1 Winter 2016 A MESSAGE FROM THE PRESIDENT Letter from the President Sarah Owens, CPOC Welcome to 2016! Who made New Year s Resolutions? My New Year s Resolution is to break my New Year s Resolution...that way I succeed at it for once! I would like to start by thanking everyone for their continued support of the PA Paraoptometric Association. We, as your Board, are excited to keep you engaged in your career as a paraoptometric. Hopefully, along the way, we can all have a lot of fun doing it too! The past few years have shown a tremendous change in the health care realm. Both for health care providers and for the everyday healthcare consumers. Current day optometry demands that paraoptometrics provide an integral role in patient care. Third party payers and your patients need assured they are receiving the best care from a highly qualified team. Doctors are increasingly more dependent upon everyone in the office to reach these goals; from the manager, technician, optician, and including the receptionist. For instance, are you entering medication prescriptions or eyeglass orders in EHR? For Doctors who participate in the Medicare & Medicaid Electronic Incentive Programs to fulfill meaningful use requirements, the AOA s review of the regulations and guidance from the Centers for Medicare & Medicaid Services recommends that only credentialed staff members with designations such as Certified Paraoptometric Technician (CPOT), Certified Paraoptometric Assistant (CPOA), and Certified Paraoptometric (CPO) enter orders related to CPOE. You as staff paraoptometrics can receive training and continuing education in order to fill this role. Any yes, everyone, Meaningful Use is not disappearing entirely. Paraoptometric certification through the American Optometric Association is your opportunity for advancement. Becoming a certified paraoptometric is easy. For testing information, simply go the AOA Paraoptometric s/staff Section online at As a member of the Pennsylvania Paraoptometric Association (PPA), you have free access to study materials in our Loaner Library. Education will be offered from Friday May 13, 2016-Saturday May 14, I am planning a wide variety of topics to be presented. Once again a charm will be presented to all attending Paras. We will have new lanyard in the spring that will also be available to all attending Paras. I look forward to seeing you all in the spring. The good news is that CMS has recognized "paraoptometric certification" acceptable for performing those particular functions. This is a great milestone for AOA paraoptometric certification and creates awareness of how valuable it is to be certified. AOA deserves a lot of credit for having the foresight to create a recognized certifying body for paraoptometrics

AOA clarifies meaningful use rule on electronic order entry Certified paraoptometric staff may perform computerized provider order entry (CPOE) to fulfill meaningful use requirements, per a clarification from AOA reinforcing the benefits of AOA associate membership. Based on AOA's review of the regulations and guidance from the Centers for Medicare & Medicaid Services (CMS), AOA recommends that doctors of optometry only allow credentialed staff members, with designations such as Certified Paraoptometric Technician (CPOT), Certified Paraoptometric Assistant (CPOA) and Certified Paraoptometric (CPO), to enter orders related to CPOE. The AOA recently clarified with CMS that doctors of optometry are responsible for determining which credentialed staff members, if any, are qualified to perform the CPOE function as a meaningful use measure in the Medicare & Medicaid Electronic Health Records (EHR) Incentive Programs. CMS reiterated that the agency is "neither qualified nor authorized to make a determination," but referred to August 2013 guidance that states "appropriately credentialed" staff can perform the function and have it count toward the measure. Given that CMS specifically indicates "laypeople" are unqualified to perform duties associated with CPOE and that those selected to assist must be certified by organizations other than the employer, ensuring that staff members tasked with entering orders receive additional training and certification such as CPOT, CPOA or CPO would be a positive first step. In addition to designating certified paraoptometrics to perform CPOE, AOA recommends documenting how those individuals were determined qualified to enter such orders. This documentation should include the individual's paraoptometric credentials, specific experience, education or training that might be relevant, information about the workflow used in the office to enter orders, and compliance with any pertinent local, state or federal laws. This information will be helpful in the event that a doctor is audited related to the EHR Incentive Program Doctors of Optometry May Use Certified Paraoptometric Staff for Electronic Order Entry CMS Defers to Physicians, Federal and State Law in Determining Who Can Enter Medication and other Orders for Meaningful Use Many doctors of optometry who participate in the Medicare & Medicaid Electronic Health Records (EHR) Incentive Programs have asked whether credentialed staff members are qualified to perform computerized provider order entry (CPOE) to fulfill meaningful use requirements. Based on AOA s review of the regulations and guidance from the Centers for Medicare & Medicaid Services (CMS), the AOA recommends that doctors of optometry only allow credentialed staff members with designations such as Certified Paraoptometric Technician (CPOT), Certified Paraoptometric Assistant (CPOA), and Certified Paraoptometric (CPO) to enter orders related to CPOE. Doctors of optometry must also personally ensure that any individual selected to enter orders has the education and training to perform the clinical expectations for this task and that designating this task to the credentialed staff member is in accordance with local, state and federal law. The AOA recently clarified with CMS that doctors of optometry are responsible for determining which credentialed staff members, if any, are qualified to perform computerized provider order entry (CPOE) to fulfill meaningful use measures for the Medicare electronic health records (EHRs) incentive programs. Untrained staff are not considered acceptable options, so AOA recommends members take advantage of resources in the AOA Paraoptometric Resource Center and document that staff with the designations of CPOT, CPOA, and CPO have adequate training and comply with local, state and federal law. CMS recently reiterated to AOA that the agency is neither qualified nor authorized to make a determination on the specific medical training and responsibilities which determine the appropriateness of a particular medical staff member to enter orders for CPOE. CMS referred AOA to the following guidance dated August 20, 2013 for more information: When meeting the meaningful use measure for computerized provider order entry (CPOE) in the Electronic
