2 February 2017 Dear Students and Parents, The purpose of this booklet is to provide information to graduating seniors about the local scholarships and awards available. The Table of Contents represents a diverse group of associations, organizations and scholarships. It is organized by category to reflect various groups of sponsors. We hope that this will make the process easier for applicants. Eligibility guidelines and application procedures are provided for each scholarship. Unless specifically otherwise indicated, applications are due by April 1, Application forms are available on NAVIANCE or in the Guidance Office. The DHS Scholarship Application (pages 56 & 57) is used for many of the scholarships; this makes the process more efficient for you and the guidance staff. However, there are several scholarships listed that require the use of the organization s own application. The criteria for awarding scholarships and the selection process are determined by the organization or sponsor. The Guidance Department functions only as a conduit for this information. Please note some scholarship applications must be mailed directly to the committees by the student. Instructions for and and deadlines must be closely followed or consideration may not be given to applications. The DHS Scholarship Application Cover Form on (page 54 & 55) must be completed and included with the applications that are submitted to the Guidance Office. The Guidance Office cannot be responsible for processing and tracking applications that are not listed on this cover form. Additional copies of this form are available in the Guidance Office. 3 A. GENERAL/MISCELLANEOUS ALIGN CREDIT UNION ANNUAL SCHOLARSHIP Align Credit Union is committed to helping members of the communities we serve achieve their educational goals. As a result, the Align Credit Union Scholarship Program was created in 1994 as a means of assisting young adults with their pursuit of higher education. Over the past 21 years, we have awarded over 240 scholarships and we are proud that the scholarship program has evolved to include our local high schools. You are eligible to apply if you are a graduating Danvers High School senior. Applicants will be judged on the following criteria: academic success, leadership roles, school involvement, community service and participation in extra-curricular activities. Completion of the Align Credit Union Scholarship Application, transcript, resume, two (2) letters of recommendation and essay. Please include an essay in 200 words or less, Describe how credit unions positively impact your community. DEADLINE: APRIL 1 st CIDZIK FAMILY SCHOLARSHIP Based on class rank (#6 ranked student) in the Danvers High School graduating class, AND who has been accepted to a 4-year college or university. [Should the 6 th ranked student NOT be accepted to a 4-year college or university, the scholarship will be awarded to the next ranked (7 th, 8 th, etc.) student who has been accepted to a 4-year college/university. Scholarship must be claimed in the year it is awarded. NONE CIRCLE CARES INSURANCE SCHOLARSHIP Awarded to a DHS senior who will attend a 4-year college or university. The applicant should demonstrate strong academic merit along with a dedicated commitment to the Danvers Community. Completion of the DHS Scholarship Application, transcript, 2 letter of recommendation, resume and essay. Essay should be in 200 words or less. Please describe as your essay topic: The dangers of distracted driving and the impact it has on the teenage community. 4 COMMUNITY SERVICE AWARD FOR S.A.D.D. MEMBERS (DARE) Awarded to a DHS graduating senior who has participated in S.A.D.D. Completion of DHS Scholarship Application, essay, transcript, two letters of recommendation and resume. DanversCARES HEALTHY LIVING SCHOLARSHIP This scholarship will be awarded to two (2) Danvers High School seniors who best embody the mission and values of DanversCARES. The recipients will have a proven track record of healthy living and positive, value-driven decision-making DHS graduating senior A GPA of 3.0 or higher is required Recipients must have an established record of service in the Danvers community. Preference will be given to students who have volunteer, work or community service experience in health-related organizations and/or who are active members of student activities that embody principles of good health, such as but not limited to: DanversCARES, SADD, SUFA, Athletics, Atlas & Athena Mentors, Beverly Hospital, etc. Completion of the DHS Scholarship Application, transcript, and two relevant letters of recommendation must be submitted along with your response to the following questions. In 300 words or less, please answer the following: How have you exemplified healthy living during your time at Danvers High School? How will your future academic/career/post-high school goals reflect a commitment to healthy living? DEADLINE: APRIL 1st GREATER BEVERLY CHAMBER OF COMMERCE SCHOLARSHIP Awarded to a Danvers High School graduating senior who will be continuing his/her education at a 2 or 4-year college/university. 