2 COURSE Head Highlights SAFE OPIoID PRescribing February 25 26, 2012 Palm Springs, CA A National Health Priority The tension between patients who are seeking pain relief and the prescription drug abuse epidemic raises concern for many primary care physicians (PCPs). In fact, the American Academy of Pain Medicine (AAPM) has identified that safe pain management is one of the top five concerns among PCPs. Add all the media attention and regulatory scrutiny that surround opioid prescribing, and many PCPs simply back away from treating pain. However, recent research cited in the 2011 Institute of Medicine s (IOM) report Relieving Pain in America shows that PCPs can play a very important part in helping people with pain. PCPs treat approximately 52% of chronic pain patients. 1 Of the chronic pain patients treated on an ongoing basis, 51% continue with PCPs. 1 Twenty-nine percent of PCPs prescribe opioids less often than they deem appropriate because of regulatory oversight concerns. 1 The PCP s response (to a pain patient s request for help) may be crucial in providing timely relief and preventing acute pain from progressing to a persistent or chronic state. 2 The IOM report strongly advocates for many of the axioms AAPM has long embraced, and as a result, AAPM is running this course concurrently with AAPM s 28th Annual Meeting, which is themed This Is Our Time. 3 Head Course Highlights A National Health Priority: The Public Health Crisis of Pain Program Goals AAPM has established four overarching goals for this program. These goals both reflect AAPM s long-term, public-healthoriented vision and provide the framework for the Safe Opioid Prescribing course. 1. Decrease the number of unintentional deaths from prescription opioid analgesics related to chronic pain treatment plans. 2. Decrease the number of emergency room admissions for prescription drug overdoses in patients prescribed opioid analgesics as part of chronic pain treatment plans. 3. Reduce the number of admissions to opioid substance abuse treatment centers for patients prescribed opioid analgesics as part of their chronic pain treatment plans. 4. No decrease in access to appropriate opioid therapy in the treatment of chronic pain. Intended Audience All PCPs and clinicians committed to minimizing the risks and reducing the number of deaths associated with opioid analgesics in the treatment of chronic pain maintaining access to the appropriate use of opioid analgesics for the millions of American adults who suffer from chronic pain. Educational Objectives After attending the Safe Opioid Prescribing course, learners will be better able to identify the known root causes related to the abuse, misuse, and unintentional deaths related to opioid analgesic use in the treatment of chronic pain understand the potential risks of abuse and diversion for both conventional and emerging opioid formulations incorporate the core components of the Federation of State Medical Boards (FSMB) Model Policy for the Use of Controlled Substances into their practices implement multifaceted interventions aimed at decreasing the risks for opioid analgesic abuse by chronic pain patients improve the implementation of best practices related to patient adherence, assessment, and evaluation, and prescriber and patient communication. Educational objectives for sessions and workshops are outlined on AAPM s website at Expected Outcomes As a result of the CME activity and after attending the Safe Opioid Prescribing course, prescribers will be able to use the knowledge of safe opioid prescribing practices to implement evidence-based clinical tools and protocols in their practice understand the prevalence of pill shopping, the significant risk it presents to prescribers, and how to use state prescription monitoring programs (PMPs) to mitigate that risk demonstrate improved knowledge of using urine drug tests for diagnosis and ongoing treatment, which includes the ability to implement uniform treatment protocols in the PCP setting, including uniform interpretation, understanding when to enact follow-up tests, using effective tools for communicating results with patients, and documenting results in the patient medical records demonstrate improved knowledge of extended-release and long-acting opioid pharmacology, including dosage forms, intervals, key instructions, drug interactions, adverse events, relative potency to oral morphine, and relative use in opioidtolerant patients. The competing public health crises of undertreated pain and prescription drug abuse continue to increase the need for safe and effective pain management strategies within the busy primary care practice. AAPM, as the premier physician organization committed to advancing the quality and practice of pain medicine, recognizes pain management and opioid therapy as a top 5 concern among PCPs. Limited professional education in effective pain management strategies leaves PCPs fearful of regulatory scrutiny and reluctant to implement chronic opioid therapy. Yet, pain medicine can be an effective and appropriate component of a comprehensive chronic-pain treatment plan. The missing component, however, is proper education for both providers and patients. 4 Program sessions and workshops Program Structure AAPM s Safe Opioid Prescribing course addresses much more than the standard how to prescribe continuing medical education (CME). The course combines highly interactive sessions presented by international pain experts from a broad base of treatment settings with robust workshops, real case studies, and lively question-and-answer sessions. It includes everything you need to acquire the expertise and confidence to help your patients with pain and make a difference in this national health crisis. The Safe Opioid Prescribing course is a 2-day course aimed at PCPs who seek to provide better care for their patients with pain from the experts in pain. 6 General Information beyond the 2-day course Several other components of the Safe Opioid Prescribing course are being created to expand its impact. Advocacy and Public Education Initiative To complement the prescriber education program, the Academy plans to collaborate with key stakeholders to organize a medication take-back event in Palm Springs, CA, adjacent to the Safe Opioid Prescribing course. This demonstrates a best practice related to reducing diversion of opioids and affords an opportunity to connect with local antidrug coalitions, parents, teachers, and law enforcement officials about the harms of prescription drug abuse and diversion and the value of take-back/safe-disposal programs. Integration of Maintenance of Licensing (MOL) This CME intervention has the potential to be a vital component of the MOL process through ongoing self-assessment and demonstration of safe opioid prescribing competence. Quality Improvement and Patient Safety Initiatives The Academy anticipates that this initiative will provide multiple opportunities to address and overcome the inherent barriers to safe opioid prescribing practices in the primary care setting. Best practices and lessons learned from this longitudinal CME activity can also be integrated into regional and national patient safety and quality improvement initiatives.
