VOTE FOR ALPHA OFFICES The Nominations Committee chaired by Jamie Manning has selected an excellent slate of candidates who are highly qualified for each vacant office and who have been active in the association. 4 Members may vote in the following ways: 1. BALLOT FOR ELECTION OF OFFICERS 2013 ANNUAL MEETING For validation purposes (AlPHA Membership), please enter the last four digits of your social security number. Ballots without the 4 digits will not be considered valid. Voting AlPHA Member XXX-XX- VICE PRESIDENT Suzanne Terrell - As social work manager in Public Health Area 9, based in Clarke County, Suzanne has worked with the Alabama Department of Public Health for 12 years. She has served on the AlPHA Board as Alternate Delegate to the American Public Health Association, and currently serves as treasurer. She was honored to receive the Guy M. Tate Award in 2005, and Social Worker of the Year in TREASURER Sonya Tinsley - An 11-year member of AlPHA, Sonya has worked as an administrative support assistant in both Madison County and Public Health Area 2, as office manager for Area 2, and currently as tobacco prevention coordinator for Area 2. She has assisted with the AlPHA silent auction for several years and served as vendor coordinator. This is her third year of service as corresponding secretary. RECORDING SECRETARY Melanie Dickens - Melanie began state employment in 2007 and is currently employed as the tobacco prevention and control coordinator in Area 1, which encompasses six counties. She received her undergraduate degree as well as her master s degree from the University of North Alabama. Melanie resides in Sheffield with her husband Pete and their daughter Gabrielle. Away from work, she enjoys spending time with her family and being an active participant in community and civic organizations. SHA ALTERNATE DELEGATE Kevin Hicks - Kevin has 19 years service with the Alabama Department of Public Health. He has worked as a senior environmentalist and currently serves as a senior radiation physicist in the Emergency Planning Branch of the Office of Radiation Control. Currently, he serves as a liaison to promote and educate the public on radiation topics. He is a South Central Public Health Leadership Institute scholar and continues to be an advocate for the mission set forth by the Alabama Department of Public Health. APHA ALTERNATE DELEGATE Ross Hudson - Ross has been a member of AIPHA for 12 years and has served in several positions on the AlPHA Board, including president and treasurer. He has enjoyed working with the talented individuals that make up the organization and would like to continue to serve them as the APHA Alternate Representative. He is the Area Social Work Director for Area 2, supervising case management staff in seven counties. SHA DELEGATE Natalie Quinney - Natalie is a Public Health Area 7 social work manager located in Marengo County. She has been employed with ADPH for 11 years and a member of AlPHA for 10 years. She has served as the corresponding secretary and alternate Southern Health Delegate for the past 3 years. In 2008 she was honored as the Public Health Social Work Manager of the Year and is a graduate of the 2008/2009 South Central Public Health Leadership Institute. 5
6 RUTH HARRELL NURSING SCHOLARSHIP APPLICATION *Awards for Summer 2013 will be announced (April 4, 2013). Applicants for the Ruth Harrell Nursing Scholarship must meet the following qualifications: Must be a member of the Association. Must hold a current Registered Nursing License. Must be accepted into a National League for Nursing accredited nursing program on a full-time or part-time basis to advance the candidate s nursing education. 1. Preference will be given to those planning to practice in Alabama. 2. Preference may be given to those expressing a desire to practice public health nursing, including but not limited to work within the Alabama Department of Public Health. 3. Past awardees may apply for a second scholarship. They will compete with all eligible applicants for that award year.
