2 ORGANISATIONAL ENVIRONMENT Bay of Plenty District Health Board is a government owned company (Hospital and Health Service), which provides both hospital and community-based health care. Currently the company provides health service from Tauranga, Whakatane and community-based venues. Reporting Relationships Clinical Director Business Leader Professional Advisor, Occupational Therapy Regional Clinical Coordinator CAMHSS Multi-disciplinary Team PRIMARY FOCUS Mental Health & Addiction Services is committed to the provision of safe and accountable acute response services and dynamic ongoing care delivery. Interventions are predicated on robust assessment and collaborative planning using an holistic approach and authentic therapeutic engagement with the client and his/her family. The Occupational Therapist will work as a member of the Mental Health & Addiction Services to provide an effective community based assessment and treatment service to the target populations within existing legal frameworks. Occupational Therapists working with Mental Health & Addiction Services understand that staff will maintain skill-diversity by adhering to training requirements and taking opportunities to collaborate with various roles within the service. 3 The Occupational Therapists will contribute to, and be guided by national strategic service directives such as the New Zealand Health Strategy. National Mental Health Sector Standards, Recovery and Strengths models, as well as local strategic directions outlined within policy and protocol. The mental health care environment is evolving to meet expectations, priorities and needs of the community and all staff are expected to creatively contribute to service changes to meet identified needs. The Occupational Therapists will provide safe, effective client care using professional knowledge and skills in accordance with DHB policies and protocols and the New Zealand Occupational Board s scopes of practice. Principal Accountabilities: 1: Clinical Practice Maintains professional practice standards of clinical practice in accordance with ethical, professional and organisational guidelines.» Will provide specialist mental health input effectively utilising evidence-based practice to assess, implement and evaluate care.» Determine client needs through comprehensive functional assessment.» Supervision, development and co-ordination of a day programme.» Implement appropriate treatment plan. Revises and adapts plan as necessary.» Fully involves clients and families with all aspects of the treatment process.» Attends regular clinical supervision.» Provides supervision (if suitably qualified).» Demonstrates a flexible approach and ability to cope with changing situations.» Assists other staff in facilitating groups. 2: Communication and Teamwork Interacts effectively with the client, family/whanau, the multidisciplinary team, community groups and agencies and other health professionals» Participates positively as a member of the multi-disciplinary team, demonstrating individual responsibility and accountability.» Works co-operatively and collaboratively to ensure a consistent approach to the attainment of the team s objectives.» Accepts referrals from other team members for Occupational Therapy specific tasks and Day Programme.» Creates effective networks among colleagues and seeks advice as required.» Identified conflict and attempts to reach a positive conclusion.» Demonstrates an ability to access information systems as appropriate. 4 3: Continuous Quality Improvement» Demonstrates a commitment to continual quality improvement activities and undertakes where appropriate.» Regular review of occupational therapy day programme with input from multidisciplinary team and Regional Clinical Coordinator. 4: Ethical and Legal Practise Clinical practice adheres to relevant legislative, company, service and professional requirements.» Holds a current Practising Certificate» Develop and maintain personal professional practice in accordance with professional standards, legislative requirements, policies and guidelines.» Is aware of legislation that impacts on mental health care delivery and practices within legal boundaries. This is to include the Mental Health (Compulsory Assessment and Treatment) Act 1992 the Privacy Act 1993 (Health Information Privacy Code 1994) and the Children, Young Person s and their Families Act 1989.» Considers ethical issues when involved in planning or contributing to decision making around client care.» Demonstrates knowledge of legal and ethical requirements pertaining to informed consent and other procedures that may impact upon the rights of clients.» Maintains currency of ethical trends, research and recommendations relating to practice and health issues.» Takes responsibility for reading and practising according to BOPDHB s policies and procedures.» Regularly records and reports statistical information and data as required. 5: Cultural Safety Care is client focused and planned in regard to ethnic, cultural, religious and other individual needs.» Demonstrates an active understanding of the Treaty of Waitangi and its application within Mental Health and the importance of Tangata Whanau.» Is aware of and upholds BOPDHB s commitment to the Treaty of Waitangi and the improvement in Maori health.» Demonstrates awareness of own cultural background, attitudes and values» Cultural and spiritual needs of clients are met with sensitivity, including those of family/whanau and significant others.» Consultation occurs with Maori Health Services, in relation to client care as appropriate.» Attends bicultural training offered by BOPDHB. 5 6: Professional Development Accepts responsibility for own professional development.» Participates in professional peer review.» Attends and participates in the Mental Health & Addiction Service inservice programme.» Undertakes additional courses to promote personal and professional development.» Maintains BOPDHB s and Mental Health & Addiction Service mandatory certifications and additional clinical skills relevant to area.» Maintains an up-to-date knowledge of care/treatment/research in the area of child and adolescent mental health and their families.» Maintains administrative requirements of Mental Health & Addiction Service.» Participates in annual performance review process.» Participates in service evaluation as required. Occupational Therapist Case management» The case manager role outlined here should be regarded as an extension of the key tasks and responsibilities outlined above.» The Occupational Therapist case manager will work within an integrated and eclectic model of care predicated on crisis resolution, strengths and Recovery philosophies that aims to promote the service-user s strengths in managing mental disorder and psycho-social sequelae and decrease the destructive potential of mental disorder and utilise person-specific risk management and relapse planning in a timely and congruent manner.» Case Manager functions include acute assessment and response and timely management of referrals, ongoing care, review and discharge planning. The case load may include clients experiencing acute mental disorder as well as specific therapeutic engagement and includes facilitating: access to care and ensuring smooth transitions along the care pathway for clients and their families resolution of distress and effective management of mental health issues re-integration with family and primary care networks Occupational Therapist, Voyagers, WHK December 2009 Page 5 of 8

6 » Occupational Therapist will be expected to utilise extensive psychological knowledge and a broad range of skills and therapeutic strategies to achieve these aims. The mix of skills that are required include: Knowledge of mental disorder Competence in comprehensive assessment, risk assessment, the use of the mental status examination and problem formulation Clinical skills in engagement, de-escalation, conflict resolution, and problem solving Ability to discuss and negotiate management plans with clinicians Ability to prioritise Excellent interpersonal skills Good written and oral presentation skills Strong client focus Sensitivity to the context and experience of to others Cultural awareness and safe practice Respect for privacy and confidentiality A professionally based attitude to mental health care Competence in the implementation of discrete therapeutic frameworks Knowledge of relevant legislation including Mental Health (Compulsory Assessment and Treatment) Act 1992, Privacy Act 1993, Health and Disability Act, Health Practitioners Competency Assurance Act, and the NZ Health Strategy (Te Tahahu; Te Kokiri) A function of BOPDHB is to provide a 24-hour service. This may at times necessitate you being required to change duties or transfer to another ward or department to ensure adequate coverage. This position description is not exhaustive and the incumbent may be requested to perform any reasonable task within the scope of the position as requested by the Clinical Nurse Leader. This position description will be reviewed from time to time in consultation with the incumbent. HEALTH PRACTITIONERS COMPETENCE ASSURANCE ACT You are required to maintain your current competency based practicing certificate. 2. You must notify Manager of any changes to scope or conditions on practice (determined by Regulatory Authority). 3. You must complete the requirements of any competency programme. 4. You must notify employer of concerns relating to the risk of harm to the public of another health practitioner practicing below the required standard of competence. 5. Know the provisions of the HPCAA as the governing legislation.
