Nebraska Hospice and Palliative Care Association (NHPCA) Mission: A statewide partnership to improve the quality of life for all Nebraskans who have chronic conditions or are near the end of life and to support the various communities who care for them. Vision: No Nebraskan lives in pain or dies badly. Nebraskans with chronic or end-of-life conditions regardless of age, live their last months to their fullest, with their wishes expressed and respected, their pain and suffering alleviated, their fears and questions heard and addressed, their relational, spiritual, cultural, and financial needs met, with their loved ones around them and supported up to and following their death. Conference Purpose The goal of this conference is to improve end-of-life care and conditions across the state of Nebraska by providing education on end-of-life issues for physicians, nurses, social workers, advocates, administrators, and others working in the end-of-life field. Who Should Attend? This is a premier regional conference for everyone caring for patients and families with chronic illness or at the end of life. Attendees include physicians, nurses, therapists, social workers, administrators, and counselors. Cyber Station/Handouts The 2015 annual conference is paperless! 7 keynote speakers. Jennifer Kennedy, M.A., B.S.N., CHC, LNC Jennifer Kennedy is the Director of Regulatory and Compliance for the National Hospice and Palliative Care Organization. She has 28 years of experience as a leader and nurse in diverse health care settings and has worked in hospice care for more than 16 years and at the National Hospice and Palliative Care Organization for eight years. She has a bachelor s degree in nursing, a dual master s degree in health education and case management, and is currently working on a doctoral degree in health care education. Jennifer speaks at national and state conferences, writes articles for trade journals, and develops education and resources for hospice and palliative care providers. David Casarett, M.D., M.A. David Casarett is a palliative care physician and health services researcher whose work focuses on improving systems of care for people with serious, life-threatening illnesses. He is a Professor of Medicine at the University of Pennsylvania, a faculty member of the Pennsylvania Department of Medical Ethics, and Director of Hospice and Palliative Care for the University of Pennsylvania. Dr. Casarett is a recipient of the Presidential Early Career Award for Scientists and Engineers, the highest honor given by the U.S. government to researchers in the early stages of their careers. Dr. Casarett is the author of two non-fiction books, Last Acts and Shocked: Adventures in the Strange Science of Bringing Back the Recently Dead. His third book, about the science of medical marijuana, will be published in May T. Marni Vos, RGS T. Marni Vos is a clean and refreshing, award-winning humorist. She is as funny as she is inspiring. A motivational speaker, she entertains as she renews and invigorates her audience to be top performers in their work and as individuals. A former educator with three degrees, she continues to educate individuals, in all walks of life, to meet their challenges with a light heart and the echo of laughter. In today s business environment, where every skill counts, humor, laughter, and creativity can provide a winning edge. T. Marni Vos became one of only seven women in 20 years to be a finalist in the prestigious San Francisco International Stand-up Comedy Competition. She has performed on Lifetime s Girls Night Out and opened for Jay Leno. She has delivered more than 1,000 keynote speeches in over 45 states, Canada, and on cruise ships. Deborah Grassman, ARNP Deborah Grassman is CEO and co-founder of Opus Peace: Healing Center, a non-profit 501(c)3 organization whose mission is to help people re-own and re-home scattered pieces of broken self lost through heartache, neglect, abuse, trauma, death, and war. She is a nurse practitioner whose career with the Department of Veterans Affairs lasted for nearly 30 years. Deborah s book, Peace at Last: Stories of Hope and Healing for Veterans, is used by multiple agencies to learn about the effects of past trauma on the quality of a person s dying process, the influence of the various cultures of war on dying veterans, stoicism, and warrior wisdom. She has a new book, The Hero Within: Redeeming the Destiny We were Born to Fulfill, which describes the processes of abiding, reckoning, and beholding our interior hero. Shandy Loberg Shandy Loberg is CEO of Shandy s Solutions, a public speaker, and an author. Shandy is a mother of four children and a devoted Christian wife. Her background and college degrees are in human resources. She has owned her own business and has been a public speaker for 13 years. Shandy has spoken for 250 different companies and reached 4,000 people with her entertaining, educational and inspiring speeches. She has written a book titled Surviving Life: How to Turn Nightmares Into Dreams. 5

