You will gain new perspectives on issues that drive your everyday work. The event is structured to combine networking and education on the hard topics that are confronting the senior care industry. The conference addresses issues facing all aspects of therapy but is pertinent to the whole range of long term supports and services professionals. MDS Expert, Maureen Wern will share insights on the implications of this change. She will also provide steps that you can take now to ensure that your patients acuity is captured accurately under the new methodology. A must attend session for all long term services and supports professionals. Presented by: Maureen Wern 10:00 am 10:15 am Networking Break 10:15 am 11:15 am Comprehensive Care for Joint Replacement (CCJR) Comprehensive Care for Joint Replacement (CCJR), covers lower extremity joint replacement procedures, specifically Medicare Severity-Diagnosis Related Groups (MS-DRGs) 469 (Major joint replacement or reattachment of lower extremity with Major Complications or Comorbidities (MCC)) and 470 (Major joint replacement or reattachment of lower extremity without MCC). Unlike voluntary bundling experiments such as Bundled Payments for Care Improvement (BPCI), CMS proposes to require all hospitals in designated Metropolitan Statistical Areas (MSAs) to participate in CCJR. Join Diane as she provides information on the rules and requirements of CCJR as well as how senior care providers can develop strategies and tactics to create a post-acute partnership. She will assess the key success drivers in bundle performance. Presented by Diane Dietz 11:15 am 11:30 am Networking Break 11:30 am 12:30 pm Working with Hospitals Partnering with hospitals is essential to driving admissions and reducing rehospitalizations. In the ever-changing world of long term care, our residents are entering our facilities with more subacute, complex diagnoses. They are younger and more fragile. Understanding this population and working closely with hospitals is the new normal. Shane has facilitated this relationship building in the Cincinnati and Dayton markets, and brings a level of expertise to a difficult subject. Presented by Shane Craycraft
4 Conference Schedule: 12:30 pm 1:15 pm Networking Luncheon (included in your registration fee) 1:15 pm 2:15 pm What s all the fuss about Antipsychotics? More than half of nursing facility residents have some form of dementia, many of whom experience behavioral and psychological symptoms associated with dementia (BPSD). These behaviors, which can pose significant challenges for the staff and residents, are often best treated with non-pharmacologic measures such as environmental modifications. In 2012, CMS launched a nursing facility quality initiative that included a goal to decrease the off-label use of antipsychotics. Unfortunately, Ohio remains at the bottom of the list in reducing antipsychotics. Deb Kriner will share information on the heightened scrutiny by the Federal government (CMS, OIG, DOJ) as well as identify areas concerning antipsychotic drugs that surveyors will focus on. She will share liability concerns with antipsychotics and how to mitigate them as well as effective strategies to reduce antipsychotics. CMS has communicated that Ohio will be one of the states they focus on due to our lack of progress when compared to other states. Therapy will play a large role in assisting with the reduction through interventions that are more therapeutic versus pharmaceutical. Presented by Deb Kriner 2:15 pm 2:30 pm Networking Break 2:30 pm 3:30 pm Cardiac Rehab and COPD LOS Reduction Partnering with hospitals and physicians to reduce the length of stay for residents by utilizing the physician s protocols is important for the residents in our centers. Effectively managing subacute diagnoses that often leads to avoidable rehospitalizations is necessary with the changing payment systems. COPD is a diagnosis that often times, if not managed appropriately can have a negative outcome for the patient. Understanding the fragility of a patient s condition and managing their care in accordance with physician direction and oversight especially with regard to their therapy plan is essential in positive outcomes. This session will give you both the facility s and the therapist s role in this process. Presented by Scott Unverferth and Keri Romes

He is a Registered Nurse, a Licensed Nursing Home Administrator and an active advocate for quality. Participation in the profession has led him to be a named an AHCA Future Leader and Board Member of the Ohio Health Care Association along with various other committee roles. He worked with the Ohio Department of Aging Person Centered Care Coalition and at one time sat on the AHCA Survey and Enforcement Committee. He has served in many positions throughout his healthcare career including DON, Administrator, VP of Operations and is currently the Corporate Director of Clinical Operations for HCMG. Diane Dietz Diane Dietz is the Assistant Executive Director of the Ohio Health Care Association. Diane has over 30 years experience in long term care. Debra Kriner, RN, C Debra Kriner of Sylvania, Ohio is owner and operator of d kriner & associates LLC, which was established in 1994 as a consultative median between long-term care and governmental agencies. Debra has demonstrated expertise in crisis management and system analysis. Debra works with several law firms reviewing medical records and providing expert testimony. Debra also provides educational programs, which focus on the regulatory arena of long-term care and enjoys providing these seminars for various organizations throughout the Midwest. Her past twenty years of experience includes dayto-day operations as a director of nursing and direct care nurse in both long term care and pediatrics. She has been involved extensively the past fifteen years with Ohio Health Care Association serving on many regulatory and nursing committees. She currently is serving on the Board of Trustee for the Ohio Health Care Association. She also serves on the Regulatory, Clinical, and Disaster Planning committees for AHCA. Kerri Romes Kerri Romes has been in the long term healthcare industry for over 10 years. She is a licensed Occupational Therapist who has been working with the geriatric population as a STNA, a restorative aide, and a therapist. She has 10 years of therapy clinical experience in long term care, home health, and inpatient rehab settings. Kerri has a passion for developing innovative treatment programs to assist the long term residents remain at their highest functional level. Scott Unverferth, LNHA Scott Unverferth has been in long term care for 15 years, and has his MBA in Organizational Leadership. He is a licensed nursing home administrator in Ohio and Pennsylvania. He has held roles as STNA, Administrator, Regional Manager, and Director of Operations for HCF Management. Scott serves on various health related board of directors and has been active on the Facility Standards Committee at OHCA for several years Maureen Wern, CEAL, CNAC Maureen Wern, CNAC, CEAL is the President of Wern and Associates, Inc. and a recognized expert in long-term care consulting and education. With over 35 years of hands-on experience and industry expertise, Maureen utilizes her in-depth knowledge and experience to help providers understand and address issues associated with the MDS, RUGs and case mix management. She is also known for her expertise in QAPI, survey readiness, staffing, clinical services and overall facility management. Maureen is a frequent, well-respected presenter for the Ohio Health Care Association. She currently serves as an OHCA Board member, a member of the OHCA Executive Committee and the Chair of the OHCA Convention Committee.
