2 PRESENTATION OVERVIEW: In today s rehabilitation environment, organizations are challenged to reduce readmissions and acute transfers, decrease length of stay and ensure safe discharges of our patients to home. More than ever, clinicians and leaders must design and implement evidence-based initiatives that improve patient safety and outcomes. This one-day conference will showcase interdisciplinary collaborative projects and technological solutions for successful transition of persons with SCI from acute injury to lifetime wellness and recovery and provide attendees with real skills to improve patient outcomes across the care continuum. LEARNING OBJECTIVES: At the conclusion of this activity, participants will be able to: Identify evidence-based team strategies that reduce common medical and hospital acquired conditions of persons with spinal cord injury. Identify safe techniques and employment of technology to mobilize persons with spinal cord injury. List resources and strategies that enhance functional engagement in community living for persons with spinal cord injury. 3 CHALLENGE ACCEPTED: SAFELY NAVIGATING THE CARE CONTINUUM FOR PATIENTS WITH SPINAL CORD INJURY CONFERENCE AGENDA MORNING SESSIONS 7:30 to 8:00 a.m. Registration and Continental Breakfast 8:00 to 9:00 a.m. Keynote: Safety = Quality in Today s Healthcare Environment Jerry Zuckerman, MD, Chief Quality and Patient Safety Officer, Einstein Healthcare Network As Chief Quality and Patient Safety Officer at Einstein Healthcare Network, Dr. Zuckerman is responsible for all quality improvement and patient safety initiatives, as well as infection prevention and control activities for Einstein Medical Center Philadelphia, Einstein Medical Center Elkins Park and MossRehab. In this keynote session, Dr. Zuckerman will present information about programs designed to eliminate patient harm and promote patient safety. Different frameworks to employ that positively impact outcomes will be introduced and discussed. 9:00 to 10:00 a.m. PART ONE: Medical Complications after Spinal Cord Injury: Safely Identifying and Addressing Problems Marlyn S. Ramos Lamboy, MD, Physiatrist, Spinal Cord Specialty Program, MossRehab In this two-part session, learners will gain insight into medical complications that individuals with SCI can face. Part One addresses neurogenic bowel, neurogenic bladder and respiratory dysfunction. Learners will be able to identify presenting symptoms of these complications and gain understanding of treatment options available and what appropriate next steps can or should be taken. 10:00 to 10:15 a.m. BREAK 10:15 to 11:15 a.m PART TWO: Medical Complications after Spinal Cord Injury: Safely Identifying and Addressing Problems Wesley Chay, MD, Clinical Director, Spinal Cord Specialty Program Inpatient Unit, MossRehab In Part Two of this two-part session, attendees will gain insight into medical complications that individuals with SCI can face including skin wounds, pain, spasticity, autonomic dysreflexia, heterotopic ossification and fractures. Identifying presenting symptoms and gaining understanding of treatment options available as well as appropriate next steps will be discussed. 11:15 a.m. to 12:15 p.m. Safety Risks of Severe Psychiatric Symptomatology in Spinal Cord Injury Units of Care Inder Dashan Kalra, MD, Psychiatry Consultation Liaison, MossRehab; and Marni Nutkowitz, PsyD, Neuropsychologist, Spinal Cord Specialty Program, MossRehab Patients who are admitted to SCI inpatient rehabilitation units often have co-morbid psychiatric diagnoses. Some of these clinical syndromes, such as mood, personality, impulse control and eating disorders can pose significant threat to the safety of these individuals and can interfere with their willingness and ability to adhere to life-sustaining treatment. A single case study will be presented to delineate transdisciplinary strategies employed to ameliorate deleterious symptoms, enhance treatment adherence and minimize the risk of patient self-injurious behavior. 12:15 to 1:15 p.m. LUNCH AFTERNOON BREAKOUT SESSIONS Participants will be able to attend three of the six lectures listed below. 1:15 to 3:45 p.m. (each session is 45 minutes with a 15-minute break after the second session) A: MANAGING SAFETY CONCERNS IN COMMUNITY BASED ADAPTED RECREATION ACTIVITIES AJ Nanayakkara, Chief Executive Officer, Global Abilities Foundation Safety concerns are sometimes overlooked in an effort to increase participation in recreation by individuals with disabilities. However, a focus on safety is vital to a participant s long-term health. By educating participants about safety concerns and protocols, therapists and program managers can ensure that participants with disabilities gain the long-term benefits of an active lifestyle. B: TIPS ON COORDINATING A SAFE DISCHARGE PLAN Yadira Roman, BSW, MSCC, Social Worker/Medical Care Manager, SCI Unit, MossRehab It is crucial to establish a relationship between patients, families and team members in order to truly assess the existing social family supports. If you gain clear information from your evaluation, you can create a strong rapport with your patient and help them comprehend the changes coming in the future. C: DRIVING SAFELY POST SPINAL CORD INJURY Dennis Magrann, OTR/L, CDRS, Manager, Driving Program, MossRehab and Houman Ziai, OTR/L, CDI, DRS, Driving Program, MossRehab Resuming previous life roles and activities for individuals with spinal cord injury can be challenging; particularly, obtaining accessible transportation that safely takes into consideration new physical limitations. This presentation will highlight the ins and outs of returning to independent accessible driving, which includes the evaluation process, driver training, adaptive equipment, funding sources, state regulations and vehicle modifications. D: UTILIZING SMART HOME TECHNOLOGY: SAFETY, INDEPENDENCE AND BURDEN OF CARE Amanda Quinque, MS, OTR/L, ATP, Occupational Therapist, SCI Team Leader, MossRehab This presentation will explore the integration of smart home technology into the daily lives of persons living with spinal cord injury. We ll discuss the many uses, benefits and adaptability of commercially available products in home control, safety, communication and independence in daily life roles as well as how this technology can greatly impact the overall quality of life for persons with SCI. 4 E: USE OF TECHNOLOGY FOR SAFE AND EARLY PATIENT MOBILIZATION FOLLOWING SPINAL CORD INJURY Naomi Greenberg, PT, DPT, GCS, ATP, Physical Therapist, SCI Team Leader, MossRehab The concept of safe patient handling is a significant concern for many healthcare organizations in order to prevent injuries to both patients and staff members. The design of newer technology allows for the development of innovative treatment techniques for early mobilization of patients after a spinal cord injury. By incorporating technology into therapy sessions, therapists and nurses can find various means to increase patients functional skills in order to facilitate a safe discharge home. F: ACHIEVING AN A ON THE PATIENT SAFETY HARM SCORE CARD Shari Goodman, RN, MSN, CRRN, Clinical Manager, SCI Unit, MossRehab and Beth Jacobs, RN, CCM, CRRN, SCI Specialist/Clinical Coordinator, Spinal Cord Specialty Program, MossRehab This presentation will focus on two quality indicators identified by CMS as important measures to determine the safety of our patients. MossRehab implemented team based strategies to limit harm which resulted in successful outcomes. We will share detail of the Medical Early Warning System (MEWS) and how it reduced our unplanned transfers to acute care and a transdisciplinary hospital-wide model to limit your risk for hospital acquired pressure injuries (HAPI), reduce PI incidence rates and manage IRF-PAI reporting.