The presentation will look at the congressional and regulatory issues related to federally funded hospice care and the future of hospice and palliative care. Fascinating, thoughtprovoking, and funny, this session is an exploration of how all of these scientific breakthroughs are changing the way we think about life and death. The purpose of this activity is to enable the learner to understand who adult children and codependents are, what personal behaviors they exhibit, how they became that way, and the lifelong impact that these precipitating factors create in their lives. Gain an understanding of the dysfunctional family roles that grow out of these early experiences and how they remain a major aspect of one s personal life. This training will allow participants to select from a list of behaviors that are characteristic of adult children; given a dysfunctional family role, list two characteristics of this role; and describe at least three treatment strategies for adult children and codependents. A2: Becoming a Conversation Starter for End-of-Life Discussions Jackie Bates, R.N., CHPN, Owner and Founder, The Next Conversation, Bennet, Neb. The purpose of this activity is to enable the learner to become a conversation starter and helper in the discussion process as patients and families try to make health care decisions, especially regarding a terminal illness. This training will allow participants to diagram three key points on a timeline where critical health care decisions are made, identify motivating reasons (the timeline) for patients to become an active participant in their health care decisions, and formulate and apply their own personal style as a conversation starter for end-of-life discussions. A3: ALS Basics Shannon Todd, MSW/MPA, CSW, Care Services Specialist, The ALS Association Mid-America Chapter, Omaha, Neb. The purpose of this activity is to enable the learner to understand and recognize the clinical features of Amyotrophic Lateral Sclerosis (ALS), as well as 8 (continued on next page)

The learner will also understand the programs offered by The ALS Association and how The ALS Association can assist both clients and a hospice facility caring for a person with ALS. This training will allow participants to describe the clinical features of ALS including risk factors, common onsets, and how ALS is diagnosed; identify symptoms of ALS and explain common recommended management of these symptoms; and identify the organizational purpose of The ALS Association and describe the services provided to people with ALS and their families by The ALS Association. A4: Hospice Approach to the Pulmonary Patient Bob Bleicher, M.D., Medical Director, HoriSun Hospice, Lincoln, Neb. The purpose of this activity is to highlight the common and less common fatal pulmonary diseases such as pulmonary fibrosis, lymphangioleiomyomatosis, pulmonary hypertension, and Hepato-Pulmonary syndrome. It will also highlight respiratory failure from all causes including ALS, stroke, and head trauma. Participants will learn about these illnesses, their treatment, and the final symptom complex of dyspnea, cough, wheezing, chest pain, hemoptysis, and anxiety. Ventilator management in hospice will also be discussed, as well as terminal ventilator weaning. This training will allow participants to describe fatal pulmonary diseases, describe symptoms and signs of end stage pulmonary diseases, and describe treatment plans for pulmonary conditions. 2:00 p.m. - 2:15 p.m.... Break 2:15 p.m. - 3:15 p.m.... Concurrent Sessions [B1-B4] B1: Communication Approaches for Supervisors Sen. Colby Coash, B.A., Nebraska State Senator, District 27, Lincoln, Neb. This session will help participants identify their communication style and recognize how their strengths make them leaders and bring value to a team. Participants will leave with new strategies for communicating with their staff, peers and stakeholders. This training will allow participants to explain their personal communication style, practice applying their style to their work environments, leverage their strengths as they compare to other styles, and apply their communication awareness to future work through identified behavior changes. B2: Patient Safety: Why Do Medication Errors Happen and What Can I Do? Edward DeSimone, R.Ph., Ph.D, FAPhA, Professor of Pharmacy Sciences, Creighton University, School of Pharmacy and Health Professions, Omaha, Neb. The purpose of this activity is to enable the learner to understand the major causes of medication errors. The learner will be introduced to the National Coordinating Council for Medication Error Reporting Program and the guidelines for reporting medication errors. Learn risk management techniques that can be utilized to minimize medication errors. This training will allow participants to list and describe the five major categories of medication errors, describe the Medication Error Index, and list and describe at least three risk management techniques in order to reduce medication errors. B3: ALS and End of Life Shannon Todd, MSW/MPA, CSW, Care Services Specialist, The ALS Association Mid-America Chapter, Omaha, Neb. The purpose of this activity is to enable the learner to understand when patients with Amyotrophic Lateral Sclerosis (ALS) should be considered for hospice. The discussion will also include ALS-specific considerations on timing of hospice. The learner will be able to recognize ALS end-of-life symptoms and management techniques. The session will also explain challenges and needs of the ALS caregiver at the end of life and provide tools to assist caregivers at this stage of the disease. This training will allow participants to be able to describe the clinical features of ALS and define hospice criteria for ALS, identify end-of-life symptoms of ALS and explain recommended management of these symptoms, and identify end-of-life issues and characteristics of ALS caregivers. The purpose of this activity is to enable the learner to recognize the ethics principle of respect for autonomy within the context of end-of-life care and learn how to apply a decision-making tool to assist when conflicts occur. Participants will be able to describe the principle of respect for patient autonomy within the context of end-of-life care, identify barriers and challenges of respect for autonomy within the context of end-oflife care, and apply the patient-centered decisionmaking diagram to promote respect for autonomy. (This session meets the requirements for one hour of ethics continuing education for certified social workers.) 