Purpose The purpose of this conference is to provide long-term care and assisted living leaders (including homes for the aged, adult foster care and other independent and supportive living settings) with quality, implementable standards of practice to promote facility/center management, operations, continuity of workforce and optimal resident care and service. Target Audience This conference is designed for executive-level leadership including facility/center owners, corporate-level consultants (CEO, CFO, COO, etc.), regional directors, administrators, executive directors, directors of nursing, nurse managers, directors of finance and other long-term care and/or assisted living management personnel (including homes for the aged, adult foster care and other independent and supportive living settings). Continuing Education Participants will have the opportunity to earn up to 10 licensed nursing home administrator contact hours. This continuing education activity is appropriate for adult foster care providers and certified assisted living directors for a recommended maximum of 10 clock hours of professional development. Participants may also earn up to 10 nurse contact hours. HCAM is an approved provider of continuing nursing education by the Wisconsin Nurses Association, an accredited approver by the American Nurses Credentialing Center s Commission on Accreditation. 2

Dr. Josh Luke National Bundled Payment Collaborative Skilled nursing facility (SNF) and assisted living (AL) owners, administrators and directors of nursing will not want to miss this cutting edge presentation from a best-selling author and one of the leading healthcare futurists in the country. This presentation, led by a former SNF & AL administrator, who also served as a hospital CEO, breaks down which bundled payment models are most relevant to SNF and AL, as well as the most appropriate way to position post-acute care for each bundled payment model. The presentation provides great detail not just on all of the hospital penalty programs, but shares how hospitals and SNFs are being penalized when post-acute care length of stay is excessive and when SNFs routinely discharge to home health. The Care Plan Act will be a major focus, and strategies on how SNFs best align with referral sources and help them avoid penalties, while avoiding (recently announced) SNF penalties for over-utilization as well. With the ever-evolving state and federal labor and employment laws, it is critical that employers keep up-to-date to avoid legal liability. Simultaneously, we are challenged with re-organization at the State and changes to state law permitting us to hire physicians. This session organizes the changes and offers suggestions to implement and manage change. Recommended for skilled nursng audiences only ANTIBIOTICS: A LOOK AT THE INCREASED SCRUTINY OF ANTIBIOTIC USE IN NURSING HOMES Bill Vaughan Remedi SeniorCare Rob Shulman Remedi SeniorCare Antibiotic use in nursing homes has come under intense scrutiny due to studies that suggested antibiotics are overused and poorly managed. Misuse of antibiotics can cause profound clinical implications for the residents and adversely affect the regulatory health of the facilities who administer them. This presentation, given by a board certified geriatric pharmacist and the former Chief Nurse of Maryland s state survey agency, will provide an overview of current and proposed federal regulations which impact antibiotic use. Best clinical practices designed to reduce the risks associated with antibiotics will also be explored. Finally, antibiotics stewardship will be discussed from both a clinical and operational perspective. Recommended for skilled nursing audiences only THE SKILLED SHIFT: HOME HEALTH TO HOME CARE & ASSISTED LIVING Dr. Josh Luke National Bundled Payment Collaborative In this session, long-term care providers will learn about Medicare Spending per Beneficiary (MSPB) and its impact on the hospital. Long-term care providers will learn how they can have an impact on the hospital s penalty and how to open doors through the referral sources to effectively manage this process. The specifics of the Care Plan Act and how it impacts MSPB. Finally, this session will discuss the proposed Post-Acute MSPB penalty and how long-term care providers can minimize exposure. Explore risk arrangements for these bundled payment models and how to decide if your facility is ready to make this type of contract. Also, the presenters will provide an update on recent changes in Medicare and Medicaid reimbursement and status of the MI Health Link pilot program. CMS penalty periods are almost always a three year look back so the time to act is now as SNF s are being measured based on current performance. This session will explore which two key indicators impact a SNF s post acute Medicare Spending per Beneficiary exposure, and how a SNF can begin to address them with relative ease in a short amount of time, to ensure optimal coordinated patient care.
