1 CBSPANNER Volume 10, Issue 4 Winter 2012 President s Message Diane Swintek, BSN, RN, CPAN This is the time of year when my mind turns to reflecting on the passage of time. My favorite holiday, Thanksgiving Day, is approaching as I write this. It is the time I reserve for giving thanks for my good fortune: I am thankful that I am employed in the nursing specialty that I love, that I am in good health, routine aches and pains aside, and that I have health insurance to cover any health related expenses; I am thankful that I have an employer In this issue: Page 1! Photos from CBSPAN Fall Seminar!! that recognizes the standards and practice recommendations put forth by ASPAN as being necessary to follow for safe patient care; and I am most thankful to have had an opportunity to serve the perianesthesia nurses of CBSPAN as your President for the last 2 years. The holidays are a particularly poignant time for nurses and their patients. Each of us has a family that we would much prefer to be spending that day with, but for those few hours we are together, as caregiver and patient, we are a family. Last year I was on-call for Thanksgiving and was called in to recover patients who had emergency surgery. The most memorable was an older lady who was going to her daughter s home for Thanksgiving dinner and tripped over a curb while getting into the car. Now she was in the hospital having a fractured hip repaired. The patient s daughter was giving instructions over the phone to her daughter, a high school senior, about when to remove the turkey from the oven and how to prepare the rest of the planned feast. The three of us, patient, daughter, and nurse, talked about previous Thanksgivings and how we all enjoyed that particular holiday. I told them I was thankful that I could be there to ensure that the patient recovered from anesthesia safely. The daughter was thankful that her mother was not more seriously injured. In September, my leadership journey included attending ASPAN's Component Development Institute 2012 held in St. Louis, Missouri. ASPAN sponsors CDI annually to assist components with membership growth and development. ASPAN s leaders discussed topics such as recruitment, budget planning, running a board meeting, understanding the Representative Assembly, applying for the Gold Leaf Award, and more. In addition, the use of electronic media for information sharing, including electronic newsletters, facebook pages, and websites was discussed. It was an insightful experience to listen to other components and have open discussion on similar triumphs and challenges. It was a time to exchange ideas and develop an open dialogue among colleagues and ASPAN leaders. In October, the DC District of CBSPAN and Medstar Georgetown University Hospital joined forces to host the annual 2012 Fall Seminar held at the Levy Center in Washington, DC. It was an all day seminar for registered nurses working in the pre- or post-anesthesia areas of hospitalbased perioperative units, ambulatory centers or procedural areas. This year s theme was Aspire to Make a Difference. The goal of the conference was to provide educational opportunities for nurses who practice in these specialty areas and enhance their knowledge for providing safe care and improving patient outcomes. Topics included Maximizing Surgical Outcomes, Regional Blocks, Legislative Affairs, Science of Pre-warming, Evidence Based Practive vs. Research, and Leadership Principles. There were nurses from the DC, Maryland, Delaware and Northern VA region networking and exchanging ideas. Finally, we had an opportunity to participate in the CBSPAN panel discussion on ASPAN, CBSPAN and ABPANC. This year I will be looking forward to working with all of you in different capacities. I will be mentored by our previous leaders, Diane Swintek our current president, and Tanya LaCompte, our Region 5 director, in my new role. I want to thank all of you for your support and guidance. I hope to see all of you at our PANAW celebration in Baltimore in February. 4 Page 4 President s Report continued thankful that her daughter could be with her through this experience. It is a special duty to be a nurse working to care for patients on the day of a holiday and one that we all embrace when first we start the journey to becoming a nurse. Lastly, I am thankful for the leadership team at CBSPAN. We come from a wide variety of nursing experiences staff nurse, PCC, resource nurse, educator, manager, and supervisor. We work well as a team and always have the best interests of our members in mind when making decisions regarding CBSPAN activities. I could not be an effective leader without your wise counsel and participation. I often reflect on how I became so intimately involved in CBSPAN. The answer is that this group of nurses has always been most welcoming and inclusive of its members. The nurses are assertive, compassionate, confident, and accomplished. They freely share best practices and offer answers to questions. I know I have matured as a