THE OAKS AT Richfield Living Alleghany Room 3706 Knollridge Road Salem, VA A Tale of Transformation March 26, 2014 It may be the most important journey you ever make, one that will transform your organization from a skilled care facility to a true home for elders where they enjoy the privacy, choice, autonomy, sense of well-being and pleasures in daily living they deserve. Come hear about their journeys and take the first important steps of your own. Learn about the Fours Stages of Culture Change and identify your organization s stage in the process. Led by Megan Hannan, MSW Nursing and Home: Integrating Quality Care with Life May 21, 2014 Do you believe that making our own decisions is an essential element of a satisfying life?
