also a resource on CMS rules and helps members deal with the myriad of CMS regulations that impact skilled nursing care centers. Prior to joining NCHCFA in 2008, Eric spent three years as a facility surveyor with the North Carolina Division of Health Service Regulation. Originally from Michigan, Eric, brings more than fifteen years of experience as a social worker and a marital and family therapist. He graduated from Hope College in 1992 and received a Masters of Social Work from Wayne State University in Detroit in Karen O. Lennon Director of Education and Member Services karenl@nchcfa.org Phone Extension 204 Karen is responsible for the selection, planning and presentation of NCHCFA s ongoing educational seminars held at various times 16

17 throughout the year. She works with the Association s Trade Associate Members and also assists with the planning and delivery of the Annual Convention & Expo and Summer Symposium. She oversees the maintenance of the membership database, coordinates new member recruitment, and assists the Association facility members as needed with any questions or concerns. In addition, Karen prepares the annual Membership Directory for publication and is responsible for the accounts receivable and accounts payable operations for the Association. Prior to joining NCHCFA in 2010, Karen worked as a commercial lines insurance auditor with Travelers Insurance. Karen earned her BA in Communications from East Carolina University. Adam Sholar President and CEO adams@nchcfa.org Phone Extension 205 Adam is responsible for the effective day-to-day management of the Association and its resources. Adam is the Chief Executive Officer of the Association and is responsible directly to its Board of Directors. He is also responsible for representing the Association before the North Carolina General Assembly and Executive Agencies. Donna Snyder Administrative Assistant donnas@nchcfa.org Phone Extension 200 Donna is responsible for answering all incoming calls and greeting visitors. She provides administrative support to the staff and specifically to Education and Membership Services, processing all seminar registrations, confirmations, and CEU forms. She also assists with the publishing of the weekly newsletter UPDATE. Prior to joining NCHCFA in 2015, Donna worked in various capacities in the legal and healthcare fields. 17

18 William A. Turner Director of Information Systems & Technology Phone Extension 208 William is responsible for acquisition, adoption, maintenance and management of areas related to the Association s information systems, technology strategy, publications, graphics design, communications, as well as the development of NCHCFA s online presence through various tools and Web sites. He is also the publishing editor for UPDATE, the Association's weekly newsletter, placing the newsletter online each week, as well as securing electronic delivery. William is also the publications and graphics designer for the Association. Prior to joining NCHCFA in 2002, William worked in IT Director/Specialist positions with Metromedia, Nortel, IBM specifically in the areas of international communications and quality assurance testing, as well as contract consulting in the area of disaster recovery. William received his BSBA in Decision Sciences and Certificate in Information Systems from the School of Business at East Carolina University. Extensive certification and training in automation, integration, management, quality assurance, and testing have been obtained in over 25 years of consulting. In his spare time, William is a Christian author/designer/teacher and nationally recognized studio photographer. 18

19 Polly G. Welsh, RN-BC, MPH Executive Vice President Phone Extension 203 Polly focuses on long term care policy, regulation, and quality issues, serving on many advisory committees, task forces and other government policy collaboratives with the Division of Health Service Regulation, the Division of Medical Assistance, the Division of Aging, within the Department of Health and Human Services. Polly conducts statewide, as well as national, educational sessions on leadership, quality, and regulatory issues. Prior to joining NCHCFA in 1993, Polly worked for the State Survey Agency where she was responsible for implementation of OBRA 87 in NC s Medicare/Medicaid Certification program. Polly has also held both administrative and staff positions in both acute and long-term care facilities. Polly has been a Registered Nurse for over 40 years and has a certification as a Gerontological Nurse from the American Nurses Credentialing Center. She is a graduate of North Carolina State University where she earned a Bachelor of Science in Health Occupations Education. Polly received her Master of Public Health, Long Term Care Training Continuum, from East Carolina University in Her professional affiliations and achievements include: Member, North Carolina Institute of Medicine; Past Member, Editorial Board, North Carolina Medical Journal; Past Member, Board of Directors, NC Foundation for Nursing Excellence; and recipient, 2012 North Carolina s The Order of the Long Leaf Pine. 19

27 Special Recognition Awards Special Award Winners NCHCFA honors the special people who have made a significant contribution to long-term care in North Carolina. All of the individuals selected to receive the following awards work within long-term care. NCHCFA s time-honored awards program provides opportunity to acknowledge front line, unlicensed personnel who have demonstrated outstanding achievements in the long-term care profession. The NCHCFA Annual Awards Program honors individuals who are the backbone in our nursing facilities because the support they provide to our residents. Nominations are submitted by the facilities and a panel of judges from the Education and Programs Committee selects one winner from each district. Each winner is honored during the Association s Annual Convention and Expo which is highlighted by the announcement of the J.R. Garrett, Jr. Direct Care Worker of the Year Award and the Riley W.