woman and as a nurse through my association with them. I hope you will one day choose to go on this same journey, becoming a leader of this fine society. CBSPAN has 596 members!! WANTED: Articles, news, pictures for the next issue of the CBSPANNER. Each 2me I a;end ASPAN s Na2onal Conference, I come home energized and have a renewed dedica2on to my perianesthesia prac2ce. It is great to see old friends and meet new friends from all over the country. The speakers are mo2va2onal and present a wide range of topics including clinical, hot topics, research, Evidence Based Prac2ce and leadership. I par2cularly enjoy hearing new informa2on or hot topics. One of the sessions I a;ended was presented by J. Michael Verlander MD: The Role of Anesthesia in Preven2on of Surgical Site infec2ons With SSI (surgical site infec2ons) contribu2ng to 1-3% of pa2ents per year/or about 500,000 cases, preven2on is a hot topic. Sta2s2cs show that SSIs drive healthcare costs up $5-10 billion per year, increase hospitaliza2on up to a week, and double ICU admissions as well as mortality rates. Some of the measureable and controllable factors that influence risk of SSI: 1. An2bio2c Prophylaxis 2. Tissue Oxygena2on- normocarbia 3. Maintenance of normothermia 4. Glycemic control 5. Pain Control 6. Smoking Recommenda2ons: o Administer an2bio2cs before /as close to cut 2me as possible. Repeat dose if surgery >4 hours o Opera2ng Room use FIO2 of 80% and maintain EtCo2 levels within normal range. In PACU use O2 in at risk pa2ents. o Begin warming in pre- op and use warming prior to induc2on to maintain normothermia. o A;empt to maintain periop blood glucose in 150 mg/dl range o U2lize regional anesthesia for at risk pa2ents. o Use chlorhexidine/alcohol surgical scrub; clipping vs. shaving o Use hand washing, clean needleless ports o Minimize blood transfusions/use PRBC stored < 2 weeks o Insist pa2ents avoid smoking on day of surgery Diane Swintek, BSN, RN, CPAN Are You Prepared for the Digital Age of Healthcare? My facility is preparing to introduce the electronic health record (EHR) to the perioperative and perianesthesia care areas. Currently, only the Phase I and Phase II PACU areas have not been utilizing a digital record. However, now our entire facility, and the whole health system of which my Medical Center is a member, is instituting one system. In an effort to prepare myself for this initiative, I joined the Informatics Specialty Practice Group (SPG) at ASPAN. This year at National Conference I was able to attend the SPG meeting and educational session. Matthew Byrne, the chair of the Informatics SPG, wrote an article about the coming of the EHR. The article appeared in the February 2012 issue of JoPAN. In it, the question is asked if ASPAN has, or plans to create, a competency for Informatics. This issue was the burning question about which the SPG members brainstormed. How do we use the EHR in our practice and ensure that what is being documented meets the Standards and Practice Recommendations set forth by ASPAN? The group also dared to dream that ASPAN could create an application that perianesthesia nurses would be able to download to their phone, tablet, or ipad, that addresses clinical and organizational issues. Wouldn t it be terrific if you could access the Redi- Ref electronically and not have to search out the last person that used that reference? I know many of you are already using an EHR and I wonder if it meets all your documentation needs. Does the use of the computer in providing patient care rob you of one-on-one time with that patient? That is my pressing concern. Will I be able to use the coming technology and still continue to provide safe, high quality, competent patient care. Only time will tell, but I intend to prepare myself in the meantime and learn all I can about the functionality of the EHR chosen for my Medical Center. I also know that the other members of the Informatics SPG will be able to help me navigate through any problems I encounter once we have gone live!

Medstar Franklin Square Medical Center Registration is now open: From the Editor: In an on-line mailing I receive from Wesley Seminary where I completed my Parish Nursing course, I read the words of one of the professors: What concentrated wisdom from your chosen field would you want to pass on? What a thought-provoking question!! After giving it a LOT of thought, for me it comes down to caring for the patient-- body, mind and spirit. If any one of those aspects of the patient are out of sync, then it becomes difficult for the patient to heal totally. So often we only focus on the physical and mental parts of healing the patient. We forget the soul, the spirit. If the patient is feeling overwhelmed, depressed, unable to cope with no where to turn, his/her recovery is going to be less than satisfactory. Perhaps my recognition of this phenomenon is what led me to Parish Nursing several years ago, and which I continue to do today even though I am retired from PACU nursing. Just a thought...
