Newsletter of the Connecticut Society of Perianesthesia Nurses Volume 39 Number 2 Summer 2017 INSIDE President s Message 1 Vice President s Message 2 Region 4 Director 3-4 Editor s Note 5 Governmental Affairs 5 CSPAN News 7-9 ASPAN News 10 ABPANC News 11 CSPAN Contacts 12 CSPAN members at ASPAN National Conference From Our President Lucille Mentone BSN, RN, CAPA I recently returned from Indianapolis and the ASPAN National Conference. Your conference! Sadly there were only 4 of us there from Connecticut. As always, the conference was energizing and informative. We also had a great time networking with each other, the speakers and nurses from all over the country. This will be my last message as president and I will return to my well worn theme of membership and participation on the board. Your board! I am very happy to have some new members and to welcome a returning member this year. But let me get to the down and dirty benefits of serving. Diane and I traveled to this past conference free of charge. The board paid for our flight, our room, our food and even the parking at the airport. We do that every year for the President and Vice President. All board members have their registration for National Conference reimbursed. All board members attend our conferences free of charge. Two members of the board attend Leadership Development Weekend free of charge every year. Our government affairs representative is given the opportunity to attend Nurse in Washington. Again, all expenses paid! In this time of economic hardships for hospitals this is a great perk. Yes, we do lots of work to make this the great organization that it has become but we have fun doing it. I promise you, if you take the leap you will not regret your decision! Indianapolis - Courtesy L. Mentone My year as president was a pleasure. Thank you to my Vice President, Diane Perrone. She made it easy for me and did much of the hard work. Thanks to Jeanne Prota, our Past President, who kept me on the straight and narrow! Thank you to all the board members for their untiring support and willingness to serve. And thanks to our members at large. We couldn t be the organization we are without you. 1
Vice President s Message Diane Perrone ADN, BA, RN, CAPA T he 36 th ASPAN National Conference hosted in Indianapolis in early May celebrated and validated our perianesthesia practice. Inspired by dedication, passion, leadership, research, education, and advocacy for patients and nurses. I felt renewed and empowered in my own practice. Networking with other PACU nurses from around the country reinforced the comradery among PACU nurses as we face the same challenges and successes whether we are from Connecticut or Alaska. As we participated in lectures and presentations by wonderful nurse speakers, most of whom continue to work in direct patient care, the education component of ASPAN came through loud and clear. As an ASPAN member, please look for the FAQ (Frequently Asked Questions) under the Clinical Practice Tab of the ASPAN home web site. Valuable and accessible, perianesthesia nurses from around the world submit practice questions that are answered by experts in perianesthesia nursing practice. Many of the questions posted are questions you may have already asked yourself! Questions currently listed and answered include issues regarding staffing, acuity, and standards of care. Use this information to strengthen your practice, feed your curiosity, and form the foundation for additional questions. Also under the Clinical Practice tab are ASPAN Position Statements. Position Statements, according to ASPAN, address current issues effecting clinical practice and offer, a representation of the organization s viewpoint or stand on particular issues. The current position statements are works in progress. They expose our practice to collaboration with other medical and research disciplines, and evidence based research. Some of the Position Statements discussed at National include: Do Not Resuscitate Advanced Directives, Acuity Based Staffing for Phase l, and Air Quality and Occupational Hazard Exposure Prevention. Check them out and start a discussion at work. Again, feed your curiosity. Explore the Education tab on the ASPAN home page for on-line learning opportunities. Offered are full-day, half-day and two hour live webcasts. Also offered are on-demand education modules such as the perianesthesia certification review bundle, and on-demand library with individual topics of interest to PACU nurses. New this year is ASPAN Select, which promotes small group learning. One to three prerecorded sixty to ninety minute modules can be presented for CEU s. The small group s host registers participants through ASPAN and arranges the venue and AV equipment. In person conferences are alive and well at both the national and component level. CSPAN S own Spring Conference 2017 was very well attended and very well received. It was wonderful to network with PACU nurses from around our state and come together not only to learn new things and keep current on relevant topics, but also to support Safe Haven, an organization that supports women in children leaving abusive situations. CSPAN hosted three ASPAN national speakers in addition to a presenter from CSPAN and a Physician Assistant (PA) from St. Francis Hospital in Hartford. Topics included opioid crisis, pediatric airway emergencies, ERAS, anesthetic agents, sepsis and uncommon complications in ambulatory surgical post-op patients. We also received great ideas for topics for future conferences. Please continue to contact CSPAN 2 Continued page 6-