6.0 contact hours awarded. There are four attributes to our health: the physical, the mental, the emotional, and the spiritual. Both the patient and the healer know that stress is the number one medical bill in this country. Stress is not an event; it is how we choose to perceive an event. Perceiving the world around us with humor, hope, joy, and faith, not only assists the health care provider, but also the patient. This training will allow participants to be able to discuss the physiological responses of humor, respiration, ventilation, heart rate, blood pressure and flow, muscle tension, and hormones; identify what stress is and how we perceive it; and examine ways of incorporating positive attitudes into our daily life, not only in our professional lives, but our personal lives as well. 5:00 p.m.... Soul injury can subtly and not-so-subtly rob veterans of their vitality. The source of soul injury is unmourned grief from fallen comrades and other losses, as well as unforgiven guilt and shame over things veterans think they should or should not have done. Family members of combat veterans are also affected; their wounds, too, often go unnoticed and untended. This presentation will provide insight about soul injury and ways to respond. This training will allow participants to identify at least three reasons why soul injuries surface as veterans age, identify the essential elements of unmourned grief and unforgiven guilt, verbalize the role of stoicism in disconnecting from soul pain, and identify the essential elements of a soul injury ceremony. 10:15 a.m. - 10:45 a.m.... American culture overvalues youthfulness and undervalues aging. Acute illness is viewed compassionately and treated aggressively; chronic illness receives less attention, support, and sympathy. As members of our death-denying, youth-inspired society, we unconsciously adopt these values for ourselves. It is never too late or too early to make the acquaintance of the wise elder archetype that awaits redemption. This session will help you discover ways to access the interior sage awaiting your acquaintance. This training will allow participants to be able to identify cultural barriers that interfere with healthful aging and recognize our ageless, interior wise person and how it can be used to help us inhabit ourselves more completely. C2: Customizing Comfort Care to Individual Patient Needs: A Toolkit Approach Deborah Marks Conley, MSN, APRN-CNS, GCNS-BC, FNGNA, Gerontological Clinical Nurse Specialist; Assistant Professor; NICHE and AgeWISE Director, Nebraska Methodist Hospital, Omaha, Neb. Barbara Johnson, AD, R.N., Staff Development Nurse Oncology Surgery Unit, Methodist Hospital, Omaha, Neb. This session will provide a step-by-step comfort care plan for patients at the end of life in the acute care setting. It offers opportunities for interprofessional collaboration and provides patients and families with care that is consistent with their values and preferences. Components of the comfort care plan, as well as a potpourri of comfort care resources, supplies and options developed by this hospital will be presented. This training will allow participants to be able to describe comfort care plan development for inpatient use, examine the interprofessional comfort care plan, and examine a toolkit approach to comfort care in an inpatient setting. C3: Creating Opportunities for Discussing Endof-Life Planning: The Begin with the End in Mind Series Julie Masters, Ph.D., Professor and Chair, Department of Gerontology, University of Nebraska at Omaha, Omaha, Neb. A person s ability to communicate is an important survival tool in their pursuit of personal and professional goals. Effective communication helps us interact with the outside world throughout our entire life, no matter what age or ailments we may have. It helps us better understand a person or situation, enabling us to resolve differences, build trust and relationships, and perform effectively as a unit. Proper communication can create an environment where creative ideas, problem solving, affection, and caring can flourish. This training will allow participants to explain the differences between understanding and hearing, discuss how two-way communication is necessary to survive successfully, and apply survival tools which ensure more effective understanding of the message they are trying to convey. However, most people don t take time out of their busy lives to see the jewels the elderly can provide. Shandy will entertain and enlighten the listeners to such jewels as resiliency, forgiveness, commitment, and perspective by using some of her own humorous and entertaining experiences with the elderly. This training will allow participants to formulate a plan to challenge themselves to look at their lives with a new self-discovery attitude, describe the survival tool of forgiveness, explain the survival tool of commitment, and paraphrase the survival tool of perspective contact hours awarded.