4 ParaEyes Page 4 Winter 2016 Health Records (EHR) Incentive Programs, does an individual need to have the job title of medical assistant in order to use the CPOE function of Certified EHR Technology (CEHRT) for the entry to count toward the measure, or can they have other titles as long as their job functions are those of medical assistants? If a staff member of the eligible provider is appropriately credentialed and performs similar assistive services as a medical assistant but carries a more specific title due to either specialization of their duties or to the specialty of the medical professional they assist, he or she can use the CPOE function of CEHRT and have it count towards the measure. This determination must be made by the eligible provider based on individual workflow and the duties performed by the staff member in question. Whether a staff member carries the title of medical assistant or another job title, he or she must be credentialed to perform the medical assistant services by an organization other than the employing organization. Also, each provider must evaluate his or her own ordering workflow, including the use of CPOE, to ensure compliance with all applicable federal, state, and local law and professional guidelines.i CMS previously indicated on Sept. 4, 2012 that a layperson would be unable to perform these duties: it is apparent that the prevalent time when CDS (clinical decision support) interventions are presented is when the order is entered into CEHRT, and that not all EHRs also present CDS when the order is authorized (assuming such a multiple step ordering process is in place). This means that the person entering the order could be required to enter the order correctly, evaluate CDS either using their own judgment or through accurate relay of the information to the ordering provider, and then either make a change to the order based on the CDS intervention or bypass the intervention. We do not believe that a layperson is qualified to do this.ii Given that CMS specifically indicates that laypeople are unqualified to perform duties associated with CPOE and that those selected to assist must be certified by organizations other than the employer, ensuring that support staff members that are chosen to enter orders receive additional training and certifications such as Certified Paraoptometric Technician (CPOT), Certified Paraoptometric Assistant (CPOA), and Certified Paraoptometric (CPO) would be a positive first step. For doctors of optometry who designate staff members to enter orders into EHRs, AOA recommends enrolling the staff member(s) as AOA Associate members, which is free, and documenting how those individuals were determined to be qualified to enter orders. This documentation should include the individual s paraoptometric credentials, specific experience, education or training that might be relevant, information about the workflow used in the office to enter orders, and compliance with any pertinent local, state or federal laws. In the event that a physician is audited related to the EHR incentive program, this documented information could be helpful.

Submit educational materials to Commission on Paraoptometric Certification for approval. Oversee the recording of continuing education hours and awards, also recourse library and economic task force.. Attend meetings and report to the president/board. Secretary/Treasurer- Duties Keep accurate records of the proceedings of all board of directors and general membership meetings, submit the minutes to the president for approval within two weeks of the meeting, Attend all PPA board meetings and general membership meetings. As treasurerperform all duties assigned by the president. Be custodial of funds, legal and fiscal documents and securities received in the name of the association. Present a written, detailed financial report to the general membership. Collect and deposit all monies and dues for the association. Keep a complete and correct list of all members. Vice President of Communication/ Public Relation- Edit, publish and distribute the associations newsletter. Attend all PPA Board meetings. The V.P. of Public Relation chairs the Paraoptometric of the year award, and promotes the Girl Scout Patch Program. Attend all PPA board and general membership meetings. 9 ParaEyes Page 9 Winter 2016 Healthy Eye Food Recipes Carrot-Cumin Soup Eye-healthy nutrients: beta-carotene, vitamin A Serves 4 2 tablespoons olive oil 1 medium yellow onion, chopped 2 garlic cloves, minced 1 pound large carrots, peeled and cut into 1-inch pieces (about 2½ cups) 2½ cups vegetable broth ½ teaspoon ground cumin ¼ teaspoon ground coriander ½ teaspoon sea salt Freshly ground black pepper 1 teaspoon freshly squeezed lemon juice ½ cup plain Greek yogurt 2 tablespoons toasted cumin seeds, for garnish 1. In heavy saucepan, heat olive oil over medium-high heat and sauté onion for 2 minutes, then add garlic and sauté an additional minute. Add carrots, broth, cumin, coriander, salt, and pepper. Bring to a boil. Reduce heat, cover, and simmer until carrots are tender, about 15 minutes. 2. Purée soup with an immersion blender or, working in batches, purée in blender or food processor until smooth, then return to the saucepan. Whisk in lemon juice and yogurt. Season to taste. Ladle into bowls and sprinkle with cumin seeds.