5 THE GEORGE KOUTSOS FAMILY SCHOLARSHIP AWARD Awarded to a Danvers High School graduating senior who will be pursing a career in Finance/Economics. Must be involved in the DECA program. The recipient must have demonstrated academic achievement and possesses qualities of drive, innovation and leadership. Completion of the DHS Scholarship Application, transcript, resume, two letters of recommendation and essay. DEADLINE: APRIL 1 ST PEOPLE S UNITED COMMUNITY FOUNDATION OF EASTERN MASSACHUSETTS Awarded to selected Danvers High School students who exhibit a variety of interests combining academic achievement and participation in extra-curricular activities. Students should be well respected by their peers, consistently demonstrate 100% effort, and achieve beyond expectations. Financial need is considered. Completion of DHS Scholarship Application, transcript, two letters of recommendation, resume and essay. MICHAUD MITSUBISHI SCHOLARSHIP AWARD Awarded to a Danvers High School graduating senior who has been accepted to a four-year college. This recipient must have shown great improvement in their academics and attitude from freshman to senior year and has a positive outlook in pursuing their education in college. Financial need will be considered. Completion of the DHS Scholarship Application, transcript, resume, two (2) letters of recommendation, and essay. Please explain in the essay reason why you are worthy to be the recipient of this award. 5

6 THE PURITAN LAWN MEMORIAL PARK EDUCATIONAL FOUNDATION SCHOLARSHIP Applicants must be high school seniors who have had to put family first and education second throughout their high school career, and who (1) reside within Essex, Middlesex, or Suffolk counties; (2) will be pursuing full-time education beyond the twelfth grade; and (3) need additional financial assistance to continue education. Eligible candidates will include those students who may not have achieved their academic potential because of circumstances beyond their control as an example, students who have placed family responsibilities ahead of education. Completed applications should be mailed directly to: Puritan Lawn Memorial Park Educational Foundation, Inc. 185 Lake Street Peabody, MA Deadline: Please check website for deadline WORLD LANGUAGE NATIONAL HONOR SOCIETY SCHOLARSHIP Awarded to members of the Danvers High School World Language National Honor Society who have taken French or Spanish and plan to continue studying world language. Selection is based on service to the society and the extent to which language study will be pursued in college. Completion of DHS Scholarship Application, essay, transcript, resume and a Language Teacher Recommendation Form (form included in the DHS Scholarship Instruction & Forms Booklet, to be completed by language teacher), and completion of an essay stating how and to what extent the applicant plans to pursue world languages in college, and what services he/she provided for the Society during high school years. 6

IN MEMORIAM DON BARNETT MEMORIAL SCHOLARSHIP Awarded to a graduating senior who has demonstrated academic achievement and personal commitment to school and community, with an emphasis on community. This scholarship is renewable for four-years. Completion of Don Barnett Memorial Scholarship Application, transcript, resume, two letters of recommendation, and essay. Please explain in the essay Describe your community service and what it has meant to you to give back to others? (Essay no more than 200 words) Application available on NAVIANCE or in the guidance office. MARGARET and DOM BARILLARO SCHOLARSHIP Awarded to a DHS graduating senior who has been accepted at a four-year college, and who has demonstrated academic achievement, involvement in extra curricular activities, character, and financial need. Completion of DHS Scholarship Application, essay, two letters of recommendation, resume, transcript, and evidence of financial need ( Student Aid report from FAFSA). Applicants of the BARILLARO and WILLIS E. & HELEN L. THORPE MEMORIAL SCHOLARSHIPS must complete and submit the financial information Sheet in order to be considered for these scholarships. 8 JENNIFER ANN BELL MEMORIAL SCHOLARSHIP Awarded to a DHS graduating senior who has demonstrated academic achievement and personal commitment to school and community. Please complete the Jennifer Ann Bell Scholarship Application available on NAVIANCE or in the Guidance Office. BROOKS FAMILY SCHOLARSHIP The Scholarship Committee selects recipients based on academic achievement, financial need, and general appearance of the scholarship application. Strong consideration is given to essay questions (Part III), accuracy and orderliness of answers to questions of fact (Part I and II). Application available on NAVIANCE or on file in Guidance Office. - Two complete copies of this application packet must be submitted in order to be considered for this scholarship. - All questions on the application must be answered; transcript and the most recent Federal Income Tax Return (pages 1 & 2 only) must be included. *An application may be rejected if it does not include all of the documentation above. - A detailed description of this scholarship can be found in Appendix I. THE P MICHAEL CARNEY SCHOLARSHIP FUND Awarded to a Danvers High School graduating senior who will be pursing a career in the military. Considerations include: Exhibited leadership ability, A commitment to excellence and the fair treatment of all, Family member of a veteran will be considered but not necessary, Financial need will be considered but is not necessary. 