All registration forms or completed online registration must be postmarked/ ed by this date for early registration fees to apply. Regular registration fees apply to online registrants and forms received up to October 15. Forms and online registration received after that date will be charged the on-site registration fee. Payment: Check, money order, agency PO, or credit card (MC, VISA, Discover, American Express) will be accepted as payment. Please make checks payable to Stockton Continuing Studies/NJOTA Annual Conference and mail to above address. Confirmation: If you supply a readable, active address, you will receive an confirmation of your registration. Otherwise, your cancelled check or credit card bill is your confirmation. Your certificate of CEU/PDU can also serve as a receipt. Cancellation: In the event of inadequate enrollment, the college reserves the right to cancel events. Therefore, it is especially important to register early. Any cancellation by a registrant must be received at least 3 days prior to the event for full refund. Cancellation requests made within 3 days of an event will receive 50% refund. All refunds are subject to a $10.00 processing fee. No refunds will be made for cancellations on day of conference. If, due to any unforeseen circumstances, the conference program must be revised, every effort will be made to make an appropriate substitution in line with the high quality of the conference offerings. Refunds will not be provided under these circumstances. 1