ASPAN Region 4 Director s View Amy Dooley MS, RN, CPAN, CAPA Be the Best You Can Be! I don t know about you, but, frequently I feel as if I am a waitress at work. The patients ask me for water, food, or when their room will be ready. This really came to light the other evening as I was out for dinner and started thinking about the skills of the waitress that are similar to a nurses job. Here s what I came up with. First of all, both have to be able to prioritize our work. The managing of several tables of customers and managing several patients at different levels of awareness requires decision making skills and the ability to know what is the most important activity to do RIGHT Now! A waitress and a nurse have to meet the social expectations of the customers. Eating at a restaurant means tips for the wait staff and the tips generally go up if they are nice, friendly, and give fast service. Nurses are being measured these days by their HCAHPS Scores which requires a positive patient experience based on how the nurse treated them. Fair or unfair the results means a great deal to the institution so nurses now have to be sensitive to how their patient perceives them. You are the face of the institution. Your waitress must bear the brunt of the slow kitchen when your food doesn t come in a timely fashion. He/she is also held responsible when the bartender is delayed pouring your next glass of wine. They must answer for events they are not responsible for. How many times do nurses have to bear that similar mantra? The doctors are not always around to answer questions, the kitchen may be delayed delivering food, the hospital is full so getting a room assignment is not timely, and visiting hours are restricted for privacy issues. The nurse hears all those issues from the patients and needs to be mindful of their answers to be positive and supportive. The social norms of workplace culture is similar. Both nurses and waitresses work beyond a 9-5 job and includes weekends and holidays. There is no such thing as working from home. Your colleagues expect you to show up for work and must adjust the workload if someone cannot come to work. Of course the major difference is that nurses are working with patients and not customers. The most important and major skill is assessment which leads to follow through. No one will die if a waitress makes a mistake, but, a nurse cannot say the same thing. Nurses save lives bottom line. So how do waitresses and nurses become the best they can be? How do waitresses get better tips and how can nurses improve HCAHPS scores? I have a few suggestions. First of all pay attention to details. Sure the waitress gets your drink right away and takes your food order, all expected. Then the small things begin to add up. He/she forgets to refill your water, doesn t ask if you d like another beverage, forgets to bring the extra sauce you asked for, and doesn t ask how your food is once it gets delivered. These are not major gaffs, it s just that they all add up and sends a message that you the customer are not that important to the waitress. Nurses are held to the same detail by patients. Their vital signs are taken, medications reviewed, urines measured, and 3 - Continued next page
Continued from page 3 assessment done. But what about those little things asking if the patient is warm enough, if they need help to reposition, asking if they have any questions, pausing and asking family present if they need anything too. These are not the major items that every patient needs to have done. These are the details that make a difference to the patient. Meet the patient/customer where they are at. Perhaps the customer has to catch a plane in 25 minutes. They will want their food & check at a quicker pace than someone with all evening to eat. Maybe the customers are celebrating a private event and they want to be left alone. The wait staff needs to figure out what the customer wants and meet them there. Likewise, a nurse needs to meet the patient where they are at. Perhaps they are extremely nervous and want reassurance. Perhaps they have just gotten bad news and need support. Perhaps they don t like losing control and are rude to the nurse. These are the scenarios that nurses experience and if they meet the patient where they are at, the patient will feel that commitment to them and have a better experience. True, not everyone will feel happy, but if nurses can improve 85% of patients then that is a big goal. So my closing thoughts to be the best that you can be, is to reach your patient at his/her level, be consistent with the message you give, pay attention to details, and treat them like you d like your family member to be treated. Congratulations to the newly Certified PeriAnesthesia Nurses CPAN Lisa Zbikowski Connie Wall Charina Glori Donna Douglas CAPA Debra Gratton (Permission to reprint from Amy Dooley) CSPAN Board Update CSPAN board held it s annual Installation Dinner at Baci Grill in Cromwell, to bring in the new Board of Directors and Committee Members. Welcome all! Don t be shy about asking to get involved! President - Diane Perrone, Past President - Lucille Mentone, Vice President, Education Chair, & Corresponding Secretary - Bridget Rich, Treasurer - Joanne King, Secretary - Sue Bryan, Governmental Affairs - Lauren Kelly, Editor - Pat Moore, Membership Chair/ webmaster - Bonnie Murray, Education - Lori Lyles, Maryann Berluti, and Nancy Merrow 4