9 JOSEPH CIRUOLO MEMORIAL SCHOLARSHIP In 1958, at the age of thirteen, Joseph Ciruolo came to United States as an immigrant from Italy. Due to lack of specialized programs for English Language Learners, he was placed in grades below his age group, struggled to learn through his academic years. Joseph was a young man with limited financial opportunities. He did not underestimate himself and valued his education. Through hard work and effort, Joseph turned his hard luck into a successful career as a small business owner, operating several different establishments during his time as a restaurateur. He taught himself the importance of working hard, saving, and most importantly, the value of education to get started on the path to success. The Joseph Ciruolo Memorial Scholarship grants this scholarship opportunity to a Danvers High School graduating senior who exhibits a strong desire to better themselves through continuing education. With a strong desire to learn, as well as a plan in place to achieve long-term goals, is integral to the candidate seeking this scholarship. In your essay of 500 words or less, please tell us about yourself, your accomplishments (both academic and personal) and your plans for the future. Please be as detailed as possible when describing your goals and how you intend to achieve them. DEADLINE: April 1 st PAUL COLEMAN, Sr. MEMORIAL SCHOLARSHIP A four-year Danvers High School student who demonstrates academic achievement, personal commitment to school and community and GPA of 85% or above. DEADLINE: April 1 st WALTER J. DEMBOWSKI, JR. MEMORIAL SCHOLARSHIP Awarded to a DHS graduating senior. The recipient must be a well-rounded individual with a GPA of 3.0 or higher, significant athletic experience, participation in the arts, personal traits that reflect humility, loyalty, kindness, determination, sportsmanship and be respected by peers and faculty. 10 CORNELIUS F. DUNN MEMORIAL SCHOLARSHIP Awarded to DHS graduating seniors who have demonstrated acts that exemplify the famous Dunn motto, Ladies and Gentlemen Always. The recipient must be pursuing higher education. NONE BRIAN A. FEARER MEMORIAL SCHOLARSHIP Awarded to a DHS student who has demonstrated determination in the pursuit of academic merit and personal excellence. It shall be given to a student planning a career in journalism, literature or communication. Financial need is not a requirement. A letter addressed to Mrs. Nathan Fearer is required, with your transcript, two letters of recommendation, and resume. Please include in this letter What college have you been accepted to and what is your intended major? FINN FAMILY SCHOLARSHIP Abbott Financial Management established the Finn Family Scholarship Fund in memory of their dear friends Geoffrey and Shelia Finn. Geoff and Shelia Finn were dedicated and involved members who made tireless contributions to the Danvers community. Mr. Finn was the founding father of Danvers Youth Soccer in the late 1970 s, which Mrs. Finn was a member of the Danvers Garden Club, as well as the religious education director for St. Mary s Church. Collectively, the Finn s worked to improve the Danvers community through their leadership and volunteer efforts. With all three children growing up playing soccer in Danvers, the sport had a special place in their hearts. While academics always came first in the Finn household, soccer and community service were also highly valued. Awarded to a DHS or SJP graduating senior with a GPA of 3.0 or higher who is continuing their education, a dedicated soccer player and individual displaying strong community service. The recipient must demonstrate academic achievement through consistent effort, and show respect, kindness and compassion for others through their beliefs, personal code of conduct, and actions. Johnson, III 77 Conant Street Danvers, MA DEADLINE: April 1 st JOHN THOMAS LOCKE MEMORIAL SCHOLARSHIP John Thomas Locke achieved his life-long dream and became a Special Agent with the Federal Bureau of Investigation in Boston. He was able to achieve this goal through his drive and determination. It is the sure footedness that comes with having proved you can meet life, and deal with all its ups and downs. John did this every day of his life, and believed in always moving in a positive direction. 12 ROBERT J. LYNCH MEMORIAL SCHOLARSHIP The recipient of this award should show an interest in leadership roles such as management at work and/or team leadership skills. Be an active member of the Danvers community by volunteering to coach or instruct others in anyway, exemplify a strong work ethic by expressing a desire to mentor others, build, maintain and keep positive relationships with people of all walks of life, show a deep sense of pride and commitment in family and community. RITZER MEMORIAL SCHOLARSHIP Colleen Ritzer was a beloved Danvers High School Math teacher and a lifelong resident of Andover. A 2007 graduate of Andover High School, she graduated Magna Cum Laude in 2011 from Assumption College with a bachelor of arts in math and a concentration in secondary education and a minor in psychology. Colleen, the oldest of three children, received tremendous support and encouragement from her large and loving extended family as she pursued a career in teaching. 13

