The Medical Neighborhood, an expanded concept of patient-centered care, recognizes that the effectiveness of the PCMH care model in achieving integrated care is dependent on the coordination of subspecialists, specialists, and other health care entities involved in patient care. To be eligible for documentation of credit, participants must attend the full activity and submit a completed evaluation form. Participants who complete the evaluation online at PCMHCongress. com within 4 weeks of the live meeting will immediately receive documentation of credit. ACCREDITATION In support of improving patient care, the National Committee for Quality Assurance (NCQA) is accredited by the American Nurses Credentialing Center (ANCC), the Accreditation Council for Pharmacy Education (ACPE), and the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing education for the healthcare team. This live educational activity has been approved for AMA PRA Category 1 Credit. Continuing nursing education contact hours will be provided to participants.