10 ParaEyes Page 10 Winter 2016 Lentil and Orange Salad Makes 2 cups. Serves 1 as a meal or 2 as a side dish. Ingredients 1/2 cup green lentils 3/4 cups low sodium chicken broth 1 orange ½ red pepper, sliced 2 green onions, thinly sliced 1/4 cup fresh mint or 2 tsp dried, divided 1 tbsp olive oil 1 tbsp lime juice 1 tbsp apple cider vinegar Salt & Pepper Directions 1) Cook lentils. Bring chicken broth to a boil. Add just under ½ cup raw lentils, rinsed well. Return to boil, lower heat, and simmer partially covered for approximately 30 minutes or until cooked through. Drain if necessary. 2) While lentils are cooking, prepare the dressing. With the back of a spoon, smash half of the mint leaves with the lime juice, apple cider vinegar and salt and pepper. Add olive oil slowly, whisking briskly. 3) Using a fine grater, zest the orange. 4)Remove the rest of the orange peel using a knife, then chop the orange into bite sized pieces. 5) Mix lentils with orange, orange zest, pepper, green onion and the remaining mint. Gently toss with the dressing. Enjoy immediately or chill for up to 2 days. A serving of this food or recipe has 50% or more of the eye foods daily target of omega 3 fatty acids.
11 ParaEyes Page 11 Winter 2016 Whole Wheat Penne with Spinach and Gorgonzola Ingredients: 10 oz. uncooked whole wheat penne pasta Olive oil cooking spray 1 1/2 cup yellow onion, diced (~ 1 medium onion) 3 large clove garlic, minced 1/2 cup chicken broth 3 Roma tomatoes, chopped (~2 cups) 1 (6-ounce) bag fresh baby spinach 1/3 cup fresh basil, chopped or 1 teaspoon dried basil Salt and pepper to taste 2/3 cup crumbled gorgonzola cheese 1/3 cup pine nuts (optional) Directions 1.Cook pasta according to package directions, without salting water. 2.While pasta is cooking, spray a large, non-stick frying pan with cooking spray. Heat over medium-high heat. Add onions, then stir and cook until slightly transparent, approximately 5 minutes. Add garlic, stir and cook for another minute. Add broth and let simmer for 3 minutes. Add tomatoes, toss, and simmer for 2 minutes. Add spinach and basil, cook and stir for approximately 2 minutes, or until leaves wilt. Remove from heat and salt/pepper to taste. 3.Drain pasta and add to spinach mixture. Thoroughly toss. Serve on a platter and top with gorgonzola and pine nuts. Especially rich in: lutein, zeaxanthin, folate, and zinc, with modest amounts of vitamin C. Most of the omega-3s are from plant sources, which are not quite as good as the EPA and DHA in fish. Also rich in: iron, fiber, B vitamins, calcium, potassium, and copper

17 ParaEyes Page 17 Winter 2016 Why Career Fulfillment Matters Find a job you love and you shall never have to work a day in your life. I m sure you ve heard this saying before. If you ve ever worked at a job you love, you have to agree! Hopefully, you are currently working in a rewarding and exciting career. There are many reasons career fulfillment is important, here are few: Career Success is a Significant Component of Personal Success Many are taught, in order to be viewed as successful, we must attain a certain level of professional achievement. Our self-worth has become tied to our professional worth. We Spend a Large Percentage of Our Lives at Work From the age of 18 to 65, most of us work an average of eight hours a day, five days a week. That s a significant percentage of the time most will spend on Earth. Imagine spending a majority of your life in a professional career you don t enjoy. Career Is Closely Tied to Identity In our society, right or wrong, we tend to put people in boxes. Career is a common way of doing this. There s a widespread practice of stereotyping people based on what they do for a living. People might make snap judgments, but they re just as quick to let themselves be pleasantly surprised. When you re proud of the work you do, it shows. Others hear it in your voice and see it on your face.