14 Colleen began teaching at Danvers High School in September of 2012 and through her passion for enlightening young minds, made an immediate impact in the lives of her students. She quickly earned a reputation as a passionate and popular teacher. Following her passing, one student summarized Colleen s dedicated approach to teaching, sharing, In the two short months I ve had Ms. Ritzer as a teacher, there are two things I have learned: Math is as good as you make it and being nice to people can leave a greater impact on their life than you think. In honor of that passion, and to create a lasting legacy, Colleen s family established the Colleen E. Ritzer Memorial Scholarship to benefit graduating Andover and Danvers High School seniors who demonstrate a passion for teaching, academic excellence and love of family. Applicants must plan to purse a degree in the field of education at an accredited four-year college or university in the fall of All students who plan to major in education or a specific subject (i.e. math, science, etc.) with a concentration in education are encouraged to apply for these competitive scholarships that are awarded to well-rounded individuals who share Colleen s passions of family and teaching. TALBOT MEMORIAL SCHOLARSHIP FUND Lynda Talbot lost a twenty-three year courageous battle with breast cancer in August Awarded to a Danvers High School graduating senior who has demonstrated academic achievement and personal commitment to school and community. THORPE MEMORIAL SCHOLARSHIP Awarded to deserving DHS graduates pursuing further education at a four-year institution. Applicants of the BARILLARO and WILLIS E. & HELEN L. THORPE MEMORIAL SCHOLARSHIPS must complete and submit the financial information Sheet in order to be considered for these scholarships. 16 ANNE TURCOTTE MEMORIAL SCHOLARSHIP : The Anne Turcotte Memorial Scholarship will award two scholarships in the amount of $1,000 each to the pair of graduating DHS seniors who best reflect the selfless, hard-working and generous nature of Anne Turcotte. : Applicants must include the DHS Scholarship Application, resume, two (2) letters of recommendation, transcript, and 500-word essay (described further below). Essay Description The Anne Turcotte Memorial Scholarship Committee will award scholarships to a pair of Danvers High School graduating seniors who best exemplify the qualities of Anne Turcotte, who passed away March 2013 at age 50. The Anne Turcotte Memorial Scholarship honors Anne s life and her indelible legacy. She was a daughter, a wife, a mother, a grandmother, a sister, a friend, a colleague, and a benefactor. She was vivacious, fiery and lived life to the fullest. She was your loudest fan and most ardent supporter, but watch out to anyone who threatened the people she loved. She opened her home to numerous transients with a warm meal, comfortable quarters and a healthy serving of advice like it or not. Anne was always on the go, whether it was a night out with the girls, a boat excursion to Kettle Cove, or any event where she could marvel at her beloved grandchildren. She worked tirelessly to advance tourism in Salem, often traveling abroad herself whilst collecting friends and hearts along the way. But it was at home where Anne seemed happiest. There she hosted countless parties and impromptu get-togethers rejuvenated, it seemed, by the laughter and love of the family and friends who surrounded her. And in her year-long battle with cancer, Anne epitomized dignity, grace and compassion wanting only the best for all who she was to leave behind. All that she asked was that her grandchildren remember her, which this scholarship helps to ensure. To be considered an Anne Turcotte Memorial Scholarship recipient, write a 500-word essay about someone who impacted your life the way Anne impacted her family, friends and even complete strangers. Explain how you know this person, the qualities they share with Anne and in what special ways this person changed your life. In particular, show how this person s undying sacrifices, generosity and love forever changed you. PHILIP A. 18 DANVERS GIRLS SOFTBALL OTTO MOULTON SCHOLARSHIP The Danvers Girls Softball/Otto Moulton Scholarship philosophy is that the individual considered for the award be one who has a love of softball while also displaying good sportsmanship and teamwork.