Editor s Note Pat Moore MSN, RN, CAPA Governmental Affairs Lauren Kelly MSN, RN, CPAN S imply...amazing! As a member of the CSPAN community you have had numerous valuable opportunities presented to you over the past year for your annual membership dues of thirty dollars. The CSPAN Spring Conference and membership meeting this past April was outstanding according to the survey reviews. The speakers all received rave reviews, several members were awarded VISA gift cards, two members were awarded a free entry to next years conference, and as a group we donated over $ to Safe Haven of Greater Waterbury, Inc. as well as towels, linens, diapers, and personal essentials. Last fall CSPAN hosted a FREE Wine Social at Gouveia Vineyard for members to network, talk shop, and just have fun. Our next social is in the works for September! Last fall CSPAN hosted a free half day conference for certified members! CSPAN offers two scholarship opportunities to members and if you become active as a Board member- opportunities abound from ASPAN as well. Getting involved is as simple as asking and a great way to hone a few new skills. I am having a great time writing a little and getting used to Microsoft Publisher and Bonnie (our website technician) has learned WordPress and website design. A fall certification review is coming and we encourage members to apply for one of CSPAN s scholarships. Believe in yourself, be proud of yourself, be proud of your profession.become CAPA or CPAN certified. You can do it! Nursing Advocacy: Beyond the Bedside As nurses we have the obligation to advocate for our patients, not only at the bedside but on a broader scale as well. Provision 3 of the American Nurses Association Code of Ethics states The nurse promotes, advocates for, and protects the rights, health and safety of the patient. We are fortunate to have professional organizations that make our collective voice heard. The theme of this year s Connecticut Nurses Association Legislative Education Day on March 22nd was Advocacy: Patient, Professional, and Political. The message was that nurses have an ethical and professional responsibility of using our knowledge and experience to influence health policy and help protect the public. Participants learned how a bill becomes a law. There was also discussion of current Connecticut legislation on everything from the opioid crisis to the use of tire rubber on municipal and public playgrounds, and how nurses are involved in shaping this legislation. 6 Gov. Affairs Continued- The Connecticut Hospital Association (CHA), though not nurse-driven, also serves as an advocate through a number of initiatives: promoting patient safety, ensuring equitable care, building a strong workforce of healthcare professionals, and securing fair and adequate hospital reimbursement. One current threat to the latter is the Governor s proposed budget that severely cuts and taxes Connecticut hospitals. The CHA s Action Center electronic letterwriting campaign is a quick and easy way for us to urge our legislators to protect hospitals. This is urgent as our legislators will be releasing their budgets soon! If you haven t done so already, please follow the link below (or copy and paste to your browser) to send them a letter. You may use the pre-typed message, add your own thoughts, or compose your own message. Here is your opportunity to advocate! vvsrc=%2fcampaigns%2f52506%2frespond VP message Continued from page 2 with additional suggestions. Thank you to everyone who attended, our speakers, participants who presented posters and all who helped to make this conference a huge success. Finally, I learned at the National Conference about the important role of PACU nurses in advocacy. ASPAN now has two nurse liaisons on The Joint Commission. The nurses perspectives will add value to many perianesthesia issues including addressing post-op pain in the PACU. ASPAN also has a seat at the table with the national ERAS society, OSHA s team evaluating waste anesthetic gases and other work place hazards in the PACU and the Council on Surgical and Perioperative Safety. Ongoing research will support our evidence based practice on important PACU practices such as patient assessment, pain management, and obstructive sleep apnea. Thank you for all you do, as you take great care of your patients. You represent the face of PACU nursing to patients and their families. You are dedicated, passionate leaders who advocate for patients. Please continue to hone your practice and seek new solutions through evidence based practice via research and reach out to CSPAN and ASPAN for support.
